경추 견인기 부착 여부에 따른 발 형태별 보행 시 족저압에 미치는 영향 (The Effects of Neck Traction and Foot Type on Plantar Pressure Distribution during Walking)

  • Objective: The purpose of this study was to investigate the effect of neck traction and foot type on plantar pressure distribution during walking. Method: Total of 24 data were collected from women working with a computer for more than 6 hours every day. Three groups by foot type were divided: Pes Planus, normal foot, and Pes Cavus. Depending on the foot type and cervical traction, plantar pressure variables were measured; CA, MF, PP, and CT. Each variable was divided into 12 masks. MANOVA was performed for the difference of plantar pressure variables by foot type, and a paired t-test was performed for the cervical traction within groups. Results: The total CA decreased in the Pes Planus (p<.001) and Pes Cavus (p<.05) groups. MF increased in the big toe (p<.01) and 2nd toe (p<.05) of the normal foot, and MF-3rd metatarsal decreased (p<.01). The MF-2nd toe (p<.01) and 3rd toe (p<.05) of Pes Cavus decreased. The PP decreased in 2nd toe (p<.05), 3rd toe (p<.01), and 4th toe (p<.05) of the Pes Cavus. In normal foot, the PP-3rd metatarsal (p<.05) and PP-4th metatarsal (p<.01) reduced. In Pes Planus, PP decreased in the hindfoot (p<.05). In Pes Cavus group wearing a neck-tractor, the CT-hindfoot increased (p<.05). Conclusion: There was a significant change in the plantar pressure change by foot type after neck traction. When walking with a neck-tractor, the heel impact was alleviated in the Pes Planus, and the Pes Cavus showed the smooth and effective propulsion in the push-off. Overall, weight acceptance was effectively performed when walking with neck-traction. It was also found that the neck-tractor corrects the alignment of the neck, thereby creating a more stable gait pattern.

  • This paper describes the possibility of analyzing gait pattern from the variation of the lower leg electrical impedance. This impedance is measured by the four-electrode method. Two current electrodes are applied to the thigh and foot., and two potential electrodes are applied to the lateral aspect. medial aspect, and posterior position of lower leg. We found the optimal electrode position for knee and ankle joint movements based on high correlation coefficient, least Interference, and maximum magnitude of impedance change. From such features of the lower leg impedance, it has been made clear that different movement patterns exhibit different impedance patterns and impedance level.

  • Purpose: The purpose of this study was to investigate change of muscle activities during level walking, stairs and ramp climbing in old adults. Methods: Twelve old adults were recruited and agreed this study. Muscle activity was measured by MP150 system (BIOPAC System Inc., Santa Barbara, USA). Statistical analysis was used one-way ANOVA to know the difference according to gait conditions (level walking, stairs and ramp ascending) Results: In stance phase, muscle activities of low extremities with old adults were generally significant difference in ramp ascending. In swing phase, rectus femoris and biceps femoris activity in old adults generally more increased during stairs ascending and tibialis anterior and gastrocnemius activity in old adults generally increased during ramp ascending. Conclusion: These results indicate that stair and ramp climbing is different muscle recruit pattern to level walking.

흰쥐 좌골신경에 신경파괴제 주입 후 초래되는 하지 운동과 신경조직학적 변화 (Functional and Histopathological Changes Following Injection of Neurolytic Agents on Rat's Sciatic Nerve)

  • Background: This study was undertaken to observe the functional changes of the hind limb and histopathological changes in the sciatic nerve after an injection of alcohol or phenol, which are commonly used neurolytic agents, highlighting the time of recovery. Methods: Forty-eight Sprague-Dawley rats weighing 200-300 g were used for the experiment. Histopathological changes under the electron microscope, were observed in the distal part of the sciatic nerve, into which 0.1 ml of alcohol or phenol was injected. This was severed in 3 rats of each group at 10 minutes, 1 hour, 24 hours, 3 days, 1, 2, 4 and 6 weeks later. The functional changes in the hind limbs were observed for 6 weeks by noting their walking pattern. Results: Following the injection of alcohol or phenol into the right sciatic nerve, the right hind limb showed a severe pronounced motor weakness and obvious gait changes. About 2 weeks later, gradual improvement of gait changes began, and after 6 weeks, the motor weakness and gait changes were no longer perceptible in both groups. The findings of any histopathological change were similar in both alcohol or phenol groups. At 10 minutes after injection, destructive lesions were confined to the unmyelinated fibers and the myelin sheath of small the myelinated fibers. On the 3rd day and at 1 week, pathologic changes in axonal fibers and Schwann cells were in being phagocytized in spite of myelin restitution. From 2 to 4 weeks, axonal regeneration and remyelination appeared at the same time a myelin disintegration and axonolysis. At 6 weeks, neural regeneration was similar to that of the contralateral control group. Conclusions: These results suggest that functional and histopathological changes, after injection of neurolytics into the peripheral nerves, are quite similar in both alcohol and phenol groups. The progression of functional and histopathological changes become more obvious according to the time interval following the injection. Consequently, side effects that develop following the use of alcohol or phenol may begin to improve around the time that nerve regeneration occurs, i.e., two to four weeks later.

발의 과도한 회내 상태가 슬관절 근력과 보행에 미치는 영향 (Effects of excessive Pronation of the Foot on Knee joint Strength and Gait)

  • BACKGROUND: This study aimed to determine the changes in muscle strength and walking ability in patients who complained of knee instability due to excessive pronation of the foot. METHODS: Twenty patients (ten men and ten women) who complained of instability of the knee joint due to excessive pronation of the foot participated in the experiment. In the experimental group, the internal rotation of the tibia caused by excessive adduction of the foot was maintained as external rotation, and the joint state was to recognize the movement of the joint position changed through maintenance of the muscle. This exercise was performed five times for each patient, and the muscle strength maintenance was performed for 20 seconds. In the control group, stretching and range of motion (ROM) exercises were performed. For the stretching exercise, one specific motion was performed for 20 second, and the ROM exercise was performed to confirm the change in muscle strength in the knee joint area and walking ability. RESULTS: The knee flexion and extension strength in the patients with excessive pronation of the foot differed significantly from those in the subjects from the control group (p<.05). Further, the before-after comparison of the step time and length in the evaluation of walking ability, which affects overall postural movement due to knee joint instability, revealed a significant difference between the experimental and control groups (p<.05). CONCLUSION: The patients that were subjected to manual therapy and ROM exercise for the knee joint showed improved knee joint muscle strength and walking ability compared to the subjects from the control group.

척수신경손상 환자의 보행훈련 전.후의 능동적 근육제어의 변화 (Voluntary Motor Control Change after Gait Training in Patients with Spinal Cord Injury)

  • 본 연구에서는 보행훈련 전후에 실시된 보행속도 측정 결과와 누운 자세에서 능동적 발목 배굴시의 근전도 활동 측정 결과를 비교하였다. 9명의 불완전 척수손상 환자들이 3개월 동안 하루 20분 주 5회로 이루어진 체중부하 감소장치 및 트레드밀을 이용한 보행훈련 (STAT supper-ted treadmill ambulation training)에 참가하였다. 보행속도 실험과 능동운동 실험은 보행훈련 시작 및 종료된 시점의 동일한 날, 혹은 같은 주에 실시되었다. 환자의 능동운동시의 표면근전도 값을 측정하였고. 동일한 운동시 열명의 건강한 피실험자들로부터 측정된 근전도값을 기준값으로 비교하는데 사용하였다 하지 열곳의 근육에서 측정된 표면근전도 값은 반응벡터 (response vector)라고 명명되었고 크기 및 근육 제어 분포 패턴을 관찰하는데 사용되었다. 척수손상 환자들의 보행속도는 0.47$\pm$0.35 m/s에서 0.68$\pm$0.52 m/s로 유의한 증가를 보였다. 근전도 해석결과에서는 우측 발목관절 배굴시에 근전도의 증가를 보인 6인의 환자들의 값은 109.7$\pm$148.5 $\mu\textrm{V}$에서 145.9 $\pm$ 180.7 $\mu\textrm{V}$ 로 증가하였으나 유의한 수준은 아니었다 (p〈0.055). 또한 좌측 발목의 배굴에서 측정된 근전도에서는 단 2명만이 상관계수와 근전도 증가를 보였다 보행훈련 후의 환자들의 근육 활동의 변화는 개개인의 상태에 따라 매우 다양하게 변하였고. 보행훈련 후의 상태는 보행훈련 전의 상태에 매우 의존적임을 알 수 있었다 정량적인 해석을 위해 새로 도입된 반응벡터의 사용은 척수손상 환자들의 보행훈련 전후에 환자의 상태를 평가. 예측, 혹은 훈련의 방향 설정시에 유용하게 사용될 수 있는 가능성을 보여주었다.ne(chloroform)와 toluene 등이 가장 자주 검출되는 성분이었다. 대부분의 aromatic hydrocarbons, alkyl benzenes및 다른 유기용매 성분은 검출농도 이하이다.한 대사적인 문제와 생활 양식에 의한 영향이 클 것으로 추정된다. 그러므로 BMI가 높은 노인들에게 부족되고 있는 영양소에 대한 연구가 이루어져야 할 것이다. 특히 비타민 A, E B 복합체는 노인성 질병과 관련하여 그 기능이 규명되어야 할 영양소이다. 이는 항산화효소의 증가에 의한 지질 과산화물 생성삼소와 PGE$_2$및 TXA$_2$등의 유의적인 감소가 유선암세포의 증식을 억제하는 인자로 작용할 수 있는 가능성을 시사하였다. 또한 지방산 투여 에 따른 과산화물 생성과 항산화 시스템이 유선암 세포에서는 상호기전적으로 작용하지 않았으나 이는 농도와 배양시간에 따른 CLA의 항암효과가 in vivo 실험에서 재검증 할 필요가 있음을 시사한다. 또한 장시간의 세포배양 실험시 PGE$_2$는 반감기가 짧고 다른 대사물로 쉽게 전환되는 불안전한 물질이므로 새로운 측정방법의 개발이 필요하겠다. 더욱이 CLA에 의한 항암효과가 정확히 어떤 기전에 의한 것인지 보다 깊이 있는 연구가 필요하지만 본 연구에서 얻은 에이코사노이드 대사의 변화 등은 유선암 예방 연구에 기초적인 자료가 될 것으로 생각된다.들어가 면서 점차 감소하여 대조군과 유사한 양상을 나타내었으나, 전체적으로 대조군보다 높은 수치를 나타내었다. 반면에 5, 10 $\mu\textrm{g}$ L$^{-1}$의 농도에 노출된 세포는 초기에 매우 낮은 탄소동화율을 보였으며, 5 $\mu\textrm{g}$ L$^{-1}$의 농도에 노출시킨 세포의 경우 12일째부터 단위세포 당 탄소동화율이 매우 크게 증가하였다. 본 연구의

  • Purpose : The purpose of this study was to investigate characteristics of the muscle activities during level walking and stairs ascending in young adults. Methods : Fifteen young adult were recruited this study. Muscle activity (BIOPAC System Inc., Santa Barbara, U.SA). Statistical analysis was difference between level and stair walking. Results : In stance phase, muscle activity of low extremity generally more increased during ramp ascent both young adults. In swing phase, muscle activity of low extremity generally more increased during stairs ascent in young adults. Conclusion : These results indicate that stair and ramp ascent is more difficult task than level walking in young adults. Muscle activity was more changed at ramp ascent. In the future, we suggest that studies of stair and ramp gait pattern regarding ambulatory patient with disabilities be further studied and an appropriate stairs and ramp inclination will be indicated.

Transcallosal Ipsilateral Motor Pathway from the Unaffected Hemisphere in a Patient with Traumatic Brain Injury

  • Purpose: We presented with a patient with traumatic brain injury who showed a transcallosal ipsilateral motor pathway from unaffected motor cortex to affected extremities, as evaluated by diffusion tensor tractography (DTT). Methods: One patient and six age-matched normal subjects were enrolled. A 42-year-old left hemiparetic male who suffered from brain injury by motor vehicle accident 9 years ago showed a leukomalactic lesion in the right corona radiata and parieto-temporal lobe. His left extremities were completely paralyzed initially, but recovered slowly over 2 years. At the time of the evaluation, he was able to grasp and release an object, and to walk with spastic gait pattern. DTT was performed using 1.5 T with a Synergy-L Sensitivity Encoding head coil. DTT was obtained with termination criteria of FA <0.2 and an angle change > $45^{\circ}c$. Results: The motor tracts of the unaffected (left) hemisphere of the patient and control subjects originated from the motor cortex and descended along the known corticospinal tract without any transcallosal tract. By contrast, the tract of the affected (right) hemisphere originated from the left premotor cortex, descended through the left corona radiata, and then crossed the mid-portion of the corpus callosum. The tract then descended through the known corticospinal tract pathway to the right medulla. Conclusion: We conclude that the transcallosal ipsilateral motor pathway from the unaffected hemisphere appeared to contribute to the motor recovery in this patient.
