Surgical Treatment of Recalcitrant Wart (난치성 사마귀의 수술적 치료)

  • Purpose: A wart is caused by epidermal infection with the human papilloma virus. Although wart naturally disappears in some cases, it require treatment because of pain, aesthetic problem, and the possibility of malignant change. Conventional non-surgical treatment cannot be a fundamental solution for the pain and has such disadvantages as frequent recurrence and difficulties in achieving a satisfactory outcome. A surgical procedure was performed on patients with wart and the procedure had a good outcome. Methods: We investigated the gender, age, lesion site, mean treatment duration, and presence or absence of recurrence in 21 patients with a wart within the period of January 2007 to July 2011. For local lesions, primary closure, including subcuticular suture after the excision, was performed. If the defect size was too big to do primary closure, we performed rotation flap. For wide multiple lesions, a split thickness skin graft was performed. Results: Among the 21 patients, 12 patients were male and 9 patients were female, and their mean age was 42 years (SD=17.38, range: 11~75 years). The lesion site was the foot in 10 patients, the hand in 8 patients, the face in 2 patients, and the scalp in 1 patient. The mean treatment duration was 13.5 days (SD=4.36, range: 6~15 days) for the primary closure or rotation flap, and 18.5 days (SD=2.12, range: 17~20 days) for the skin graft. 20 patients were cured without recurrence. No recurrence was observed in the patients who underwent primary closure or rotation flap. One of the two patients who underwent a skin graft of their wart that had covered their entire palm had local recurrence in part of her finger tips. Conclusion: We performed surgical procedure on recalcitrant wart. As a results, we can treat it with short treatment duration, low recurrence rate and less scarring and get high patient satisfaction.

A Case of Descending Necrotizing Mediastinitis (하행 괴사성 종격동염의 치험례)

  • Purpose: Cervical necrotizing fasciitis tends to involve the deep soft tissues and spread caudally to the anterior chest and mediastinum, often resulting in major complications and death. It may rapidly spread into the thorax along fascial planes, and the associated diagnostic delay results in this descending necrotizing mediastinitis. So, aggressive multidisciplinary therapy with surgical drainage is mandatory. We present a very rare case of descending necrotizing mediastinitis with literature review. Methods: A 53 years old male visited our department 7 days after trauma in neck. His premorbid conditions and risk factors of necrotizing fasciitis were concealed hepatoma, trauma history, chronic liver disease, and nutrition deficit. Computed tomographic scans of the head and neck region were performed in this patient : signs of necrotizing fasciitis, were seen in the platysma, sternocleidomastoid, trapezius muscle and strap muscles of the neck. Fluid accumulations involved multiple neck spaces and mediastinum. At the time, he diagnosed as necrotizing fasciitis on his neck and anterior chest. Necrotic wound was excised serially and we treated this with the Vacuum - assisted closure(VAC, Kinetics Concepts International, San Antonio, Texas) system device. After appropriately shaping the sponge and achieving additional 3 pieces drainage tubes in the pockets, continuous negative pressure of 125 mmHg was applied. The VAC therapy was utilized for a period of 12 days. Results: We obtained satisfactory results from wide excision, abscess drainage with the VAC system, and then split thickness skin graft. The postoperative course was uneventful. Conclusion: The refined technique using the VAC system can provide a means of simple and effective management for the descending necrotizing mediastinitis, with better cosmetic and functional results. Finally, the VAC system has been adopted as the standard treatment for deep cervical and mediastinal wound infections as a result of the excellent clinical outcome.

Correction of Burn Scar Contracture: Indication and Choice of Free Flap (화상 반흔구축 재건 시 유리피판술의 적응증 및 적절한 피판의 선택)

  • Purpose: Most burn scar contractures are curable with skin grafts, but free flaps may be needed in some cases. Due to the adjacent tissue scarring, local flap is rarely used, and thus we may consider free flap which gives us more options than local flap. However, inappropriate performance of free flap may lead to unsatisfactory results despite technical complexity and enormous amount of effort. The author will discuss the points we should consider when using free flaps in treating burn scar contractures Methods: We surveyed patients who underwent free flaps to correct burn scar contractures from 2000 to 2007. We divided patients into two groups. The first group was those in which free flaps were inevitable due to exposure of deep structures such as bones and tendons. The second group was those in which free flap was used to minimize scar contracture and to achieve aesthetic result. Results: We performed 44 free flap on 42 patients. All of the flaps were taken well except one case of partial necrosis and wound dehiscence. Forearm free flap was the most common with 21 cases. Most of the cases(28 cases) in which free flaps were inevitable were on the wrist and lower limbs. These were cases of soft tissue defect due to wide and extensive burns. Free flaps were done in 16 cases to minimize scar contracture and to obtain aesthetic outcome, recipient sites were mostly face and upper extremities. Conclusion: When using free flaps for correction of burn scar contractures, proper release and full resurfacing of the contracture should be carried out in advance. If inadequate free flap is performed, secondary correction is more challenging than in skin grafts. In order to optimize the result of reconstruction, flap thickness, size and scar of the recipient site should be considered, then we can achieve natural shape, and minimize additional correction.

  • There are serious damage of people and properties every year due to landslides that are occurred by heavy rain. Because these phenomena repeat and the heavy rain is not an atmospheric anomaly, the counter plan becomes necessary. The study area, Ulsan, is one of the seven metropolitan, and largest cities of Korea and has many large facilities such as petrochemical complex and factories of automobile and shipbuilding. So it is necessary assess the landslide hazard potential. In the study. the three steps of landslide hazard assessment techniques such as susceptibility, possibility, and risk were performed to the study area using GIS. For the analyses, the topographic, geologic, soil, forest, meteorological, and population and facility spatial database were constructed. Landslide susceptibility representing how susceptible to a given area was assessed by overlay of the slope, aspect, curvature of topography from the topographic DB, type, material, drainage and effective thickness of soil from the soil DB, lype age, diameter and density from forest DB and land use. Then landslide possibility representing how possible to landslide was assessed by overlay of the susceptibility and rainfall frequency map, Finally, landslide risk representing how dangerous to people and facility was assessed by overlay of the possibil. ity and the population and facility density maps The assessment results can be used to urban and land use plan for landslide hazard prevention.

Cortical Network Activated by Korean Traditional Opera (Pansori): A Functional MR Study

  • The Pansori is a Korean traditional vocal music that has a unique story and melody which converts deep emotion into art. It has both verbal and emotional components. which can be coordinated by large-scale neural network. The purpose of this study is to illustrate the cortical network activated by a Korean traditional opera, Pansori, with different emotional valence using functional MRI (fMRI).Nine right-handed volunteers participated. Their mean age was 25.3 and the mean modified Edinburgh score was +90.1. Activation tasks were designed for the subjects to passively listen to the two parts of Pansories with sad or hilarious emotional valence. White noise was introduced during the control periods. Imaging was conducted on a 1.5T Siemens Vision Vision scanner. Single-shot echoplanar fMRI scans (TR/TE 3840/40 ms, flip angle 90, FOV 220, 64 x 64 matrix, 6mm thickness) were acquired in 20 contiguous slices. Imaging data were motion-corrected, coregistered, normalized, and smoothed using SPM-96 software.Bilateral posterior temporal regions were activated in both of Pansori tasks, but different asymmetry between the tasks was found. The Pansori with sad emotion showed more activation in the light superior temporal regions as well as the right inferior frontal and the orbitofrontal areas than in the right superior temporal regions as well as the right inferior frontal and the orbitofrontal areas than in the left side. In the Pansori with hilarious emotion, there was a remarkable activation in the left hemisphere especially at the posterior temporal and the temporooccipital regions as well as in the left inferior and the prefrontal areas. After subtraction between two tasks, the sad Pansori showed more activation in the right temporoparietal and the orbitofrontal areas, in contrast, the one with hilarious emotion showed more activation in the left temporal and the prefrontal areas. These results suggested that different hemispheric asymmetry and cortical areas are subserved for the processing of different emotional valences carried by the Pansories.

Effects of Amino Acid-enriched Ruminally Protected Fatty Acids on Plasma Metabolites, Growth Performance and Carcass Characteristics of Hanwoo Steers

  • This study was conducted to determine the effects of amino acid-enriched ruminally protected fatty acid (AARPFA) on plasma fatty acids and amino acids, growth performance and carcass characteristics of Korean native steers (Hanwoo) by simultaneous supply of fatty acids and limiting amino acids (methionine and lysine). Eighteen finishing Hanwoo steers, 18 months of age and weighing an average of $459.0{\pm}38.9\;kg$, were used for studies of the metabolism of plasma fatty acids and amino acids during supplementation of AARPFA. Also, 45 finishing Hanwoo steers, 16 months of age and weighing an average of $408.6{\pm}26.5\;kg$, were used for growth performance and carcass characteristics during supplemention of AARPFA. There were three treatments which comprised a basal diet supplemented with AARPFA at 0 g (T1), 50 g (T2) or 100 g (T3), respectively. Concentrations of saturated, unsaturated and total fatty acids in plasma were increased in T3 compared with other treatments (p<0.05). Concentrations of methionine and lysine in plasma were linearly increased with increasing levels of AARPFA (p<0.01). Average daily gain, dry matter intake and feed conversion ratio were not different among the treatments. Marbling score measured by ultra-sound scanning was higher in T3 than in T1 at 24 months of age (p<0.05). Rib eye area, back fat thickness, yield index and yield grade score were similar across the treatments. Marbling score and quality grade score were higher in T3 compared with other treatments (p<0.01). Thus, plasma fatty acids, methionine and lysine metabolism were affected by supplementing with 100 g of AARPFA which also had positive effects on marbling score and meat quality grade of finishing Hanwoo steers.

Moisture Content Change and Heat Distribution Characteristics of Veneer Heated by Microwave (마이크로파 가열 단판의 함수율 변화 및 열분포 특성)

  • The analysis of shape before and after heating, Surface moisture content, Moisture weight change and surface heat distribution by fixed type microwave heating in forms of flat veneer and veneer roll of Korean pine, pitch pine, larch and yellow poplar was conducted. The results were as follows: In case of flat veneer, the quality after microwave heating was comparatively good, but it was somewhat warped. These phenomena may be due to transformation by nonuniform drying stress and stronger effect of local irradiation on the veneers when heating veneer owing to the characteristics of fixed type microwave equipment. In case of the features of roll-shaped veneer heated by microwave, the quality after heating was comprehensively excellent. Especially there was no warping unlike flat veneer. Heat distribution and diffusion were also very stable for roll-shaped veneer and such heat distribution had much influence on surface moisture content and moisture weight loss. Accordingly, the veneer roll would show sufficient drying efficiency in fixed type microwave equipment through a scrutinized examination on generating power and irradiation time according to species and thickness of veneer.

Step-Coverage Consideration of Inter Metal Dielectrics in DLM Processing : PECVD and $O_3$ ThCVD Oxides (이층 배선공정에서 층간 절연막의 층덮힘성 연구 : PECVD와 $O_3$ThCVD 산화막)

  • An investigation on the step-coverage of PECVD and $O_3$ ThCVD oxides was undertaken to implement into the void-free inter metal dielectric planarization using multi-chamber system for the submicron double level metallization. At various initial aspect ratios the instantaneous aspect ratios were measured through modelling and experiment by depositing the oxides up to $0.9{\mu}m$ in thickness in order to monitor the onset of void formation. The modelling was found to be in a good agreement with the observed instantaneous aspect ratio of TEOS-based PECVD oxide whose re-entrant angle was less than $5^{\circ}$. It is demonstrated that either keeping the instantaneous aspect ratio of PECVD oxide as a first layer less than a factor of 0.8 or employing Ar sputter etch to create sloped oxide edge ensures the void-free planarization after$O_3$ ThCVD oxide deposition whose step-coverage is superior to PECVD oxide. It has been observed that $O_3$ ThCVD oxide etchback scheme has shown higher yield of via contact chain than non etchback process, with resistance per via contact of $0.1~0.3{\Omega}/{\mu}m^2$.

Development of Spatial Landslide Information System and Application of Spatial Landslide Information (산사태 공간 정보시스템 개발 및 산사태 공간 정보의 활용)

  • The purpose of this study is to develop and apply spatial landslide information system using Geographic information system (GIS) in concerned with spatial data. Landslide locations detected from interpretation of aerial photo and field survey, and topographic , soil , forest , and geological maps of the study area, Yongin were collected and constructed into spatial database using GIS. As landslide occurrence factors, slope, aspect and curvature of topography were calculated from the topographic database. Texture, material, drainage and effective thickness of soil were extracted from the soil database, and type, age, diameter and density of wood were extracted from the forest database. Lithology was extracted from the geological database, and land use was classified from the Landsat TM satellite image. In addition, landslide damageable objects such as building, road, rail and other facility were extracted from the topographic database. Landslide susceptibility was analyzed using the landslide occurrence factors by probability, logistic regression and neural network methods. The spatial landslide information system was developed to retrieve the constructed GIS database and landslide susceptibility . The system was developed using Arc View script language(Avenue), and consisted of pull-down and icon menus for easy use. Also, the constructed database can be retrieved through Internet World Wide Web (WWW) using Internet GIS technology.

High Frequency Properties of Fe93.5Si6.5 Magnetic Powder/Epoxy Composite Film (Fe93.5Si6.5 자성분말/에폭시 복합재 필름의 고주파 특성)

  • Composites of $Fe_{93.5}Si_{6.5}$ powder and epoxy were prepared using a thermal curing process. Scanning electron microscope (SEM), vibrating sample magnetometer (VSM) and network analyzer were used to analyze the structure, electromagnetic properties and microwave absorption of the composites. Results show that the saturation magnetization depends on the fraction of the $Fe_{93.5}Si_{6.5}$ powder in the composite, which affects initial permeability. It is believed that the eddy current loss is a dominant factor over 1 GHz and that the resonance frequency of the composite decreases with increasing fractions of $Fe_{93.5}Si_{6.5}$ powder. Finally, reflection loss was calculated from the permeability and permittivity of these composites. Composite with 50 wt.% $Fe_{93.5}Si_{6.5}$ powder fractions and 5 mm thickness showed reflection loss below -20 dB from 3.66 GHz to 4.16 GHz. Therefore, it is believed that thin Fe-Si/epoxy composites may be a good candidate for microwave absorption application.
