  • Objectives: The aim of this study is to analyze the utilization behaviour and influencing factors of complementary and alternative medicine (CAM) therapies among elderly people with chronic diseases. Methods: The study population was selected among 9 welfare facilities for the aged in 2 cities among Daegu Metropolitan City & Gyeongsangnam Province by two-stage cluster sampling. 250 senior citizens participated in a face-to-face interview using a structured questionnaire. Logistic regression analysis was performed to examine the association between the utilization of CAM therapies and various characteristics of the elderly. All statistics were analyzed using the PASW (ver 18.0). Results: Among 233 elderly peoples, 70.4% (164 persons) had used various kinds of CAM therapies (including nutritional methods, pharmacologic and biologic treatments, etc.) more than once during the last year. 48.8% (64 persons) ~ 60.7% (88 persons) of the elderly used CAM therapies without health and medical experts' counsel. The elderly with perceptions of CAM therapy used it 2 times more than those without knowledge of CAM therapies. The number of chronic diseases was more likely to increase the usage of nutritional methods (OR=2.92, 95% CI: 1.07-7.97), manipulative and body-based practices (OR=5.85, 95% CI: 1.97-17.34), pharmacologic and biologic treatments (OR=2.92, 95% CI: 1.49-8.17). Elderly with diabetes used nutritional methods 3.76 (95% CI: 1.49-9.47) more than elderly without diabetes. Conclusions: CAM therapies use in the aged with chronic diseases appears common. The findings suggest that the clinical efficacy and safety of CAM therapies on medical management of chronic disease may be investigated and that patient-physician communication need to be strengthened.

  • Lymphangioleiomyomatosis, a rare disease in women of childbearing age, is the result of benign nodular hypertrophy of the smooth muscle of the lypmhatics and other tissues of the abdomen and thorax. We report a 36-years-old woman with pulmonary and retroperitoneal lymphangioleiomyomat.osis who responded with hormone treatment. She developed vaginal pruritis and a pelvic ultraound was done given her significant past medical history. Ultrasound examination demonstrated a large mass in the right side of her pelvis. Therefore she was admitted to St. Michael's Hospital in Toronto for laparoscopy. Result of cytology was to be consistent with the diagnosis of retroperitoneal lymphangioleiomyomatosis. High resolution CT sacn of the thorax demonstrated multiple small cystic lesions, without associated nodularity compatible with a diagnosis of pulmonary lymphangioleiomyomatosis. She has been taking Provera tablets 100mg po tid since Dec. 15, 1993. We have given her a prescription for Depo provera 500mg IM monthly since she came back to Korea. and made arrangements for regular follow up monthly. We performed chest X-ray, CT of chest(high resolution), abdomen and pelvis, pulmonary function tests and arterial blood gas analysis. Chest X-ray and CT findings showed no significant change since July. 20, 1993.

  • Edentulous patients with severe alveolar bone resorption have trouble with using traditional complete denture. In order to overcome these problems, implant-retained overdenture was developed. SFI-bar$^{(R)}$ system can save time and cost compared to other existing bar systems which need complicated laboratory procedures because it can be adjusted directly in a patient's mouth. A 55-year-old male, who had experienced a fractured lower old implant-retained overdenture, wanted a durable and painless denture. The fractured Locator$^{(R)}$ attachments were removed and edentulous mandible was restored with SFI-bar$^{(R)}$. A 77-year-old female with a medical history of the Parkinson's disease and severely absorbed alveolar bone of mandible, wanted to wear a retentive mandibular denture without pain. After placing two implants in front of mental foramen, two adaptors were connected to two implants and a tube bar was connected to the adaptors. A female part fitted to the bar was attached to the new denture. These clinical reports describe two-implant-retained overdenture using the SFI-bar$^{(R)}$ system in mandibular edentulous patients. Since the patients were satisfied esthetically and functionally during 2 years' observation, we would like to report cases.

  • Symptoms of summer patch were observed on Kentucky bluegrass (Poa pratensis L.) cv. "Midnight II" from mid-June in 2015 in Seoul, Korea. The symptoms appeared as leaf blight, root rot, and frog-eye patch, which are typical of summer patch. To identify the causal agent of these symptoms, a pathogen was isolated from diseased leaves and roots, and the cultural, morphological, and phylogenetic characteristics were analyzed. The isolate reached 50-60 mm on potato dextrose agar (PDA) after 10 days as a white-grey mycelium with septa, and became olive-green or brown from the center. Phialide-like structures were observed at the ends of hyphae, and conidia were rarely observed. A phylogenetic analysis was conducted based on large subunit (LSU) and RNA polymerase II large subunit (RPB1) sequences. According to this analysis, the isolated pathogen was confirmed to be Magnaporthiopsis poae. In a pathogenicity test, summer patch symptoms were observed at 20 days after inoculation using the same grass cultivar. This is the first report of summer patch disease caused by M. poae on cool season grass in Korea.

  • Purpose: To evaluate the factors affecting hospital utilization for respiratory diseases by ecological study design and GIS tool. To raise the social concern for respiratory disease by the result. Methods: Hospital admission data supported by national health insurance cooperation were transformed to spread sheet data format and combined with air monitoring dataset. Air pollution data were collected from the annual report of air monitoring published by Korea Ministry of Environment. Socioeconomic statistics including population density, age distribution, forest ratio etc.. were filed using Korea National Statistical Office database. Multiple linear regression analysis was performed to evaluate the factors affecting hospital utilization for respiratory diseases. Analytical unit was 52 cities. Results: The factors affecting hospital utilization for respiratory diseases were the proportion of population 60 years and over, seaside city, $O_3$ level, smoking rate. Conclusions: However, outdoor pollutants monitoring data and smoking rate have weakness in reflecting individual exposure. Further research is required to propose more illustrative means to evaluate causal relationship between air pollution and respiratory health effect factors.

  • Recently, translational research (TR) in health technology (HT) has been considered as an emerging alternative research system for the improvement of human health. TR from bench to bedside involves a strong bidirectional relationship between basic science discovery and clinical practice. To support R&D planning and policy in HT effectively, the performance of TR programs was analyzed and evaluated in a R&D project on health and medical technology. TR programs were classified into three parts: unilateral TR, bilateral TR and multilateral TR. Bibliometrics and citation analysis were performed to assess research papers and gather information for the performance analysis of TR programs. In addition, both quantitative and qualitative analysis were successfully carried out using ISI Web of Science, Google Scholar Citations, SCOPUS and Knowledgematrix. In conclusion, the performance analysis of TR programs could significantly improve the efficiency of R&D plans, R&D management and evaluation for a safe and healthy life.

  • Objectives: This study aims to identify the influential factors on the pulmonary function of targeted general residents in the areas surrounding Gwangyang Steel Mill. Methods: An PFT (Pulmonary Function Test) was conducted from May 2007 to November 2007 on 974 target residents (438 male, 536 female), including an exposed group (674 people) who resided within a radius of 5 km from Gwangyang Steel Mill in Gwangyang-si, Jeollanam-do and a control group (300 people) who resided outside a radius of 15 km. A survey related to personal characteristics, life habits, respiratory diseases and allergic symptoms, medical histories and living environments of the residents was also conducted to identify influential factors on pulmonary function. Results: As %$FEV_1$ and %FVC of the exposed group are 99.17% and 96.98%, respectively, and those of the control group are 105.47% and 101.91%, respectively, with the PFT values of the exposed group being lower than those of the control group (p<0.001), it turns out that the pollution in the industrial complex is likely to trigger a reduction in the pulmonary function of local residents. The odds ratio analysis result for asthma diagnosis history indicates that the odds ratios tend to be higher among surveyed residents who are living near above-average traffic volumes and are using beds, where it is statistically meaningful that especially the odds ratios are higher if the residences are closer to roads (p<0.01, CI=1.12-4.52). The factors that affect the FEV1 are identified as smoking, passive smoking, asthma diagnosis history, nasal congestion symptoms and allergic eye disease ($R^2$=0.154, p<0.001). The factors that affect the FVC are identified as smoking, passive smoking, asthma diagnosis history and allergic coryza ($R^2$=0.158, p<0.001). In addition, the analysis result for the relevance of air pollution levels to pulmonary function ($FEV_1$, FVC) shows that FEV1 and FVC tend to decrease as the concentration of $O_3$ increases. Conclusions: We believe that this study may provide preliminary data for the development of preventive measures for health effects on the residents and environmental health control measures for environmental pollution in the industrial complex area.

  • Hypertension is the most prevalent disease worldwide and is itself a risk factor for cerebral, cardiac, and renal diseases. The inconsistency of candidate genes suggested by previous genomewide association studies (GWASs) may be due to not only differences in study design and genetic or environmental background but also the difference in the power of analysis between continuous traits and discrete traits. We analyzed 352,228 single nucleotide polymorphisms (SNPs) in 8842 unrelated Koreans obtained from Ansan and Ansung cohorts. We performed a series of GWA analyses using three different phenotype models; young hypertensive cases (278 subjects) versus elderly normotensive controls (680 subjects); the upper 25% (2211 hypertensive cases) versus the lower 25% of the SBP distribution (2211 hypotensive controls); and finally SBP and DBP as continuous traits (8842 subjects). The numbers of young hypertensive cases and elderly normotensive controls were not large enough to achieve genomewide significance. The model comparing the upper 25% subjects to the lower 25% of subjects showed a power that was approximate to that of QTL analysis. Two neighboring SNPs of the ATP2B1 gene, rs17249754 (SBP, p=$2.53^{-10}$; DBP, p=$1.28{\times}10^{-8}$) and rs7136259 (SBP, p=$1.30{\times}10^{-9}$; DBP, p=$6.41{\times}10^{-8}$), were associated with both SBP and DBP. Interestingly, a SNP of the RPL6 gene, rs11066280, revealed a significant genomewide association with SBP in men only (p=$3.85{\times}10^{-8}$), and four SNPs located near the MAN2A1 gene showed a strong association with DBP only in elderly men aged 60-70 years (e.g., rs6421827, p=$4.86{\times}10^{-8}$). However, we did not observe any gene variant attaining genomewide significance consistently in the three phenotype models except for the ATP2B1 gene variants. In general, the association signal with blood pressure was stronger in women than in men. Genes identified in GWASs are expected to open the way for prevention, early diagnosis, and personalized treatment of hypertension.

  • Glucagon-like peptide-1 (GLP-1) has a broad spectrum of biological activity by regulating metabolic processes via both the direct activation of the class B family of G protein-coupled receptors and indirect nonreceptor-mediated pathways. GLP-1 receptor (GLP-1R) agonists have significant therapeutic effects on non-alcoholic fatty liver disease (NAFLD) and steatohepatitis (NASH) in animal models. However, clinical studies indicated that GLP-1 treatment had little effect on hepatic steatosis in some NAFLD patients, suggesting that GLP-1 resistance may occur in these patients. It is well-known that the gut metabolite sodium butyrate (NaB) could promote GLP-1 secretion from intestinal L cells. However, it is unclear whether NaB improves hepatic GLP-1 responsiveness in NAFLD. In the current study, we showed that the serum GLP-1 levels of NAFLD patients were similar to those of normal controls, but hepatic GLP-1R expression was significantly downregulated in NAFLD patients. Similarly, in the NAFLD mouse model, mice fed with a high-fat diet showed reduced hepatic GLP-1R expression, which was reversed by NaB treatment and accompanied by markedly alleviated liver steatosis. In addition, NaB treatment also upregulated the hepatic p-AMPK/p-ACC and insulin receptor/insulin receptor substrate-1 expression levels. Furthermore, NaB-enhanced GLP-1R expression in HepG2 cells by inhibiting histone deacetylase-2 independent of GPR43/GPR109a. These results indicate that NaB is able to prevent the progression of NAFL to NASH via promoting hepatic GLP-1R expression. NaB is a GLP-1 sensitizer and represents a potential therapeutic adjuvant to prevent NAFL progression to NASH.
