Part 3. Clinical Practice Guideline for Airway Management and Emergency Thoracotomy for Trauma Patients from the Korean Society of Traumatology

  • The following key questions and recommendations are presented herein: when is airway intubation initiated in severe trauma? Airway intubation must be initiated in severe trauma patients with a GCS of 8 or lower (1B). Should rapid sequence intubation (RSI) be performed in trauma patients? RSI should be performed in trauma patients to secure the airway unless it is determined that securing the airway will be problematic (1B). What should be used as an induction drug for airway intubation? Ketamine or etomidate can be used as a sedative induction drug when RSI is being performed in a trauma patient (2B). If cervical spine damage is suspected, how is cervical protection achieved during airway intubation? When intubating a patient with a cervical spine injury, the extraction collar can be temporarily removed while the neck is fixed and protected manually (1C). What alternative method should be used if securing the airway fails more than three times? If three or more attempts to intubate the airway fail, other methods should be considered to secure the airway (1B). Should trauma patients maintain normal ventilation after intubation? It is recommended that trauma patients who have undergone airway intubation maintain normal ventilation rather than hyperventilation or hypoventilation (1C). When should resuscitative thoracotomy be considered for trauma patients? Resuscitative thoracotomy is recommended for trauma patients with penetrating injuries undergoing cardiac arrest or shock in the emergency room (1B).

DPPA2 Protein Expression is Associated with Gastric Cancer Metastasis

  • Gastric cancer (GC) as the fourth most common cause of malignancies shows high rate of morbidity appropriating the second leading cause of cancer-related death worldwide. Developmental pluripotency associated-2 (DPPA2), cancer-testis antigen (CT100), is commonly expressed only in the human germ line and pluripotent embryonic cells but it is also present in a significant subset of malignant tumors. To investigate whether or not DPPA2 expression is recalled in GC, our aim in this study was to elucidate DPPA2 protein expression in gastric cancer. Fifty five GC tumor and their related margin normal tissues were recruited to evaluate DPPA2 protein expression and its probable associations with different clinicopathological features of the patients. DPPA2 was overexpressed in GC cases compared with normal tissues (P < .005). While DPPA2 expression was detected in all GC samples, its high expression was found in 23 of 55 tumor tissues (41.8%). Interestingly, 50 of 55 normal samples (90.9%) were negative for DPPA2 protein expression and remained 5 samples showed very low expression of DPPA2. DPPA2 protein expression in GC was significantly correlated with lymph node metastasis (p = 0.012). The clinical relevance of DPPA2 in GC illustrated that high level expression of this protein was associated with lymph node metastasis supporting this hypothesis that alteration in DPPA2 was associated with aggressiveness of gastric cancer and may be an early event in progression of the disease. DPPA2 may be introduced as a new marker for invasive and metastatic GCs.

중증외상환자의 주 손상 부위별 특성과 간호활동 (Characteristics and Nursing Activities of Severe Trauma Patients Regarding the Main Damaged Body Parts)

  • Purpose: The purpose of this study was to identify the characteristics and nursing activities of severe trauma patients regarding damaged body parts in Busan Regional Emergency Medical Center. Methods: A survey using a 'trauma patient information questionnaire and a list of nurse activities' was conducted with 133 patients over 15 points ISS on EMR from June 1, 2011 to May 31, 2012. The collected data were analyzed by the SPSS/WIN 12.0 program. Results: Almost all of the subjects were men, and the mean age was 48.8. The amount of road traffic accidents was 60.4%, and the mean RTS and ISS were 6.08, and 23.14 points. Nursing activities in common were airway management, assessment of LOC & GCS, and EKG monitoring. Most of head and neck trauma patients were cared for manasing using intracranial pressure: each patience had the following assessed: pupil size and light reflex, they were checked the leak of CSF, kept $30^{\circ}$ head elevation, and administered medications. Some of chest trauma patients were treated for chest tube and central venous catheter insertion. Partial abdominal trauma patients were administered analgesic and cared for using arterial pressure measurement. Part of the limbs and pelvis trauma patients were given a blood transfusion. Conclusion: Based on the results, the characteristics and nursing activities were specific according to the specific damaged body parts.

Yonsei Evolutionary Population Synthesis for Old Stellar Systems

  • We present the Yonsei Evolutionary Population Synthesis (YEPS) models for spectroscopic and photometric evolutions of simple and composite stellar populations. The models are based on the most up-to-date Yonsei-Yale stellar evolutionary tracks and BaSel 3.1 flux libraries, and provide integrated spectroscopic quantities of Lick/IDS system including high-order Balmer absorption-lines. Special care has been taken to incorporate the systematic variation of horizontal branch (HB) morphology as functions of metallicity, age, alpha-element mixture, and helium abundance of simple stellar populations. Our models for normal-helium stellar populations indicate that the realistic modeling of HB and alpha-element brings about 5 Gyr and 0.1 dex differences in age and metallicity estimations, respectively, compared to those without these effects. The HB effect does not depend on the specific choice of stellar libraries and alpha-element enhancements, and this effect is non-negligible even in the metal sensitive absorption indices, such as Mg2 and Mg b. Comparison of the models to observations reveals that the HB and alpha-element effects are critical in understanding otherwise inexplicable phenomena found in globular cluster systems in the Milky Way and nearby galaxies, including the observed bimodality of the line strengths of globular clusters in massive galaxies. In addition, we found that helium-enhanced stellar populations, which are the major sources of extreme HB stars, bring about increased FUV, NUV fluxes, and thus the model colors of those filters become extremely blue. Age dating based on the YEPS model with normal-helium stellar populations reveals that the evidence for 'downsizing' of elliptical galaxies is found not only in the local field but also in Coma cluster, and that the mean age of elliptical galaxies in Coma cluster is about 1.4 Gyr younger than the mean age of those in the local field. We also find that our models with helium-enhanced subpopulations can naturally reproduce the strong UV-upturns observed in giant elliptical galaxies assuming an age similar to that of old GCs in the Milky Way.

Synthetic Maternal Stress Hormone Can Modulate the Expression of Hox Genes

  • All living things have been developed efficient strategies to cope with external and internal environmental changes via a process termed 'homeostasis'. However, chronic prenatal maternal stress may significantly contributes to pregnancy complications by disturbing hypothalamic-pituitary-adrenal (HPA) axis and the automatic nervous system (ANS), and results in unfavorable development of the fetus. Dysregulation of these two major stress response systems lead to the increased secretion of the glucocorticoids (GCs) which are known to be essential for normal development and the maturation of the central nervous system. As Hox genes are master key regulators of the embryonic morphogenesis and cell differentiation, we aimed to determine the effects of dexamethasone, a potent synthetic glucocorticoid, on gene expression in mesenchymal stem cell C3H10T1/2. Analysis of 39 Hox genes based on reverse transcription PCR (RT-PCR) method revealed that the expression patterns of Hox genes were overall upregulated by long dexametasone treatment. These results indicate that maternal stress may have a deleterious effect on early developing embryo through the stress hormone, glucocorticoid.

Homing and Restorative Effects of Bone Marrow-Derived Mesenchymal Stem Cells on Cisplatin Injured Ovaries in Rats

  • Premature ovarian failure (POF) is a long-term adverse effect of chemotherapy treatment. However, current available treatment regimens are not optimal. Emerging evidence suggests that bone marrow-derived mesenchymal stem cells (BMSCs) could restore the structure and function of injured tissues, but the homing and restorative effects of BMSCs on chemotherapy injured ovaries are still not clear. In this study, we found that granulosa cell (GC) apoptosis induced by cisplatin was reduced when BMSCs were migrated to granulosa cells (GCs) in vitro. Chemotherapy-induced POF was induced by intraperitoneal injection of cisplatin in rats. BMSCs labeled with enhanced green fluorescent protein (EGFP) were injected into the rats via the tail vein to investigate the homing and distribution of BMSCs in vivo. The number of BMSCs in the ovarian hilum and medulla was greater than in the cortex, but no BMSCs were found in the follicles and corpus lutea. In addition, the BMSCs treatment group's antral follicle count and estradiol levels increased after 30 days, compared with the POF group. Hence, our study demonstrates that intravenously delivered BMSCs can home to the ovaries, and restore its structure and function in POF model rats.

  • Objective: This is a case report on the effect of Korean traditional medicine on deep stupor to semi-coma with intracerebral and intraventricular hemorrhage. Methods: The patient was treated by Korean traditional medicine such as acupuncture, moxibustion, and herbal medicine. The change of mental state was evaluated by the Glasgow Coma Scale (GCS). We used the Modified Medical Research Council Scale (MRC scale) and the Modified Barthel Index (MBI) to evaluate the effectiveness of the treatment. Results: After treatment, the physical function and ability to perform activities of daily living were improved. The Modified Barthel Index increased from 0 to 40 after treatment with Korean medicine. Conclusions: According to this study, Korean traditional medicine can be effective for the treatment of quadriparesis in patients with deep stupor to semi-coma in intracerebral and intraventricular hemorrhage.

High Monocarboxylate Transporter 4 Protein Expression in Stromal Cells Predicts Adverse Survival in Gastric Cancer

  • Background: Increasing evidence suggests that stromal monocarboxylate transporter 4 (MCT4) and carbonic anhydrase IX (CA IX) may play key roles in tumor development. However, their clinical value remains largely unexplored in gastric cancer (GC). The present study aimed to determine clinicopathological significance and prognostic values of stromal MCT4 and CA IX in GC. Materials and Methods: Specimens from 143 GC patients were immunohistochemically stained using polyclonal anti-MCT4 and anti-CA IX antibodies. Expression was correlated with patient clinicopathologic characteristics and survival data. Results: High stromal MCT4 expression was detected in 72 of 143 (50.3%) GCs and high CA IX in 74 (51.7%). Both high stromal MCT4 and CA IX were correlated with advanced TNM stage (p=0.000; p=0.000). High CA IX expression was positively related to depth of invasion (p=0.022) and positive lymph nodes (p=0.002) as well. Survival analysis indicated high expression of stromal MCT4 to be an independent factor in predicting poor overall survival (OS) (HR and 95%CI=1.962, 1.032-3.729, p=0.040) and disease free survival (DFS) (HR and 95%CI=2.081, 1.158-3.741, p=0.014) of GC patients. However, high CA IX expression exhibited no significant predictive value. Conclusions: These findings suggest that high expression of stromal MCT4 and CA IX proteins is significantly correlated with GC progression. High stromal MCT4 heralds worse outcome of GC patient, suggesting a novel candidate prognostic marker and therapeutic target.

Aberrant Expression of the Autocrine Motility Factor Receptor Correlates with Poor Prognosis and Promotes Metastasis in Gastric Carcinoma

  • AMFR, autocrine motility factor receptor, also called gp78, is a cell surface cytokine receptor which has a dual role as an E3 ubiquitin ligase in endoplasmic reticulum-associated degradation. AMFR expression is associated with tumor malignancy. We here investigated the clinical significance of AMFR and its role in metastasis and prognosis in gastric cancer. Expression of AMFR, E-cadherin and N-cadherin in cancer tissues and matched adjacent normal tissues from 122 gastric cancer (GC) patients undergoing surgical resection was assessed by immunohistochemistry. Levels of these molecules in 17 cases selected randomly were also analysed by Western blotting. AMFR expression was significantly increased in gastric cancer tissues, and associated with invasion depth and lymph node metastasis. Kaplan-Meier analysis showed AMFR expression correlated with poor overall survival and an increased risk of recurrence in the GC cases. Cox regression analysis suggested AMFR to be an independent predictor for overall and recurrence-free survival. E-cadherin expression was decreased in gastric cancer tissues; conversely, N-cadherin was increased. Expression of AMFR negatively correlated with E-cadherin expression, whereas N-cadherin expression showed a significant positive correlation with AMFR expression. AMFR might be involved in the regulation of epithelial-mesenchymal transition, with aberrant expression correlating with a poor prognosis and promoting invasion and metastasis in GCs.

무선 인지 셀룰러 망에서 자원예측에 의한 가드채널 할당기법의 성능개선 (Performance Improvements in Guard Channel Scheme by Resource Prediction for Wireless Cognitive Radio-Based Cellular Networks)

  • 본 논문에서는 셀룰러 기반 무선 인지망에서 핸드오프 호를 위한 가드채널할당기법(guard channel scheme)의 자원이용률의 저하를 개선하기 위해 무선인지 기술을 사용하여 자원 이용률을 향상시키면서, 무선 인지망의 부 사용자(cognitive radio user) 손실률을 줄이는 방법을 제안한다. 제안한 방법에서는 핸드오프 호의 전용채널(guard channel)을 부 사용자가 이용하도록 하고, MMOSPRED(Multi-Media One Step Prediction)기법으로 핸드오프 호의 발생을 예측한 다음, 핸드오프 호가 발생하면 핸드오프 호의 전용채널을 넘겨주고, 부 사용자는 예약된 자원으로 스펙트럼 핸드오프를 하여 서비스를 완료 하도록 한다. 시뮬레이션을 통하여, 제안한 기법의 자원 이용률과 부 사용자의 손실률에 대한 성능이 기존의 가드채널 할당기법과 모바일의 이동성을 예측하지 않은 채널할당기법 보다 우수함을 보인다.
