Performance of mid-upper arm circumference to diagnose acute malnutrition in a cross-sectional community-based sample of children aged 6-24 months in Niger

  • BACKGROUND/OBJECTIVES: Accurate, early identification of acutely malnourished children has the potential to reduce related child morbidity and mortality. The current World Health Organisation (WHO) guidelines classify non-oedematous acute malnutrition among children under five using Mid-Upper Arm Circumference (MUAC) or Weight-for-Height Z-score (WHZ). However, there is ongoing debate regarding the use of current MUAC cut-offs. This study investigates the diagnostic performance of MUAC to identify children aged 6-24 months with global (GAM) or severe acute malnutrition (SAM). SUBJECTS/METHODS: Cross-sectional, secondary data from a community sample of children aged 6-24 months in Niger were used for this study. Children with complete weight, height and MUAC data and without clinical oedema were included. Using WHO guidelines for GAM (WHZ < -2, MUAC < 12.5 cm) and SAM (WHZ < -3, MUAC < 11.5 cm), the sensitivity (Se), specificity (Sp), predictive values, Youden Index and Receiver Operating Characteristic (ROC) curves were calculated for MUAC when compared with the WHZ reference criterion. RESULTS: Of 1161 children, 23.3% were diagnosed with GAM using WHZ, and 4.4% with SAM. Using current WHO cut-offs, the Se of MUAC to identify GAM was greater than for SAM (79 vs. 57%), yet the Sp was lower (84 vs. 97%). From inspection of the ROC curve and Youden Index, Se and Sp were maximised for MUAC < 12.5 cm to identify GAM (Se 79%, Sp 84%), and MUAC < 12.0 cm to identify SAM (Se 88%, Sp 81%). CONCLUSIONS: The current MUAC cut-off to identify GAM should continue to be used, but when screening for SAM, a higher cut-off could improve case identification. Community screening for SAM could use MUAC < 12.0 cm followed by appropriate treatment based on either MUAC < 11.5 cm or WHZ < -3, as in current practice. While the practicalities of implementation must be considered, the higher SAM MUAC cut-off would maximise early case-finding of high-risk acutely malnourished children.

CHIME Zircon Age of the Gamaksan Alkaline Meta-Granitoid in the Northwestern Margin of the Gyeonggi Massif, Korea, and its Tectonic Implications (경기육괴 북서 연변부 감악산 알칼리 변성화강질암의 CHIME 저어콘 연대와 지체구조적 의의)

  • We carried on CHIME zircon age dating for the Gamaksan alkaline meta-granitoid (GAM) from the northwestern margin of the Gyeonggi massif, and obtained a timing of regional metamorphism at $247{\pm}14Ma$ (n=103, MSWD=0.92). The age is compatible with Permo-Triassic regional metamorphic ages from the Imjingang Belt which has been regarded as possible eastward extension of Triassic collisional belt in China. Considering an extensional ductile shearing of the Gyeonggi (Kyonggi) Shear Zone which deformed GAM occurred at 226 Ma with temperature condition about $500^{\circ}C$ (Kim et al., 2000), and the Late Triassic to Early Jurassic Daedong Group unconformably overlies on top of the ductile shear zone, cooling rate of GAM over the period can be estimated as $18{\sim}10^{\circ}C/Ma$. Since new zircon begin to pow at temperature higher than upper-amphibolite facies condition (${\sim}700^{\circ}C$), cooling rate of GAM from peak metamorphism (247 Ma) to deposition of the Daedong G.oup (${\sim}$Early Jurassic) would be higher than $10^{\circ}C/Ma$. Such rapid cooling rate is compatible with that reported from exhumation stage of the Dabie-Sulu Belt, and supports an idea that the Gyeonngi massif is a part of Permo-Triassic orogenic belt in East Asia.

The Relationship between Daesoon Thought and Prophecies of Jeong Gam: Emphasizing the Chinese Poetic Sources Transfigured by Jeungsan (대순사상과 『정감록』의 관계 - 증산이 변용한 한시 전거(典據)를 중심으로 -)

  • It has been suggested that Jeungsan's prophetic poem that starts with the verse "For about seven or eight years, there will be a castle in the ancient country [七八年間古國城] ⋯" originally comes from Prophecies of Jeong Gam (鄭鑑錄). Despite Jeungsan, himself, obviously having been critical of that text, this claim has become the basic grounds for discourse suggesting that Jeungsan was not only interested in Prophecies of Jeong Gam but also considerably influenced by the text. However, the claim itself was formulated due to misunderstandings of the Chinese poems that had been included in A Compilation of Secret Prophecies Hidden in the Family-clan of Seogye (西溪家臧訣). These poems pursue a different ideological orientation than the poem from Prophecies of Jeong Gam. Ultimately, the Chinese poem in the verse 84 the chapter titled, Prophetic Elucidations in The Canonical Scripture of Daesoon Jinrihoe cannot provide a basis for the claim that Jeungsan was strongly influenced by Prophecies of Jeong Gam. This claim that Prophecies of Jeong Gam made a deep impact on Jeungsan and Daesoon Thought was based on three other texts outside of those that appear within verse 84 of Prophetic Elucidations. The first supposedly-related line is: "Heaven opens at the period of the Rat (Ja 子), Earth opens at the period of the Ox (Chuk 丑), humankind starts at the period of the Tiger (Ihn 寅)." This line comes from from Shao Kangjie's Book of Supreme World Ordering Principles (皇極經世), and the line could be quoted idiomatically as an expression in the Joseon Dynasty. Accordingly, attempts to relate Daesoon Thought to Prophecies of Jeong Gam are a distortion that arise from the assumption that Jeungsan had a significant interest in Prophecies of Jeong Gam. The second related line is "At the foot of Mount Mother (母岳山), a golden icon of Buddha has the ability to speak [母岳山下 金佛能言]." That line is nearly identical to the verse "On the summit of Mount Mother, a golden icon of Buddha has the ability to speak [母岳山頭 金佛能言]." Yet, Jeungsan changed '頭 (du, the summit)' to '下 (ha, the foot or under)' and express his own unique religious prophecy. This allusion to the prophecies of Jeong Gam is actually a criticism designed to disprove the earlier prophecy. Third, is the verse, "The form of Buddhism, creation of daoism, and propriety of Confucianism [佛之形體仙之造化儒之凡節]," which is characteristically related to Daesoon Thought. This verse can only be found in the prophetic text, Prophecies of Chochang (蕉蒼訣), and it is provided a main source when alleging that Prophecies of Jeong Gam was an influence on Daesoon Thought. However, considering the context of Prophecies of Chochang and the year of its publication (it is assumed to be compiled after 1950s), this does not hold water as Jeungsan had already passed into Heaven several decades before that time. This disqualifies the verse from being a basis for asserting Prophecies of Jeong Gam as an influence on Daesoon Thought. Contrary to the original assertion, there is a considerable amount of evidence that Prophecies of Chochang absorbed aspects of Daesoon Thought, which were simply revised in a novel way. There is no truly compelling evidence underpinning the argument that Prophecies of Jeong Gam had a unilateral impact on Daesoon Thought. There seems to be a great deal of confusion and numerous misinterpretations on this matter. Therefore, the claim that Daesoon Thought, as developed by Jeungsan, was influenced by the discourse on dynastic revolution and feng shui contained in Prophecies of Jeong Gam should be re-examined at the level of its very premise.

Catabolic Enzyme Activities and Physiological Functionalities of Lactic Acid Bacteria Isolated from Korean Traditional Meju (재래식 메주에서 분리한 유산균들의 각종 효소활성 및 기능성)

  • Three kinds of Korean traditional Meju were selected and the counts of microorganisms in these Meju were determined. The counts of total aerobic bacteria, lactic acid bacteria and yeasts and molds were 107~108, 106~108 and 107~108 cfu/g, respectively in three Meju and lactic acid bacteria were important microorganisms in the fermentation of Meju. Therefore, we isolated three kinds of dominant lactic acid bacteria from these Meju. They were identified as Leuconostoc mesenteroides (98%, Lm-SMm), Lactobacillus plantarum (99%, Lp-SMm) and Lactococcus lactis (98%, Ll-GAm). Then, enzyme activities and physiological functionalities of three lactic acid bacteria were investigated. Protease, lipase and $\alpha$-amylase activities were detected in three lactic acid bacteria, Ll-GAm showed relatively higher activities than other two lactic acid bacteria. Lm-SMm, Lp-SMm and Ll-GAm showed 45, 48 and 60% of antioxidative activity to 1,1-diphenyl-2-picryhydrazyl (DPPH), and exhibited 45, 67 and 70% of inhibitory effects in HT-29 human colon cancer cells, respectively. These results indicate that three lactic acid bacteria isolated from traditional Meju, especially Ll-GAm are applicable to Meju preparation for soybean paste industry.

Donguibogam or DongUiBoGam: Is It Still a Different Book? - A Study on the Translation of Donguibogam into English Orthography -

  • Introduction: Up to now there has been no unified orthography of Donguibogam (東醫寶鑑) in English. Too manydifferent spellings on Donguibogam are seen in the articles on it. Therefore, we present a solution to the spelling problem of Donguibogam in this study. Discussion: To verify the problem of various spellings of Donguibogam we looked at over 54 articles that have spellings of Donguibogam published from 2000 to 2008, and compared those 29 different spellings with each other. To know the historical background of Donguibogam spelling, we looked over many translations that foreigners used, but we could just find fragmentary references to Donguibogam. We compared various spellings with the official orthography of the Korean government. We discussed the appropriateness of various spellings. There is no unified spelling in the DongUiBoGam center and in the conference articles of the Korean Society of Medical History, which recently celebrated the 400th year of DongUiBoGam publication. Conclusion: It is suggested that the proper orthography should be "Donguibogam: Principles and Practice of Eastern Medicine". Hereafter all articles should follow this spelling without exception for more consistent study.

Uranium Enrichment Determination Using a New Analysis Code for the U XKα Region: HyperGam-U

  • HyperGam-U was recently developed to determine uranium enrichment based on ${\gamma}$- and X-ray spectroscopy analysis. The $XK_{\alpha}$ region of the uranium spectrum contains 13 peaks for $^{235}U$ and $^{238}U$ and is used mainly for analysis. To describe the X-ray peaks, a Lorentzian broadened shape function was used, and methods were developed to reduce the number of fitting parameters for decomposing the strongly overlapping peaks using channel-energy, energy-width, and energy-efficiency calibration functions. For validation, eight certified reference material uranium samples covering uranium enrichments from 1% to 99% were measured using a high-resolution planar high-purity germanium detector and analyzed using the HyperGam-U code. When corrections for the attenuation and true coincidence summing were performed for the detection geometry in this experiment, the goodness of fit was improved by a few percent. The enrichment bias in this study did not exceed 2% compared with the certified values for all measured samples.

  • First, we analyze the contents of Dong-Eui-Bo-Gam. Second, from the view of the 'Hyung-Sang(형상:body form)' theory, we try to interpret the acupuncture points chosen in each chapter with a base on the contents of the book. We study on the on 'Hyung-Sang(형상: body form)' thee of acupuncture points in each chapter on 'Jung(정)' and 'Ki(기)' in Dong-Eui-Bo-Gam(동의보감). The acupuncture points on epilepsy in 'Jung(정)' chapter are come from the purview of 'Jung(정)' type, man and 'Shin(신)' type. In the cade of hypochondria, it is often considered 'Ki(기)' pe and 'Shin(신)' type. The acupuncture points on all kind of Ki(기)-desease are come from the purview of the point of 'Ki-Hae(기해)'. The acupuncture points on congestion of the head are come from the purview of indigestible type or weak type of person and woman, and the acupuncture points on 'belch 'are come form the purview of 'KE(기)' type and 'Shin(신)' type. The acupuncture points on polypnea are come from the purview of having phlegm, weak type, and long necked type.

Preference of Physical Microhabitat on the $1^{st}$-class Endangered Species, Gobiobotia naktongensis inhabiting the Gam Stream, Tributary of the Nakdong River

  • The study was conducted in 2007~2008 in order to understand preference of physical microhabitat on the $1^{st}$-class endangered species, Gobiobotia naktongensis inhabiting the Gam Stream, tributary of Nakdong River. The total number of fish caught from the study sites was 3,671 representing 7 families 24 species. There were 8 Korean endemic species including Odontobutis platycephala, and 2 introduced species (Carassius cuvieri, Micropterus salmoides) were found. According to investigation and analysis of physical microhabitat on Gobiobotia naktongensis caught in the Gam Stream, a total of 57 individuals were found at shallow depth (0.14~0.46 m) and run (0.239~0.585m $sec^{-1}$). As a result of sieve analysis, stream beds consisted of about 1% gravel and 99% sand (83.4% coarse sand, 15.6% find sand). Therefore, Gobiobotia naktongensis seemed to inhabit shallow-run with coarse sand bed than deep-pool microhabitat. The findings indicate preference of physical microhabitat on Gobiobotia naktongensis, and it is important to enhance efficiency of fish conservation and ecological restoration with understanding species-specific characteristics in microhabitat including protected species.

  • Following former study(A Study of the Perception of SoGal in the early through mid Chosun Dynasty-Focusing on "UiBangYuChi(醫方類聚)" and "DongUiBoGam(東醫寶鑑)", there were medical attainments in this study on SoGal(消渴) of "UiRimChualYo(醫林撮要) SamSoMun(三消門)". 1. SoGal(消渴) theory of "UiRimChualYo(醫林撮要) SamSoMun(三消門)" presented the lack of Bodily Liquids and Blood theory(津液 血不足論) overcoming the limits of the fire/heat theory(火熱論). 2. In the "UiRimChualYo(醫林撮要) SamSoMun(三消門)", the concept of SoGal was started to change simple symptoms into the diseases including ethology, pathology and prognosis etc. 3. In the "UiRimChualYo(醫林撮要) SamSoMun(三消門)", the prescriptions of JaEum(滋陰) were thought significantly. On the other hand the prescriptions of HuaYul(火熱) were excluded at all. 4. The use of warm herb like BuJa(附子) was showed through GaGamSinGiHwan(加 減腎氣丸) and succeeded in "DongUiBoGam" the kidney's fire(坎火衰少論) lack theory. The futuristic percetion of "UiRimChualYo(醫林撮要) SamSoMun(三消門)" influenced "DongUiBoGam(東醫寶鑑)" was able to have progress ideas about SoGal.

Nonlinear Prediction using Gamma Multilayered Neural Network (Gamma 다층 신경망을 이용한 비선형 적응예측)

  • Dynamic neural networks have been applied to diverse fields requiring temporal signal processing such as system identification and signal prediction. This paper proposes the gamma neural network(GAM), which uses gamma memory kernel in the hidden layer of feedforward multilayered network, to improve dynamics of networks and then describes nonlinear adaptive prediction using the proposed network as an adaptive filter. The proposed network is evaluated in nonlinear signal prediction and compared with feedforword(FNN) and recurrent neural networks(RNN) for the relative comparison of prediction performance. Simulation results show that the GAM network performs better with respect to the convergence speed and prediction accuracy, indicating that it can be a more effective prediction model than conventional multilayered networks in nonlinear prediction for nonstationary signals.
