PNF 통합 패턴 운동이 만성 요통 노인 환자의 균형에 미치는 영향 (The effects of combination patterns exercise of proprioceptive neuromuscular facilitation on balance in chronic low back pain elderly patients)

  • 본 연구에서는 스프린터와 스케이터 패턴을 통합한 PNF 통합 패턴 운동이 만성 요통 노인 환자의 정적 균형과 동적 균형의 변화를 알아보고자 실시하였다. 본 연구는 만성 요통의 질환이 있는 노인 환자 34명을 대상으로 하였고, 34명을 무작위로 각각 PNF 통합 패턴 운동군(17명)과 스위스볼 운동군(17명)으로 나누었다. 운동전후에 정적 균형 능력 측정을 위해 GOOD BALANCE system(Metitur Oy, Palokka, Finland, 2003)을 사용하였다. 동적 균형 능력 측정을 위해 기능적 팔 뻗기 검사(functional reach test; FRT)와 일어나 걸어가기 검사(timed up and go test; TUG)를 시행하였다. 정적 균형 능력과 동적 균형 능력의 거의 대부분의 항목에서 유의한 차이를 나타내었다(p<.05). 군간 비교에서도 다수의 항목에서 스위스볼 운동군과 PNF 통합 패턴 운동군 사이에 유의한 차이를 나타내었다(p<.05). 이와 같은 연구 결과로 볼 때 PNF 통합 패턴 운동은 만성 요통 노인 환자의 균형 능력을 개선시키는데 효과적임을 알 수 있었으며, 임상에서 PNF 통합 패턴 운동을 이용할 수 있는 기초 자료를 충분히 제공하였다고 할 수 있을 것이다.

Production of Bovine Nuclear Transfer Embryos Using Fibroblasts Transfected with Single-Chain Human Follicle-Stimulating Hormone Gene

  • Human follicle-stimulating hormone (hFSH) is a pituitary glycoprotein that regulates follicular development and ovulation. Clinically, hFSH has been used to induce follicular growth in infertile women. The hormone is composed of heterodimers, including a common ${\alpha}$ subunit among the gonadotropin family and a hormone-specific ${\beta}$ subunit. Since assembly of the heterodimer is a rate-limiting step in the production of functional hFSH, transgenic clone cows carrying a single-chain hFSH transgene may efficiently produce functional hormone. Genes encoding the ${\alpha}$ and ${\beta}$ subunits of hFSH were linked using the C-terminal peptide sequence from the ${\beta}$ subunit of human chorionic gonadotropin. Bovine fetal fibroblasts were transfected with the gene construct, including the goat ${\beta}$-casein promoter and a single-chain hFSH coding sequence. Transfected fibroblasts were transferred into enucleated oocytes, and individual nuclear transfer (NT) embryos developed to the blastocyst stage were analyzed for the transgene by polymerase chain reaction. Seventy eight blastocysts (30.8%) were developed from 259 reconstructed embryos. Among these blastocysts, the hFSH gene was detected in 70.8% (34/48) of the embryos. Subsequent transfer of hFSH-transgenic clone embryos to 31 recipients results in 11 (35.5%) early pregnancies. However, all fetuses were lost before reaching day 180 of gestation. The results from this study demonstrated that bovine NT embryos carrying single-chain hFSH could be produced, and further extensive studies in which NT embryos are transferred to more recipients may give rise to single chain hFSH-transgenic cows for biomedical applications.

노인요양병원 입원환자의 TUG, FRT와 SPPB와의 상관관계 연구 (Correlations between SPPB, FRT, and TUG in Hospitalized Frail People - The Timed Up and Go test, the Functional Reaching Test, and the Short physical performance battery -)

  • Purpose: The purpose of this study was to determine the correlations, if any, between SPPB (Short physical performance battery), FRT (Functional reach test), and TUG (Timed up and go test) in hospitalized frail people. Methods: Seventy frail elderly patients (male/female: 16/54) participated in this study. All subjects could walk with or without an assisting device and they had no mental problems. Subjects were scored on physical functioning using the SPPB, FRT, and TUG. The data were analyzed using Pearson Correlation Coefficients. Results: There was a statistically significant difference between (a) SPPB and (b) FRT (p<0.01) and TUG (p<0.01). SPPB and FRT scores showed a positive correlation (r = 0.38) and SPPB and TUG scores showed a negative correlation (r = -0.56). Conclusion: SPPB, FRT, TUG scores are correlated with each other. From these results, we conclude that the SPPB is a useful method for predicting walking, balance, and physical function in frail elderly people.

The Coordinative Locomotor Training Intervention Strategy Using the ICF Tool to Improve the Standing Posture in Scoliosis: A Case Report

  • Purpose: This study was examined to improve the standing posture of a scoliosis client using the ICF Tool. Methods: For examination, the study subject was a 16-year-old female student diagnosed with 3curve-pelvic (3CP) type scoliosis. Information about her were collected through a client interview and based on international Classification of Functioning, Disability and Health (ICF). The ICF core set was for post-acute musculoskeletal conditions, and the ICF level 2 items suggested by National Rehabilitation Information Center (NARIC) were added to the recommendations for scoliosis. For evaluation, the ICF assessment sheet was used to identify the interaction among the problems. For the diagnosis, the client's functional problems were described in ICF terms. For the prognosis, the global goals for reaching the client's functional activity and participation level were presented as the long-and short-term goals. For the intervention, a coordinative locomotor training program composed of warm-up, main exercise, and cool-down was applied 3 times a week, 50 minutes a day, for 5 weeks. For the outcome, the differences between before and after the intervention were compared with the ICF qualifier and are shown with the ICF evaluation display. Results: Clinical advantages were observed in body function and structure (7° decrease of thoracic angle, 7 score increase of trunk muscle power, 6.47s improve of one leg standing, 4 score decrease of neck pain). The activity for maintaining the standing posture, in which the client had a primary limitation, was improved. Conclusion: Applying the coordinative locomotor training program is expected to improve scoliosis client's standing posture.

The Effects of Closed Kinetic and Open Kinetic Chain Exercises Using Knee Reposition Sense in Chronic Stroke Patients

  • Purpose: This study aimed to determine the effects of open kinetic chain exercise (OKCE) and closed kinetic chain exercise (CKCE) using knee reposition sensing on balance, strength, and knee joint reposition sense (JPS) in chronic stroke patients. Methods: Twenty-nine hemiplegic patients participated in this study. Participants were randomly divided into 3 groups, CKCE, OKCE, and controls, with 9, 10, and 10 participants, respectively. The CKCE group completed CKCE using knee reposition sensing, whereas the OKCE group completed OKCE using knee reposition sensing. The control group completed conventional physical therapy. Results: Significant differences between the CKCE and OKCE groups were apparent for all outcomes except the functional reaching test. The CKCE group displayed significant improvements in knee JPS versus the OKCE and control groups (p<0.01). The OKCE group displayed significant improvements in knee extensor muscle strength versus the CKCE and control groups (p<0.05). The CKCE and OKCE groups displayed significantly improvements in static balance versus the control group (p<0.05). Conclusion: CKCE and OKCE improved balance, strength, and knee JPS. Additionally, CKCE might provide a more useful intervention benefit than OKCE for increasing knee JPS, a weight-bearing task. OKCE was sufficient to improve the knee extensor muscle strength.

다양한 지지면에서의 요부안정화운동이 만성편마비환자의 균형에 미치는 변화 (The Changes of Lumbar Stabilization Exercise on Balance with a Variety of Supporting Surface in Chronic Hemiplegia)

  • Purpose : The purpose of this study was to demonstrate the effects of lumbar stabilization exercise on static and dynamic balance performance with unstable supporting surface and an even surface. Methods : The subjects of this experimental study were 30 patients among hospitalized stroke patients. They were randomly divided into two groups: unstable supporting surface group(USS; n=15) and an even surface group(ES; n=15). The lumbar stabilization exercise program for patients in two groups went through 40 minute exercise sessions three times a week for 8 weeks. To analyze the effects of a variety of supporting surface, Functional reaching test(FRT), Berg's balance scale(BBS), and Overall stability index(static balance) were measured before and after the exercise. The paired t-test was used to compare mean differences between before and after exercis and the independent t-test was used to compare mean differences between groups. Results : After 8 weeks exercise program, there were significant differences between before and after exercise in FRT, BBS, and static balance. But there were not significant differences in balance between two groups. Conclusion : This study suggest that lumbar stabilization exercise on unstable supporting surface and an even surface can improve FRT, BBS, and a static balance.

뇌졸중 환자에서 반복적인 양측성 운동학습 적용이 상지기능에 미치는 영향

  • Chronic upper extremity hemiparesis is a leading cause of functional disability after stroke. The purpose of this study were to identify effects of a 6weeks repetitive bilateral arm training on upper motor function and the reorganization of motor network. Four chronic stroke patients participated in this study. They performed for 6 consecutive weeks, 3 days a week, 30 minutes a day. In the single group study, four 5-minute periods per session of bilateral arm training were performed with the use of a custom-designed arm training machine. The results of this study was as follows. 1. Following the 6weeks period of RBAT, patient exhibited a improvement in FMA and BBT. 2. Following the 6weeks period of RBAT, it showed improvement in reaching time, symbol digit substitution and finger tapping speed of KCNT. 3. fMRI activation after RBAT showed a focal map in lesional cortical area and perilesional motor areas. These fMRI data suggest that hemodynamics response to RBAT reflect sensorimotor reorganization in contralateral hemisphere. In conclusion, these date suggest that improved upper extremity function induced by repetitive bilateral arm training after stroke is associated with reorganization of motor network as a neural basis for the improvement of paratic upper extremity function.

안전방출밸브 개발과 용량인증 사례 (Experience for Development and Capacity Certification of Safety Relief Valves)

  • The purpose of this study is localization of safety relief valves for Nuclear Service. The safety relief valve is the important equipment used to protect the pressure vessel, the steam generator and the other pressure facility from overpressure by discharging the operating medium when the pressure of system is reaching the design pressure of the system. We developed design technology used FEM ' CFM about safety relief valve for Nuclear Service according to ASME (or KEPIC) Code and KHNP's Technical Specification. To prove validity of a design technology, actually, we manufactured and inspected and tested the sample products designed according to a developed technology. The capacity qualification test was achieved according to requirement of ASME(or KEPIC) Code by NBBI and the functional qualification test was achieved according to ASME QME-1 for operating condition in technical specification of KHNP by NLI. Therefore we have to achieve the development of safety relief valves for Nuclear Service with our own technologies.

  • Purpose : The purpose of this study was to examine the effects of Rhythmic Stabilization(RS) and Combination of Isotonic(CI) of Proprioceptive Neuromuscular Facilitation on balance in patients with hemiplegia. Methods : Ten patients with hemiplegia were trained with RS and CI for six weeks. Two Standard Scale(TSS), Functional Reach Test(FRT), Time Up and Go test(TUG) and Berg Balance Scale(BBS) were used to prove their improvement of balance. Results : Body weight bearing decreased on less affected side and increased on more affected side. Distance of forward reaching for FRT increased. Time for TUG decreased. BBS score increased. They all showed significant difference. Conclusion : This study suggests that balance training with RS and CI of Proprioceptive Neuromuscular Facilitation improved balance in patients with hemiplegia.

탄력저항을 이용한 하지근력 강화 운동이 여성노인의 정적 및 동적 균형능력에 미치는 영향 (Effects of Lower Extremity Muscle Strengthening Exercise Using Elastic Resistance on Balance on Elderly Women)

  • Purpose: The purpose of study was to test the effect of lower extremity muscle strengthening exercise using elastic resistance on balance on elderly women. Methods: Twenty four women who were over 65 years old were randomly assigned to either the experimental group that received the exercise programme (n=12) or to the control group (n=12) that continued normal activities except programme. Exercises included the following 7 different activities; chair squat, knee flection, extension, hip flection, extension, calf muscle raise, ankle dorsiflection. All the participants were subject to 3 tests, including One-Leg Standing Test, Functional Reaching Test, and Up & Go Test. The measurements were made before the excercise, 4 and 8 weeks after the exercise. The data were analyzed by using the two way repeated ANOVA. Results: The results were as follows; As compared with change of balance capacity between groups, a significant difference was shown in the up and go test (p<.05), but not in balance performance clinical test of OLST (p>.05) and FRT (p>.05). Conclusion: The lower extremity muscle strengthening exercise using elastic resistance for the elderly women had some positive effects on the balance. Therefore this intervention can be broadly applied to other elders for preventing falls.
