프로바디마사지가 뇌병변 장애우의 신체적 특성과 대동작 기능 및 관절 가동범위에 미치는 영향 : 사례연구 (Effects of the Probody Massage on the Physical Characteristics, Gross Motor Function and ROM in Youth with Cerebral Palsy : Case study)

  • Objective : The purpose of this study was to investigate the effects of the Probody Massage Program on the physical characteristics, gross motor function and ROM (Range Of Motion) of children with cerebral palsy. Method : The subjects of this study were two children who have been diagnosed with first grade cerebral palsy that utilized T development support center located in B Metropolitan city for 8 weeks, twice a week, to carry out the Probody Massage Program for 30 minutes. Physiological reactions (height, weight, BMI, blood pressure (an index of inflammation), pulse rate) and large operating functions (sitting, crawling and the joints' range of motion as an angle of the shoulders' upper limb articulation) were measured pretest, after 4 weeks, and after 8 weeks. Results : The Probody Massage Program showed positive changes in physical characteristics (blood pressure, sitting, and crawling), gross motor function (upper limb shoulder movement), joint range of motion, height, body weight, metabolic activation and blood circulation of children with cerebral palsy. Conclusion : We believe making a practical impact on the growth and development, functional recovery of daily life, and improvement of quality of life of children with cerebral palsy by utilizing Probody Massage Program improves blood pressure (an index of inflammation), pulse, sitting, crawling, and the joints' range of motion as an angle of the shoulder joints' upper limb movement of children with cerebral palsy.

네덜란드 RE;MIND 디자인의 개념적·비평적 사용미학 (The Dutch RE;MIND Design's Conceptual and Critical Aesthetics of Use)

  • Aesthetics of use for modern people can be regarded as being weighted toward universal and rational function since it has focused on an 'automation and efficiency' and 'smartness' in terms of technological and human engineering position. Thus, aesthetics of use as an individual agent has remained in minimal level. This study attempts to seek conceptual model of design regarding the behavior of users from the systematization of critical theories about the usability. To do that, 'HERTZAIN TALES' written by Anthony Dune and the concept and system of Droog Design have been used as a core standard. In line with the various experiments by artist groups such as Fluxus, Memphis group and the work of product semantics, and inherent characteristics of Droog design, which has surfaced from the 1990s, concepts that has mentioned in the Hertzain tales have been organized into main texts. By overcoming cognitive bias inherent in the material culture and by overcoming an existing usability based on immateriality and dialectic solution, the autonomy of critical design and instrumentality of Droog design have been systemized. By interpreting the design aspect of 12 pieces that have been submitted to the RE;MIND section, the core of this study, it has been confirmed that conceptual and critical design's aesthetics of use From the interpretation of the design aspect of 12 pieces that have been submitted to the RE;MIND section, the core of this study, it has been confirmed that conceptual and critical design's aesthetics of use has established the autonomy and independence of the behavior of users, created functional articulation of materials and secured new conceptual model and thus, could be an effective methodical clue for creating an emergent design.

현대건축에서의 단일체적 조형 성향에 관한 연구 (A Study on the Monolithic Characteristics of Contemporary Architecture)

  • 본 연구는 다원화된 현대건축에서 나타나는 현상의 하나인 단일체적(monolithic) 표현 성향의 특성에 관한 것이다. 다양한 건축적 표현 중에서 단일체 건축은 형태적 산만함이나 분열, 이론에 치중한 형태적 복합성의 난해함을 대치하려는 의도로 해석되고, 현시대의 불안정성, 불확정성, 유동성 등이 강조되는 아방가르드적인 건축이론이나 작품경향에 반해 건축의 위상과 자율성 확립을 통한 반발이기도 하다. 이런 단일체 건축은 단순하고 경제적인 구조를 빈틈없는 외피로 감싼 형태적 엄격함을 보여주면서 인식이 용이한 이미지를 구축함으로써 독자적 아이덴티티를 갖춘 형태의 자립형 건축이라 할 수 있다. 외관의 추상적 형태를 이용해 형태묘사적인 아이덴티티를 구축하거나, 크기나 스케일을 과장함으로써 존재감을 강화시키기도 하며, 색, 재료, 마감, 단면형태 등을 이용하여 내부공간과 외부공간을 중재하는 표피를 전략적으로 탐색하기도 한다. 이런 단일체 건축은 오늘날의 다중적이고 불확정적인 상황에서 건축적 아이덴티터를 확립하기 위한 노력의 일환으로 받아들이는 것이 합당할 것이다.

  • Background and Objectives : The current study aimed to 1) compare of Lingual function and alveolar sounds between 4-year-old Korean children with and without ankyloglossia, 2) investigate the correlation between ① maximum lingual length-protrusion(MLL-P) and percentage of consonants corrects(PCC) focused on alveolar sounds, ② lingual movement and PCC focused on alveolar sounds, ③ MLL-P and lingual movement. Materials and Method : Twenty-two 4-year-old children participated as subjects in the study: a control group of 11 normal children and an experimental group of 11 children who were previously diagnosed as having ankyloglossia. They were measured for lingual function (lingual length, lingual movement) and the performances of speech articulation. Results : Children with ankyloglossia displayed significantly shorter MLL-P than 4 year normal children. Experimental group displayed significantly worse lingual movement, lower PCC in Picture consonants test, and lower PCC of alveolar sounds than control group. Ther was significantly high correlation between MLL-P and lingual movement of experimental group. Conclusion : This paper describes clinical measure and functional aspects of the tongue. Such baseline analysis provides a more definitive appraisal of lingual function as well as a more objective basis for diagnosis and treatment of ankyloglossia.

Integrating 3D facial scanning in a digital workflow to CAD/CAM design and fabricate complete dentures for immediate total mouth rehabilitation

  • PURPOSE. To integrate extra-oral facial scanning information with CAD/CAM complete dentures to immediately rehabilitate terminal dentition. MATERIALS AND METHODS. Ten patients with terminal dentition scheduled for total extraction and immediate denture placement were recruited for this study. The patients were submitted to a facial scanning procedure using the in-office PritiMirror scanner with bite registration records in-situ. Definitive stone cast models and bite records were subsequently submitted to a lab scanning procedure using the lab scanner (iSeries DWOS; Dental Wings). The scanned models were used to create a virtual teeth setup of a complete denture. Using the intra-oral bite records as a reference, the virtual setup was incorporated in the facial scan thereby facilitating a virtual clinical evaluation (teeth try-in) phase. After applying necessary adjustments, the virtual setup was submitted to a CAM procedure where a 5-axis industrial milling machine (M7 CNC; Darton AG General) was used to fabricate a full-milled PMMA immediate provisional prosthesis. RESULTS. Total extractions were performed, the dentures were immediately inserted, and subjective clinical fit was evaluated. The immediate provisional prostheses were inserted and clinical fit, occlusion/articulation, and esthetics were subjectively assessed; the results were deemed satisfactory. All provisional prostheses remained three months in function with no notable technical complications. CONCLUSION. Ten patients with terminal dentition were treated using a complete digital approach to fabricate complete dentures using CAD/CAM technology. The proposed technique has the potential to accelerate the rehabilitation procedure starting from immediate denture to final implant-supported prosthesis leading to more predictable functional and aesthetics outcomes.

구개인두성형술 후 음성의 음향학적 변화 (The Acoustic Changes of Voice after Uvulopalatopharyngoplasty)

  • The primary sound produced by the vibration of vocal folds reaches the velopharyngeal isthmus and is directed both nasally and orally. The proportions of the each component is determined by the anatomical and functional status of the soft palate. The oral sounds composed of oral vowels and consonants according to the status of vocal tract, tongue, palate and lips. The nasal sounds composed of nasal consonants and nasal vowels, and further modified according to the status of the nasal airway, so anatomical abnormalities in the nasal cavity will influence nasal sound. The measurement of nasal sounds of speech has relied on the subjective scoring by listeners. The nasal sounds are described with nasality and nasalization. Generally, nasality has been assessed perceptually in the effect of maxillofacial procedures for cleft palate, sleep apnea, snoring and nasal disorders. The nasalization is considered as an acoustic phenomenon. Snoring and sleep apnea is a typical disorders due to abundant velopharynx. The sleep apnea has been known as a cessation of breathing for at least 10 seconds during sleep. Several medical and surgical methods for treating sleep apnea have been attempted. The uvulopalatopharyngoplasty(UPPP) involves removal of 1.0 to 3.0 cm of soft palate tissue with removal of redundant oropharyngeal mucosa and lateral tissue from the anterior and sometimes posterior faucial pillars. This procedure results in a shortened soft palate and a possible risk following this surgery may be velopharyngeal malfunctioning due to the shortened palate. Few researchers have systematically studied the effects of this surgery as it relates to speech production. Some changes in the voice quality such as resonance (nasality), articulation, and phonation have been reported. In view of the conflicting reports discussed, there remains some uncertainty about the speech status in patients following the snoring and sleep apnea surgery. The study was conducted in two phases: 1) acoustic analysis of oral and nasal sounds, and 2) evaluation of nasality.

설소대 절제술이 설운동과 발음에 미치는 영향 (THE EFFECT OF LINGUAL FRENECTOMY ON THE TONGUE MOTION AND SPEECH)

  • We have examined 20 tongue-tie patients who had been operated the lingual frenectomy in Dept. of Oral & Maxillofacial surgery, Pusan National University for the effect of the lingual frenectomy on the tongue motion and speech, and divided patients into groups by age. Each group was separated as follows ; the age of 5-6, 7-9, 10-12, 13-15, and 16-18. We measured the frenal lengh, the range of tongue motion and evaluated the speech pre- and postoperatively. The preoperative patient groups have larger tongue tips and smaller lingual frenums than normal ones. In the preoperative patient group, there was significant obstacle of the protrusive and superior movement of the tongue. The exception was the laterotrusive movement on both sides. There was the lower value of the Picture Consonant Articulation Test(PCAT) in the preoperative 5 to 6 year-old group compared with normal group. In other age groups, there was no significant difference. The range of postoperative PCAT in all age groups has become as similiar as that of normal group. The 5 to 6 year-old group which had significant difference in PCAT was improved in PCAT. In conclusion, we propose that the PCAT as well as anatomical tongue size and functional tongue movement is the basis of the indication of lingual frenectomy and the 5-6 year-old is the optimal time of the lingual frenectomy for improvement of the tongue movement and the speech.

성 로렌조 교회의 건축과 증축계획에 관한 연구 (A Study on the Architecture and Extension Project for the Church of St. Lorenzo)

  • The purpose of this paper is to study on the architecture and extension project for the Church of St. Lorenzo. This study is composed of a through analysis of the extension elements on the church building and extension type for extension project of the Church of St. Lorenzo. The results of study are as follows: 1. The extension project for the Church of St. Lorenzo was basically consisted of the concept of a horizontal extension plan through both the interior and exterior space of the church building. This project was the plan of concept of additional or affixing extension through existing spaces with necessary spaces to make up for the previous simple space and form as well as functional aspect. 2. The unit spaces of the nave, the aisle, the small chapels bilaterally adorning the aisles, the transept and the chapels around transepts, and the sacristies by the extension project for the Church of St. Lorenzo is composed of regulation, balance and harmony as geometric space composition according to simple proportion system on whole and parts as well as parts and parts, through the definite articulation by each space zoning. 3. The most important innovative aspect different to previous churches in the extension project for the Church of St. Lorenzo is the compositive system of each other symmetrical spaces through long centrical axis of central part, applying the module system for horizontal arrangement. In particular, the use of regular module on repetition and proportion of rhythm from architectural composition system was precisely composed with the alter that consists of visual focus to express more accurate perspective.

구강암과 구인두암의 절제술 후 전완유리피판술을 이용한 재건술 (Reconstruction with Radial Forearm Free Flap after Ablative Surgery for Oral Cavity and Oropharyngeal Cancers)

  • Background and Objectives: Surgical ablation of tumors in the oral cavity and the oropharynx results in a three dimensional defect because of the needs to resect the adjacent area for the surgical margin. Although a variety of techniques are available, radial forearm free flap has been known as an effective method for this defect, which offers a thin, pliable, and relatively hairless skin and a long vascular pedicle. We report the clinical results of our 54 consecutive radial forearm free flaps used for oral cavity and oropharynx cancers. Materials and Methods: We reviewed the medical records of patients who were offered intraoral reconstruction with a radial forearm free flap after ablative surgery for oral cavity and oropharyngeal cancers from August 1994 to February 2003 and analyzed surgical methods, flap survival rate, complication, and functional results. Among these, 20 cases were examined with modified barium swallow to evaluate postoperative swallowing function and other 8 cases with articulation and resonance test for speech. We examined recovery of sensation with two-point discrimination test in 15 cases who were offered sensate flaps. Results: The primary sites were as follows : mobile tongue (18), tonsil (17), floor of mouth (4), base of tongue (2), soft palate (2), retromolar trigone (3), buccal mucosa (1), oro-hypopharynx (6), and lower lip (1). The paddles of flaps were tailored in multilobed designs from oval shape to tetralobed design and in variable size according to the defects after ablation. This procedures resulted in satisfactory flap success rate (96.3%) and showed good swallowing function and social speech. Eight of 15 cases (53.3%) who had offered sensate flap showed recovery of sensation between 1 and 6 postoperative months (average 2.6 month). Conclusion: The reconstruction with radial forearm free flap might be an excellent method for the maximal functional results after ablative surgery of oral cavity and oropharyngeal cancers that results in multidimensional defect.

과도한 치관높이 공간을 가진 환자에서 유지관리를 고려한 임플란트 수복증례 (Implant restoration considering maintenance for a patient with excessive crown height space)

  • 치관높이 공간이 과도할 경우 임플란트 몸체 혹은 구성물의 실패와 같은 고정성 보철물의 생역학적 합병증이 증가하며 치관 높이 공간이 12 mm 이상으로 과도한 경우는 고정성 보철물 선택에 유의하여야 한다. 고정성 보철물을 제작 시 치아들이 길어지므로 심미적 부위에서는 치은 색조를 띠는 재료를 사용해야 할 필요가 있다. 이러한 경우, 유지와 위생 관리가 적절하지 못하게 형성된 고정성 보철물은 음식물, 치태, 치석을 정체시키고 염증과 감염이 발생될 수 있으며 만성 염증과 감염으로 인해 외과적인 개선이 필요할 수 있다. 본 증례에서, 과도한 치관높이 공간을 가진 환자에게 발음 및 심미성을 고려하여 보철물 제작 후 보철물 주위의 치은 염증 및 부종이 발생하였으며, 그 원인이 구강위생을 어렵게 하는 보철물의 디자인으로 판단되어 치간공극을 충분히 형성한 임시 보철물을 장착 후 2개월 동안의 관찰 기간을 거치며 특이할 만한 이상 소견이 없음을 확인하였고 자가 위생 관리가 용이한 디자인으로 지르코니아 하부구조에 도재를 축성한 고정성 보철물로 구강회복을 시행하였으며, 일련의 치료과정을 통하여 부적절한 연조직 반응이 관찰 되지 않고 기능적인 면에서 만족스러운 결과를 얻을 수 있었다.
