PNF를 이용한 체간안정화운동이 뇌졸중 환자의 체간조절능력과 균형, 보행에 미치는 영향: 단일사례연구 (Effects of Trunk Stability Exercise by using PNF on Trunk Control Ability and Balance, Gait in a Patient with Hemiplegia: A Single Case Study)

  • Purpose: Deficits in lower-extremity function and trunk control ability have a negative impact on individuals with hemiplegia. This case report aimed to describe the effect of trunk stability exercises using proprioceptive neuromuscular facilitation (PNF) on trunk control ability, balance, and gait in a patient with hemiplegia. Methods: A 77-year-old man with hemiplegia and trunk and lower extremity impairment participated in this four-week training intervention. Results: The patient demonstrated improvements in trunk control ability, balance, and gait performance. Outcome measures (Fugl-Meyer Assessment Lower Extremity (FMA-LE), Trunk Control Test, Berg Balance Scale, Timed Up and Go test, 10 Meter Walk test) were measured before and after the training program. Conclusion: The results of this case suggest that a trunk stability exercise using a PNF program may improve trunk control ability, balance, and gait in a patient with hemiplegia.

  • Purpose : To describes the important aspects of knee joint movement and function used when applying PNF technique to the lower limb. Method : The knee was a very important roles in the lower limb movement and ambulation. This study summarizes the physiologic movement of knee to the PNF lower extremity patterns. Result : The tibiofemoral joint is usually described as a modified hinge joint with flexion-extension and axial rotation by two degrees of freedom movement. These arthrokinematics are a result of the geometry of the joints and the tension produced in the ligamentous structures. The patellofemoral joint is a sellar joint between the patella and the femur. Stability of the patellofemoral joint is dependent on the passive and dynamic restraints around the knee. In a normal knee the ligaments are inelastic and maintain a constant length as the knee flexes and extends, helping to control rolling, gliding and translation of the joint motions. Conclusions : It is important to remember that small alterations in joint alignment can result in significant alterations in patellofemoral joint stresses and that changes in the mechanics of the patellofemoral joint can also result in changes in the tibiofemoral compartments. Successful treatment requires the physical therapist to understand and apply these arthrokinematic concepts. When applied to PNF low extremity patterns, understanding of these mechanical concepts can maximize patient function while minimizing the risk for further symptoms or injury.

The Biofeedback Scapular Stabilization Exercise in Stroke Patients Effect of Muscle Activity and Function of the Upper Extremity

  • Purpose: This study attempts to understand the effect of stabilization exercise of biofeedback scapular on muscle activity and functional evaluation of the upper extremity in stroke patients. Methods: Patients were divided into two groups; a biofeedback scapular stabilization exercise group comprised of 8 patients and a task-oriented training group including another 8 patients, and 30-minute exercise was performed 5 times a week for 8 weeks. Electromyogram was used to measure muscular activity of lower trapezius, deltoid middle, and serratus anterior. Fugl-Meyer Assessment and Manual Function Test were used to evaluate functions of the muscles mentioned. Results: Significant difference was observed in the comparison group before and after exercise in muscular activity of lower trapezius, deltoid middle, and serratus anterior, Fugl-Meyer Assessment, and Manual Function Test. Conclusion: Therefore, we could see that biofeedback scapular stabilization exercise is more effective than task-oriented training in facilitating muscle activation and functional capacity of upper limb.

시설 노인을 위한 기능적 그룹활동 프로그램의 개발 및 운영 평가 (Development and Evaluation of Functional Group Activity Program on Institutionalized Aged)

  • Purpose: The purpose of this study was to identify the changes of physical function, perceptual and cognitive function, emotional function, and functional independence in the institutionalized aged according to functional group activity program (self help Tai Chi exercise plus functional task). Methods: Study subjects were 20 institutionalized aged from June to October in 2010. The subjects received functional group activity program two times a week for 15 weeks. Physical function (grip strength, coordination, lower extremity strength, balance, gait, trunk flexibility), perceptual and cognitive function, emotional function(depression, social skill), and functional independence were measured before and after the program. Results: The subjects showed significantly increased physical function (coordination, lower extremity strength, gait, trunk flexibility), perceptual and cognitive function, emotional function (depression, social skill), and functional independence. The functional group activity program may be an effective strategy for institutionalized elders to enhance their functions. Conclusion: The functional group activity program may be effective on elderly institutions which have limitation in human, material, environmental resources.

여대생의 하이힐 착용에 따른 하지 통증, 피로, 아킬레스건의 융복합 비교: 예비연구 (Comparison of pain, fatigue, and Achilles tendon in female college students wearing high heels and flat shoes: A Preliminary Study)

  • 여대생의 하이힐 착용으로 인한 근골격계 질환의 증상을 파악하기 위해 하이힐 착용과 비 착용에 따른 하지 통증, 피로, 아킬레스건 두께의 차이를 비교 분석하기 위함이다. 하이힐 착용군(n=19)과 비 착용군(n=23)을 대상으로 하지 통증, 피로, 아킬레스건 두께를 측정하였다. 대상자의 일반적인 특성과 하지 통증, 피로, 아킬레스건 두께의 정도는 평균, 표준편차를 사용하여 분석하였다. 하이힐과 비 착용에 따른 하지 통증, 피로, 아킬레스건 두께에 대한 비교는 독립 t 검정으로 분석하였다. 분석 결과, 두 군간 하지 통증(t=2.28, p=.028), 오른쪽 아킬레스건 두께(t=2.30, p=.027), 왼쪽 아킬레스건 두께(t=3.89, p<.001)에 대한 유의한 차이가 있었다. 이 연구의 결과는 다학제간 융복합적 접근이 하이힐 착용에 따른 근골격계 질환의 구조와 기능에 대한 건강 문제에 근거자료로 적용할 수 있다는 것을 보여준다. 향후, 동일한 대상자에서 하이힐 착용에 따른 추적관찰이 필요할 것으로 사료된다.

Development of a Foot Pressure Distribution Measuring Device for Lower Limb Rehabilitaion

  • It is important to train lower limb muscle strength using a tilting table to recover the lower extremity function of hemiplegia patients. It is known that the foot deformity and poor posture of hemiplegia patients can reduce the effectiveness of lower limb rehabilitation training. In this study, we developed a sensor system that can measure the foot pressure distribution of the patients for the load control of the lower extremity during lower limb rehabilitation training and it can be substituted for conventional high-cost technologies.

주상골 하강이 하지 배열의 임상적 평가에 미치는 영향 (The Effect of Navicular Drop on The Clinical Measures of Lower Extremity Alignment)

  • Purpose : This study aimed to examine the relationships among five clinical measures for functional alignment of the lower extremity. Methods : Thirty healthy subjects (15 males and 15 females) were recruited for the study. The five clinical measures of functional alignment of the lower extremity included navicular drop, quadriceps angle, internal rotation of hip, and anterior and lateral pelvic tilt angles. The level of navicular drop was calculated by the difference between the height of the navicular bone in the sitting (non-weight bearing) and standing (weight bearing) positions. The quadriceps angle and internal rotation of hip were measured using a standard goniometer with photographic markers while the subjects were lying in a prone position on a table with their knee at $90^{\circ}$ flexion. Anterior and lateral pelvic tilt angles were determined using a inclinometer. Results : Correlation and a simple linear regression analysis were used to assess relationships between the clinical measures. There were significant correlations between navicular drop and quadriceps angle (p<.05), between navicular drop and internal rotation of hip (p<.05), and between quadriceps angle and internal rotation of hip (p<.01). In simple linear regression analysis, the navicular drop appeared to be a factor affecting the quadriceps angle and internal rotation of hip (p<.05). The findings suggest that navicular drop has a great impact on lower extremity alignment. Conclusion : This study might help us to examine lower extremity function and clarify its role as a potential injury risk factor.

이중과제 훈련이 뇌졸중 환자에게 미치는 영향 : 체계적 고찰 및 메타분석 (Effects of the Dual-Task Training on Stroke Patients : A Systematic Review and Meta-analysis)

  • 목적 : 본 연구는 뇌졸중 환자를 대상으로 한 이중과제 훈련(Dual-task training)의 효과를 알아본 연구들을 분석하는 것을 목적으로 한다. 연구방법 : NDSL, RISS, PubMed, CoChrane, EMBASE를 통해 지난 10년 간 국내외 학술지에 게재된 논문들을 검색하였고, 최종적으로 10개의 논문을 선정하였다. 이를 전통적 단일계층 근거 모형에 따른 질적평가를 시행하고, Comprehensive Meta Analysis 3.0 프로그램을 통해 메타분석을 시행하였다. 결과 : 선정된 10편의 논문의 질적 수준은 모두 전통적 단일계층 근거 모형의 I과 II에 해당되어 높은 편에 속했다. 이중과제 훈련을 구성하고 있는 운동과제로는 7편의 문헌에서 보행 또는 균형 과제를 선택했으며, 3편의 연구에서 상지운동 관련 과제를 선정하였다. 메타분석을 실시한 결과 일상생활활동 수행능력이 0.65, 인지기능이 0.64로 보통(medium) 효과크기로, 하지운동 기능과 상지운동 기능은 각각 0.34와 0.22로 작은(small) 효과크기를 보였다. 일상생활활동 수행능력과 인지기능의 효과크기만 통계학적으로 유의미하였다(p<0.05). 결론 : 본 연구를 통해 이중과제 훈련이 뇌졸중 환자의 일상생활활동 수행능력 및 인지기능 회복에 유용한 중재기법이 될 수 있음을 확인하였다. 이는 국내 임상 환경에서 뇌졸중 환자에게 적절한 중재방법을 선정할 때 도움이 될 만한 자료로 활용될 수 있을 것이다.

오타고 운동 프로그램이 장애 노인의 신체적 기능과 낙상 예방에 미치는 효과 (The Effect of Otago Exercise Program on Physical Function and Fall Prevention in Disabled Elderly)

  • 본 논문은 장애 노인에게 오타고 운동 프로그램이 낙상감소에 미치는 영향과 신체기능과 자신감 회복의 효과를 확인하기 위해 시도되었다. 연구대상은 총 30명으로, 두 그룹으로 할당하여, 실험군 17명, 대조군 13명이었다. 오타고 운동군에게 오타고 운동 프로그램을 12주간 주 2회 각 50분씩 시행하였다. 측정도구는 하지근력, 균형, 유연성과 낙상효능감 변수는 오타고 운동 전과 12주간 오타고 운동 중재 후에 낙상효능감, 낙상횟수와 신체적 기능 변화를 측정하였다. 자료 분석은 일반적 특징은 기술통계로 분석하였고, 두 그룹간의 분석은 독립 T 검정을 하였고, 두 그룹내 분석은 대응 T 검정를 하였다. 연구결과 신체적 기능 변화에서 하지근력의 변화는 통계적으로 유의하였고(p< .01), 대조군은 하지근력이 감소하였으나, 통계적으로 유의하지는 않았다. 정적균형의 변화는 실험군이 유의하게 증가하였고, 대조군은 유의한 감소를 나타내었다. 유연성의 변화는 실험군에서 유의한 증가를 보였고, 대조군은 유의하게 감소하였다. 동적 균형의 변화는 실험군에서 유의한 증진을 나타내었고, 대조군의 동적 균형은 유의한 저하를 나타내었다. 낙상효능감의 변화에서 실험군은 약간의 향상을 나타내었고, 대조군은 유의한 저하를 나타내었다. 본 연구 결과 오타고 운동 프로그램이 장애노인 낙상예방에 대한 신체적기능과 낙상효능감을 증진시켜 긍정적 변화를 가져왔다는 것이 입증되었다.

Correlation of single leg vertical jump, single leg hop, and single leg squat distances in healthy persons

  • Objective: To determine the correlation among three functional tests: single leg vertical jump (SLVJ), single leg hop for distance (SLHD), and single leg squat (SLSQ). Design: Cross sectional study. Methods: Twenty healthy men (n=10) and women (n=10) with no history of lower extremity dysfucntion participated in this study and performed in university research laboratory. The procedures consisted of a general warm-up, a task-specific warm-up, actual testing, and a cool down. All participants performed the three tests in random order. Each test was performed three times for the dominant and non-dominant lower extremity (LE). SLVJ, SLHD, SLSQ were measured using a standard tape measure. Results: Statistically significant difference was presented between dominant LE and non-dominant LE in each function test (p<0.05). The strongest correlation was between SLVJ and SLSQ, 0.939 and 0.883 for dominant and non-dominant LE, respectively (p<0.05). The weakest correlation was between SLVJ and SLHD, 0.713 for dominant (p<0.05) and between SLSQ and SLHD, 0.739 for non-dominant (p<0.05). Conclusions: There is a strong correlation between SLVJ and SLSQ, suggesting that each test measures similar constructs of function and can be substitutive, while weak correlation between SLSQ and SLHD suggest these two tests do not measure the same functional components and could be paired as outcome measures for the clinical assessment of LE function. It will provide physical therapist with scientific evidence for effective test combination of LE function assessment in clinical practice.
