Deep overbite patients who do not have proper occlusal relationship may cause problems such as teeth wear and antagonist extrusion. These lead to the collapse of occlusal plane and esthetic problem. Increasing vertical dimension is frequently essential to resolve those problems. This case report demonstrates a full-mouth rehabilitation for a patient with severe deep bite that contacts surface to surface by increasing vertical dimension. Treatment procedures included diagnosis, treatment planning, implant surgery, and prosthodontic rehabilitation. Satisfactory results were obtained in functional and esthetic aspects.

Recently, digital technology has become increasingly prevalent in the dental clinic. Using a milling machine for clinic, it is possible to produce provisional restoration inside the clinic. This can promote large clinical cases such as full mouth rehabilitation with the help of a tabletop scanner, which is capable of semi-adjustable articulator equipment, and a powerful dental CAD software with excellent user convenience. In this case report, a full-mouth rehabilitation was done with digital technology to a 55 year-old female patient, who has lost vertical dimension through the attrition, and has got inclined occlusal plane with unplanned and repeated dental reconstruction. Through the design and milling of the provisional restoration in the clinic and the duplication of these provisionals by double scanning technique, a good functional and esthetic result could be achieved.

Prosthodontic decision-making process is done through analysis of clinical information and mutual communication between patient and dentist. When opposing teeth are extruded due to missing tooth or tooth defects, selection of appropriate treatment plan and predictory prognosis can be complicated in functional rehabilitation. In case of severely disharmonious occlusal plane result from extruded teeth, re-establishment of the occlusal plane is required, if necessary via prudent evaluation. In this case, class III patient had unesthetic appearance and mastication discomfort caused by disharmonious occlusal plane due to severe extrusion. Through a structured diagnostic process, appropriate treatment plan was selected. Esthetic and functional results were obtained through from full mouth rehabilitation with re-establishment of the occlusal plane.

Inadequate or unstable posterior support cause extrusion of antagonistic teeth and overloading of remaining teeth. Extrusion of antagonistic teeth result in collapsed occlusal plane and decreased prosthetic space. Also, overloading of remaining teeth increase occlusal wear or anterior extrusion of anterior teeth, which is resulting in loss of anterior guidance, disharmony of occlusion and decreased prosthetic space. In this case, careful evaluation of OVD(occlusal vertical dimension) is the general first trial to regain appropriate function and esthetics. Simultaneously, adequate anterior guidance and harmonious occlusal plane should be established in the repeatable reference position of the mandible. We report this case because severely worn dentition achieved functional and esthetic results from full mouth rehabilitation with increased vertical dimension and use of centric relation.

Excessive teeth abrasion causes pathologic changes of oral environment and masticatory system due to decrease in occlusal vertical dimension. When establishing new occlusal plane and recovering vertical dimension, accurate diagnosis and analysis are essential. In this case, after appropriate centric relation in elevated vertical dimension was taken using Gothic arch tracer, full mouth rehabilitation was performed. A 70-year-old male patient had the chief complaint that he could not eat due to teeth wear. He showed generalized teeth wear and decreased vertical dimension. Elevation of vertical dimension was planned by model analysis. According to increased vertical dimension, centric relation was recorded using Gothic arch tracer and temporary prostheses were applied. Appropriate occlusion was established by temporary prostheses for 4 months. Final prostheses were fabricated using vertical dimension adapted by temporary prostheses. Through this procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

Parkinson's disease is a neurological disorder characterized by tremor, bradykinesia, akinesia, postural instability, and muscular rigidity, which is caused by the depletion of neurotransmitters such as dopamine. Cooperative dental treatment is more challenging because of tremor of Parkinson's disease. In this case, a 47-year-old Parkinson's disease patient with chronic periodontitis was treated with full-mouth rehabilitation using conventional fixed prostheses and implant fixed partial denture, which attained satisfactory outcomes functionally and esthetically. Short term periodic follow-ups will be needed with consideration for the characteristics of Parkinson's disease such as decreased manual dexterity.

The loss of posterior occlusal support due to tooth loss is likely to lead to compensatory protrusion and labial tilt of the anterior teeth, which may be accompanied by a deep bite and a decrease in vertical dimension. The patient may suffer from a decrease in masticatory efficiency, inaccurate pronunciation, facial appearance changes, and temporomandibular joint disorder, so stable occlusal formation with support of posterior occlusion and restoration of vertical dimension is necessary. We report the case of a patient with reduction of vertical dimension, and inaccurate pronunciation due to multiple tooth loss who underwent full mouth rehabilitation with increased vertical dimension accompanied by phonetic analysis and achieved satisfactory functional and aesthetic results.
