BACKGROUND. Generalized aggressive periodontitis (GAP) is a destructive periodontal disease that can develop in young age. Only a few cases of full mouth rehabilitation, using dental implants, have been reported in a patient with aggressive periodontitis. CASE DESCRIPTION. This clinical report describes the treatment procedures and results of full mouth rehabilitation in a patient with aggressive periodontitis. After all teeth were extracted, 6 implants were placed in the maxilla and mandible, respectively. Fixed detachable implant prostheses were made. The patient was satisfied with the final results. She was followed for 10 months postloading. CLINICAL IMPLICATION. For a long-term success, continuous maintenance care is critical, as the contributing factors of the disease (such as immune factors or periodontal pathogens) may not be controlled adequately.

악교정 수술과 임플란트 보철을 통한 전악재건술의 임상적 치료는 보철과 의사에게 도전적인 과제이다. 본 증례는 골격적 부조화를 가진 다발성 우식증 환자의 진단과 치료에 있어서 치의학의 여러 분야의 협진을 보여주었다. 그 결과로 악교정 수술을 동반한 발치 후 즉시 임플란트 식립 및 조기 하중을 가하는 임플란트 보철물에 의한 전악 재건이 예견가능하고 효과적인 치료 방법이었기에 이를 보고하고자 한다.

전악 구강 수복에서 이상적인 교합평면을 형성하기 위한 기존의 진단 및 치료계획은 각각의 과정이 복잡하고 치과의사와 기공사간의 정보교환이 주관적이므로 어려움이 있다. OP finder^® system은 동공간선이 평행하지 않은 환자의 정보를 보다 객관적이고 정확하게 전달하는데 도움이 된다. 본 증례에서는 오래된 불량한 고정성 보철물과 심하게 마모된 하악 치아를 가진 환자에게 OP finder^® system을 이용하며 전악수복을 시행하였고, 적절한 교합평면 설정 및 저작기능 회복 결과를 얻었기에 보고하고자 한다.

Severe tooth wear may cause the pathologic change of the TMJ and masticatory muscles, unesthetic facial appearance, pathogenic pulp and occlusal disharmony. Treating patients with severely worn dentition often requires full mouth rehabilitation with increasing vertical dimension. Proper diagnosis and treatment planning are important for esthetic and functional definitive restorations and the long term stability of the neuromuscular system and the TMJ. In this case, 66 year-old female presented with generalized worn dentition. Based on assessment, pathologic destruction of teeth structure on entire dentition was caused by masticatory force and diet habit without loss of vertical dimension. Subsequently, 3 mm increase of vertical dimension that based on incisor for tooth restoration and esthetic improvement was determined. After 8 weeks stabilization period with temporary fixed prostheses, definitive prostheses were fabricated. After 6 months follow up period, satisfactory outcomes were attained both functional and esthetic aspects through this procedure.

Purpose: The purpose of this study was to evaluated the process of model analysis, design, and production for a full mouth prosthetic rehabilitation of function, esthetics, and pronunciation. Methods: This is a case report of class III malocclusion with a severe anterior incisor occlusion and a prosthetic treatment of patient with poor occlusion of posterior teeth. A provisional restoration based on the diagnostic wax-up was applied to the patient. And then functions, esthetics, and occlusal stability were observed during 4 week tracking period. Final restorations were delivered and evaluated based on a systematic analysis, diagnosis and treatment plan. Results: We confirms that this case study obtained the satisfactory results in terms of functions and esthetics. Conclusion : If we can give the patient the continuous evaluation and progress monitoring, we expect the dentist and the dental technician to design the prosthesis successfully.

This case study is aimed at introducing a full-mouth rehabilitation of a patient with severe tooth wear using fixed prosthesis. This is a case report of a patient with severe wear dentition with changing vertical dimension. In line with using prosthetic treatment, patient adaptation was verified with provisional restorations followed by diagnostic wax-up. Function, esthetics, and occlusal stability were verified during a 4-week follow up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory results after delivery of definitive prosthesis. Prosthetic treatment of severe wear dentition was functionally and esthetically successful. Dentists and dental technicians would be able to develop better treatment approaches using fixed dental prosthesis.

The case report describes the occlusal rehabilitation of a male patient with Angle Class III malocclusion and its effect on maximum bite force. The main complaints of patient were masticatory difficulty and poor esthetic. The patient's expectations from the treatment were a good esthetic and function with a less invasive and relatively promptly way. Therefore, increasing of the occlusal vertical dimension (OVD) and then restoring the maxillary and mandibular teeth was chosen by the patient among the treatment options. At the beginning of treatment maximum bite force of patient was measured. Then an occlusal splint was provided to evaluate the adaptation of the patient to the altered OVD. Full mouth rehabilitation with metal ceramic restorations was made. After the completion of full mouth restoration, bite force measurement was repeated and patient exhibited increased maximum bite force. Full mouth restorative treatment in a patient with Class III malocclusion could be an effective treatment approach to resolve esthetic concern and to improve masticatory function related to maximum bite force.

이전 단일 구조 지르코니아는 색조의 단조로움 등 심미성의 문제로 전치부 사용에서 어려움이 있었다. 이에 전치부 증례의 지르코니아는 코핑으로만 주로 이용되어 최종보철물은 추가적인 도재 피개가 필요했었다. 이에 따라 도재 파절 등의 문제가 존재하였으나 점차 단일 구조 지르코니아 자체의 투과도와 색조가 개선되어 도재 피개 없이도 사용이 가능해졌다. 본 증례는 63세 남성환자로 단일 구조 지르코니아를 이용하여 전악 보철 수복을 시행해 기능적으로나 심미적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

병적 치아 마모의 경우 교합면의 병리적 손상, 기능장애, 교합 이상, 심미문제, 치수병변, 턱관절 장애 등의 증상을 보인다. 이러한 경우에는 원인을 분석하고 수직고경 손실여부, 교합 양상 등을 고려하여 치료 계획을 세워야 한다. 본 증례는 심한 마모가 있는 71세 남환으로 수직고경 거상을 동반한 전악수복을 하였다. 임시 보철물을 통해 증가된 수직고경에 대한 환자의 적응도를 평가하였고, 이상 소견이 없는 것을 확인하고 최종 보철물을 장착하였다. 일련의 치료 과정을 통해 만족스러운 결과를 얻었기에 보고하는 바이다.

과도한 수평 및 수직피개를 가진 과개교합 환자에서 치질의 파괴 및 대합치 정출의 문제를 흔히 관찰할 수 있다. 이는 곧 교합평면의 붕괴와 부적절한 전방유도로 이어지며, 조화로운 교합관계의 회복이 이루어지지 않으면 다수의 치아 상실과 저작기능의 감소를 야기하게 된다. 과개교합환자의 문제를 해결하려면 교정과, 구강외과 및 보철과의 협진이 이루어져야 한다. 본 증례보고는 심한 과개교합을 가진 환자에서 수직교합고경을 증가시킨 후 완전구강회복을 이룬 증례를 보고하고자 한다. 치료는 구외 및 구내검사, 진단 및 치료계획, 진단 납형형성, 분절골 절단술, 교정적 함입 및 보철치료 순으로 진행되었다. 수직고경증가를 동반한 완전구강회복술로 환자의 심미적, 기능적 문제를 해결할 수 있었다.
