Reconstruction of Postburn Nasal Alar Defect by Paramedian Forehead Flap (화상으로 손실된 비익에 대한 전두피판을 이용한 재건)

  • In the face, the nose plays an important role in both function and appearance. Among the subunits on the nose, the alar is a critical nasal structure of high aesthetic value. Previously, many surgeons have insisted that structural support should be added to the reconstruction of alar and particularly so in the case of a full-thickness defect. In a 58-year-old male patient who had a third-degree burn injury and full-thickness defect in most of the left nasal alar, forehead flap surgery alone was successfully performed without structural support.

  • Most gastric subepithelial tumors (SETs) are asymptomatic and are often incidentally discovered during endoscopic procedures conducted for unrelated reasons. Although surveillance is sufficient for the majority of gastric SETs, certain cases necessitate proactive management. Laparoscopic wedge resection, although a viable treatment option, has its limitations, particularly in cases where SETs (especially those with intraluminal growth) are not visualized on the peritoneal side. Recent advances in endoscopic instruments and technology have paved the way for the feasibility of endoscopic resection of SETs. Several promising endoscopic techniques have emerged for gastric SET resection, including submucosal tunneling endoscopic resection, endoscopic full-thickness resection (EFTR), laparoscopic and endoscopic cooperative surgery (LECS), and non-exposure EFTR (non-exposed endoscopic wall-inversion surgery and non-exposure simple suturing EFTR). This study aimed to discuss the indications, methods, and outcomes of endoscopic therapy for gastric SETs. In addition, a simplified diagram of the category of SETs according to the therapeutic indications and an algorithm for the endoscopic management of SET is suggested.

Thermo-Structural Survivability Evaluation of a Thrust-Measuring Nozzle Extension in a Full-scale Combustor (실물형 연소기의 추력측정용 노즐확장부에 대한 열/구조적 건전성 평가)

  • The survivability of the temporary nozzle extension for an accurate thrust measurement in a full-scaled combustor has been investigated through thermal analyses. The effects of nozzle extension materials and the thickness of thermal barrier coating (TBC) have been elucidated. It has been found that thermal survivability cannot be guaranteed without TBC. The maximum temperature of the nozzle extension decreased with increasing TBC thickness. For hot firing tests, the TBC is thought to be indispensable to the thermo-structural survivability of the nozzle extension made of steel.

Free vibration analysis of FG composite plates reinforced with GPLs in thermal environment using full layerwise FEM

  • The current investigation is the first endeavor to apply the full layerwise finite element method (FEM) in free vibration analysis of functionally graded (FG) composite plates reinforced with graphene nanoplatelets (GPLs) in thermal environment. Unlike the equivalent single-layer (ESL) theories, the layerwise FEM focuses on all three-dimensional (3D) effects. The GPLs weight fraction is presumed invariable in each layer but varies through the plate thickness in a layerwise model. The modified Halpin-Tsai model is employed to acquire the effective Young's modulus. The rule of mixtures is applied to specify the effective Poisson's ratio and mass density. First, the current method is validated by comparing the numerical results with those stated in the available works. Next, a thorough numerical study is performed to examine the influence of various factors involving the pattern of distribution, weight fraction, geometry, and size of GPLs, together with the thickness-to-span ratio, thermal environment, and boundary conditions of the plate, on its free vibration behaviors. Numerical results demonstrate that employing a small percentage of GPL as reinforcement considerably grows the natural frequencies of the pure epoxy. Also, distributing more square-shaped GPLs, involving a smaller amount of graphene layers, and vicinity to the upper and lower surfaces make it the most efficient method to enhance the free vibration behaviors of the plate.

A Study on the Fabrication and Characteristic Analysis of Organic Light Emitting Device using BAlq (BAlq를 적용한 유기발광소자의 제작 및 특성 분석에 관한 연구)

  • BAlq was fabricated as for hole blocking layer in the OLED devices to investigate its electrical and optical characteristics. Device structure was ITO/$\alpha$ -NPD/EML/BAlq/Alq3/Al:Li using TYG-201, DPVBi (4, 4 - Bis (2, 2 - diphenylethen-1 - yls) - Biphenyl), Alq and DCJTB (4-(dicyanomethylene)-2- (1-propyls)6-methy 4H-pyrans) as green emitting material, blue emitting material, host material for red emission and red emitting guest material respectively. The OLED device showed optimum working voltage and electron density at 600 cd/$m^2$ when thickness of BAlq is 25$\AA$ for RGB OLED devices while their efficiencies are better at 50$\AA$ of BAlq. Red and blue color OLEDs also fabricated using 30$\AA$ thickness of BAlq and compared with those without BAlq layer. BAlq was more effective in electrical properties such as working voltage, current density and efficiency of red OLED than blue and green ones. This study describes that 30$\AA$ is optimum thickness of BAlq for best performance of full color OLED devices when using BAlq as a hole blocking material.

The Adipofascial V-Y Advancement Flap with Skin Graft for Coverage of the Full-Thickness Burns of the Gluteal Region

  • Any types of burn injury that involve more than deep dermis often require reconstructive treatment. In gluteal region, V-Y fasciocutaneous advancement flap is frequently used to cover the defect. However, in case of large burn wounds, this kind of flap cannot provide adequate coverage because of the lack of normal surrounding tissues. We suggest V-Y adipofascial flap using the surrounding superficially damaged tissue. We present the case of a patient who was referred for full-thickness burn on gluteal region. We performed serial debridement and applied vacuum-assisted closure device to defective area as wound preparation for coverage. When healthy granulation tissue grew adequately, we covered the defect with surrounding V-Y adipofascial flap and the raw surface of the flap was then covered with split-thickness skin graft. We think the use of subcutaneous fat as an adipofascial flap to cover the deeper defect adjacent to the flap is an excellent alternative especially in huge defect with uneven depth varying from subcutaneous fat to bone exposure in terms of minimal donor site morbidity and reliability of the flap. Even if the flap was not intact, it was reuse of the adjacent tissue of the injured area, so it is relatively safe and applicable.

Experimental seismic behavior of RC special-shaped column to steel beam connections with steel jacket

  • The seismic performance of the reinforced concrete (RC) special-shaped column to steel beam connections with steel jacket used in the RC column to steel beam fabricated frame structures was investigated in this study. The three full-scale specimens were subjected to cyclic loading. The failure mode, ultimate bearing capacity, shear strength capacity, stiffness degradation, energy dissipation capacity, and strain distribution of the specimens were studied by varying the steel jacket thickness parameters. Test results indicate that the RC special-shaped column to steel beam connection with steel jacket is reliable and has excellent seismic performance. The hysteresis curve is full and has excellent energy dissipation capacity. The thickness of the steel jacket is an important parameter affecting the seismic performance of the proposed connections, and the shear strength capacity, ductility, and initial stiffness of the specimens improve with the increase in the thickness of the steel jacket. The calculation formula for the shear strength capacity of RC special-shaped column to steel beam connections with steel jacket is proposed on the basis of the experimental results and numerical simulation analysis. The theoretical values of the formula are in good agreement with the experimental values.
