Background: Minimally invasive techniques for open heart surgery are widely accepted in these days. There are minimally invasive approaches by the right or left parasternal incision and another approaches by mini-sternotomy of upper or lower half or sternum. We report the safety and efficacy of minimally invasive technique with right parasternal incision compared with the routine full sternotomy. Material and Method: From April 1997 through February 1999, 20 patients(Group A) underwent minimally invasive cardiac operations. We chose 41 patients(Group B) whose preoperative diagnosis were the same and general conditions were similar and who underwent routine full sternotomy before April 1997. We compared A group and B group in many aspects. We performed routine full median sternotomy in B group but we did a minimally invasive technique through a small right parasternal incision in A group. Result: mean age was 36.1 years in both groups. In disease entities, there were 11 cases of ASD, 9 cases of mitral valve disease in group A, and 16 cases of ASD, 25 cases of mitral valve diseases in group B. In ASD, operation time, cardiopulmonary bypass time of aortic occulusion time were 263 min, 82 min, and 41 min in group A and 180 min, 53 min, and 32 min in group B. In mitral valve disease, operation time, cardiopulmonary bypass time and aortic occlusion time were 267min, 106 min, and 70min in A group and were 207 min, 82 min, and 69 min in group B. There were significant differences in operation time, CPB time, and ACC time between group A and group B. There was a significant difference in the amount of bleeding in postoperative day 1 between group A and group B of mitral diasease. However, there was no significant difference in the amount of bleeding in other comparisons. Mean length of incision was 8.7 cm in group A. There was no significant difference in postoperative complications between A group and B group. There was no mortality in either group. Conclusion: We conclude that this minimally invasive technique with right parasternal incision is cosmetically excellent but it is not effective in reducing operative time and there was no significant difference in recovery time and postoperative complications compared with routine full sternotomy.
서울중앙병원에서 1989년 개원 이래로 시행해 오던 우전측부 개흉술을 통한 심방중격결손증과 승모판막에 대한 수술 결과를 알아보고 1997년 8월부터 10월까지 보다 광범위하게 시행된 최소침습적인 개심술의 결과를 정리하여 향후 최소침습적인 수술 조작에 대한 지표로 삼고자 하였다. 우전측부 개흉으로 17예의 심방중격결손증, 4예의 승모판 성형술, 6예의 승모판막치환술, 그리고 1예의 세번째의 심장수술에서의 삼첨판과 승모판 재대치술을 시행하여 1예의 출혈로 인한 재수술을 제외하고는 이 접근으로 인한 합병증은 없었다. 1997년 8월부터 10월까지 6례의 대동맥판막 치환례중 누두흉 1례를 제외한 5례에서 4례는 상부흉골절개를, 1례에서는 횡흉골절개를 시도하였다. 같은 기간 동안에 7예의 심방중격결손증에서 우전측부 개흉술과 하부 흉골절개로 수술을 시행하여 무리없이 수술을 마칠 수 있었고 미용적인 면과 출혈량에서 특히 우수한 결과를 나타내었다. 이에 저자는 이러한 최소 침습적인 개심술이 안전하고 환자의 호응이 높아 앞으로 보다 적극적으로 시도되어야 할 것으로 결론을 내린다.
배경: 선천성 심장 질환에서의 최소 피부 절개에 대한 보고는 매우 적다. 저자등은 수술상처를 최소화하면서 통상적인 수술수기 및 시야를 확보할 수 있는 방법을 고려하였다. 대상 및 방법: 저자등은 1997년 4월부터 1997년 9월까지 선천성 심장 질환 환자 40명을 대상으로 최소 피부 절개술과 정중 전 흉골 절단을 통해서 개심술을 시행하였다. 환자 질병 분포는 성인 환자가 5명(남:여=1:4)이며 이중, 심방 중격 결손증 3명, 심실 중격 결손증 1명, 부분 심내막상 결손증 1명이었고, 소아 35명(남:여=17:18)에서는 심방 중격 결손증 4명, 심실 중격 결손증 30명, 발살바 동맥류 1명이었다. 정중 피부 절개는 흉골 두번째 늑간하부에서 검상돌기 1~2cm상방까지 실시하였다. 흉골 하부 절단에는 일반 전기톱을, 흉골 상부 절단에는 특수 전기톱을 피부 밑에 삽입하여 전장의 흉골을 절단하였다. 그리고, 흉골 견인은 좌우 & 상하 양방향에 2개의 견인기를 각각 직각으로 거치하여 수술시야를 확보하였다. 결과: 흉골길이대비 피부절개의 길이는, 성인에서는 55.0$\pm$3.5%로 절개길이가 평균 12cm(10~13.5cm)였고, 소아에서는 63.1$\pm$3.9%로 평균 7.3cm(5.2~11cm)였다. 모든 증례에서 체외 순환시 필요한 동정맥 삽관을 추가적인 서혜부 피부 절개 없이 직접 대동맥 및 상하공 대정맥에 시행할 수 있었으며 좌심방벤트관은 필요시 삽입하였다. 전 례에서 수술사망이나 합병증은 발생하지 않았으며, 최소 피부 절개술에 따른 창상 감염 및 피부 괴사, 혈종 형성, 출혈에 의한 합병증은 없었다. 결론: 이상의 결과로 최소 피부 절개술과 정중 전 흉골 절개를 이용한 선천성 심장 질환 수술은, 통상적인 체외 심폐 순환을 할 수 있으며, 외관상 미용의 효과가 뚜렷하였고, 다양한 선천성 심질환에서 안전하고 효과적으로 사용될 수 있다고 판단된다.

Background: The standard approach in treating cardiac myxoma is the median full sternotomy. With the evolution of surgical techniques, the right minithoracotomy approach has emerged as an alternative method. Since few studies have been published assessing the right minithoracotomy approach, we performed a retrospective study to compare the clinical outcomes of the right minithoracotomy approach with those of the sternotomy approach. Methods: From January 2005 to December 2014, 203 patients underwent resection of a cardiac myxoma. Patients with preexisting cardiac problems were excluded from this study. 146 patients were enrolled in this study; 83 patients were treated using a median sternotomy and 63 patients were treated using a right minithoracotomy. Results: No early mortalities were recorded in either group. Although the cardiopulmonary bypass time and aorta cross-clamp time were significantly shorter in the sternotomy group (p<0.001 and p=0.005), postoperative blood transfusions and arrhythmia events were significantly less common in the thoracotomy group (p=0.004 and p=0.025, respectively). No significant differences were found in the duration of the hospital stay, postoperative intubation time, the duration of the intensive care unit stay, and recurrence. Conclusion: The minimally invasive right minithoracotomy approach is a good alternative method for treating cardiac myxoma because it was found to be associated with a lower incidence of postoperative complications and a shorter postoperative recovery period.
Background: There are several modalities of coronary artery revascularization for multivessel coronary artery disease. Hybrid coronary revascularization (HCR) with minimally invasive direct coronary artery bypass grafting was introduced for high-risk patients, and recently, many centers have been using it. Limited incisional full sternotomy coronary artery bypass (LIFCAB) involves left internal thoracic artery (LITA)-to-left anterior descending coronary artery (LAD) anastomosis through a sternotomy with a minimal skin incision; it could be considered another technique for minimally invasive LITA-to-LAD anastomosis. Our center has performed HCR using LIFCAB, and in this paper, we report our short-term results, obtained in the past 3 years. Methods: The medical records of 38 patients from May 2010 to June 2013 were analyzed retrospectively. The observation period after HCR was 1 to 37 months (average, $18.3{\pm}10.3$ months). The patency of revascularization was confirmed with postoperative coronary angio-computerized tomography or coronary angiography. Results: There were 3 superficial wound complications, but no mortalities. All the LITA-to-LAD anastomoses were patent in the immediate postoperative and follow-up studies, but stenosis was detected in 3 cases of percutaneous coronary intervention. Conclusion: HCR using LIFCAB is safe and yields satisfactory results from the viewpoint of revascularization for multivessel disease.
Background: The aim of this study is to evaluate our institutional results of the aortic valve replacement through minimally invasive approaches compared with conventional sternotomy. Materials and Methods: From August 1997 to July 2010, 838 patients underwent primary isolated aortic valve replacement. Of them, 73 patients underwent surgery through minimally invasive approaches (MIAS group) whereas 765 patients underwent surgery through the conventional sternotomy (CONV group). Clinical outcomes were compared using a propensity score matching design. Results: Propensity score matching yielded 73 pairs of patients in which there were no significant differences in baseline profiles between the two groups. Patients in the MIAS group had longer aortic cross clamp than those in the CONV group ($74.9{\pm}27.9$ vs.. $66.2{\pm}27.3$, p=0.058). In the MIAS group, conversion to full sternotomy was needed in 2 patients (2.7%). There were no significant differences in the rates of low cardiac output syndrome (4 vs. 8, p=0.37), reoperation due to bleeding (7 vs. 6, p=0.77), wound infection (2 vs. 4, p=0.68), or requirements for dialysis (2 vs. 1, p=0.55) between the two groups. Postoperative pain was significantly less in the MIAS group than the conventional group (pain score, $3.79{\pm}1.67$ vs. $4.32{\pm}1.56$; p=0.04). Conclusion: Both minimally invasive approaches and conventional sternotomy had comparable early clinical outcomes in patients undergoing primary isolated aortic valve replacement. Minimally invasive approaches significantly decrease postoperative pain.
배경: 최근에 시작된 소절개선을 이용한 심장수은술 환자에게 미용상 효과가 좋으며, 통증이 적으며 빠른 회복을 가져온다. 본 교실에서는 그간 실시한 소절개심장수술의 현황을 임상고찰하였다. 대상 및 방법: 1997년 2월 흉골좌연종절개선으로 관상동맥우회술을 실시한 이후 1998년 11월까지 총 31례의 소절개술을 이용한 심장수술을 실시하였다. 남녀 비는 17:14였으며, 1세에서 75세까지의 연령분포를 보였다. 흉골좌연종절개술을 실시한 예는 9례로 관상동맥 질환으로 인공심폐기의 사용없이 박동상태에서 내유동맥으로 좌전행지관상동맥에 우회술을 실시하였는데, 그 중 1례는 내유동맥의 비꼬임으로 술 후 1주일 째 재수술한 경우였다. 흉골우연종절개는 1례로 승모판교련절개술 후 재발된 협착증에 대해 승모판치환술을 실시한 경우였다. 소흉골절개선으로 수술한 예는 21례로, 승모판치환 및 삼첨판성형술이 6례, 승모판치환술 5례, 중복판치환술 2례, 대동맥판치환술 1례, 좌심방점액종 1례, 심방중격결손증 2례, 심실중격결손증 2례, 우심실 자창 1례였다. 처음 5례는 T형의 흉골소절개를 실시하였으나 경험이 쌓이면서 대동맥기저부의 노출이 좋고 흉골 봉합시 안정감이 높은 화살모양의 흉골소절개를 실시하였다. 결과: 수술시간, 인공심폐 구동시간, 대동맥차단시간, 인공호흡기 사용기간, 수술 1일까지의 흉관배액양, 흉관 거치기간, 집중치료실 입원기간 등은 기존의 정중흉골절개선 예들에 비하여 큰 차이가 없었다. 수술 후 사망 예는 2례였으며 1례는 흉골소절개선으로 승모판치환 수술 1일에 대동맥 삽관부위의 파열로 출혈 사망하였고, 다른 1례는 흉골좌연종절개선으로 관상동맥우회술 후 2일에 부정맥으로 사망하였다. 사망의 원인과 수술절개선의 선택과는 직접적인 관련이 없었다. 합병증은 뇌색전증 1례, 창상의 혈종 1 례가 있었다. 결론: 소절개선으로도 정중흉골절개선과 마찬가지로 심장 수술을 효과적으로 실시할 수 있을 것으로 생각된다.
Background: Minimally invasive direct coronary artery bypass surgery(MIDCAB) has been increasing in interest along with the new techniques in myocardial immobilization for easier and safer procedures. Until the opening of the era of new techniques, adequate accuracy and good patency of grafts were debatable. Our experiences of MIDCAB were studied according to the stages of technical developments. Material and Methods: Since March 1996, 55 patients have undergone MIDCAB procedures. The patients of off-pump CABG(no cardiopulmonary bypass under full sternotomy) were excluded from the study. In the early experience(Stage I), a left anterior small thoracotomy through the left parasternal incision was performed(n=6); then an approach through the lower partial sternotomy was used(Stage II, n=33); and recently, a chest wall elevator for harvesting the internal thoracic artery and the foot plate for myocardial immobilization have been used(USSC, Norwalk, CT)(Stage III, n=16). Result: The surgical procedures of four patients in the Stage II group have been converted to conventional bypass because of the deeply seated left anterior descending coronary artery in two patients, fracture of the calcific lesion in the right coronary artery in one patient, and a cardiogenic shock during hypothermia in the other patient with ventricular dysfunction. Two patients in stage II experienced symptomatic recurrences after surgery and restenosis was verified on angiocardiography. They were managed by interventional procedures. All the other patients were doing well without symptoms, except one patients in Stage II who underwent PTCA procedure for a lesion in the circumflex artery during the follow up period. Conclusion: The new and specialized devices are essential to the development of MIDCAB surgery. MIDCAB and the hybrid procedures in multi-vessel disease are on the way to further development. So far, our experience is limited only to a single device among the many new devices for the purpose.
We report the case of a patient with mitral regurgitation complicated by type B dissection and Marfan syndrome (MFS) who was managed successfully with minimally invasive mitral valve repair. Without type A aortic dissection or aortic root dilation, MFS patients may develop mitral valve regurgitation, as in this case, and need valve surgery to improve their symptoms and long-term survival. However, it is not clear that a full sternotomy and prophylactic aortic surgery are necessary. Although retrograde perfusion to the dissected aorta is controversial, our approach minimizes the risk of future anticipated aortic surgery in MFS patients.
배경: 내시경 장비와 수술 로봇의 발달로 작은 절개를 이용한 최소 침습적 심장수술이 점점 증가하고 있으나 이에 대한 수술 결과를 고식적인 수술 방법과 비교, 분석한 국내 보고는 드문 편이다. 대상 및 방법: 2005년 12월부터 2007년 6월까지 수술용 내시경 로봇(AESOP2000)을 이용하여 심장수술을 받은 48명(A군)과 동 기간에 정중흉골절개로 수술을 받은 50명(B군)을 후향적으로 조사하여 수술 시간, 수술 결과, 술 후 통증, 회복 정도 등을 객관적으로 비교, 분석하였다. 결과: 수술 사망은 없었으며 술 후 합병증 발생률도 두 군간에 큰 차이는 없었다. 평균 수술시간($292.7{\pm}61.7$분, $264.0{\pm}47.9$분; p=0.01), 체외순환시간($(128.4{\pm}37.6$분, $101.7{\pm}32.5$분; p<0.01)은 A군에서 더 길었으나 대동맥차단시간 ($82.1{\pm}35.0$분, $87.8{\pm}113.5$분; p=0.74), 평균 인공호흡기 사용시간($18.0{\pm}18.4$시, $19.7{\pm}9.7$시; p=0.57)은 차이가 없었으며 중환자실 평균 입원시간($53.2{\pm}40.2$시, $72.8{\pm}42.1$시; p=0.02) 및 평균 재원기간($9.7{\pm}7.2$일, $14.8{\pm}11.9$일; p=0.01)은 A군에서 더 짧았다. 입원기간 중 수혈을 받은 환자는 B군에서 더 많았으며 평균 수혈양도 B군에서 더 많았으나 통계적 유의성은 없었다(p=0.06). 술 후 1주에서 4주까지 통증 점수는 A군에서 의미 있게 적었으며 일상생활의 수행 능력을 측정하는 듀크 활동상태지수 (Duke activity status Index)에 따른 기능 점수(functional status score)는 A군에서 현저하게 높았다. 술 후 심초음파 결과 승모판 성형술을 시행한 환자의 역류 정도($0.7{\pm}1.0,\;0.9{\pm}0.9$; p=0.60)와 삼첨판 성형술을 시행한 환자의 역류 정도($1.0{\pm}0.9,\;1.1{\pm}1.0$; p=0.89)는 두 군간에 통계적으로 유의한 차이가 없었다. 판막주위유출은 각각 1명의 환자에게 있었으며 그 외 판막 관련 합병증은 없었다. 결론: 본 연구 결과 로봇을 이용한 치소 침습적 심장수술은 수술 후 통증 감소, 환자의 빠른 회복, 미용적인 효과, 짧은 입원기간 등의 장점뿐만 아니라 수술 성적도 고식적 수술 방법에 비해 떨어지지 않음이 확인되었다.

- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
