Full mouth rehabilitation with dental implant utilizing 3D digital image and CAD/CAM system: case report (3차원 디지털 영상과 CAD/CAM 시스템을 활용한 전악 임플란트 수복 증례)

  • This article describes how to use digital system in a fully edentulous case that diagnosis to definitive prosthesis fabrication. While proceeding oral scan and CBCT taking, digital markers were attached on maxillary palate and lower existing denture. Using CBCT image and oral scan image, the bone contour and anatomical structures were analyzed and flapless surgical guide, customized abutment and prosthesis were made. After the osseointegration, the definitive prosthesis was fabricated using the oral scan image with scan body. It provides clinicians with a fast workflow and improves clinical efficiency.

  • Parkinson's disease is a neurological disorder characterized by tremor, bradykinesia, akinesia, postural instability, and muscular rigidity, which is caused by the depletion of neurotransmitters such as dopamine. Cooperative dental treatment is more challenging because of tremor of Parkinson's disease. In this case, a 47-year-old Parkinson's disease patient with chronic periodontitis was treated with full-mouth rehabilitation using conventional fixed prostheses and implant fixed partial denture, which attained satisfactory outcomes functionally and esthetically. Short term periodic follow-ups will be needed with consideration for the characteristics of Parkinson's disease such as decreased manual dexterity.

  • In the case of excessively worn dentition, there is often insufficient space for the prosthesis, and if physiologically acceptable, the prosthesis can be fabricated by increasing the vertical dimension of occlusion. Various methods have been introduced to determine the vertical dimension of occlusion. Clinicians have to choose a method that can comfort the patient among several methods. A removable appliance can be used as a reversible method to ensure that the determined vertical dimension of occlusion does not cause physiological problems. When making impressions of many teeth, it is often difficult to make accurate impressions at once. In this case, after making an accurate impression of the individual teeth, a transfer coping was made and a pickup impression was taken in the oral cavity to create a master cast. In this case, a fixed partial denture was fabricated and full mouth rehabilitation was performed by increasing the vertical dimension of occlusion in a patient with excessively worn dentition and lack of space for restoration. As a result of follow-up of the patient for 7 years, satisfactory results were obtained both esthetically and functionally.

  • Using computer-aided design and manufacturing technique improve quality of treatment in many aspect. This case reports the complete mouth rehabilitation of a patient with amelogenesis imperfecta utilizing digital technology. Clinical examination revealed loss of mastication due to insufficient occlusal stop, missing teeth, interdental spacing due to microdontia, insufficient overbite, and etc. Full veneer crowns for teeth were selected, followed by a fixed partial denture and implant placement was done using CAD-CAM guide template with bone graft for partially edentulous space. Definitive restorations were duplicated by double scanning provisional restorations and successfully delivered to the patient. These full mouth rehabilitation procedures resulted in satisfactory outcomes for the patient functionally and aesthetically.

A prospective study of patient satisfaction after implant supported mandibular overdenture treatment in fully edentulous patients (완전 무치악 환자에서 하악 임플란트 지지 피개의치 사용 후의 환자만족도에 관한 전향적 연구)

  • Purpose: The purpose of this prospective study was to compare the satisfaction levels of the patients worn implant supported mandibular overdentures and conventional complete dentures. Materials and methods: Full edentulous patients (n=40) who were planned to do implant supported mandibular overdentures, aged 51 to 82 years, were enrolled in this study. All subjects rated their satisfaction levels (mastication, pronunciation, esthetics, pain, and general satisfaction) of their original conventional dentures prior to treatment and their new overdentures 1 week and 12 weeks after the delivery on 10 cm visual analogue scales. Repeated-measures ANOVA was performed to compare the satisfaction level of each three period (P<.05). Results: The mean satisfaction levels of the implant supported overdentures after 1 week and 12 weeks of delivery were significantly higher than those of the conventional complete dentures in all scoring parameters (P<.05). After 12 weeks of overdenture delivery, the increase of the satisfaction levels in mastication, pronunciation, and pain and the decrease in esthetics and general satisfaction were observed in comparison with those after 1 week of delivery, however, no significant difference was found between the satisfaction levels of 1 week and 12 weeks after delivery. Conclusion: An implant supported overdenture might be one of the effective ways to give more satisfaction to patients who were not gratified with a conventional denture and eventually to improve their quality of life.

  • Purpose: The recent proliferation of the aging population, oral health care is closely related to the mental health of the elderly is very important being recognized. Provide basic data for improving the quality of life for the elderly, to determine whether the oral health of the elderly whether oral self-perception of health and any related wish. Methods: Surveys to May 20 from April 15, 2011, subjects were elderly and living in Daejeon and Chungnam 277 people were examined. All the statistics using SPSS 18.0 for the determination of statistical significance, and the significance level was 0.05. Results: 1.   • The purpose of this study was to examine the oral health states of elderly people over the age of 65 from the city of Seongnam, including the number of present permanent tooth, dental caries, rate of treatment need, CPITN and use of prosthesis, in a bid to lay the foundation for improving elderly oral health programs. The findings of the study were as follows: 1. The DMF rate of the male senior citizens stood at 98.3 percent, and that of the women numbered 99.0 percent and DMFT index 20.8. 2. The number of present permanent tooth excluded the third molars was 13. 3. Regarding CPITN, $CPITN_1$ was 74.9 percent, and $CPITN_2$ was 56.2 percent. $CPITN_3$ was 8.0 percent. 4. The rate of full denture wearing of the elderly people was 26.1 percent in the upper part and 17.8 percent in the lower part. And their rate of full denture need was 7.4 percent in the upper part and 4.3 percent in the lower part. 5. Establishing an planned abject for Oral health, Seongnam City has reviewed the necessity of development on enhancement of Oral health.

Application of 3D printer in dental clinic (치과 진료실에서 3D 프린트의 활용)

  • 3D printing is a process of producing 3d object from a digital file in STL format by joining, bonding, sintering or polymerizing small volume elements by layer. The various type of 3d printing is classified according to the additive manufacturing strategies. Among the types of 3D printer, SLA(StereoLithography Apparatus) and DLP(Digital Light Processing) 3D printer which use polymerization by light source are widely used in dental office. In the previous study, a full-arch scale 3d printed model is less precise than a conventional stone model. However, in scale of quadrant arch, a 3d printed model is significantly precise than a five-axis milled model. Using $3^{rd}$ Party dental CAD program, full denture, provisional crowns and diagnostic wax-up model are fabricated by 3d printer in dental office. In Orthodontics, based on virtual setup model, indirect bracket bonding tray can be generated by 3d printer. And thermoforming clear aligner can be fabricated on the 3d printed model. 3D printed individual drilling guide enable the clinician to place the dental implant on the proper position. The development of layer additive technology enhance the quality of 3d printing object and shorten the operating time of 3D printing. In the near future, traditional dental laboratory process such as casting, denture curing will be replaced by digital 3D printing.

  • Compared with traditional full dentures, mandibular implant overdentures have the advantage in that good support and retention can be obtained even with two implants. When manufacturing a mandibular implant overdenture using two implants, it is important to place the implant in the correct position. The long-term prognosis of overdenture is good when two implants are placed in the direction vertical to the occlusal plane and parallel to each other at the canine position. However, it is difficult to place two implants in the correct position in edentulous patients, and if you use surgical guides in these cases, you can get help in placing the implants in a prosthetically advantageous position. This case, a 57-year-old male patient, came to our hospital with all upper and lower teeth removed due to periodontal disease. Therefore, the maxilla was restored with a traditional full denture and the mandible was restored with an implant overdenture using two implants, which resulted in satisfactory functional and esthetic results.

  • In a edentulous patient, various methods can be employed for prosthetic treatment using implants, such as implant-supported fixed prostheses, overdentures, hybrid prostheses, and implant assisted removable partial denture. In this case, in a patient with moderate to severe chronic periodontitis requiring full arch extractions, implants were strategically placed using computer-guided surgery. In the maxilla, due to inadequate bone quality and quantity leading to insufficient initial stability, delayed loading was implemented, and interim prosthesis was used during the osseointegration period. In the mandible, stable initial stability was achieved, allowing for immediate loading to reduce patient discomfort. Primary stability is considered the most crucial factor for obtaining immediate loading, so a thorough clinical and radiological evaluation of the remaining alveolar bone quantity and quality must be conducted before surgery.
