B-mode ultrasound images of the carotid artery wall: correlation of ultrasound with histological measurements

  • B-mode ultrasound is being used to assess carotid atherosclerosis in epidemiological studies and clinical trials. Recently the interpretation of measurements made from ultrasound images has been questioned. This study examines the anatomical correlates of B-mode ultrasound of carotid arteries in vitro and in situ in cadavers. Twenty-seven segments of human carotid artery were collected at autopsy. pressure perfusion fixed in buffered 2.5% gluteraldehyde and 4% paraformaldehyde and imaged using an ATL UM-8 (10 MHz single crystal mechanical probe). Each artery was then frozen, sectioned and stained with van Gieson or elastin van Gieson. The thickness of the intima. media and adventitia were measured 'to an accuracy of 0.01 mm from histological sections using a calibrated eye graticule on a light microscope. Shrinkage artifact induced by histological preparation was determined to be 7.8%. Digitised ultra sound images of the artery wall were analysed off-line. The distance from the leading edge of the first interface ($LE_{1}$) to the leading edge of the second interface ($LE_2$) was measured using a dedicated programme. $LE_{1}$-$LE_{2}$ measurements were correlated against histological measurements corrected for shrinkage. Mean values for the far wall were: ultra sound $LE_{1}$-$LE_{2}$ (0.97 mm, S.D. 0.26), total wall thickness (1.05 mm, S.D. 0.37), adventitia (0.35 mm, S.D. 0.16), media (0.61 mm, S.D. 0.18). intima (0.09 mm, S.D. 0.13). Ultrasound measurements corresponded best with total wall thickness, rather than elastin or the intima-media complex. Excision of part of the intima plus media or removal of the adventitia resulted in a corresponding decrease in the $LE_{1}$-$LE_{2}$ distance of the B-mode image. Furthermore. increased wall thickness due to intimal atherosclerotic thickening correlated well with $LE_{1}$-$LE_{2}$ distance of the B-mode images. B-mode images obtained from the carotid arteries in situ in four cadavers also corresponded best with total wall thickness measured from histological sections and not with the thickness of the intima plus media. In conclusion, the $LE_{1}$-$LE_{2}$ distance measured on B-mode images of the carotid artery best represents total wall thickness of intima plus media plus adventitia and not intima plus media alone.

A Method for Observation of Benign, Premalignant and Malignant Changes in Clinical Skin Tissue Samples via FT -IR Microspectroscopy

  • Sunlight causes various types of adverse skin changes on the sun-exposed areas of the skin, in which the most hazardous one is the induction of malignant skin tumours. FT -IR spectra were obtained from specimens excised from normal skin, BCCs, SCCs, MMs, nevi, lesions of solar keratosis and Bowen's disease. Tissue samples from freshly frozen specimens were cut into 2 sections in strictly sequential order to be stained with H & E for histopathological analysis, and then to be air-dried on CaF$_2$ slide glasses for further spectral data acquisition from defined area of interest. Intra- and inter-sample variations were estimated within grouped lesion categories according to each skin component. Mean spectra for each type of tissue pathology in the 800-1800 $cm^{-1}$ / region was interpreted using the classical group frequency approach that showed the most visible differences in spectra of benign, premalignant and malignant changes directly related to protein conformation and nucleic acid bases. The relative intensity of the nucleic acid peak was increased with progression to malignancy. In addition, PCA was able to evaluate and maximise the differences in the spectra by reducing the number of variables characterizing each patient and pathology category. This type of approach to non-destructively estimate the complexity of IR-spectra of inhomogeneous samples such as skin demonstrates the advantage of FT -IR microspectroscopy to be able to observe diseased states (benign, premalignant, malignant) and distinguish them from normal against a huge background of inter- and intra-subject variability.

The Effects of Sophorae radix Extracts on CGRP Immunoreactive Neurons of Spinal Cord and Ganglia in Experimental Arthritic Rat Model (실험적 관절염 흰쥐 모델에서 고삼추출액이 척수와 척수신경절의 CGRP 면역반응 신경원에 미치는 영향)

  • To investigate the antiinflammatory and analgesic effects of Sophorae radix extracts administered to the arthritic rat model, immunohistochemical stains for CGRP in the L4, L5 and L6 spinal cord and ganglia were done, and paw swelling thickness were measured. Complete Freund,s Adjuvant(CFA) were injected to subcutaneous tissue of left foot paw of rats to induce arthritis. Sophorae radix extracts was administered immediately after CFA injection for 10 days. The spinal cord and ganglia were frozen sectioned(30㎛). These sections were stained by CGRP immunohistochemical staining method, and observed with light microscope. The results were as follows : 1. The change of paw swelling thickness of experimental group decreased from 4 day to 10day after CFA injection compared to control group. 2. The change of differential leukocytes counts of experimental group increased the ratio of lymphocytes. and decreased the ratio of neutrophils compared to control group. 3. The change of CGRP immunoreactive nerve fiber of dorsal horn of experimental group was dense stained compared to control group. 4. The number of CGRP immunoreactive neurons of L4 and L5 spinal cord of experimental group was less than in those control group. These results suggested that Sophorae radix extracts reduces the number of CGRP immunoreactive neurons and nerve fibers of spinal cord and ganglia, and decrease paw swelling thickness in arthritic rat model, which may be closely related to analgesic and antiinflammatory effects of Sophorae radix.

Expression of neurotransmitter(CRF, CRF-R and CRF-BP) related to stress in stomach and zusanli in rats (백서의 위와 족삼리에서 스트레스 관련(CRF, CRF-R, CRF-BP) 신경전달물질의 발현에 대한 연구)

  • Objective: The expression of CRF(corticotropin releasing factor), CRF-R(receptor) and CRF-BP(binding protein) in CNS neurons projecting to the stomach and ST36 using the pseudorabies virus in the rat was investigated. Methods: After survival times of 5 days following injection of PRV-Ba-Gal, The thirty rats were perfused, and their brain were frozen sectioned($30{\mu}m$). These sections were stained by PRV-Ba-Gal histochemical staining method and(or) CRF, CRF-R and CRF-BP immunohistochemical method. The common expressed areas of the brain projecting to the stomach and zusanli(ST36) following injection of PRV-Ba-Gal were observed with light microscope. Results: 1) The dense accumulation of CRF-immunoreactive terminals is seen in the area postrema, n. tractus solitarius, external zone of the median eminence, with some immunoreactive CRF also present in the internal zone. 2) Aggregates of CRF-R immunoreactive perikarya are found in area postrema, n. tractus solitarius, lateral reticular n., gigantocellular reticular n., locus coeruleues, paraventricular n. of hypothalamus, median eminence, preoptic n., arcuate n. and hind limb area of cerebral cortex. 3) Aggregates of CRF-BP immunoreactive perikarya are found in area postrema, n. tractus solitarius, lateral reticular nucleus, gigantocellular reticular n., locus coeruleues, paraventricular n. of hypothalamus, median eminence and arcuate n.. Conclusions : These results suggest that PRV-Ba-Gal labeled areas projecting to stomach and ST36 may be related to the central autonomic pathways. A part of CNS neurons projecting to the stomach and ST36 were related to expression of CRF, CRF-R and CRF-BP related to the stress in central autonomic center.

Central Localization of the Neurons Projecting to the Kidney, UB23 and GB25 Using the Pseudorabies Virus (Pseudorabies virus를 이용한 신장, 신수 및 경문에서 투사되는 중추신경계내 표지영역에 관한 연구)

  • Objective : To investigate the relation to the organs, shu points and mu points. The labeled common locations of the spinal cord and brain were observed following injection of pseudorabies virus(PRV) into the the kidney, UB23 and GB25. Methods : After survival times of 96 hours following injection of PRV, The fifteen rats were perfused, and their spinal cord and brain were frozen sectioned($30{\mu}m$). These sections were stained by PRV immunohistochemical staining method, and observed with light microscope. Results : In spinal cord, PRV labeled neurons projecting to the kidney, BL23 and GB25 were founded in cervical, thoracic, lumbar and sacral spinal segments. Dense labeled areas of cervical segments were overlap in lateral cervical n. and lamina III-V area. Thoracic segments were overlap in lateral spinal n., intermediolateral n. and lamina V-X areas. Lumbar segments were overlap in lamina I-V areas. Sacral segments were overlap in lamina IV, V and X areas. In brain, PRV labeled areas projecting to the kidney, UB23 and GB25 were overlap in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular n./rostroventrolaterai n., raphe obscurus n,, raphe pallidus n., raphe magnus n., gigantocellular reticular n., locus coeruleus, subcoeruleus n., A5 cell group and paraventricular hypothalamic n.. Conclusions : This results suggest that PRV labeled overlap areas of projecting to the kidney may be correlated to shu and mu points related to the kidney. These morphological results provide that organs-shu(transport) and mu(alarm) points interrelationship may be related to the central autonomic pathways.

Significance of Expression of Human METCAM/MUC18 in Nasopharyngeal Carcinomas and Metastatic Lesions

  • Human METCAM/MUC18, a cell adhesion molecule (CAM) in the immunoglobulin-like gene super family, plays a dual role in the progression of several epithelium cancers; however, its role in the nasopharyngeal carcinoma (NPC) remains unclear. To initiate the study we determined human METCAM/MUC18 expression in tissue samples of normal nasopharynx (NP), NPCs, and metastatic lesions, and in two established NPC cell lines. Immunoblotting analysis was used for the determination in lysates of frozen tissues, and immunohistochemistry (IHC) for expression in formalin-fixed, paraffin-embedded tissue sections of 7 normal nasopharynx specimens, 94 NPC tissue specimens, and 3 metastatic lesions. Human METCAM/MUC18 was expressed in 100% of the normal NP, not expressed in 73% of NPC specimens (or expressed at very low levels in only about 27% of NPC specimens), and expressed again in all of the metastatic lesions. The level of human METCAM/MUC18 expression in NPC tissues was about one fifth of that in the normal NP and metastatic lesions. The low level of human METCAM/MUC18 expression in NPC specimens was confirmed by a weak signal of RT-PCR amplification of the mRNA. Low expression levels of human METCAM/MUC18 in NPC tissues were also reflected in the seven established NPC cell lines. These findings provided the first evidence that diminished expression of human METCAM/MUC18 is an indicator for the emergence of NPC, but increased expression then occurs with metastatic progression, suggesting that huMETCAM/MUC18, perhaps similar to TGF-${\beta}$, may be a tumor suppressor, but a metastasis promoter for NPC.

Distribution of growth hormone-releasing factor- and somatostatin-immunoreactive neurons in the hypothalamus of the Korean squirrel(sciurus vulgalis coreae) (청서 시상하부의 growth hormone-releasing factor 및 somatostatin 면역반응신경세포의 분포)

  • This study was attempted to investigate the topographical distribution, shape and immunoreactivity of growth hormone-releasing factor(GRF)- and somatostatin(SOM)-immunoreactive neurons in the hypothalamus of the Korean squirrels(Sciurus vulgalis coreae). For the light microscopical examination of immunohistochemistry, the brains were fixed with 4% paraformaldehyde solution by means of intracardiac perfusion. And the frozen sections($40{\mu}m$ thick) were stained immunohistochemically by ABC method. Distribution of GRF immunoreactive neurons($12-17{\mu}m$) was highest in the paraventricular nucleus, moderate in the periventricular and supraoptic nuclei, and low in the arcuate nucleus and lateral hypothalamic area. Their immunoreactive fibers were found very high in the median eminence, moderately in the supraoptic, paraventricular and periventricular nuclei, and low in the arcuate nucleus and lateral hypothalamic area. SOM immunoreactive perikarya($14-18{\mu}m$) were found moderately in the periventricular nucleus near the subependymal layer of the third ventricle, and low in the arcuate and suprachiasmatic nuclei. SOM immunoreactive fibers were found high in the median eminence, and moderately or low in the arcuate and periventricular nuclei.

  • The purpose of this morphological study was to investigate the relationship to facial nerve and LI4 related to the large intestine meridian. The common locations of the spinal cord and brain projecting to the LI4 and facial nerve were observed fallowing injection of transsynaptic neurotropic virus, pseudorabis virus(PRV), into the LI4 and facial nerve of the rat. After survival times of 96 hours following injection of PRV, the rats were perfused, and their spinal cord and brain were frozen sectioned(30${\mu}m$). These sections were stained by PRV immunohistochemical staining method, and observed with light microscope The results were as follows: 1. The PRV labeled spinal cord segments projecting to the LI4 and facial nerve were founded in cervical, thoracic, lumbar and sacral segments. Dense labeled areas of each spinal cord segment were founded in lamina IV, V, X, lateral spinal nucleus, intermediolateral nucleus and dorsal nucleus. 2. The PRV labeled medulla oblongata projecting to the LI4 and facial nerve were founded in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular nucleus, rostroventrolateral reticular nucleus, medullary reticular nucleus, nucleus tractus solitarius, raphe obscurus nucleus, raphe pallidus nucleus, raphe magnus nucleus, gigantocellular nucleus, lateral paragigantocellular nucleus, and spinal trigeminal nucleus.

NECROSIS OF A PAROTID GLAND PLEOMORPHIC ADENOMA : A CASE REPORT (이하선 다형성 선종의 괴사)

  • Pleomorphic adenoma is the most common neoplasm of the parotid gland, generally presenting as a slowly growing, firm, well-circumscribed, painless nodule. It is often difficult to separate into benign and malignant categories because its bland histomorphologic and cytologic features. This tumor can be rarely associated with cystic change, hemorrhagic necrosis, or spontaneous infarction. Necrosis of lesional tissue may be associated with malignant transformation, particularly in a pleomorphic adenoma. We report a case of 50-year-old woman presented with a enlarging right parotid mass. Computed tomographic scan demonstrated a right superficial lobe mass with ill-defined border. The preoperative fine needle aspiration yielded necrotic debris and atypical squamous elements that were thought to be compatible with high degree of mucoepidermoid carcinoma. A total parotidectomy with intraoperative frozen section revealed extensive necrosis and diagnosed as malignant tumor. This tumor was finally diagnosed as a pleomorphic adenoma with necrosis on permanent sections. Caution should be exercised in evaluation of the parotid neoplasms with central necrosis to avoid misdiagnosis of such lesions as malignancy.

Central Localization of Neurons Projecting to the Uterus and Sanyinjiao(Sp6) (자궁과 삼음교에서 투사하는 충추신경계내 신경원의 표지영역에 관한 연구)

  • This experimental studies was to investigate location of labeled neurons in CNS following injection of pseudorabies virus(PRV), Bartha strain, into the uterus and Sanyinjiao(Sp6) of rats. After survival times of 4-5 days following the injection of PRV, the rats were perfused, and their brain and spinal cord were frozen sectioned($30\mu\textrm{m}$). These sections were stained by PRV immunohistochemical staining methods, and observed with light microscope. The results were as follows: 1. In the spinal cord, overlap areas of PRV labeled neurons projecting to uterus and Sp6 were observed in lamina VII, IX and X areas of cervical segments. In thoracic segments, overlap areas were observed in lamina IV, VII, X and intermediolateral n.. In lumbar segments, overlap area of PRV labeled neurons were observed in lamina I, V-VII, IX, X and intermediolateral n.. In sacral segments, overlap areas of PRY labeled neurons were observed in lamina N, V, VII, X and sacral parasympathetic n.. 2. In the brain, overlap areas of PR V labeled neurons projecting to the uterus and Sp6 were observed in lateral paragigantocellular n., rostroventrolateral reticular n., raphe obscurus n., raphe pallidus n., raphe magnus n., locus coeruleus n., Barrington's n., A5 cell group, central gray n., paraventricular hypothalamic n. and arcuate n. This results suggest that overlap areas of PRV labeled neurons of the spinal cord projecting to the uterus and Sp6 might be the first-order neurons related to the viscera-somatic sensory and sympathetic preganglionic neurons. PRV labeled neurons of the brain may be the second and third-order neurons response to the movement of smooth muscle of uterus. These PRV labeled neurons may be central autonomic center related to the integration and modulation of reflex control linked to the sensory and motor system monitoring the internal environment. These overlap areas of spinal cord and brainmay be related to autonomic centers related to regulation of uterus.
