한국농촌인구(韓國農村人口)의 종신적(終身的) 노동능력상실(勞動能力喪失) 빈도(頻度)와 원인(原因) (Frequency and Causes of Life-long Labour Force Loss in Rural Population of Korea)

  • 한국(韓國)의 종신적(終身的) 노동능력상실자(勞動能力喪失者)의 빈도(頻度)와 원인(原因)을 조사(調査) 파악(把握)함으로써 그 관리방안(管理方案)의 모색(模索)을 위한 기초자료(基礎資料)를 얻고자 함이 본(本) 연구(硏究)의 목적(目的)이다. 조사대상(調査對象)은 1974년(年) 전국대학생(全國大學生) 하계연합봉사국(夏季聯合奉仕國)의 활동지역(活動地域)이었던 81개군내(個郡內)의 대상(對象) 지역사회(地域社會)에서 제비를 뽑아 선택(選擇)된 총(總) 4,174가구(家口)의 전가일원(全家日員)인 총(總) 27,172명(名)이었다. 조사원(調査員)으로서의 대학생(大學生) 1명당(名當) 1가구(家口)씩을 방문(訪問)하여 가구원중(家口員中)의 종신적(終身的) 노동능력상실자(勞動能力喪失者)의 유무(有無), 있는 경우 그 원인(原因) 및 시작연령(始作年齡) 등을 조사(調査)하였으며 그 성적(成績)의 개요(槪要)는 다음과 같다. 종신적(終身的) 노동능력상실자(勞動能力喪失者)(가구당(家口當) $1{\sim}4$명(名))가 있는 가구(家口)는 전체적(全體的)으로 8.9%로 나타났다. 종신적(終身的) 노동능력상실자(勞動能力喪失者)의 전체적(全體的)인 조유병률(粗有病率)은 1,000당(當) 15.1로 나타났으며, 연령표준화율(年齡標準化率)로 본 성별(性別) 유병률(有病率)(1,000당(當))은 남(男) 16.3, 여(女) 13.4로 나타났다. 그리고 그 율(率)은 연령(年齡)의 증가(增加)에 따라 점차 증가(增加)되었다. 종신적(終身的) 노동능력상실(勞動能力喪失)의 원인별(原因別) 유병률(有病率)(1,000당(當))을 보면 노쇠(老衰) 10.2, 사지장애(四肢障碍) 2.4, 내장기계(內臟器系)의 만성질환(慢性疾患) 1.2, 근육골격계(筋肉骨格系)의 기타(其他) 이상(異常) 0.5, 양면맹(兩眠盲) 0.4, 척추장애(脊椎障碍) 0.2, 정신병(精神病) 0.2 및 전간(癲癎) 0.1의 순(順)으로 나타났다. 그중(中) 사지장애(四肢障碍)의 원인(原因)으로는 뇌졸중증(腦卒中症), 폴리오, 사고(事故), 관절염(關節炎) 및 전상(戰傷) 등으로 나타났다. 원인별(原因別) 및 성별(性別)로 종신적(終身的) 노동능력상실(勞動能力喪失)의 발생연령별(發生年齡別) 빈도비(頻度比)도 계산(計算) 관찰(觀察)하였다.

  • The Highway Capacity Manual specifies procedures for evaluating intersection performance in terms of delay per vehicle. What is lacking in the current methodology is a comparable quantitative procedure for ass~ssing the safety-based level of service provided to motorists. The objective of the research described herein was to develop a computational procedure for evaluating the safety-based level of service of signalized intersections based on the relative hazard of alternative intersection designs and signal timing plans. Conflict opportunity models were developed for those crossing, diverging, and stopping maneuvers which are associated with left-turn and rear-end accidents. Safety¬based level-of-service criteria were then developed based on the distribution of conflict opportunities computed from the developed models. A case study evaluation of the level of service analysis methodology revealed that the developed safety-based criteria were not as sensitive to changes in prevailing traffic, roadway, and signal timing conditions as the traditional delay-based measure. However, the methodology did permit a quantitative assessment of the trade-off between delay reduction and safety improvement. The Highway Capacity Manual (HCM) specifies procedures for evaluating intersection performance in terms of a wide variety of prevailing conditions such as traffic composition, intersection geometry, traffic volumes, and signal timing (1). At the present time, however, performance is only measured in terms of delay per vehicle. This is a parameter which is widely accepted as a meaningful and useful indicator of the efficiency with which an intersection is serving traffic needs. What is lacking in the current methodology is a comparable quantitative procedure for assessing the safety-based level of service provided to motorists. For example, it is well¬known that the change from permissive to protected left-turn phasing can reduce left-turn accident frequency. However, the HCM only permits a quantitative assessment of the impact of this alternative phasing arrangement on vehicle delay. It is left to the engineer or planner to subjectively judge the level of safety benefits, and to evaluate the trade-off between the efficiency and safety consequences of the alternative phasing plans. Numerous examples of other geometric design and signal timing improvements could also be given. At present, the principal methods available to the practitioner for evaluating the relative safety at signalized intersections are: a) the application of engineering judgement, b) accident analyses, and c) traffic conflicts analysis. Reliance on engineering judgement has obvious limitations, especially when placed in the context of the elaborate HCM procedures for calculating delay. Accident analyses generally require some type of before-after comparison, either for the case study intersection or for a large set of similar intersections. In e.ither situation, there are problems associated with compensating for regression-to-the-mean phenomena (2), as well as obtaining an adequate sample size. Research has also pointed to potential bias caused by the way in which exposure to accidents is measured (3, 4). Because of the problems associated with traditional accident analyses, some have promoted the use of tqe traffic conflicts technique (5). However, this procedure also has shortcomings in that it.requires extensive field data collection and trained observers to identify the different types of conflicts occurring in the field. The objective of the research described herein was to develop a computational procedure for evaluating the safety-based level of service of signalized intersections that would be compatible and consistent with that presently found in the HCM for evaluating efficiency-based level of service as measured by delay per vehicle (6). The intent was not to develop a new set of accident prediction models, but to design a methodology to quantitatively predict the relative hazard of alternative intersection designs and signal timing plans.

선박 충돌사고의 원인요소 간 상관관계 및 충돌시간에 따른 원인요소 분석 (Correlation Analysis of Cause factor through Ship Collision Accident, and Cause factor Analysis through Collision Time)

  • 선박의 대형화, 고속화 및 선종의 다양화는 운송수단 중 해양 운송수단의 비중을 크게 증가시켰다. 선박사고 유형 중 기관손상 다음으로 충돌이 사고발생 빈도가 높았으며, 인적 요인에 의한 사고비율이 높은 것으로 보고되고 있다. 선박 충돌사고는 한 가지 원인요소로 발생하는 경우보다 복합적인 원인요소로 발생하게 되며 여러 개의 원인요소를 재결서를 통하여 원인요소를 조사할 필요가 있다. 본 연구에서는 해양안전심판원에서 제공하는 재결서 중 선박 충돌사고에 대해서 인적 요인을 바탕으로 선박 충돌사고의 원인요소를 도출하였으며, 상관관계분석을 통하여 원인요소들을 규명하였다. 또한, 선박충돌을 인지한 시점에서부터 충돌이 일어난 시점까지 걸린 시간을 바탕으로 충돌을 피할 수 없는 충돌시점에서의 발생되는 주요 원인요소와 20분 내에 발생하는 선박 충돌사고의 원인요소를 분석하여 선박 충돌사고를 예방하고자 하였다. 상관분석은 상용소프트웨어인 Statistical Package for Social Sciences(SPSS Ver21.0)을 사용하였다. 시간 분석은 재결서를 바탕으로 상대방 선박을 인지한 시점에서부터 충돌이 일어난 시점까지 걸린 시간을 분석하였다. 선박 충돌사고의 원인요소는 2가지 이상에 의해 발생한 사고가 많았으며, 상대선박 감시소홀은 항해업무 외 다른 작업과 높은 상관관계가 있었다. 충돌을 피할 시간 여력이 없는 경우(0분)이 36.1 %이며, 경계 또는 상대 선박 감시소홀과 졸음항해 또는 음주가 원인요소이었다.

진주시 도시시설물별 화재발생 위험도 평가 (The Risk Assessment of the Fire Occurrence According to Urban Facilities in Jinju-si)

  • 우리나라의 도시화율은 점점 높아지고 있으며 이에 따라 도시인구 증가와 더불어 다양한 도시시설물들이 급속도로 도시에 집중되고 있는 실정이다. 이에 따라 다양한 재해로 인한 피해가 발생되고 있고 사회재난 중 화재는 교통사고와 더불어 도시에서 가장 많은 피해를 입히고 있다. 2015년 우리나라의 화재발생은 44,432건이 발생하여 253명의 사망자와 4,300억원의 재산피해가 발생하여 다양한 피해 저감 노력에도 불구하고 감소되는 추세를 보이고 있지 않다. 이에 본 연구에서는 국가화재정보시스템과 진주소방서를 통하여 2007년부터 2014년까지 발생한 진주시 화재자료를 수집하였으며 행정자치부의 시설물 현황 DB를 통해 진주시의 화재와 시설물의 군집성을 분석하고 화재위험도를 산출하였다. 그 결과 미국소방기술사회(SEPE:Society of Fire Protection Engineers) 기준에 따른 화재발생빈도에 대한 위험 등급을 4단계로 구분한 경우 가장 높은 A등급으로 업무시설, 위락시설, 자동차시설이 분류되었으며, 그 다음으로 U등급은 단독주택, 공동주택, 교육시설, 판매시설, 숙박시설, 집합시설, 의료시설, 산업시설, 생활서비스시설, EU등급은 기타주택이고, 마지막으로 가장 낮은 BEU등급은 위험물제조시설로 나타났다. 또한 인명피해를 기준으로 한 경우 가장 위험도가 높은 시설물은 위험물제조시설이었고 재산피해를 기준으로 한 경우는 집합시설과 산업시설이 가장 위험도가 높게 나타났다. 이상과 같이 도시에서 발생된 화재를 시설물별로 구분하여 발생빈도, 인명피해, 재산피해에 대해 위험등급을 산정하여 제시함으로서 도시공간에 분포한 시설물에 대한 화재저감대책을 수립하는데 효과적으로 활용할 수 있을 것으로 판단된다.

한 대학병원 응급실에 방문한 환자의 방사선 피폭에 관한 연구 (A Study on Exposure to radiation of the patient who visited an emergency room at a University Hospital)

  • 한 대학병원 응급실에 방문한 응급환자들이 방사선에 얼마나 피폭되는지를 알아보고자, 2006년 3월 16일 부터 31일까지 15일 동안 방문환자 200명을 임의 추출하여 방사선 피폭선량을 측정한 결과는 다음과 같다. 1. 연구대상자의 분포는 타병원전원환자 50명(25.0%), 교통사고환자 24명(8.3%), 기타사고환자 50명(25.0%), 일반환자 76명(38.0%)이었다. 2. 환자의 방사선 촬영횟수를 보면 환자 1인당 타병원, 전원환자 6.4회, 교통사고환자 14.5회, 사고환자 2.6회, 일반환자 2.4회로 교통사고환자들이 타환자군에 비해 방사선촬영 건수가 3~4배 많았다. 3. 환자의 방사선촬영종류별 피폭선량을 보면 일반촬영 28.9mGy, CT촬영 84.2 mGy, 특수촬영 1.02mGy로 CT촬영피폭이 일반촬영 비해 10배 정도 많았다. 4. 환자의 평균 방사선 피폭선량을 보면 타병원 전원환자는 24.6mGy, 교통사고환자는 55.2 mGy, 사고환자는 17.1mGy 일반환자는 17.0mGy로 타병원 전원환자와 교통사고환자가 상대적으로 피폭이 많았다. 5. 방사선촬영 부위별로 보면 일반촬영에서는 두부피폭 1.7mGy로 사고환자에서 피폭이 많았고, 흉부 2.0mGy, 복부 1.6mGy는 일반환자에게 많았으며, 척추 3.4mGy, 골반부 1.8mGy, 상지부 0.5mGy, 하지부 0.6mGy는 교통사고 환자에게 피폭이 많았으며, 통계적으로 유의한 차이를 보였다(P<0.001). 6. CT촬영에서는 타병원 및 전원환자가 두부 10.9mGy으로 많았고, 흉부와 복부는 각각 2.9mGy, 3.6mGy로 일반환자에게 많았고, 척추, 골반부 1,9mGy 2.7mGy는 교통사고환자에게 많았다. 특히 복부피폭은 통계적으로 유의한 차이를 보였다.(P<0.05) 결론적으로 한 대학병원 방문한 응급환자 특히 교통사고환자의 방사선 검사시 일반 외래 환자들의 촬영보다는 과다한 검사와 피폭이 노출선량한도가 2배 이상 증가하는 실정이다. 따라서 병원 관리자 및 방사선취급자는 환자 방사선 촬영시 노출을 최소한으로 제한하고, 방사선피폭감소를 위해 병원의 종사자인 방사선사의 기술적인 연구와 및 보조연구자 및 의료인 등 모두가 예방할 수 있는 대책이 필요할 것으로 생각한다.

주요일간지(主要日刊紙)의 보건의료관계사설(保健醫療關係社說) 내용분석(內容分析) (An Analysis on Contents of Health-Concerned Editorials in Korean Neswspapers)

  • The editorials of the leading newspapers may reflect as well as help formulate the public opinions to a significant degree. Bearing this in mind, this study was carried out to provide information useful in formulating such public health policies that could practically meet the social interests and demands in health appearing in the editorials of the newspapers. This analysis covered the editorials of 5 leading newspapers such as Donga Ilbo, Chungang Ilbo, Hankook Ilto, Chosun Ilbo and Seoul Shinmoon for about 10 years from Jan. 1st, 1970 through Oct. 31th, 1979. The major findings are as follows: 1. The total number of health-concerned editorials in the five daily papers for the period was 1,768 or occupied 6.4% of the total editorials of the same sources. The increasing trend of frequnecy of the health-concerned editorials since 1976 indicates the increment of social interests and demands in health. Analysing the contents, environmental pollution received the greatest attention in those editorials, which was followed by medical affairs, environmental sanitation, and disease control in order in terms of frequency of appearance. However, there was a tendency that the interests in the environmental pollution, medical affairs and social welfare tended to increase year by year, whereas those in environmental hygiene and disease control comparatively decreased. 2. Motives of dealing with the health-concerned editorials were provided by announcement of the governmental policies and implementation for 25.6% and by out-breaks of the relevant events for 23.9%. This tendency coincides with the general characteristics of the editorials that reflect the timely issues. Closely analysing, however, the fact that the motives engendered by the out-breaks of the relevant events or by the season concerned comparatively tended to decrease as years pass by, indicates that the editorials tend more to seek the future-oriented demands in health rather than the current issue-oriented. 3. The editorials appeared to be more concerned with the governmental policies. 95.7% of all the editorials analysed were addressed to the government. This signifies the role of government in the field of public health and medical affairs. Their attitudes toward health-related policies of the government were much more negative than the other editorials that were addressed to the government in other fields. This suggests that the governmental interests in health were neither sufficient nor fair. 4. What the editorials most stressed were; (1) increment of governmental interests in health, (2) improvement of the governmental health administration, (3) enriching the basic statistics, and (4) development of various technologies pertinent to health affairs and disease control, and so forth. However, must of their suggestions were not concrete but rather abstract and conceptual. 5. The editorials also expressed strong interests in research area. The areas of the needed research most suggested by items are; (1) development of effective health care delivery system, (2) establishment of more practical system of calculating medical cost, (3) implementation of effective policies to control degenerative diseases, (4) division of medical care services and pharmaceutical services systems, (5) effective ways to prevent Co poisoning accidents, (6) changing status of environmental pollution and its effects upon health, and (7) status of occupational diseases, and so forth. 6. There were some editorials -not small in quantity -that have risk to mislead the public opinions as well as the health policies due to lack of professional knowledge of the writers. It is desirable to establish some kind of mechanism that screens the erroneous contents of the editorials to help prevent misleading opinions.

유류오염토양이 식물식생에 미치는 영향에 관한 연구 (A Study on Effects of Oil Contaminated Soil on the Growth of Plant)

  • Oil contamination soil has been one of the most environmental social issues for decades in the inside and outside of country. The law of soil environmental preservation was carried out in the 1990s and the government controlled not only soil environment management and the remediation of contaminated soil but also promoted the development of remedial technology and cleanup business of contaminated soil by national policy. In addition to agriculture areas, the main oil contaminated sites are a gas station, oil reservoir, petro-chemical complex, site of railway carriage base and military camp. The contamination-frequency of agriculture area and effect sites are low but it has significantly important area on account of producing food for human beings. Therefore, we should be concerned about oil contamination damage of agriculture area. The oil contamination damage of agriculture area influenced drop of birth and breeding since the oil directly adheres to seeds and farm products even diffusion of contaminated soil to cultivation area. The studies of the crops and the food vegetation has not enough detailed data caused by the incident of oil contamination. This study investigated the effect of oil in germination and growth of selected plant seeds. In this study, we try to verify whether the oil contamination by accidents on farmland influenced the damage of farm produce and the mutual relation both oil contaminated soil or the vegetation of crops. The impact of oil on plant development was followed by phytotoxicity assessments. The plants exhibited visual symptoms of stress, growth reduction and perturbations in developmental parameters. The increase of the degree of pollution induced more marked effects in plants, likely because of the physical effects of oil. The relationships between the phytotoxicity contents of plants and growth reduction suggest a chemical toxicity of fuel oil. In addition, while cleaned up the contaminated soil under the standard of contaminated soil we examined it was suitable for region standard and it may have practical possibility for fill material of construction of afforestation and molding soil of landfill.

KORA를 활용한 정수장 염소 취급시설의 운영조건에 따른 피해범위 변화 평가 (Evaluation of Damage Range Variation Based on Operation System of Chlorine Facility in Water Purification Plant using KORA)

  • 정수장에서 염소 누출 사고 시 피해를 최소화하는 방안을 연구하였다. 장외영향평가의 위험도는 영향범위내 주민수와 사고 발생 빈도의 조합으로, 취급시설 운전온도, 긴급차단밸브 설치 위치 및 시설의 밀폐정도에 따른 피해범위 변화를 분석하여 피해범위 내 주민 수를 줄일 수 있는 최적의 조건을 제시하였다. 피해범위 산정은 KORA(Korea Off-site Risk Assessment Supporting Tool) 프로그램을 이용하였고, 운전온도가 낮아질수록, 용기의 브렌치 배관마다 긴급차단밸브가 설치될수록, 시설의 밀폐수준이 높을수록 피해범위가 감소하였다. 최악의 사고시나리오에서 감소율은 각각 17.6%, 71%, 34.5% 이며, 대안의 사고시나리오에는 각각 31.6%, 69.0%, 34.8% 이다.

한.양방 협진체계 개발에 관한 연구 (A Study on Cooperative Medical Treatment System between Oriental and Western Medical Practitioners)

  • A study on cooperative medical treatment system between oriental and western medical practitioners was conducted from March through August 1997 in order to determine a productive model of cooperation of two medical parts for treatment of patients. Questionnaires were distributed to 195 medical doctors(M.D.) and 195 doctors of oriental medicine(O.M.D.) working in clinics and hospitals in six major cities. Statistical analysis tools used for this study were frequency, t-test and multiple regression by using SPSS/P.C package. The results are summarized as follows: 1. The respondents were composed of male docotors(78%) and female doctors(22%) and 68.2% of M.D. and 97% of O.M.D. were interested in the other medical part. The doctors of both disciplines had some limitation on treatment of patients but they were reluctant to refer their patients to other doctors in different discipline. 2. M.D assumed that oriental medicine was more effective for chronic diseases, and O.M.D. thought that western medicine was more effective for acute diseases. 92.3% of O.M.D.s responded that even though they do not utilize western medical technology for diagnosis, they believed the results of diagnoses by western medical technology. 3. 60.5% of O.M. and 93.3% of O.M.D. said that cooperative medical treatment system could be necessary for patients and it would be effective for control of diseases and 69% of western medical doctors and 99% of oriental medical doctors agreed that oriental medical practice would be more effective for cerebellar vascular accidents than other diseases. 77.4% of western medical doctors and 92.3% of oriental medical doctors responded that the students of two different disciplines have to be taught on the other disciplines. 82.6% of western medical doctors and 83.3% oriental medical doctors agreed that it would be necessary to have collaborative research between scientists in two disciplines. 81.5% of M.D.s and 93.3% of O.M.D.s believed that they had prejudice and distrust on other discipline 4.90% of the doctors were not satisfied with the government medical policies on health insurance, legal matters, and health delivery system. 5. 75.4% of M.D.s and 50.2% of O.M.D.s said that two medical disciplines should be integrated, but they were skeptical on that. 75.3% of M.D.s thought that western hospitals should employ O.M.D.s Finally this study recommended that western medical students study oriental medicine and the vice versa, and M.D.s and O.M.D.s should have seminars and workshops to exchange knowledge, and experiences. It is also recommended that medical laws be revised to allow medical doctors refer the patients whom they can not handle to oriental medical doctors and the vice versa.

외상환자에 있어서 일차조사로서 경추부 전산화 단층 촬영의 예측인자 (Predictive Factors for MDCT as a Primary Survey in Traumatic Cervical Spine Injury)

  • Purpose: Missing cervical pathology after injury may lead to disability and influence long-term survival. Controversies continue to evolve concerning the initial screening methods used to predict cervical spine injury. Through a retrospective chart review, we attempted to analyze and propose factors predictive of cervical trauma. Methods: Of all the patients who had visited the Emergency Department of Korea University, from January 2009 to December 2009, a retrospective review of the clinical records of the 217 patients who had undergone cervical spine computed tomography was done. We investigated whether we could predict the need for cervical spine computed tomography shortly after presentation in trauma patients by comparing the group with fractures and group without fractures and by finding risk factors showing significant differences between the two groups that might be used as guides in decision making. Results: Of the 217 subjects who underwent cervical spine computed tomography scans, 33 were identified with fractures of the cervical spine while 184 were not. The most common mechanisms of trauma, in order, for those with fractures were falls, followed by traffic accidents. We found that the injury severity score, multiple injuries, a high-energy injury mechanism, neurologic deficit, and pain and tenderness of the cervical spine showed statistically significant differences between the two groups. Conclusion: Fractures of the cervical spine that are not observed with simple radiography occur with a relatively high frequency in trauma patients. Consideration should be given to the risk factors for cervical spine fracture, and if pertinent, cervical spine computed tomography should be performed with speed for early diagnosis of cervical spine fractures.
