A Case Report of Child with Brachial Plexus Palsy Due to Birth Injury (분만손상으로 인한 상완신경총마비 환아의 치험 1례)

  • Objectives The purpose of this study is to know the effect of acupuncture on brachial plexus palsy due to birth injury. Methods We decided to treat one week of Vojta therapy and occupational therapy, but other were combined, and then add three weeks of acupuncture treatment as well. Results Right clavicle fracture at birth brachial plexus injury due to decreased locomotion of the right upper extremity, muscle weakness in children aged 7 months to Vojta therapy, occupational therapy and acupuncture, when performed in conjunction mobility increase of the shoulder joint, elbow support improvement, recovery of motor function of the hand grip, etc. to obtain a significant motor function recovery improvement. Conclusion Acupuncture was a good treatment of choice for the better shoulder, joint and hands movements.

Minimally invasive distal biceps tendon repair: a case series

  • Background: Distal biceps tendon repairs are commonly performed using open techniques. A minimally invasive distal biceps tendon repair technique using a speculum and hooded endoscope was developed to improve visualization, reduce soft-tissue dissection, and minimize complications. This paper describes the technique and reports the outcomes of 75 minimally invasive distal biceps tendon repairs. Methods: The operation reports and outcomes of 75 patients who underwent distal biceps tendon repair using this technique between 2011 and 2021 were retrospectively reviewed. Results: Median time to follow-up was 12 months (interquartile range [IQR], 6-56 months). Primary outcomes were function as measured by the Disabilities of Arm, Shoulder and Hand Score (DASH) questionnaire, and rate of complications. Median DASH score was 1.7 of 100 (IQR, 0-6.8). There were 2 of 75 (2.7%) re-ruptures of the distal tendon. There were no cases of vascular injury, proximal radius fracture, or posterior interosseous nerve, median, or ulnar nerve palsy. Conclusions: In this series, minimally invasive distal biceps repair was safe and effective with a low rate of major complications. Recovery of function, as indicated by low DASH scores, was satisfactory, and inconvenience during recovery was minimized. Level of evidence: IV.

A computational analysis of the scarf angle on a composites repair

  • This study examined the relationship between the scarf angle and stress distribution, and estimated the strength recovery via a finite element analysis. The following conclusions were drawn from this study. Resin will fracture due to a tensile load with a high scarf angle, which is similar to the patch repair method. An applied stress can be loaded to a repaired laminate if the scarf angle is $5^{\circ}$. The Von-Mises stress increases with decreasing scarf angle, with the exception of a scarf angle of $30^{\circ}$, where the scarf angle can indicate the rates of shear and normal stresses. Strength recovery can be better if the scarf angle is decreased to a lower angle. However, scarf machining requires more time, a high skill level and considerable expense. Therefore, a scarf angle of $5^{\circ}$ is the most effective for a repair. These results may provide a guide for engineers wishing to formulate a standard for repair. The scarf angle needs to be carefully managed for a more efficient composite repair.

  • This case report documents the rescue, clinical presentation, treatment, and recovery of pneumothorax in a female Korean water deer (Hydropotes inermis argyropus) following a vehicular collision. Severe injuries, including a confirmed rib fracture, prompted an extensive treatment plan. Computed tomography imaging confirmed pneumothorax, particularly in the left lung, necessitating thoracocentesis to alleviate the accumulated air. Post-procedural monitoring demonstrated gradual recovery, with the water deer exhibiting restored appetite after the 2nd day of thoracocentesis. Successful recovery marked by natural pneumothorax resolution allowed for treatment cessation on the 13th day after injury.

  • Fracture of the carpal scaphoid bone is the most common fracture of the carpus. Unfortunately. nonunion are common since the symptoms do not alert patients to seek early medical treatment and the diagnosis is easily missed. Fracture of the scaphoid is a common condition whose management remains controversial. The conservative treatment has many complications, these include inability to work while in plaster, stiffness of the wrist afterwards, muscle wasting, weakness and malunion. So various operative treatments of scaphoid fracture have been developed. Open reduction by use of screws suggested by Maclaughlin first in 1954. In 1984 a new and simple operative technique has been developed to provide rigid internal fixation for all types of fractures of the scaphoid by T.J. Herbert. This involves the use of a double-treaded bone screw which provides good fixation that, after operation, a plaster cast is rarely required and most patients are able to return to work within a few weeks. Authors have experienced 10 cases of scaphoid fracture and accomplished good result in all cases by internal fixation using Herbert screw. The results are as follows : Of these 10 fractures, 2 were fresh fractures and 8 were non unions of scaphoid fracture. In nonunion cases, the time interval between fracture and operation was from 5 months to 5 years. The postoperative immobilization period was average of 4 weeks. Short period of immobilization achieved early functional recovery of the wrist. The bony unions in roentgenogram were seen from 3 months to 9 months after operation. In 2 cases the fracture gap was seen after 9 months. But in these cases the symptoms such as pain and range of motion of wrist were improved.

Supplementation of Safflower Seed Powder and Extracts Enhances Bone Metabolism in Rib-Fractured Rats

  • The current study investigated the effect of Korean safflower (Carthamus tinctorius L.) seed powder and its water and ethanol extracts on bone metabolism during recovery from rib-fracture induced by surgical operation in rats. 10-week-old male Sprague-Dawley rats weighing about 320 g were divided into 9 groups after arrival: 10d control (AIN 76 semi-purified diet), 10d safflower seed powder (10d SS-powder), 10d safflower seed ethanol extract (10d SS-EtOH), 10d safflower seed water extract (10d SS-$H_2O$), 20d control (AIN-76 semi-purified diet), 20d safflower heed powder (20d SS-powder), 20d safflower seed ethanol extract (20d SS-EtOH), 20d safflower seed water extract (20d SS-$H_2O$), and 20d sham-operation (20d sham), The dietary level for all the supplements was 5% based on the raw material weight. The rats were fed the experimental diets for 10 days before the rib fracture operation and for a further 10 or 20 days after the operation. A number 9 rib was fractured surgically and a sham-operation also performed. The rats were then sacrificed on the l0th or 20th day after the operation. The body weight initially decreased after the operation in all the rib-fractured groups, then gradually recovered. The concentrations of plasma osteocalcin were higher in the control group than in all the safflower-supplemented groups 10 and 20 days after the rib-fracture (p < 0.05). The bone-specific ALP (alkaline phosphatase) activity was significantly higher in the SS-EtOH group than in the other groups 20 days after the rib-fracture (p < 0.05). The level of urinary DPD (deoxypridinoline) was significantly higher in the SS-EtOH and SS-$H_2O$ groups than in the other groups 10 days after the rib-fracture. When comparing the PTH (parathyroid hormone) and calcitonin levels, the SS-$H_2O$ group exhibited the highest PTH level among the groups 10 and 20 days after the rib-fracture. Thus, it was concluded that the bone turnover during the fracture-healing period was more rapid in the rats supplemented with safflower seed powder or its fractions than in the control rats. Furthermore, the SS-$H_2O$ fraction was identified as the most effective in stimulating bone remodeling, as bone resorption and bone formation were both significantly increased during fracture healing when compared to the control group.

  • Fiber-reinforced composites not only have a direction of thermal expansion coefficient, but also inevitably suffer thermal stress effects due to the difference between the manufacturing process temperature and the actual use temperature. The damage caused by thermal stress is more prominent in the case of thick composite laminates, which are increasingly applied in the aerospace industry, and have a great influence on the mechanical function and fracture strength of the laminates. In this study, the dimensional reduction and thermal stress recovery theory of composite beam structure having high slenderness ratio is introduced and show the efficiency and accuracy of the thermal stress comparison results between the 3-D finite element model and the dimension reduction beam model. Efficient recovery analysis study will be introduced by reconstructing the thermal stress of the composite beam section applied to the thermal environment by constructing the dimensional reduction modeling and recovery relations.

Operative Treatment of Fractures of the Midshaft Clavicle using Locking Compression Plate (Locking Compression Plate를 이용한 전위성 쇄골 간부 골절의 수술적 치료)

  • Purpose: To assess the effectiveness of the Locking compression plate (LCP) after open reduction for the treatment of the displaced clavicular shaft fracture, the clinical and radiologic outcome of the patients who were managed with the LCP for internal fixation after open reduction has been analyzed. Materials and Methods: We reviewed 26 cases with a displaced clavicular shaft fracture treated by internal fixation using Locking compression plate after open reduction between May 2003 and November 2004. The patients were followed up for at least six months period, and final postoperative outcome was evaluated using clinical results based on Kang's criteria, radiologic signs of fusion. Results: All fractures united by an average of 9.3 weeks without delayed union and showed fast recovery of motion fraction and shoulder function. In addition,24 cases without the fractures of proximal humerus recovered to normal range of shoulder notion within 2.9 weeks. Clinically, according to Kang's criteria, the outcome was good or better in 22 patients. The complications included shoulder joint dysfunction in two cases and keloid formation in one case, and no other complications were observed. Conclusion: The internal fixation using LCP for the treatment of displaced clavicular shaft fracture is a safe, reliable method of treatment, with few complications, and offers rapid recovery of shoulder joint function and bone union.

Internal Fixation of Long Bone Fractures in 6 Calves (송아지에서 발생한 긴뼈골절의 내고정 치료 6례)

  • The simple and easy treatment with cast or splint has been applied to stabilize long bone fractures in farm animals. Applying cast or splint is relatively easier than surgical fixation and may temporarily stabilize fracture site. However, the internal fixation provides strong stabilization and early recovery of limb function. In this report, we describe six cases of limb fractures in calves repaired by internal fixation. Fracture sites are various and affected ages are between 1 day and several months old. In some cases, patients were treated with cast or splint before admitting to Veterinary Medical Center. In these patients, regardless of initial treatment fractures were not immobilized. Consequently we decided to treat these patients with open reduction and internal fixation. The fracture repair with implants(bone plate, screw, cerclage wire, and intramedullary pin) were effective and satisfactory in calves which had been failed by application of cast. Although the cost of orthopedic treatment in farm animal is one of the most important considerations from the practical point of view, internal fixation has many benefits in newborn and young calves. And simplicity of postoperative management is also practically significant benefit expected from infernal fixation In conclusion, we suggest that internal fixation will produce better result than external coaptation in calve.

  • The pediatric condylar fracture occurs very frequently in the mandible, but this injury is occasionally ignored due to difficulty of diagnosis and no cooperation of patient. The adequate initial diagnosis and active treatment must be performed because delayed and improper treatment lead to possible severe complication such as TMJ ankylosis and reta rdation of mandibular development. Most pediatric condylar fracture is mainly performed by the conservative or functional treatment, but it may be required open reduction according to cases. In this study, activator is applied for functional treatment of pediatric condylar fracture and fair recovery of jaw function is acquired.
