한국산 홍화씨분말 보충식이의 급여가 골절된 흰쥐의 골대사지표에 미치는 영향 (The Effects of Korean Safflower(Carthamus tinctorious L.) Seed Powder Supplementation Diet on Bone Metabolism Indices in Rats during the Recovery of Rib Fracture)

  • This study was carried out to investigate the effects of Korean safflower(Carthamus. tinctorious L.) seed powder supplementation on bone metabolism during the recovery of rib-fracture induced by surgical operation in rats. Male Sprague-Dawley rats of 10 weeks old, weighing 370$\pm$5g, were divided into two groups after arrival : the control group(C group, AIN-76 semifurified diet) and safflower seed group(S group, AIN-76 semifurified diet+10% safflower seed powder). They were fed experimental diets for 12 days before the rib fracture operation and for 30 more days after the operation. A number 9 rib was fractured surgically and sham-operation was also performed. After the rib-fracture operation, rats were sacrificed on the 8th, 11th, 16th, 21st, and 30th day, respectively. The body weights were decreased on the 8th day after operation in both groups but recovered gradually thereafter. The fractured sections of the S group were more strongly adhered and more rapidly repaired than those of C group. Effects of safflower seed powder supplementation on indices of bone metabolism during the repairing of rib-fracture can be summarized as following. There were no differences between C and S group. In the levels of PTH and calcitonin, which regulate the concentration of plasma calcium. However, PTH level on the 8th day was significantly higher than that on the 16th day in control group. There were also no differences between groups or within groups in the levels of calcitonin. Osteocalcin levels, one of the bone-formation indices, were similar in two groups. Activities of total-ALP in plasma were highest at the 8th day in both groups and decreased thereafter, except on the 30th day in control group. The activities of bone-specific ALP that is directly related to bone calcification, showed higher levels in the S group than in the C group throughout the experimental period. They were significantly increased in the S group on the 11th, 16th, and 21st days. In comparisons of plasma calcium and phosphate levels, Ca levels were tended to be lower in the S group than in the C group and P levels showed the opposite trend. It seems that the fractured ribs were more rapidly repaired in rats supplemented with safflower seed powder than in control rats, possibly due to stimulation of bone calcification from increased activity of bone-specific ALP.   • In the treatment of mandibular condyle fracture, conservative treatment using closed reduction or surgical treatment using open reduction can be used. Management of mandibular condylar fractures remains a source of ongoing controversy in oral and maxillofacial trauma. For each type of condylar fracture,the treatment method must be chosen taking into consideration the presence of teeth, fracture height, patient'sadaptation, patient's masticatory system, disturbance of occlusal function, and deviation of the mandible. In the past, closed reduction with concomitant active physical therapy conducted after intermaxillary fixation during the recovery period had been mainly used, but in recent years, open treatment of condylar fractures with rigid internal fixation has become more common. The objective of this review was to evaluate the main variables that determine the choice of an open or closed method for treatment of condylar fractures, identifying their indications, advantages, and disadvantages, and to appraise the current evidence regarding the effectiveness of interventions that are used in the management of fractures of the mandibular condyle.

  • In clinical practice, recurrent Achilles ruptures have been noted to occurr at the original ruptured site. However, reports of new developed fresh rupture of the Achilles tendon in other sites are is extremely rare. Our report is about one uncommon case of a traumatic calcaneal tuberosity avulsion fracture following augmented repair, which was performed using the Krackow locking loop technique. We performed open reduction and intra-osseous fixation using a suture anchor. This procedure was done through the primary longitudinal incision for the calcaneal avulsion fracture fragment. After 6 months of follow-up, our patient has achieved a complete functional recovery and he can normally perform daily and work-related tasks without pain.

  • The influence of subsequent annealing treatment on the dynamic deformation and the fracture behavior of submicrocrystalline Al-4.4%Mg alloy is investigated in this study. After inducing an effective strain of 8 via equal-channel angular pressing at $200^{\circ}C$, most of the grains are considerably reduced to nearly equiaxed grains of $0.3{\mu}m$ in size. With an increment of various subsequent heat treatments for 1 hour, resultant microstructures are found to be fairly stable at temperatures up to $200^{\circ}C$, suggesting that static recovery will be dominantly operative, whereas grain growth is pronounced above $250^{\circ}C$. The results of tensile tests show that yield and ultimate tensile strength decrease, but elongation-to-failure and strain hardening rate increase with an increase in annealing temperatures. The dynamic deformation and the fracture behavior retrieved with a series of torsional tests are explored with respect to annealed microstructures. Such mechanical response is analyzed in relation to resultant microstructure and fracture mode.

  • Return of facial nerve function is important in patients with facial nerve paralysis by trauma. Sometimes, delay in diagnosis of facial nerve paralysis make recovery of facial nerve function difficult. Traumatic facial palsy mostly occur after temporal bone fracture in unilateral. Temporal bone fracture after head trauma are divided into the three group; longitudinal fracture, transverse fracture and mixed fracture. The most common symptoms are hearing impairment, bloody otorrhea, loss of consciousness and facial nerve paralysis. The early care of temporal bone fracture involves facial nerve paralysis. And there has been many discussion and study in the treatment of the immediate or delayed facial palsy ; examply, surgical approach, time and methods. We have managed a patient with unilateral facial nerve paralysis after longitudinal temporal bone fracture in mastoid process and conservative facial nerve decompression was performed. We have obtained good result and report this case with review of literatures.

관절 함몰형 종골 골절에 대한 비관혈적 정복 및 경피적 내고정의 수술적 치료 결과 (The Result of Closed Reduction and Percutaneous Screw Fixation for Intra-articular Calcaneal Fracture of Joint Depression Type)

  • Purpose: To evaluate the results of treatments by percutaneous Acutrak screw fixation for intra-articular joint depression type fracture of calcaneus. Materials and Methods: Thirteen cases with intra-articular joint depression type fracture of calcaneus, from September 2004 to March 2006, were reviewed. There were 9 males and 4 females with 52.5 years old mean age (range: $31{\sim}74$ years old). The average follow-up period was 18 months (range: $8{\sim}32$ months). Steinmann pins and Freers were used for closed reduction. After closed reduction, Acutrak screws and K-wires were inserted. The patients were evaluated with Creighton-Nebraska health foundation assessment sheet for calcaneal fracture, the extent of recovery of Bohler angle, fragment size, and state of subtalar joint. Results: Clinical results according to Creighton-Nebraska health foundation assessment sheet for calcaneal fracture were excellent in 6 cases (46%), good in 4 cases (30%), fair in 2 cases (15%), and poor in 1 case (7%). Average preoperative Bohler angle was $7.6^{\circ}$ (range: $2^{\circ}{\sim}13^{\circ}$). Average postoperative Bohler angle was $24.4^{\circ}$ (range: $4^{\circ}{\sim}33^{\circ}$). There were no soft tissue complications. There were one mild subtalar arthritis and one moderate subtalar arthritis. Conclusion: We think that closed reduction and percutaneous Acutrak screw fixation with or without K-wire is a good option for joint depression type fracture of calcaneus.

운동선수의 제5 중족골 피로골절 (Fifth Metatarsal Stress Fracture)

  • Fractures located at the metaphyseal/diaphyseal junction at the base of the fifth metatarsal were first described by Sir Robert Jones in 1902. However, ever since, there has been disagreement and debate regarding the diagnosis, classification, pathomechanics, the incidences, and potential causes of delayed unions and nonunions, and the optimal method of treatment. It appears to be widely agreed that proximal fractures of the metaphyseal/diaphyseal region of the fifth metatarsal are prone to delayed union or even nonunion. Several classifications of proximal fifth metatarsal stress fractures have been devised. Torg et al. classified fractures involving the proximal part of the diaphysis of the fifth metatarsal into three types. The Torg classification is a good grading system that can be used to determine the type of surgery needed as well as for the prediction of prognosis. The ''plantar gap'' might add to the decision-making process for surgery and improve the prediction of patient prognosis. In addition, the new classification using 'plantar gap' might be used for classification of fifth metatarsal stress fracture. Fifth metatarsal stress fractures can be treated conservatively or surgically, and excellent results have been reported for surgery with rapid recovery in athletes. Intramedullary screw fixation has become a popular form of fixation for fifth metatarsal stress fractures. Bone grafting presents the problems of a longer recovery time and additional skin incision for harvesting. The modified tension band wiring is an useful and simple option for surgical treatment of challenging fifth metatarsal stress fractures.

분말야금법으로 제작한 NiAl합금의 기계적성질 및 형상기억특성 (Mechanical Properties and Shape Memory Characteristics of NiAl Alloys by Powder Metallurgy)

  • The composition of martensite transformation in NiAl alloy is determined using pure nickel and aluminum powder by vacuum hot press powder metallurgy, which is a composition of martensitic transformation, and the characteristics of martensitic transformation and microstructure of sintered NiAl alloys are investigated. The produced sintered alloys are presintered and hot pressed in vacuum; after homogenizing heat treatment at 1,273 K for 86.4 ks, they are water-cooled to produce NiAl sintered alloys having relative density of 99 % or more. As a result of observations of the microstructure of the sintered NiAl alloy specimens quenched in ice water after homogenization treatment at 1,273 K, it is found that specimens of all compositions consisted of two phases and voids. In addition, it is found that martensite transformation did not occur because surface fluctuation shapes did not appear inside the crystal grains with quenching at 1,273 K. As a result of examining the relationship between the density and composition after martensitic transformation of the sintered alloys, the density after transformation is found to have increased by about 1 % compared to before the transformation. As a result of examining the relationship between the hardness (Hv) at room temperature and the composition of the matrix phase and the martensite phase, the hardness of the martensite phase is found to be smaller than that of the matrix phase. As a result of examining the relationship between the temperature at which the shape recovery is completed by heating and the composition, the shape recovery temperature is found to decrease almost linearly as the Al concentration increases, and the gradient is about -160 K/at% Al. After quenching the sintered NiAl alloys of the 37 at%Al into martensite, specimens fractured by three-point bending at room temperature are observed by SEM and, as a result, some grain boundary fractures are observed on the fracture surface, and mainly intergranular cleavage fractures.

Sensory recovery after infraorbital nerve avulsion injury

  • The infraorbital nerve is a branch of the trigeminal nerve. Injury to the infraorbital nerve can be caused by trauma, including various facial fractures. Due to this nerve injury, patients complain of numbness and pain in the entire cheek, the ala of nose, and upper lip. In general, spontaneous sensory recovery is expected after decompressive surgery. If nerve transection is confirmed, however, neurorrhaphy is typically performed. Here, we present a case in which microsurgery was not performed in a patient with Sunderland grade V avulsion injury of the infraorbital nerve due to a facial bone fracture. Gradual nerve function recovery was confirmed to be possible with conservative treatment and rehabilitation alone. These findings suggest that the nerve function recovery can be expected with conservative treatment, even for severe nerve injury for which microsurgery cannot be considered.

설상형 관절내 종골 골절에 있어서 경피적 Acutrak 나사 고정을 이용한 수술적 치료 (Percutaneous Acutrak Screw Fixation of Intra-articular Tongue Type Fracture of Calcaneus)

  • Purpose: To evaluate the results of treatments by percutaneous Acutrak screw fixation for intra-articular tongue type fracture of calcaneus, especially in elderly patient or patients with underlying disease including diabetes mellitus. Materials and Methods: Seven cases with intra-articular tongue type fracture of calcaneus, from November 2003 to February 2005, were reviewed. There were 6 males and 1 female with 65 years old mean age (range: $61{\sim}71$ years old). The average follow-up period was 16 months (range: $10{\sim}25$ months). Two Steinmann pins were used for closed reduction. After closed reduction was done like Essex-Lopresti's method, two Acutrak screws were inserted. The third Acutrak screw was inserted at the sustentaculum tali away from sinus tarsi to fix the primary fracture line. The patients were evaluated with Salama scale, the extent of recovery of Bohler angle, state of subtalar joint. Results: Clinical results according Salama scale were excellent in 5 cases, and good in 2 cases. Average preoperative Bohler angle was $13^{\circ}$ (range: $12{\sim}15^{\circ}$). Average postoperative Bohler angle was 31o (range: $23^{\circ}-40^{\circ}$). There were no soft tissue complications. There were two mild subtalar arthritis. Conclusions: We think that percutaneous Acutrak screw fixation is a good option for tongue type fracture of calcaneus in elderly patient or patients with underlying disease including diabetes mellitus.
