Development of the Diagnostic Worksheet for Mathematics Academic Counseling (수학학습 상담을 위한 진단 검사지 개발 연구)

  • In this research, The objective of the present study was to develop a preliminary diagnostic worksheet for use in consultations for learning mathematics. In order to achieve this, the worksheet was constructed with questions designed to assess the students. Through standardization, diagnostic worksheets for primary school students in grades 5 and 6 and secondary school students in grades 7 and 8 were produced. The diagnostic worksheet was divided into three sections, consisting of the psychology of learning mathematics in section 1, the methodology in learning mathematics in section 2, and personal preferences in learning mathematics in section 3. The psychology of learning mathematics was composed of questions on factors such as, "confidence in math learning ability," "math anxiety," and "attitude in learning mathematics." Moreover, factors in methodology in learning mathematics were "self-management in learning mathematics" and "math learning strategies." Those for personal preferences in learning mathematics asked about "motivation" and "preferences" with questions about "math learning habits" and "management methods for learning math." This diagnostic worksheet can be used as basic material in consulting students on learning mathematics.

A Need Analysis for Medical Supporting Service as a Part of Community-based Hospice Palliative Care (지역사회기반 호스피스완화케어에서의 의료지원서비스 요구분석)

  • Objectives : This study was conducted to investigate the need of medical supporting service (MSS) as a part of community-based hospice palliative care from the view point of beneficiaries and providers. Methods : This study adopted a methodological triangulation design. A questionnaire regarding intention to use MSS was completed by 175 patients under home-based cancer patient management program. And three focus groups consisted of hospice nurses, public health physicians, and public officials were interviewed to obtain the perceived needs, obstacles, and solutions of MSS. Results : Mean age of home-based cancer patient was 70.18 year old, 48.0% of them were living alone. Only 53.7% of them were treated pain and 93.7% intend to take pain medication prescribed by public health physician. All participants of focus group interviews agreed necessity and importance of MSS. Physicians' lack of confidence and unwillingness to prescribe opioid to terminal patients was the biggest obstacle to provide MSS in the public health center. Conclusions : The necessity and demand of MSS for community-dwelling cancer patients were verified. MSS is urgent issue to meet their needs.

Effects of an Integrative Art Therapy Program on Self-Efficacy and Conflict Coping Strategies in Marriage Migrant Women (결혼이주여성을 위한 통합예술치료 프로그램이 자기유능감 및 갈등대처방식에 미치는 효과)

  • The purpose of this study was to examine the effects of an integrative art therapy program for marriage migrant women to improve self-efficacy, learn conflict resolution strategies, and establish close family relationship. The participants of the program were limited to marriage migrant women residing in A City who lived in Korea for more than two years. 30 women for the experimental group and 31 women for the control group were randomly sampled. Eight sessions were offered. A pre-test was conducted in the first session and a post-test was done one month after the program was finished. The results of the program suggested that the experimental group had a higher effect on self-efficacy and conflict resolution strategies than the control group. The integrative art therapy program for marriage migrant women conducted in this study aims for marriage migrant women to recover confidence and actively establish happy marital and family relationship by providing an opportunity to make success experience. Moreover, the program is intended to be applied to the field to build up the foundation of family and social adjustment in marriage migrant women.

An Analysis of Research Reported in Korea on the Empowerment (임파워먼트에 관한 국내 간호논문분석)

  • This study was for understanding about the current situation of internal nursing research related to empowerment and for suggesting the direction the research on empowerment should improve. 17 nursing research articles between 1991 and 2001 were analyzed and their analysis results showed that the research on empowerment tends increasing in number. Looking at study methods, survery and research had a majority of researchs and the target of research was mainly nurses. Empowerment concept is a process that it helps to be able to assert the control about factors which influence on human life. This process suggested the precondition and result on empowerment in view of nursing that it includes broader systematic, structural and social responsibility in making individual taking responsibility on his health in management. Empowerment is to enhance the power and its basic starts from his own empowerment. The contents of his own empowerment is to improve his own ability(including spiritual, physical, and social ability), to connote also his own confidence enhancement which looks at himself as positive and strong, and to run after the his internal repletion through strengthening of his positive aspect. Based on above his own empowerment, the empowerment that builds the capability of group or organization is eventually to strengthen the spirit of individual and organization. Finally, this concept means that it can have an usefulness about nursing practice, education, administration and research. With the foundation of this research analysis, although the research on empowerment is largely limited to nurse, in future this should be applied to various nursing targets. In addition, the research on diverse applying methods including program develoment which bases on this concept should be made.

Impacts of the Journal Evaluation Program of the Korean Association of Medical Journal Editors (KAMJE) on the Quality of the Member Journals

  • Background: In 1997 the Korean Association of Medical Journal Editors (KAMJE) instituted a program to evaluate member journals. Journals that passed the initial evaluation were indexed in the KoreaMed. Here, we report changes in measures of quality of the KAMJE member journals during the last 20 years. Methods: Quality measures used in the study comprised 3 assessment categories; self-assessment by journal editors, assessment of the journals by KAMJE reviewers, and by Korean health science librarians. Each used detailed criteria to score the journals on a scale of 0 to 5 or 6 in multiple dimensions. We compared scores at baseline evaluation and those after 7 years for 129 journals and compared improvements in journals indexed vs. not-indexed by the Web of Science (Science Citation Index Expanded; SCIE). Results: Among 251 KAMJE member journals at the end of 2015, 227 passed evaluation criteria and 129 (56%) had both baseline and 7-year follow-up assessment data. The journals showed improvement overall (increase in median [interquartile range; IQR] score from baseline, 0.47 [0.64]; 95% confidence interval [CI], 0.44-0.61; P < 0.001) and within each category (median [IQR] increase by editor's assessment, 0.17 [0.83]; 95% CI, 0.04-0.26; P = 0.007; by reviewer's, 0.45 [1.00]; 95% CI, 0.29-0.57; P < 0.001; by librarian's, 1.75 [1.08]; 95% CI, 1.77-2.18, P < 0.001). Before the foundation of KAMJE in 1996, there were only 5 Korean medical journals indexed in the MEDLINE and none in SCIE, but 24 journals in the MEDLINE and 34 journals in SCIE were indexed by 2016. Conclusion: The KAMJE journal evaluation program successfully contributes improving the quality of the member journals.

An Analysis of the Achievement Test in the King Sejong Institute: Current Status of Applicants and their Performance (세종학당 성취도 평가 응시 현황 및 결과 분석 연구)

  • The purpose of this study is to analyze Language Achievement tests of King Sejong Institute which have been carried out from 2014 to 2017. Language Achievement tests of King Sejong Institute has been developed since 2014, the test is operated in 99 institutes of 46 countries now (As of first half of 2017) When this study analyzes the result of evaluation for 4 years, it was found that the number of nations enforcing the evaluation, institutes and examinees has continued its growth. In the early stage of evaluation, the examinees from Asian regions take up majority but the recent proportion of European region is getting bigger gradually. In addition, only beginner level evaluation was carried out in the early stage but recently its range is expanded to the intermediate level. This Language Achievement tests of King Sejong Institute can be utilized very valuable data which can diagnose the present and future of oversea Korean language education. In order to elevate public confidence as Korean language achievement test, this study suggested, first, increase the feedback effect of evaluation, second, establish the learning history information of examinees along with the test scores, third, conduct the training for evaluator in order to increase the validity and reliability of the evaluation, fourth, seek to utilize the results of the achievement test.

Factors associated with Pediatric Delirium in the Pediatric Intensive Care Unit (소아중환자실 입원 아동의 섬망 발생 관련 요인)

  • Purpose: This study aimed to investigate incidence of delirium in the pediatric intensive care unit (PICU) and to analyze associated risk factors. Methods: The participants were 95 patients, newborn to 18 years, who were admitted to the PICU. The instruments used were the Richmond Agitation Sedation Scale (RASS), and the Cornell Assessment of Pediatric Delirium. Data analysis was performed using the descriptive, $x^2$ test, t-test, and logistic regression analyses. Results: The incidence of delirium in children admitted to the PICU was 42.1%. There were significant differences according to age ($x^2=14.10$, p=.007), admission type ($x^2=7.40$, p=.007), use of physical restraints ($x^2=26.11$, p<.001), RASS score ($x^2=14.80$, p=.001), need for oxygen ($x^2=5.31$, p=.021), use of a mechanical device ($x^2=9.97$, p=.041), feeding ($x^2=7.85$, p=.005), and the presence of familiar objects ($x^2=29.21$, p<.001). Factors associated with the diagnosis of delirium were the use of physical restraint (odds ratio [OR]=13.82, 95% confidence interval [CI]=4.16~45.95, p<.001) and the presence of familiar objects (OR=0.09, 95% CI=0.03~0.30, p=.002). Conclusion: Periodic delirium assessments and intervention should be actively performed. The use of restraints should be minimized if possible. The caregiver should surround the child with familiar objects and ensure a friendly hospital environment that is appropriate for the child.

Vegetable and Nut Food Groups are Inversely Associated with Hearing Loss- a Cross-sectional Study from the Korea National Health and Nutrition Examination Survey (채소류 및 견과류와 난청과의 연관성: 2013년 국민건강영양조사 자료활용)

  • Objectives: A cross-sectional study was conducted to investigate the associations between food groups and hearing loss. Methods: Data of 1,312 individuals were used from the Korea National Health and Nutrition Examination Survey 2013. Hearing loss was determined with a pure tone average (PTA) of greater than 25 dB in either ear. The PTA was measured as the average hearing threshold at speech frequencies of 0.5, 1, 2, and 4 kHz. The dietary intake was examined with a food frequency questionnaire with 112 food items. The food items were classified into 25 food groups. A weighted logistic regression was used to investigate the association. Results: Individuals in the highest tertile of vegetables and nuts food groups were less likely to have hearing loss than those in the lowest tertile [Odds Ratio (OR) = 0.58 (95% Confidence interval (CI) 0.38-0.91), P = 0.019; OR = 0.59 (95% CI 0.39-0.90), P = 0.020, respectively], after adjusting for confounding variables of age, sex, body mass index, drinking, smoking, diabetes, hypertension, and physical activity. Conclusions: In this cross-sectional study, we observed that high intake of vegetables and nuts food groups revealed significant inverse associations with hearing loss, after adjusting for confounding variables among 1,312 participants.

Performance evaluation study of a commercially available smart patient-controlled analgesia pump with the microbalance method and an infusion analyzer

  • Background: Patient-controlled analgesia (PCA) has been widely used as an effective medical treatment for pain and for postoperative analgesia. However, improper dose errors in intravenous (IV) administration of narcotic analgesics from a PCA infusion pump can cause patient harm. Furthermore, opioid overdose is considered one of the highest risk factors for patients receiving pain medications. Therefore, accurate delivery of opioid analgesics is a critical function of PCA infusion pumps. Methods: We designed a microbalance method that consisted of a closed acrylic chamber containing a layer and an oil layer with an electronic balance. A commercially available infusion analyzer (IDA-5, Fluke Co., Everett, WA, USA) was used to measure the accuracy of the infusion flow rate from a commercially available smart PCA infusion pump (PS-1000, UNIMEDICS, Co., Ltd., Seoul, Korea) and compared with the results of the microbalance method. We evaluated the uncertainty of the flow rate measurement using the ISO guide (GUM:1995 part3). The battery life, delay time of the occlusion alarm, and bolus function of the PCA pump were also tested. Results: The microbalance method was good in the short-term 2 h measurement, and IDA-5 was good in the long-term 24 h measurement. The two measurement systems can complement each other in the case of the measurement time. Regarding battery performance, PS-1000 lasted approximately 5 days in a 1 ml/hr flow rate condition without recharging the battery. The occlusion pressure alarm delays of PS-1000 satisfied the conventional alarm threshold of occlusion pressure (300-800 mmHg). Average accuracy bolus volume was measured as 63%, 95%, and 98.5% with 0.1 ml, 1 ml, and 2 ml bolus volume presets, respectively. A 1 ml/hr flow rate measurement was evaluated as 2.08% of expanded uncertainty, with a 95% confidence level. Conclusion: PS-1000 showed a flow accuracy to be within the infusion pump standard, which is ± 5% of flow accuracy. Occlusion alarm of PS-1000 was quickly transmitted, resulting in better safety for patients receiving IV infusion of opioids. PS-1000 is sufficient for a portable smart PCA infusion pump.

Incidence of incisional hernia following liver surgery for colorectal liver metastases. Does the laparoscopic approach reduce the risk? A comparative study

  • Backgrounds/Aims: No reports to compare incisional hernia (IH) incidence between laparoscopic and open colorectal liver metastases (CRLM) resections have previously been made. This is the first comparative study. Methods: Single-center retrospective review of patients who underwent CRLM surgery between January 2011 and December 2018. IH relating to liver surgery was confirmed by computed tomography. Patients were divided into laparoscopic liver resection (LLR) and open liver resection (OLR) groups. Data collection included age, sex, presence of diabetes mellitus, steroid intake, history of previous hernia or liver resection, subcutaneous and peri-renal fat thickness, preoperative creatinine and albumin, American Society of Anesthesiologists (ASA) score, major liver resection, surgical site infection, synchronous presentation, and preoperative chemotherapy. Results: Two hundred and forty-seven patients were included with a mean follow-up period of 41 ± 29 months (mean ± standard deviation). Eighty seven (35%) patients had LLR and 160 patients had OLR. No significant difference in the incidence of IH between LLR and OLR was found at 1 and 3 years, respectively ([10%, 19%] vs. [10%, 19%], p = 0.95). On multivariate analysis, previous hernia history (hazard ratio [HR], 2.22; 95% confidence interval [CI], 1.56-4.86) and subcutaneous fat thickness (HR, 2.22; 95% CI, 1.19-4.13) were independent risk factors. Length of hospital stay was shorter in LLR (6 ± 4 days vs. 10 ± 8 days, p < 0.001), in comparison to OLR. Conclusions: In CRLM, no difference in the incidence of IH between LLR and OLR was found. Previous hernia and subcutaneous fat thickness were risk factors. Further studies are needed to assess modifiable risk factors to develop IH in LLR.
