두부전방전위자세에 자세교정법과 호흡보조근운동의 적용 시 CVA와 호흡의 변화 (Change of Craniovertebral Angle(CVA) and Respiration on Application Correction Method of Posture and Breathing Accessory Muscle Exercise in Forward Head Posture(FHP))

  • Purpose : This study was analyzed to researched the improvement of the posture and breathing ability on correction method of posture and breathing exercise in Forward Head Posture(FHP) Method : Eighten forward head posture subjects participated in this study. The control group applied to correction method of posture and the experiment group applied to correction method of posture with breathing exercises. Results : The results showed significant improvement in Craniovertebra-Angle on each two group(P<0.05). All the other result showed non-significant in respiratory(Tidal volume, Expiratory residual volume, Inspiratory residual volume) But value of result is slightly improved in after exercise. Conclusion : CVA angle is increased in each group but not increased between experiment group and control group. Total Volume(TV), Inspiratory Reserve Volume(IRV) and Expiratory Reserve Volume(ERV) are a little increased.

Effect of upper thoracic mobilization on cervical alignment in stroke patients with forward head posture: A case study

  • The purpose of this study was to identify the effect of upper thoracic mobilization on cervical alignment in stroke patients with forward head posture. The subject's of this study were two stroke patients with forward head posture and a cervical curve angle (four-line Cobb's method; FLCM) less than $40^{\circ}$. The intervention, central posterior-anterior (PA) mobilization, was applied to the T1-T4 vertebrae (upper thoracic spine) following the Maitland concept. This mobilization was applied three times per week for four weeks. In the results, the cervical curve angle (FLCM) increased for both subject 1 and 2. However, Jochumsen method score was decreased in subject 1, while it was increased in subject 2. These results demonstrate that upper thoracic mobilization had the positive effect on the cervical curve angle but not on Jochumsen method score. These findings suggest that PA mobilization on the upper thoracic spine could correct cervical curve angle measured by FLMC in stroke patients with FHP.

Analysis of Correlation Coefficient between head posture and muscle stiffness of cervical extensor muscles

  • 이 연구의 목적은 머리자세와 목폄근육들의 근긴장도 및 근경직 사이에 상관관계를 검증하기 위해 실시되었다. 연구대상자는 20대 성인 80명으로 시상면에서 평소 머리자세와 목폄근육들에 근긴장도와 근경직을 측정하였다. 측정된 머리자세는 Image J를 사용하여 머리척추각, 머리회전각, 앞쪽어깨각을 분석하였다. 연구결과, 머리척추각이 감소할수록 위등세모근의 근긴장도 및 근경직이 유의하게 증가하고, 머리회전각이 증가할수록 위등세모근의 근긴장도 및 근경직과 유의하게 증가하는 상관관계가 있었다(p<.05). 그리고 연구대상자의 머리척추각을 기준으로 정상머리자세와 앞쪽머리자세로 재분류하여 비교한 결과, 앞쪽머리자세는 정상머리자세보다 위등세모근의 근긴장도 및 근경직이 유의하게 높은 특징을 확인하였다(p<.05). 이 연구결과는 임상에서 앞쪽머리자세의 평가 및 자세교육을 위한 기초자료로 활용되길 기대한다.

두경부(Head & Neck)종양에서 Forward IMRT 유용성에 관한 고찰 (The usefulness of Forward IMRT for Head and Neck Cancer)

  • I. 목적 방사선 치료계획에 있어서 정상조직의 선량과 치료부위 선량의 분포는 매우 중요하다. 또한 치료부위의 균일한 선량분포를 얻기 위하여 여러 가지 방법을 이용하고 있다. 특히 두경부(Head & Neck) 종양의 방사선 치료 시 체표 윤곽의 변화가 심하여 이에 따른 선량 불균일 보정이 필요하다. 기존의 치료방법으로는 parotid gland 와 spinal cord 의 tolerance dose 이하를 유지하면서 planning target volume(PTV)에 충분한 치료 선량을 전달하기에는 여러 가지 어려움이 있다. 이에 본원에서는 Forward IMRT를 이용한 방사선 치료의 유용성 및 실용적인 정도관리 방법에 관한 연구를 하였다. II. 대상 및 방법 본 연구는 두경부(Head & Neck) 종양 비인두암(nasophrygeal cancer)의 방사선 치료가 요구되는 환자를 대상으로 하여 2차원적인 치료계획과 dynamic Mulit-Leaf Collimator(dMLC)를 이용하여 partial block technique(PBT)방법을 적용한 Forward IMRT를 위한 치료계획을 dose volume histogram(DVH)로 비교 분석하였다. 또한 정도관리(quality assurance, QA)를 위하여 필름과 pinpoint chamber를 이용하여 정확한 선량 평가를 실시하였다. III. 결과 2차원적인 치료계획과 Forward IMRT를 적용한 치료계획의 DVH를 비교 분석한 결과 Forward IMRT를 적용한 치료계획이 Rt, Lt parotid gland와 spinal cord에 들어가는 선량을 좀더 줄일 수 있었다. 이러한 Forward IMRT의 시도로 조사면의 방사선 세기가 정상조직의 보호를 개선시키고 치료의 최적화를 이룰 수 있었다. Inverse IMRT에 비해 기존의 3차원적 치료계획 장치를 이용할 수 있고 비교적 단순한 방사선 세기 패턴이므로 정도관리가 용이하였다. IV. 결론 Forward IMRT는 2차원적인 치료법에 비하여 PTV에는 균일한 선량분포를 이루면서 정상조직에는 tolerance dose 이하로 선량을 전달 할 수 있는 치료기법이었다.

전방머리자세를 가진 초등학생에게 복합운동프로그램이 자세변화와 보행, 균형능력에 미치는 영향: 사례연구 (The Effects of Complex Exercise Program on Postural Change, Gait and Balance Ability in Elementary School Students with Forward Head Posture - Case Study)

  • Background: The purpose of this case study was to investigate into the effect of complex exercise program on the postural change, gait and balance ability in elementary school students with forward head posture. Methods: Four patients with forward head posture were recruited. They were evaluated pre-treatment, and after 6weeks, using neck disability index (NDI), numeric pain rating scale (NPRS), balance ability, foot pressure (fore foot/rear foot peak pressure ratio, F/R ratio), gait ability (cadence, toe out angle, stance phase). Results: First, the angle of forward head posture (craniovertebral angle; CVA and cranialrotation angle; CRA) was decreased in all subjects. The NPRS and NDI were decreased in all subjects. Also, The cadence, toe out angle and F/R ratio were increased in all subjects. The stance phase of gait cycle was positively change in all subjects. Lastly, the static balance ability improved in all subjects. Conclusion: According to the results above, the complex exercise program for students with forward head posture can help improve the postural change, gait and balance ability. Also, the complex exercise program was able to select interventions depending on the patient's condition and the desired goal.

The Effects of Head Support on Muscle Activity and Pain in a Forward-leaning Posture

  • Background: Because a forward-leaning posture can cause increased back muscle activity and pain. Therefore, an innovative method to reduce back muscle activity and pain is required. Objects: This study aimed to investigate the effects of a head support on muscle activity and pain in a forward-leaning posture. Methods: A total of 14 male and 16 female students (average age, 21.65 ± 2.37 years; height, 166.15 ± 7.90 cm; and weight, 60.65 ± 9.00 kg) were recruited for the experiment. Two of them were excluded due to musculoskeletal disorders. The muscle activity and pain in the forward-leaning posture were assessed while participants washed dishes for 7 minutes with and without a head support. The condition of using a head support was randomly performed with a 5-minutes break. To confirm a lumbar flexion angle of 30° during the experiment, myoVIDEO was used, and surface electromyography was used to measure muscle activity. Pain was assessed using a 10-point visual analog scale (VAS). The Wilcoxon signed-rank test was used to analyze the data, with p < 0.05 indicating statistical significance. Results: The cervical, thoracic, and lumbar erector spinae muscle activities significantly decreased with the use of the head support, but there was no significant change in the gluteus maximus. There was a significant decrease in the VAS score for the lumbar erector spinae (p < 0.05), but there was no significant change in the VAS score for the cervical region. Conclusion: The use of a head support in a forward-leaning posture reduced cervical, thoracic, and lumbar erector muscle activity and pain. Therefore, it could be recommended during working in a forward-leaning posture, such as during dishwashing, cooking, and working as a factory employee.

호흡방법에 따른 전방머리자세의 목근육 활성도 변화 (Effects of Breathing Methods on Neck Muscle Activation in Subjects with a Forward Head Posture)

  • Purpose: This study attempts to determine the effects of applying three kinds of breathing exercises for four weeks on the neck muscle activation of subjects with a forward head posture. Methods: A total of 30 adults aged in their twenties (15 men and 15 women) with a forward head posture who voluntarily agreed to participate after listening to the purpose and procedure of this research were chosen as the subjects of this study. The subjects were randomly divided into either the diaphragmatic breathing exercise (DBE) group, the abdominal drawing-in maneuver (ADIM) group, or the abdominal expansion method (AEM) group according to the breathing intervention scheme. Each group included ten subjects. The muscle activity of the sternocleidomastoid, scalenus anterior, and splenius capitis was measured in all the groups prior to the intervention, two weeks after the intervention, and four weeks after the intervention. All the interventions were implemented for 30 minutes a day, three times a week, for a total of four weeks. Results: No significant between-group difference was observed in terms of the change in neck muscle activity according to the four-week intervention scheme. Further, there was no interaction between the intervention period and the intervention scheme in relation to the change in neck muscle activity. Conclusion: The results of this study suggest that abdominal expansion exercise is as effective as other breathing exercise methods for subjects with a forward head posture. We therefore expect that abdominal expansion exercise can be used as a scheme for the prevention of symptoms as well as therapy for patients with a forward head posture.

운동 트레이닝과 카이로프랙틱의 복합처치가 전방 머리 자세와 거북목 대상자의 악력과 경추부 근력에 미치는 영향 (Effects of Exercise Training and Chiropractic on Grip Strength and Cervical Muscle Strength of Subjects with Forward Head Posture and Turtle Neck)

  • PURPOSE: Forward head posture and turtle neck are common musculoskeletal disorders of the cervical vertebrae. The purpose of this study was to investigate the effects of combined exercise training and chiropractic on the grip strength and cervical muscle strength of subjects with forward head posture and turtle neck. METHODS: The subjects were divided into two groups: forward head posture (n=9) and turtle neck (n=9). Both groups performed combined exercise training and chiropractic. The subjects were instructed about the exercise training once a week. This training was performed for 30 minutes every day for 8 weeks, and the chiropractic was performed for 15 minutes once a week. The cervical muscle strength and grip strength were measured. Two-way repeated measured ANOVA was performed for statistical analysis. RESULTS: In changes to the left grip strength, the main effect over time was significant, but the interaction effect and the main effect in the groups were not significant. In changes to the right grip strength, no difference was found to be statistically significant. In changes to the cervical muscle strength, no difference was found to be statistically significant. CONCLUSION: The increases of grips strengths and cervical muscle strength in forward head posture group were greater than turtle neck group but there were not found to be statistically significant.

Effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction

  • This study was designed to provide basic data for developing exercise program that helps correcting posture by knowing the effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction. In this study determined subjects whether they had forward head posture or not. On the basis of the New York state posture rating, if a subject's posture is match up with the normal standard posture, gives 5 points and if the posture is slightly get out of the normal standard posture, gives 3 points and if the posture is apparently get out of the standard, gives 1 points. When determining the forward head posture, if talus, humerus and outer ear center are on the same line, it is determined as normal and if outer ear center is off the line less than 1.0cm, it is a slight deformation and if outer ear center is off the line more than 1.0cm, it is a high deformation. In the study selected people who have more than 1 cm gap between two vertical lines start from outer ear center and acromion separately as subjects. Length between the ideal alignment line measured by using goniometer and temporal region showed statistically significant decrease as $2.36{\pm}1.07cm$ before the intervention and $1.06{\pm}0.88cm$ after the intervention. After 4 weeks of neck and chest extensor muscle exercise, the group who exercised both showed increase in range of neck joint motion and neck flexion of the forward head posture. Meanwhile the group who only exercised neck extensor muscle only and the group who only exercised chest extensor muscle didn't showed statistically significant result. That only the group who exercised both muscles showed significant result is the different with studies before. Because this study didn't target patient who had a lesion, couldn't compare effect of the conservative manner and exercise. However, this study provides the fact that the group who exercised both neck and chest muscle had more effect than the control group.

  • Purpose : The purpose of this study was to investigate the effect of 2 weeks of backward walking exercise (BWE) on cervical angle and gait parameters in college students with forward head posture. Methods : Fifteen subjects participated in the experiment. All the participants had a craniovertebral angle (CVA) of 55 degrees or less. The purpose of the study was explained to all the subjects prior to participation, and volunteered to take part in the study. A camera capable of taking pictures of the lateral plane was installed at a distance of 1.5 meters from each subject. Images of forward head posture were obtained before and after performing the BWE, and the CVA and craniorotational angle (CRA) were compared pre- versus post exercise. Foot pressure and gait parameters (step length of left and right, stride length, stance of left and right, swing of left and right, step time of left and right, and stride time) were measured using a rehabilitation treadmill. The subjects performed the BWE for 2 weeks. The exercise program consisted of a 5-minute warm-up exercise, 20-minute main exercise, and 5-minute cool-down exercise. In the main exercise, the treadmill speed was set to 2.4 km/h in the first week and 3.4 km/h in the second week. A paired t test was used to compare the CVA and CRA and gait parameters before and after the exercise. Results : Comparison of the CVA and CRA before and after the BWE revealed a significant difference post exercise, with a marked improvement in forward head posture after the exercise (p<.05). Conclusion : Based on the results of this study, the BWE is considered to be an effective exercise for the forward head posture. Also, additional research is needed to shed light on the impact of the BWE on gait parameters.
