The oval technique for nipple-areolar complex reconstruction

  • Background Nipple-areolar complex (NAC) reconstruction is the final stage of breast reconstruction. Ideal reconstruction of the NAC requires symmetry in position, size, shape, texture, pigmentation, and permanent projection, and although many technical descriptions of NAC reconstruction exist in the medical literature, there is no gold standard technique. The technique devised by the authors is very versatile, with excellent results, and it enables 1-step reconstruction with optimal results in terms of shape and nipple projection. Methods Our technique consists of a combination of modified local flaps and a full-thickness skin graft. Patients were observed for 18 months to estimate the amount of retraction. This procedure was performed in 40 patients, four of them bilaterally. The duration of the follow-up was 30 months. Complications occurred in 10% of patients, and included infections (5%), ischemia (2.5%), and hematoma (2.5%). Results No cases of total nipple necrosis were reported. The NAC shape remained optimal in all cases, with a very small reduction of the vertical and horizontal diameters of the areola, which maintained its designed round shape well, and negligible retraction in the diameter and projection of the nipple. Conclusions The oval technique represents a major step forward, involving a combination of existing techniques, such as the C-V flap and the cutaneous graft, to achieve excellent results regarding areola shape and nipple projection, significantly reducing the cases of nipple ischemia. These results were substantially obtained through subcutaneous equatorial sutures, skin grafting, and flattening of the apexes of the flap.

  • 비파괴검사기법에 활용되고 있는 X-선 디지털 영상합성법(digital tomosynthesis)에서 단층영상의 선명도를 향상시킬 수 있는 방법을 개발하였다. 기존의 SAA (shift-and-add) 알고리즘은 blur artefact로 인하여 재구성된 단층영상이 매우 흐린 단점이 있다. 본 연구에서는 SAA에서 blur artefact가 발생되는 물리적 메커니즘에 착안하여, 최초 재구성된 단층영상에서 관심있는 단층의 데이터를 모두 0의 값으로 대체한 후 이를 다시 FP (forward projection) 및 BP (backprojection)를 수행하여 관심있는 단층에서의 blur artefact를 추출 보정하여 단층영상을 복원하고자 하였다. 개발한 알고리즘을 검증하기 위해 실제 실험 및 몬테칼로(Monte Carlo) 시뮬레이션을 통해 기존 SAA 방법과 비교하였으며, 단층영상의 선명도가 크게 향상됨을 확인하였다.

Human Face Recognition using Multi-Class Projection Extreme Learning Machine

  • An extreme learning machine (ELM) is an efficient learning algorithm that is based on the generalized single, hidden-layer feed-forward networks (SLFNs), which perform well in classification applications. Many studies have demonstrated its superiority over the existing classical algorithms: support vector machine (SVM) and BP neural network. This paper presents a novel face recognition approach based on a multi-class project extreme learning machine (MPELM) classifier and 2D Gabor transform. First, all face image features were extracted using 2D Gabor filters, and the MPELM classifier was used to determine the final face classification. Two well-known face databases (CMU-PIE and ORL) were used to evaluate the performance. The experimental results showed that the MPELM-based method outperformed the ELM-based method as well as other methods.

사각관 이상유동 분석을 위한 전기적 캐패시턴스 토모그라피 코드 개발 (Development of an Electrical Capacitance Tomography Code for Analysis of Two-Phase Flow in the Rectangular Pipe)

  • A computer code for Electrical Capacitance Tomography (ECT) is developed to sense the cross sectional phase distribution of two-phase flow in the rectangular pipe in which the tomography sensor furnished by the insulated wall, electrodes, and electric field screen. The computer code had two steps for the image reconstruction. In the forward projection step, the sensitivity matrix was constructed based on the electric field calculated by the finite difference method. In the backward projection step, the sensitivity matrix and the measured capacitances were used to reconstruct the cross sectional image. Several algorithms including LBP, TR, ITR, and PLI were employed to find the proper one for the two-phase flow analysis. Since the dielectric constant of the water in two-phase flow is sensitive to the thermal parameter such as, temperature and pressure, the developed code was evaluated to find their accuracy, speed of calculation, and sensitivity to the variation of the dielectric constant. It was found that the iterative methods are superior to the direct methods for the image reconstruction, and the PLI method was the best in the variation of the dielectric constants.

하악골 운동시 과두의 이동범위에 관하여 (THE NORMAL RANGE OF CONDYLAR MOVEMENT)

  • The purpose of this study was to investigate the normal range of condylar movement of normal adults. The author has observed roentgenographic images of four serial positions of condylar head taken by modified transcranail lateral oblique projection. The serial positions are centric occlusion, rest position, 1 inch open position and maximal open position. The results were obtained as follow; 1. Inter-incisal distance was 46.85㎜ in maximal open position. 2. The length between the deepest point of glenoid fossa and summit of condylar head in rest position was wider than that in centric occlusion by 0.8㎜. 3. In 1 inch open position, condylar head moved forward from the standard line in 12.64㎜ of horizontal direction and moved downwards from the standard line in 1.84㎜ of vertical direction. 4. In maximal open position, condylar head moved forward from the standard line in 19.06㎜ of horizontal direction and moved downwards from thestanard line in 0.4㎜ of vertical direction. 5. In centric occlusion, the width between glenoid fossa and margin of condylar head was greater in the posterior portion than in the anterior portion by 0.4㎜. 6. Except for the estimated figures of 1 inch open position, all of the estimated figures was greater in male than in female.

  • The styloid process is normally a small, slender, pointed, downward and forward projection of the temporal bone. It is developed from the ossification of the Reichert's cartilage of the second branchial arch. In the adult, It is about 2.5cm in length and lies between the internal and external carotid arteries and lateral to the tonsillar fossa. Elongation of the styloid process , ossification of the stylohyoid or slylomandibular ligament causes recurrent nonspecific throat discomfort, foreign-body sensation, dysphagia, referred otalgia or facial pain. Symptoms are provoked by swallowing, turning head, carotid compression, and posttonsillectomy. Recently, the authors have experienced two cases of Eagle's syndrome. So we report them with review of literature.

시공간 상관성을 이용한 적응적 움직임 추정 (Adaptive motion estimation based on spatio-temporal correlations)

  • Generally, moving images contain the various components in motions, which reange from a static object and background to a fast moving object. To extract the accurate motion parameters, we must consider the various motions. That requires a wide search egion in motion estimation. The wide search, however, causes a high computational complexity. If we have a few knowledge about the motion direction and magnitude before motion estimation, we can determine the search location and search window size using the already-known information about the motion. In this paper, we present a local adaptive motion estimation approach that predicts a block motion based on spatio-temporal neighborhood blocks and adaptively defines the search location and search window size. This paper presents a technique for reducing computational complexity, while having high accuracy in motion estimation. The proposed algorithm is introduced the forward and backward projection techniques. The search windeo size for a block is adaptively determined by previous motion vectors and prediction errors. Simulations show significant improvements in the qualities of the motion compensated images and in the reduction of the computational complexity.

  • This study is an inspection of press forming product and mould using reverse engineering system. The inspection process on production field involves a lot of errors because of the hand-work so we focussed on improving the measured precision through performing the effective inspection using 3D non-contact scanner. By so doing that, we improved the precision of press forming product by analyzing the cause following the inspection result. Through the inspection, we applied it to the reverse engineering and we could improve the inspection process.

하나의 비디오 입력을 위한 모습 기반법과 모델 사용법을 혼용한 사람 동작 추적법 (Human Motion Tracking by Combining View-based and Model-based Methods for Monocular Video Sequences)

  • 사람의 동작을 믿을 수 있게 따라가는 것은 감시용 비디오나 사람과 컴퓨터간의 사용자 인터페이스 개발에 있어서 필수적이다. 이 논문은 모습 기반법(appearance-based method)과 모델 사용법을 혼용하여 사람을 추적하는 새로운 방법에 관한 논문이다. 하나의 비디오 입력이 화소 단위 및 물체 단위로 처리된다. 화소 단위의 처리에 있어서 개별 화소색을 분류하는 훈련방법으로, 가우스 혼합 모델(Gaussian mixture model)을 사용하였다. 물체 단위의 처리에 있어서 사람 몸에 대한 삼차원 모델링을 하고, 모델 몸체를 투사면(projection plane)에 투사시켰다. 투사된 몸체와 배경을 제외한 영상과 계산 기하 방법을 사용하여, 화소보다 작은 단위로 겹쳐지는 면적을 계산하였다. 우리의 방법은 정방향 기구학 (forward kinematics)을 사용하므로 역방향 기구학(inverse kinematics)을 사용하는 방법과 달리 계산 결함(singularity)을 갖지 않는다. 이 논문에서는 사람의 동작을 추적하기 위한 문제를 비선형 방정식 문제로 바꾸었다. 비선형 방정식의 비용 함수는 전경(foreground)의 영상 실루엣(silhouette)과 투사된 삼차원 모델 몸체의 실루엣의 겹쳐지는 면적이다. 화소 단위의 영상을 화소를 하나의 면적으로 계산함으로써, 겹쳐지는 면적에 대한 실수 단위의 계산은 계산 기하를 사용하였다. 이 논문의 방법은 다양한 사람 동작을 인식하기 위하여 사용되었다. 비디오에 나타나는 사람 동작 추적은 매우 우수하다.

르포씨 1형 골절단술후 코와 상순의 연조직 변화의 삼차원 컴퓨터 단층촬영을 이용한 정량적 측정에 관한 연구 (QUANTITATIVE ASSESSMENT OF NASAL AND UPPER LIP CHANGES AFTER LE FORT I OSTEOTOMY SURGERY USING A 3-DIMENSIONAL COMPUTED TOMOGRAPHY)

  • Objective: To evaluate nasal and upper lip changes after Le Fort I surgery by means of images taken with a three-dimensional computed tomography (3D-CT). Methods: Fifteen patients (9 female and 6 male, mean age 21.9 years) with preoperative and postoperative 3D-CT were studied. The patients underwent maxillary movement with impaction or elongation, and advancement or setback. With the 3D-CT which presents reconstructive soft tissue images, preoperative and postoperative measurement and analysis were performed for nasal tip projection angle, columellar angle, supratip break angle, nasolabial angle, interalar width, internostril width, columella length and nasal tip projection. Results: Postoperative interalar and internostril widening was significant for all categories of maxillary movement. However, there was little significant relation in all parameters between the amount and direction of maxillary movement. Interestingly, movement of the maxilla with upward did show a little decrease in the columellar angle, supra tip break angle and nasolabial angle. Also movement of the maxilla with forward did show a little advancement in the upper lip position. Conclusion: Changes to the nose clearly occur after orthognathic surgery. There was a significant increase in postoperative interalar width and internostril width with maxillary movement. However, no clear correlation could be determined between amount of change and maxillary movement. Interestingly, maxillary impaction did show a little decrease in the columellar angle, supra tip break angle and nasolabial angle. In addition, we used 3D-CT for more precise analysis as a useful tool.
