인위적인 기도협착이 동맥혈액가스에 미치는 영향 (Experimental Study of the Influence of the Upper Airway Obstruction on the Blood Gas Analysis)

  • 상부기도가 갑자기 폐쇄증을 일으켜 심한 호흡곤란증을 호소하는 환자에 대하여 응급으로. 기도 및 호흡을 재확보하여야하며 이와같은 예를 임상에서 가끔 직면하게 된다. 이러한 환자에게 적절한 기도확보는 생명을 유지시킬 수 있다. 저자는 토끼를 대상으로 기존 기관의 직경을(약 3.4mm) 약 1/3(1.2mm), 1/4(0.8mm) 및 1/6(0.6mm)로 협소시켜 생리적 변화를 추적하였다. 결과는 다음과 같았다. 1) 혈액가스의 분석결과 직경을 약 1/3로 감소시켰던 군에서 생리적 변동이 별로 없었다. 2) 직경을 약 1/4, 1/6로 감소시켰던 군에서는 $PaO_2,$ $PaCO_2$ 및 pHa에 뚜렷한 변화를 나타내어 저산소혈증, 과탄산혈증 및 대사성산증을 나타내었다. 3) 호흡저항은 모든 군에서 뚜렷하게 증가하여 1회 호흡량도 현저하게 감소하였는데 16G(직경 1.2mm 호흡로)에서는 호흡수의 증가로 폐포환기가 적당하게 영위됨에 따라서 혈액가스 및 vital signs의 변동이 별로 없던 것으로 보아 토끼에서는 이 정도의 호흡로 폐쇄에는 1시간까지 견딜 수 있음을 알았다.

구인두 폐색을 유발한 자발성 편도선 혈종 치험 1예 (Spontaneous Tonsillar Hematoma Causing Oropharyngeal Obstruction)

  • Spontaneous tonsillar bleeding is a rare condition in ENT unit. Almost reported cases have been related to infection of tonsils. Prior to introduction of antibiotics, spontaneous tonsillar bleeding was usually related to superficial capsular bleeding due to acute or chronic tonsillitis. The presented case is a 36-year old healthy man without history of acute, chronic tonsillitis, and coagulation disorder who complained of dyspnea and oropharyngeal foreign body sensation after vomitting. Examination revealed a reddish polypoid mass of the right tonsil. Furthermore, the mass pushed uvula and tongue base, caused nearly total obstruction of oropharynx. All rountine laboratory test results were within normal limits. Computed tomography (CT) showed low density mass of attached in upper pole of right tonsil without enhancement. We performed tonsillectomy including the reddish polypoid mass under general anesthesia. The pathology revealed lymphoepithelial tissue with reactive hyperplasia. This is the first reported tonsillar hematoma presenting as a large oropharyngeal mass which was caused by vomitting.

Multiple Congenital Vascular Anomalies In a Lakeland Terrier: Computed Tomographic Angiographic Evaluation

  • A 3-month-old intact male Lakeland terrier was presented with recurring regurgitation after removing cervical esophageal foreign body by endoscopy. Blood and urine analysis, radiography, ultrasonography, fluoroscopic esophagography, computed tomographic angiography (CTA) were performed. In radiography and fluoroscopic esophagography, vascular ring anomaly was considered as the primary cause of megaesophagus, and CTA with gas-inflation of the esophagus was performed. Compressed esophagus, persistent right aortic arch (PRAA), aberrant left subclavian artery (LSA), and a venous structure which was confirmed in surgery to be incomplete type persistent left cranial vena cava (PLCVC) connected with the left side azygos vein were observed. Left deviation of the trachea was also revealed in CT, which implies the compression by left ligamentum arteriosum. Therefore, type 3 PRAA with left ligamentum arteriosum and aberrant LSA, was considered as a prior differential diagnosis. Surgical repair was performed and the clinical signs improved. This report describes CTA characteristics of combination of PRAA with aberrant LSA, incomplete PLCVC and Lt. azygos vein in a dog. Although not every vascular anomaly does induce clinical sign, some types can complicate the surgical procedure, and cause clinical signs. Therefore, thorough evaluation of vascular anomalies in the thorax is important, and CTA is a useful method in identifying multiple vascular anomalies in dogs.

배양된 Pseudomonas aeruginosa의 소프트 콘택트 렌즈 표면부착에 대한 미세구조적 연구 (A Study of Ultrastructure on attachment of Soft Contact Lens Surface of Incubated Pseudomonas aeruginosa)

  • 소프트 콘택트렌즈는 취급이 간단하고 착용감이 좋고 미용효과를 가지며 운동과정에서 좋은 부착상태를 가지므로 피검자들은 시력교정으로 많이 사용되고 있다. 그러나 사용의 부주의로 눈에 부작용을 일으킬 수 있다. 본 연구는 배양된 P. aeruginosa의 소프트 콘택트렌즈 표면부착에 대한 미세구조적 상태를 규명하기 위해서 저자들은 주사전자현미경을 이용해서 소프트 콘택트렌즈 표면을 관찰하였다. 샘플 제작과정에서 $OsO_4$의 후고정과 tannic acid에 염색한 것이 가장 좋은 관찰이 되었다. 균의 모양은 막대모양으로 섬모가 형성되어 있었다. 후고정을 하지 않는 샘플에서는 많은 이물질과 인공산물로서 균의 동정이 어려웠다.

응급실 내원 아동에 대한 분석 (A study on the pedicatric accident)

  • Children's accident is a largely preventable public health problem. Little is known. however, about population-based incident and outcome of pediatric accident. From 1997.9 through 1998,8. admission data from emergency center in I city were collected. 1418 patient from 0 through 13 years of age were selected. All children with unintensional accidental problems were identified through coded sheet which categorizes epidemiologic characteristics. The specific purposes of this study are analysis about the characteristics of pediatric accidents. And it aims to produce the basic data necessary for accident prevention policy development. The results of this study were as follows; 1. The number of male children$(62.6\%)$ were higher than female children$(37.4\%)$ 2. The age group from 1 to 3 years represents the highest proportion$(45.4\%)$ of every accidents except on traffic accident. 3. The highest proportion of accident were as follows occured during the June-August$(34\%)$, Sunday$(22.6\%)$, and 17-21 p.m. $(37.2\%)$ 4. The main causes of accident include general trauma$(70.9\%)$, environmental accident$(l6.8\%)$. and traffic accident$(l2.1\%)$, 5. Preschool age group represents more than half$(65.4\%)$ of traffic accident. 6. environmental injury includes burns $(46.6\%)$, foreign body$(43.6\%)$, exposure to poisonous materials$(6.3\%)$. and bite(3.3) This results could be used to develope prevention programs and assist in accident prevention system development. And also these data substantiate that accident prevention program decrease safety-related injury rate in preschool age group must be concentrated on enhancing access to a system to have a significant effect. Furthermore, it is necessary for accident prevention. So several suggestions are described here: 1. Development of parent's educational program for accident prevention and safety education should be done actively. 2. Home safety surveillance system should be initiated. 3. The initiation of children's accident report system could be contribute the analysis and the reduction of accident.

산양삼 혼입숙성 감식초 섭취에 의한 흰쥐의 혈중지질 감소와 항산화능 개선 (Ripened Persimmon Vinegar with Mountain-cultivated Ginseng Ingestion Reduce Blood Lipids and Enhance Anti-oxidants Capacity in Rats)

  • 본 연구에서는 산양삼과 감식초의 융합소재에 의한 혈중 지질 및 항산화능을 분석하고자 하였다. 4년근 산양삼을 4년 숙성된 감식초에 혼입 숙성시킨 후, 5배 희석하여 쥐에게 경구투여하였다. 집단은 대조군(CON), 감식초군(PV), 산양삼+감식초 융합소재군(MPV)으로 각각 구분하였다. 체중은 MPV가 가장 낮게 나타났으며, 저장 지방량은 PV 및 MPV가 CON에 비하여 낮았다. 혈중 지질은 CON에 비하여 PV, MPV가 낮았으며, 고밀도지단백콜레스테롤은 높았다. 간의 Cu, Zn-SOD 및 GPx는 MPV>PV>CON의 순서로 나타났으며, 유의차가 있었다. 특히 MPV에서 가장 높았다. 간의 MDA는 CON, PV >MPV로 나타났으며, 유의차가 있었다. 이러한 결과를 볼 때, 산양삼과 감식초의 융합소재는 지질과산화를 효과적으로 억제하는 건강식품으로서 활용이 가능할 것이다.

유전자재조합 인간 골형성단백2 및 생흡수성고분자를 이용한 골형성유도체의 개발 (DEVELOPMENT OF BONE REGENERATING MATERIAL USING BONE MORPHOGENETIC PROTEIN(rhBMP-2) AND BIORESORBABLE POLYMER)

  • We tested the bone regenerating capacity and histologic response of bioresorbable matrix-type implant, which was made with Poly(lactide-co-glycolide)(PLGA) and bone apatite for the carrier of bone morphogenetic protein(BMP). The critical size defect of 8mm in diameter was created at the calvaria of SD rats(n=18), and repaired with polymer implant with $15{\mu}g$ of rhBMP-2(n=9) or without it(n=9). At 2 weeks, 1 months after implantation, the animals were sacrificed(3 animals at every interval and group) and histologically evaluated. The calvarial defect which was repaired with polymer with BMP healed with newly formed bone about 70% of total defect. But that without BMP showed only 0 to under 30% bony healing. Inflammatory response was absent in both group through the experimental period, but there's marked foreign body giant response though it was a little less significant in polymer with BMP group. As the polymer was resorbed, the space was infiltrated and replaced by fibrovascular tissue, not by bone. In conclusion, our formulation of bioresorbable matrix implant loaded with bone morphogenetic protein works good as a bone regenerating material. However, it is mandatory to devise our system to have better osteoinductive and osteoconductive property, and less multinucleated giant cell response.

힐링 공간디자인의 기초연구 (Basic Study of Space Design for Healing)

  • 힐링 공간의 디자인은 인간에게 기와 여유로움, 정신적 해방감을 제공한다. 지친 몸을 회복시키며 삶의 건장함을 제공해 준다. 힐링이란 의미와 외국의 사례 등을 통하여 힐링의 중요성과 가치를 확립하여 보았다. 인간이 자연에서 왔다가 자연으로 돌아가는 것은 지극히 필연적이라 생각되며, 살아가는 동안 목표를 세워서 조금씩 완성하고 극복해 나가는 과정을 끊임없이 계속하면서 큰 목표점에 도달하게 된다. 자연에서 온 만큼 자연의 분위기를 만들어 그 속에서 생활함으로써 치유의 효과를 누릴 수 있는 것이다. 건축의 내부에 목재를 사용하는 것이 가장 편안한 힐링의 조건이 되며, 일본의 사례에서도 알 수 있듯이 건축물의 외부 전체를 목재로 완성한 써니힐스의 건물은 마치 까치의 집을 연상케 하기도 한다. 실내의 책상이나 책장도 목재로 하여 보다 따뜻한 분위기를 연출하고 관엽식물을 심으면 힐링의 효과는 더욱 커진다. 천장에는 구름의 그림이나 나무, 해 등의 그림을 그리고 벽의 색은 기분을 차분하게 하는 파란색 계통이나 부드러운 핑크색을 적용하면 힐링의 효과는 배가된다. 본 연구를 통하여 힐링의 기본인 자연과 접하고 자연의 소재를 활용하여 생활공간을 형성하는 것이 힐링의 효과를 가져 올 수 있다는 것을 알 수 있었다.

편도와 접근을 통한 측방인두간극 이물제거의 치험 증례 (REMOVAL OF FOREIGN BODY IN THE LATERAL PHARYNGEAL SPACE VIA TRANSTONSILLAR APPROACH)

  • Lateral pharyngeal space is one of potential fascial planes of head and neck, that may become involved by various pathological processes, such as infection, inflammation and neoplasm. The calcified stylohyoid ligament with styloid process is also located in this space, so this space is more acquainted with Eagle's syndrome in oral and maxillofacial field. During the mandibular transbuccal fixation procedures of 29-year old female patient who had right condylar neck and left parasymphysis fracture, we had lost one 10.0 mm miniscrew. After confirming the location of the lost miniscrew from different angled plain skull radiographies, we tried to find it in the lateral pharyngeal space via transtonsillar approach at the time of plate removal operation. This case report is aimed to share our valuable experience of the effective approach way to the lateral pharyngeal space, which has many advantages, such as short operative time, minimal bleeding, fast post-operative recovery, and less morbidity. The related literature is also reviewed.

생선가시의 전산화 단층촬영 자료에 삼차원 재구성법을 적용한 연구 (Three Dimensional Reconstruction on Computed Tomograms of Fish Bones)

  • Background and Objectives This study was conducted to gather three dimensional images from computed tomogram in detecting and gaining information about fish bones. Materials and Methods Thirty-nine fish bones and 2 chicken leg bones were used. Bones sandwiched between the cotton sheets were placed into the 3 plastic boxes. Computed tomogram of the bones in the boxes were reconstructed three-dimensionally by Xelis (Infinitt, Korea) to make images of MPR, MIP and VR images. The images were compared with real chicken bones and fish bones as to possibility of detection, finding of location and orientation, and evaluation of shape and calculation of size. Results All 41 bones were detected in all reconstructed images. Distance to the bones from reference point can be measured and orientation can be checked. Shapes were similar between matched images of all real bones. Conclusion The results indicate the usefulness of 3D reconstruction technique in getting informations about fish bones.
