Clinical Efficacy of Gluteal Artery Perforator Flaps for Various Lumbosacral Defects

  • Purpose: Soft tissue defects in the lumbosacral area can be challenging to treat, and various methods to accomplish this have been proposed, including the use of perforator flaps. Herein, we present our experience with superior gluteal artery perforator (SGAP) and inferior gluteal artery perforator (IGAP) flaps for the reconstruction of lumbosacral defects. Materials and Methods: From March 2013 to July 2016, 28 cases (27 patients) of lumbosacral defects were treated by reconstruction with SGAP or IGAP flaps. The defects were caused by pressure sores (21 cases), burns (3 cases), tumor resection (2 cases), scars (1 case), or foreign body infection (1 case). Reliable perforators around the defect were found using Doppler ultrasound. The perforator flaps were elevated with a pulsatile perforator and rotated to cover the defects. Results: Twenty-three SGAP and 5 IGAP flap reconstructions were performed. The mean flap size was $9.2{\times}6.1cm^2$ (range, $5{\times}3cm^2$ to $16{\times}10cm^2$). Donor sites were closed by primary closure. Partial flap necrosis occurred in two cases, and minor complications of wound dehiscence occurred in 3 cases, which were healed by primary closure. The mean follow-up period was 4.4 months (range, 1~24 months). Conclusion: Gluteal-based perforator flaps can be safely harvested due to pliability and reliable vascularity in the gluteal area, reducing donor site morbidity without sacrificing the underlying muscles. Thus, these flaps are useful options for the reconstruction of lumbosacral defects.

Effect of Oral Water Intake to Globus Pharyngeus with Non-Organic Diseases (인후두 이물감을 호소하는 환자에서 경구 수분섭취가 증상 호전에 미치는 영향)

  • Background and Objectives : Globus pharyngeus is a sensation of foreign body in the throat. There are many studies evaluate relationships between globus pharyngeus and organic diseases such as laryngopharyngeal reflux, esophageal motility disorders as well as psychotic causes. But, Also many patients without etiologic causes complain of globus sensation. The authors performed a study that evaluate association between oral water intake and symptoms of globus sensation on the basis of a belief that pharyngeal dehydration due to lack of oral water intake causes globus sensation. Materials and Methods : A survey using visual analogue scale to evaluate symptoms was performed with 73 patients with globus pharyngeus. Results : Patients who drink water less than 500 mL per day (p=0.04) and less than five times per day (p=0.02) were improved after 6 months due to education and doctor's recommendation. Conclusion : Frequent and enough water intake should be recommended to resolve symptoms of globus sensation.

Related factors to dental care utilization and oral health status in immigrant workers in Korea (외국인 이주노동자의 구강건강수준과 치과 의료이용 관련요인)

  • Objectives: The purpose of the study is to investigate the related factors to dental care utilization, oral health behaviors, and oral health status in immigrant workers in Korea. Methods: The subjects were 504 foreign immigrant male workers over 20 years old who visited Daegu labor consultation center for oral health survey and oral examination. The questionnaire included 5 questions of socioeconomic characteristics, 8 questions of oral health practice behavior, 6 questions of dental clinic visit, 8 questions of social relations and Korean language proficiency. The question for health behavior was measure by body mass index(BMI). Social relations and Korean language proficiency instrument was modified by Seol from "Family welfare survey in Korean international marriage" and scored by Liker 5 scale. Results: The oral health examination of the immigrant workers was as follows: decayed teeth - 76.6%, filling teeth - 27.4%, missing teeth - 69.8%, dental caries experience above five or more - 60.2%, periodontal pocket tissues - 58.9%. Simplified Oral Hygiene Index was very poor and accounted for 49.0%. Dental care utilization experience was closely associated with social relation indexes including attendance in family events, household stuff help, financial help and counseling for hard work(p<0.01). Dental care utilization experience proportionally increased with proficiency in Korean literacy including speaking, listening, and writing abilities of Korean language(p<0.01). Conclusions: In order to improve the oral health condition of the immigrant workers, it is important to provide social network, Korean language proficiency support, and health insurance coverage through economic burden reduction by the Korean government.

  • Hwabyung is a culture-bound anger syndrome in Korea. It manifests as one or more of a wide range of physical symptoms, in response to emotional stress. This case describes a 26-year old woman diagnosed as Hwabyung by HBDIS. She complained chiefly of binge eating and sleep disorder. She also complained of chest discomfort, abdominal discomfort, foreign body sensation in the throat, burning sensation and so on. We treated the patient with herbal medication, acupuncture, cupping, and moxa. After treatment, her condition improved. Standardized patients (SP) were used in teaching and practicing interviewing and physical examination skills for This report suggests that traditional Korean treatment such as herbal medication, acupuncture, cupping, moxa is effective for the treatment of Hwabyung patients.

  • Recently, the smart health care industry has been rising rapidly and interest and efforts for public health have been greatly increased. As a result, the public does not visit medical specialists and medical facilities, but the desire to check their health condition in everyday life is increased. Therefore, many domestic and foreign companies continuously research and develop wearable devices that can measure body activity information anytime and anywhere And the market. Especially, it is used for heart activity measurement device using pulse wave sensor and electrocardiogram sensor. However, in this study, a monitoring system that can detect cardiac activity using cardiac sounds, heart sound measurement rather than pulse wave measurement and electrocardiogram measurement, was performed and its performance was evaluated. Experimental results confirmed the predictability of cardiac heart rate and heart valve disease during daily living.

  • Eagle's syndrome is a disease caused by an elongated styloid process or calcified stylohyoid ligament. Eagle defined the disorder in 1937 by describing clinical findings related to an elongated styloid process, which is one of the numerous causes of pain in the craniofacial and cervical region. The prevalence of individuals with this anatomic abnormality in the adult population is estimated to be 4% with 0.16% of these individuals reported to be symptomatic. Eagle's syndrome is usually characterized by neck, throat, or ear pain; pharyngeal foreign body sensation; dysphagia; pain upon head movement; and headache. The diagnosis of Eagle's syndrome must be made in association with data from the clinical history, physical examination, and imaging studies. Patients with increased symptom severity require surgical excision of the styloid process, which can be performed through an intraoral or an extraoral approach. Here, we report a rare case of stylohyoid ligament bilaterally elongated to more than 60 mm in a 51-year-old female. We did a surgery by extraoral approach and patient's symptom was improved.

The Effects of Calcium sulfate Membrane on the Periodontal Wound Repair of Horizontal Dehiscence defects in Dogs (성견 열개형 수평 결손부에서 Calcium Sulfate 차단막이 치주조직 치유에 미치는 영향)

  • The present study investigates the effects of calcium sulfate graft on the periodontal healing in intrabony periodontal defects of dogs. Following the general anesthesia with 30mg/kg pentobarbital injected intravenously, the first premolar was extracted and full-thickness periodontal flap was elevated from the second premolar to the fourth premolar. The portion of premolars coronal to the alveolar crest was removed and mesial and distal roots were separated. Exposed root canals were sealed with Caviton and covered completely with flaps sutured. Following the healing period of 12 weeks, the surgical sited were uncovered and $4{\times}4mm$ intrabony defects were surgically created. Those defects with calcium sulfate graft following the root planing was designated as the test sites and those with flap surgery-only were designated as control sites. The animals were sacrificed after 8 weeks and the healing was histologically analyzed. The results were as follows. 1. No foreign body reaction or inflammation were observed in either groups. Calcium sulfate was completely resorbed in the test sites. 2. New cementum was observed coronal to the notch in both groups. Connective tissue fibers were oriented parallel to the root surface in the controls. Connective tissues were formed in large amount in the sites. 3. Test sites showed marked amount of new bone formation while the control sites showed minimal bone gain. 4. Root resorption was observed in coronal portions of th control Sites. The results suggest that calcium sulfate is a biocompatible graft material with a potential for new bone and cementum formation.

Effect of poly(lactide-co-glycolide) (PLGA) on bone regeneration in rabbit calvaria (토끼 두개골 결손부의 골재생에 대한 poly(lactide-co-glycolide) (PLGA) 의 영향)

  • Purpose: The purpose of this study is to histologically and histomorphometrically evaluate the effect of PLGA on bone regeneration compared with bone graft material. Methods: The experimental study was conducted in 10 rabbits with 2 different healing periods of 2 and 4 weeks. Following surgical exposure of the calvarium, 4 circular bone defects with a diameter of 4.6mm were formed. Rabbits were divided into control group, test groups I, and II. 10 defects assigned to the test group Ⅰ were grafted with Nu-oss and other 10 defects assigned to the test group II were grafted with PLGA. The rest of the defects were in the negative control group. At 2nd and 4th week after surgery, 10 rabbits were sacrificed through intracardiac perfusion and then specimens were obtained. Histological analysis was performed following staining with trichorme and transversal sectioning of the calvarial bone. Results: A group which used PLGA showed tissue reactions characterized by severe inflammation, rather than distinctive new bone formation. Conclusions: The present experimental investigations have failed to prove any beneficial effects of PLGA. PLGA used in this study exhibited foreign body reactions and a less favorable pattern of new bone formation in comparison to control group. Conclusion: PLGA did not function as scaffold. Further investigations of many types of micro PLGA that could improve its potential in GBR procedures are needed.

Development of a Coping Scale for Emergency Situations with Elementary School Students (학령기 아동의 응급상황에 대한 대처 측정도구 개발)

  • Purpose: This study was conducted to develop an instrument to measure coping in emergency situations. Methods: The process included construction of a conceptual framework, generation 60 primary items, verification of construct validity and extraction of final items. The.60 preliminary items were reviewed for content validity by seven experts and were tested to evaluate inter-item correlation coefficient by three groups of elementary school students. From November 1 to December 10, 2008, data were collected from 920 elementary school students according to residential characters (major city, small town, rural area). Item analysis, factor analysis with rotation Varimax and Cronbach's alpha were used to analyze the data. Results: 1) There were 45 items in the final instrument categorized into 8 factors. 2) The factors were labeled as "Call for help and check" (8 items), "Care for common symptoms" (6 items), "Appropriate response for the situation" (5 items), "Evacuation" (6 items), "Care for tissue damage" (8 items), "Care for injury by heat, cold or foreign body" (6 items), "Offer help" (4 items) and "Rescue & escape" (2 items). 3) Cumulative percent of variance was 53.10% and eigen values ranged from 1.04 to 13.38. 4) Cronbach's alpha for the total was .943 and ranged from .527 to .869. Conclusion: Validity and reliability of the scale are confirmed in this study showing its utility for measuring coping in emergency situations with elementary school students. Utilization of the scale will also contribute to designing appropriate coping education programs for elementary school students.

Measuring Sport Tourist Motivation: Implications for Sport Tourism Distribution

  • Purpose - The purpose of this study was to develop a motivation scale for participation sport tourism and to produce implications of potential use of MSPST for sport tourism distribution emphasizing the needs of sport tourists and the functions required to satisfy those needs. Research design, data, and methodology - The Motivation Scale for Participation Sport Tourism (MSPST) was developed in three stages. A literature review generated 8 dimensions with 42-items in the first stage. Second, an expert review phase refined the initial item pool, which resulted in 35 items. Result - Exploratory factor analysis was employed to produce an 8-factor, 28 item pool. The reduced version was confirmed via structural equation modeling, indicating an acceptable model of fit. The final MSPST consisted of 8 dimensions of motivation, including friendship, family, solitude, challenges, intrinsic, achievement, nature, and competition. Conclusions - The MSPST is a valid and reliable scale of tourists' motives for participating in sports. The results supported the suggested measures of motives associated with participation sport tourism regarding construct, convergent and discriminant validity. A body of knowledge about motives provides insights for policy-makers seeking to support distributional industries for sport tourism and finally to promote economy on both regional and national levels.
