Use of elevator instruments when luxating and extracting teeth in dentistry: clinical techniques

  • In dentistry, elevator instruments are used to luxate teeth, and this technique imparts forces to tooth particles that sever the periodontal ligament around tooth roots inside the socket and expand alveolar bone around tooth particles. These effects can result in extraction of the tooth particles or facilitate systematic forceps extraction of the tooth particles. This article presents basic oral surgery techniques for applying elevators to luxate teeth. Determination of the optimal luxation technique requires understanding of the functions of the straight elevator and the Cryer elevator, the concept of purchase points, how the design elements of elevator working ends and tips influence the functionality of an elevator, application of forces to tooth particles, sectioning teeth at furcations, and bone removal to facilitate luxation. The effectiveness of tooth particle luxation is influenced by elevator tip shape and size, the magnitude and the vectors of forces applied to the tooth particle by the tip, and sectioning and bone removal within the operating field. Controlled extraction procedures are facilitated by a dental operating microscope or the magnification of binocular surgical loupes telescopes, combined with co-axial illumination.

굴곡성 기관지경을 이용한 성인의 기도내 이물 제거 (Fiberoptic Bronchoscopy for Removal of Endobronchial Foreign Bodies in Adults)

  • Foreign body aspiration occurs uncommonly in adults. A review of recent reports reveals only a few case reports on this topic. We have experienced 8 cases of endobronchial foreign bodies in adults from June 1988 to February 1991 which were removed successfully with fiberoptic bronchoscope and biopsy forceps. Only one of them had the primary disorder predisposing aspiration. Foreign bodies were located in right lower lobe (3 cases), right intermediate (2 cases), left lower lobe (2 cases) and left upper lobe bronchus (1 cases). Removal of endobronchial foreign boides in adult would be accomplished by fiberoptic bronchoscopy successfully.

기도내 이물의 외과적 처치에 관한 연구 (The Surgical Management of Aspirated Foreign Bodies in Airways)

  • The inhalation of a foreign body into the tracheobronchial tree is a child is a life-threatening accident. Forgotten foreign bodies In the airways cause chronic pulmonary infections, allergic asthma, bronchiectatic changes, and lung abscess Foreign bodies that cannot be grasped by bronchoscopic forceps should be removed by thoracotomy and bronchotomy. This report describes our experience In 16 patients who were treated to remove inhaled foreign bodies from the airways after repeated bronchoscopies had been unsuccessful from 1963 to 1981 at the department thoracic surgery of Seoul National University Hospital. The surgical procedures are as follows: 12 patients bronchotomy, 2 patient pneumonectomy, 2 patients lobectomy. In these cases, the foreign bodies are 8 metallic material, 3 plastic material, 1 bean, 1 black snail, and 3 cases of no records. Bronchotomy must be performed as soon as possible in order to avoid more advanced pathologic changes In the Involved lung, such as bronchiectasis, fibrosis or abscess.

An Improved Technique for Pulmonary Endarterectomy

  • We report a modified technique for pulmonary endarterectomy (PEA) on a 67-year-old man with chronic thromboembolic pulmonary hypertension (CTEPH) who presented with dyspnea. He was referred to our medical center for coronary artery bypass grafting. CTEPH had not been detected in his first visit to another medical center, but upon re-evaluation, the diagnosis was confirmed. PEA was performed with a modified method, which seems to be safe and suitable for the removal of clot and fibrotic materials. Iatrogenic dissection was performed with normal saline injection in the pulmonary artery, and then, the clot was removed completely. Although the technique may not be applicable for all cases, it can be used as an alternative to using an aspirating dissector and a pair of forceps.

  • 기도 이물은 유아나 소아에서 특히 많으며 성문으로 들어갈 수 있는 크기라면 우리 주위에 있는 어떤 물체라도 이물이 될 수 있으며 따라서 기도 이들의 종류도 다종 다양하다. 그리고 특수하게 생긴 이물의 개재부위가 다양해지는 것을 감안할 때 우리는 판자 치료에 있어서 획일적이고 동일하게 처리될 수 없음은 알 수 있다. 저자는 최근에 유아 및 소아에서 후두의 이물로 생선뼈 1례, 기관지 부분에서 걸린 피리 1례 및 기관 이물로 철사 1례를 경험하였기에 문헌적 고찰과 아울러 보고하는 바이다.

A Case of Unexpected Clinostomum complanatum Infection Initially Presenting as Foreign Body in Pharynx

  • A 46-year old man visited our outpatient clinic with complaint of foreign body sensation in throat after consuming raw freshwater fish 5 days ago. Laryngoscopic examination revealed a motile worm attached on posterior pharyngeal wall. The worm was removed using biopsy forceps under transnasal endoscopy and evidently identified as Clinostomum complanatum after microscopic examination. Patient's subjective foreign body sensation of throat and hyperemia of laryngeal mucosa remained for approximately 2 weeks post-removal, which were eventually resolved after administration of non-steroidal anti-inflammatory drug and anti-refluxant drug for 2 weeks. Treatment was ended at three weeks since the first visit. C. complanatum infections in humans are rare, and only four cases have been reported in Korea. Symptoms resembling pharyngitis or laryngitis occurs by consumption of raw, infected freshwater fish and treatment is done by mechanically removing the parasite.

Foreign body aspiration during dental treatment under general anesthesia: A case report

  • Foreign body aspiration in dental clinics is the most common cause of respiratory emergencies. There are no reports on foreign body aspiration during dental treatment under stable general anesthesia because the patient neither has voluntary movements nor reflex actions. This is a case report on the fall of a prosthesis in the larynx, which occurs rarely under general anesthesia. During the try-in procedure, the prosthesis slid from the surgeon's hand and entered the retromylohyoid space, and while searching for it, it passed down the larynx to the endotracheal tube balloon, leading to a dangerous situation. The prosthesis was promptly removed using video-assisted laryngoscope and forceps, and the patient was discharged without any complications.

  • Airway foreign body aspiration in children can lead to accidental death, due to the foreign body itself or the removal procedure. Depending on its location, removal of the foreign body can be challenging. Here, we present a case of successful removal of a foreign body from the left upper lobar bronchus via ventilating bronchoscopy with a rigid bronchoscope and Fogarty arterial embolectomy catheter. Tracheobronchial foreign bodies in locations that are difficult to reach with forceps, due to an acute angle or the small diameter of the pediatric bronchial tree, can be effectively removed with a Fogarty arterial embolectomy catheter.

  • Various techniques are well documented to obtain anatomic reduction, such as reduction forceps, manual reduction, or a combination of these methods. However, these techniques have inherent drawbacks. We propose a new intra-operative technique for anatomic reduction using screw-wire traction for open reduction and internal fixation of mandibular fractures.

  • 1년 6개월령의 수컷 비글견이 3일간의 연하통증과 식욕부진으로 내원하였다. 환축은 병력청취와 방사선학적 검사결과 낚시바늘에 의한 식도와 위내 이물로 진단되었다. 위식도 내시경을 시행한 결과 하나의 낚시바늘은 흉부식도의 심장기저 부에 나머지 하나의 낚시바늘은 위 분문부에 위치하고 있었다. 그리고 두 낚시바늘은 하나의 낚싯줄로 서로 연결되어 있었다. 연결된 낚싯줄을 끊고 위내 낚시바늘을 제거하기 위해 위 절개술을 시행하였다. 위 절개술 후 식도내 바늘을 제거하기 위해 세 갈래, 다섯 갈래 내시경 포셉과 생검 포셉을 이용한 내시경적 제거를 시도하였으나 낚시바늘이 식도의 점막에 깊게 박혀있어 견인에 실패하였다. 낚시바늘을 더욱 단단히 잡아줄 수 있는 장치를 위해 정형외과용 스테인리스 와이어(16G)를 올가미 포셉 형태로 제작한 뒤 내시경을 통한 시야가 확보된 상태에서 바늘에 걸어 제거하는데 성공하였다. 시술 10일 후 환축은 회복되어 퇴원조치 되었다.
