Purpose: To evaluate the clinical outcomes and radiographic results of open reduction and internal fixation for intraarticular calcaneal fractures. Materials and Methods: We reviewed 20 cases of calcaneal fractures managed with open reduction and internal fixation from March 2003 to January 2005. We used the computed tomographic classification system proposed by Sanders et al to classify these fractures. Preoperative and postoperative Bohler's angle, heel height (calcaneal facet height) and calcaneal length, calcaneal width were measured. The Creighton-Nebraska Health Foundation Assessment score was used for clinical evaluation. Results: There were 12 cases of type II fractures, 5 of type III fractures and 3 of type IV fractures. The mean clinical score was 84.3 for type II, 82.6 for type III and 56.1 for type IV. The mean preoperative $B{\ddot{o}}hler$ angle was $6.1^{\circ}$ and final was $22.8^{\circ}$. The mean preoperative calcaneal facet height was 76.6 mm and final was 80.3 mm (The mean calcaneal facet height was changed from preop 76.6 mm to postop 80.3 mm). The mean preoperative calcaneal length was 88.2 mm and final was 92.6 mm. The mean preoperative width was 38.1 mm and final was 35.6 mm. Conclusion: Open reduction and internal fixation showed good results for type II and III fractures, but for type IV fractures the clinical result was significantly worse than the other types. However, type IV fractures still had restoration of (should be restored in) $B{\ddot{o}}hler's$ angle, calcaneal facet height, calcaneal length and width which may be helpful in later subtalar fusion.
본 연구는 6년 이상의 유도경력 소유자인 Y.대학교 유도전공자를 대상으로 맞잡기 타입과 받기신장에 따른 허벅다리걸기 동작시 3차원 영상분석법을 이용하여 운동학적 변인의 특성을 분석하는데 그 목적이 있다. 유도 맞잡기 타입과 받기신장에 따른 허벅다리걸기 동작은 비디오카메라 2대를 사용하여 촬영(30frame/sec.)하였으며, 분석을 위한 운동학적 변인들은 시간, 자세, 중심변인이며, 실험 결과에 대한 분석과 논의를 통하여 다음과 같은 결론 및 제언을 얻을 수 있었다. 허벅다리걸기 동작의 맞잡기 타입에 따른 유의한 차이는 3국면 소요시간, 걸기1단계 공격둔부각, 걸기1단계 공격무릎각, 걸기2단계 공격무릎각, 걸기2단계 지지무릎각, 걸기1단계 공격발목각, 걸기2단계 공격발목각, 걸기2단계 지지발목각에서 유의한 차이(p<.05)를 보였으며, 타입에 따른 걸기2단계 지지둔부각에서만 신장이 큰 받기에 대해서 A, B타입 모두 유의한 차이(p<.05)를 보였다. 종합해보면, 허벅다리걸기를 수행할 때 맞잡기와 상대신장이 허벅다리걸기 기술에 영향을 미친다는 것을 알 수 있었으며, 상대의 신장이 작을 경우 목 뒤를 잡는 타입 A가 시간이 단축(<.05)되었다. 따라서 경기 중 상대의 신장이 작다면 목 뒤를 잡는 것이 유리하다. 이러한 사실들을 종합해 볼 때, 유도 경기 중 허벅다리걸기를 수행할 대 맞잡기가 많은 영향을 주었다. 또한 기술의 핵심단계인 3(걸기)국면에서도 받기신장에 의한 차가 더 크게 나타나 받기신장 역시 많은 영향을 주었다. 단계별 변인들의 변화량에 있어서는 전체적으로 모든 변인들이 변화량에서 큰 차이를 나타내지는 않았다고 할 수 있다. 따라서 맞잡기와 받기신장은 상당한 관계가 있음을 알 수 있었으며 유도 경기 중 상대에 따른 다양한 잡기와 신장을 고려한 기술이 요구된다.
본 논문은 족하수 환자를 대강으로 휴대가 간편하고 사용이 편리한 피드백 제어형 전기자극기를 제안하는 데 그 목적이 있다. 이 시스템은 교통사고, 산업재해, 뇌졸중 등으로 인하여 하지마비를 동반한 환자에서 마비된 하지근의 위축 및 괴사를 방지하고 혈액순환을 증가시킬 뿐만아니라 보행을 보조하여 안정된 활동을 가능케 한다 피드백 제어형 전기자극기는 환자가 오랜 시간 보행시 전극의 임피던스 변화나 근피로에 의해 족하수증이 다시 발생하게 되면 관절각의 변화가 관절각 센서에 의해 검출되고 이때의 오차는 PID 제어에 의한 자극전압의 변화를 가져와 설정된 위치에 발목이 도달하도륵 자극레벨이 자동 조절된다. 자극조건은 자극주파수 40 Hz, 자극펄스 0.2 ms, 30∼80 V이다. 족하수 환자 5명을 대상으로 42일 동안 첫째 1주일 동안에는 하루에 15분을 자극하고, 다음 주기는 1주일 단위로 하루에 30분, 60분, 70분, 120분씩 점차적으로 시간을 증가하면서 자극하였다. 이때 근력은 27.7%가 향상되었고, 근 피로도는 22.9%가 감소되었다. 이런 결과를 종합할 때 피드백 제어형 전기자극기는 족하수 환자의 보행기능 및 운동기능을 회복하는 데 효과적이라 하겠다.
Purpose: The purpose of this study is to analyze the clinical and radiological results of minimally invasive percutaneous screw fixation in intra-articular calcaneal fractures and its complications. Materials and Methods: This study is based on 30 intra-articular calcaneal fractures that treated by index operation from June 2005 to November 2006 with at least 6 months follow-up. We assessed the clinical and radiological outcomes and complications. Results: According to Sanders classification, there were 8 in type IIA, 7 in type IIB, 3 in type IIIAB, 6 in type IIIBC, 6 in type IV. And according to Essex-Lopresti classification, there were 9 in the tongue type, 21 in the joint depression type. Average follow-up period was 14.6 months (range: 6-23 months). Average interval between from injury to operation was 2.3 days. Average AOFAS score was 87.7 (range: 52-92). Satisfactory results were obtained in 22 cases (73.3%) by AOFAS score and in 20 cases (66.7%) by VAS score (mean: 3.4). Radiological results improved from 8.7 to 20.3 degrees in the Bohler angle and from 40.2 mm to 52.1 mm in calcaneal height. Postoperative complications were 2 skin and soft tissue problems and 1 sural neuropathy. Conclusion: Minimally invasive percutaneous screw fixation may be useful alternative surgical method in the management of Sanders type II and III calcaneal fractures, which is possible to achieves the anatomical restoration and minimizes postoperative complication in patients with high risks of soft tissue compromise and allows relatively early operation.
Purpose: The purpose of this study was to evaluate the effect of web-based diabetic education on glycosylated hemoglobin(HbAlc) and self-care in people with type 2 diabetes. Method: Twenty-five patients were requested to input the blood glucose level weekly at http://www.biodang.com by cellular phone or wired Internet for one year. The researcher sent optimal recommendations weekly to each patient using the short message service for cellular phones and wired Internet. Results: Patients had a significant mean decrease in HbAlc, from 8.0% at pre-test to 6.9%, after three months and rested at 6.8% until 12 months. Patients had a significant mean increase in physical exercise adherence, from 3.3 days at pre-test to 4.1 days per week, after three months. Patients had a significant mean increase in foot care adherence, from 4.8 days at pre-test to 6.9 days per week, after one year. Conclusion: These findings indicate that Internet education could be used to improve HbAlc, physical exercise, and foot care adherence in type 2 diabetic patients.
Purpose: To evaluate the clinical results and determine appropriate methods of surgical treatment about type II talar neck fracture. Materials and Methods: Among nineteen patients who received surgical treatment for type II talar neck fracture from May 2000 to May 2005. Fourteen patients with a follow-up period of more than 1 year were divided into two groups. Six patients reduced by closed reduction (Group A) with screw fixation and eight patients reduced by open reduction with screw fixation. We analyzed preoperative, postoperative and follow-up simple radiographs and reviewed patient hospital records retrospectively. Clinical results were evaluated by Hawkins scoring system. We analyzed pain, limp, range of motion of ankle and subtalar joint. Results: Five patients (83.3%) in group A and seven patients (87.7%) in group B had excellent and good clinical results. There were no complications including avascular necrosis, delayed union, nonunion. Conclusion: Closed reduction with screw fixation of talar neck fracture shows correct reduction and satisfactory results. But because of short term period of follow-up, we need long term results.
Purpose: This study was performed retrospectively and to compare the clinical outcomes of the surgical management using Antiglide plating and lateral plating for Danis-Weber type B isolated lateral malleolar fractures. Materials and Methods: Between March 2004 and March 2009, 92 patients were followed up for more than 6 months after surgical treatment. 49 patients were treated with Antiglide plating, whereas the remaining 43 patients underwent lateral plating. The results of the two groups were analyzed both radiographically and clinically using Ankle scoring system. Results: Both groups were comparable for age, sex distribution, time to surgery, and operation time. Bone union was noted at average of 8 weeks in all fractures. The mean ankle score was similar in both groups (94% versus 93%). Conclusion: Antiglide plating for Danis-Weber type B isolated lateral malleolar fracture is one of the good methods which has some advantages such as early ankle motion, ambulation and smaller operative incision.
Purpose: To evaluate the results of open reduction and internal fixation for displaced talar neck fracture. Materials and Methods: Fourteen patients were followed for more than 1 year after open reduction and internal fixation of displaced talar neck fracture. The mean age was 41.5 years, and the mean follow-up period was 3.8 years. There were 10 type II, 3 type III and 1 type IV fractures per Hawkins. There were 3 open fractures. Clinically AOFAS ankle-hindfoot scale and Hawkins criteria were utilized. Radiologically quality of reduction, duration of bony union, avascular necrosis and posttraumatic arthritis were evaluated. Results: At last follow-up, AOFAS scale was mean 89.1 points. There were 8 excellent, 4 good, and 2 fair results according to Hawkins criteria. Radiologically anatomical reduction was obtained in 13 cases. Mean duration of bony union was 11.8 weeks. There were 3 avascular necrosis and 2 post-traumatic arthritis. There was no significant difference in the incidence of avascular necrosis between early operation group and surgically delayed group. Conclusion: Accurate open reduction and rigid internal fixation seem to be prerequisites for satisfactory treatment of a displaced talar neck fracture.
The aim of this study was to investigate the effect of complex Korean medicine treatment especially Buja-tang and Aconitum ciliare decaisne pharmacopuncture on patients with type 1 complex regional pain syndrome caused after a traffic accident. The patients was treated with Buja-tang, Aconitum ciliare decaisne pharmacupuncture, acupuncture and physical therapy during 20 days of hospitalization. Numerical rating scale (NRS), short form McGill pain questionnaire (SF-MPQ), body temperature measurement of the foot, manual muscle test (MMT) were used for assessment. After treatments, NRS of burning pain in the left ankle decreased from 8 to 4-5 and NRS of burning pain in the right ankle decreased from 7 to 4-5. Also, SF-MPQ results, foot temperature measurement and MMT showed improvement during hospitalization. This study shows that complex Korean medicine treatment especially Buja-tang and Aconitum ciliare decaisne pharmacopuncture are effective in alleviating symptoms in patients with type 1 complex regional pain syndrome.
Fractures located at the metaphyseal/diaphyseal junction at the base of the fifth metatarsal were first described by Sir Robert Jones in 1902. However, ever since, there has been disagreement and debate regarding the diagnosis, classification, pathomechanics, the incidences, and potential causes of delayed unions and nonunions, and the optimal method of treatment. It appears to be widely agreed that proximal fractures of the metaphyseal/diaphyseal region of the fifth metatarsal are prone to delayed union or even nonunion. Several classifications of proximal fifth metatarsal stress fractures have been devised. Torg et al. classified fractures involving the proximal part of the diaphysis of the fifth metatarsal into three types. The Torg classification is a good grading system that can be used to determine the type of surgery needed as well as for the prediction of prognosis. The ''plantar gap'' might add to the decision-making process for surgery and improve the prediction of patient prognosis. In addition, the new classification using 'plantar gap' might be used for classification of fifth metatarsal stress fracture. Fifth metatarsal stress fractures can be treated conservatively or surgically, and excellent results have been reported for surgery with rapid recovery in athletes. Intramedullary screw fixation has become a popular form of fixation for fifth metatarsal stress fractures. Bone grafting presents the problems of a longer recovery time and additional skin incision for harvesting. The modified tension band wiring is an useful and simple option for surgical treatment of challenging fifth metatarsal stress fractures.
