  • This case study showed improvement both in pain and gait disturbance caused by SCH. KM can be a positive treatment for reducing pain or improving gait disturbance caused by SCH, and it can be considered a treatment option for SCH. Future studies with a larger number of cases and longer period of follow-up in SCH with gait disturbance and rigidity are needed.

The Effects of c-Fos Expression on Ultrasound Treatment in Sciatic Nerve Crush Damaged Rats (초음파 치료가 좌골신경 압좌 손상된 흰쥐의 c-Fos 발현에 미치는 영향)

  • This study was performed to evaluate the effects of low-intensity ultrasound application to the peripheral nerve injury animal model on enhancement of nerve regeneration and functional recovery. Using aseptic microsurgical techniques, the sciatic nerve of adult male Sprague-Dawley rats was crushed at the outside of right mid-thigh for 30 seconds with fine forceps. Beginning just after surgery, various continuous-wave ultrasound treatments with intensities of 0.2 W/$cm^2$, 0.5 W /$cm^2$ and 1.0 W /$cm^2$ operated at 1 MHz or sham treatment were applied to the opposite inside of the crush site for 1 minute every other day with a transducer moving speed of 2cm/sec. For evaluation of the progress of sciatic nerve regeneration, c-Fos expression in the lumbar spinal cord (L4-5) dorsal horn was investigated. c-fos expression was markedly increased at 1hour after sciatic nerve crush injury, then gradually decreased thereafter. The c-fos expressions were significantly decreased (p<0.05) in all the experimental groups in comparison with the control group until 3days post-crush, and the degrees of decrease were higher in 0.5 W/$cm^2$ and 1 W/$cm^2$ intensity ultrasound application groups. It is suggested that low-intensity ultrasound application to an animal model of sciatic crush injury may suppress pain transmission and promote nerve regeneration, and which may result in delayed progress of muscle atrophy and accelerated progress of muscle recovery and eventually may result in accelerated and improved foot function recovery.

  • The purpose of this study was to compare and analyze kinetic variables of lower limbs according to types of ankle taping in drop landing. For this, targeting seven male basketball players (average age: $20.8{\pm}0.74yrs$, average height: $187.4{\pm}3.92cm$, average weight: $79.8{\pm}7.62kg$) with no instability of ankle joints, the drop landing motion was conducted according to three types of inelastic taping (C-type), elastic taping (K-type), and no treatment (N-taping). Based on the result, the next conclusion was reached. First, the effect of taping for the players with stable ankles was minimal and the high load on ankle joints offset the fixing effect of inelastic taping. Thus the inelastic taping for the players with stable ankles did not have an effect on the control of dorsal flexion during one-foot landing. Second, increasing angular velocity by increasing the movable range of knee joints disperses impact forces, yet inelastic taping restricted the range of knee joint motion and at the same time increased angular velocity, adding to a negative effect on knee joints. Third, inelastic taping induced inefficient motion of Lower limbs and unstable impact force control of ankle joints at the moment of landing and produced maximum vertical ground reaction force, which led to an increase of load. Therefore, inelastic ankle taping of players whose jump actions occur very often should be reconsidered. Also, it is thought that this study has a great meaning in proving the problem of inelastic taping related to knee pain with unknown causes.

  • Objectives : This study is designed in order to evaluate oriental medical treatment of deveation of the eye and mouth caused trauma. Methods : The authors observed patient by Yanagihara's unweighted grading system for operated acupuncture treatment, herbal medicine treatment and physiotherapy. Conclusion : 1. Deveation of the eye and mouth is caused by trauma ; intra cranial trauma, intra temporal bone trauma, extra, temporal bone trauma, etc. Cardinal symptom is palsy of Facial muscle, slobbering, articulation disorder, epiphora, ear pain, hyperacusis, laterality hypogeusia. 2. Deveation of the eye and mouth patient by Lt. temporal bone Fx. is seen evaluate of Yanagihara's total score ; from S to 35. 3. Deveation of the eye and mouth patient by facial nerve inhury is seen evaluate of Yanagihara's total score ; from 10 to 30. 4. Traumatic Deveation of the eye and mouth patient evaluate by oriental medical treatmend ; acupuncture treatment, herbal medicine treatment and physiotherapy. This is based on sil(賞) of stomach channel of foot yangming & larhe intestine channel of hand Yangming.

A Case Report of Osteochondral Lesions of the Left Talus after Ankle Sprain Treated with Complex Traditional Korean Medical Care (염좌로 유발된 좌측 거골 골연골 병변 1례에 대한 한방 복합치료 증례 보고)

  • Objectives : The purpose of this report is to introduce a case of osteochondral lesions of the left talus caused by ankle sprain and suggest complex traditional Korean medical care with warm needling therapy as a possible method of conservative treatment. Methods : A 20-year-old male with osteochondral lesions of the left talus complaining of ankle pain and restricted range of movement was treated mainly with warm needling therapy and other combined Korean medicine therapy from January 5, 2015 to January 12, 2015. Improvements of symptoms were measured by American Orthopaedic Foot & Ankle Society Score (AOFAS Score), Visual Analog Scale (VAS) and Range of movement (ROM). Results : During 8 days of treatment, the patient showed gradual improvement in AOFAS score, VAS at rest and at weight bearing position, as well as ROM of the ankle. Conclusion : According to the result, warm needling therapy is a possible conservative treatment for osteochondral lesions of the talus. Further studies are needed regarding possible longterm effects.

Secondary adrenal insufficiency caused by sorafenib administration in a patient with hepatocellular carcinoma

  • Sorafenib (Nexavar) has been regarded as a treatment for unresectable hepatocellular carcinoma (HCC), with side effects that include hand-foot skin reaction, diarrhea, rash, fatigue, hypertension, nausea, anorexia, weight loss, and alopecia. Thyroid disorder, such as endocrine side effect, has also been reported. However no case involving adrenal insufficiency has been reported. Here, we report a case of adrenal insufficiency which occurred after taking sorafenib in a patient with HCC. A 56-year-old man visited our hospital due to right upper quadrant abdominal pain and he was diagnosed as multiple disseminated and unresectable HCCs with portal vein invasion; therefore transarterial chemoembolization was performed and sorafenib administration was started. Two months later, he was admitted to the hospital complaining of severe fatigue. The laboratory results showed cortisol of <$0.2{\mu}g/dL$ and adrenocorticotropic hormone of <1.00 pg/mL. The patient had no history of taking steroids or herbal medications. Secondary adrenal insufficiency was diagnosed and prednisolone 10 mg per day was started immediately; as a result, fatigue remarkably improved. This may be the first report indicating a possible association between sorafenib and adrenal insufficiency and it implies that the possibility of adrenal insufficiency should be considered in patients taking sorafenib who complain of severe fatigue.

Anti-nociceptive and anti-inflammatory effects of Geranii Herba (현지초(玄之草)의 진통(鎭痛) 및 항염증(抗炎症) 효과)

  • Objectives : The present study investigated the anti-nociceptive and anti-inflammatory properties of the water extract of Geranii Herba (The stem and leaves of Geranium thunbergii Sieb. et Zucc.) in the animal models of pain and inflammation. Methods : We evaluated the anti-nociceptive and anti-inflammatory activities of Geranii Herba extract (GHE) using the writhing test, tail-flick test, carrageenan-induced paw edema and xylene-induced ear edema models. Two dose of GHE (100 and 1000 mg/kg) was administrated orally to the mice. Control group received normal saline and ibuprofen (50 mg/kg) was used as a positive control drug. Results : GHE 1000 mg/kg treated group showed an increased tail-flick response time in the tail-flick test and inhibitory effect on writhing syndrome induced by acetic acid. Treatment with GHE at the same dose inhibited ear edema induced by xylene and foot edema induced by carrageenan toxicity. Conclusion : The results demonstrate that GHE has anti-nociceptive and anti-inflammatory effects in the various models of nociception and inflammation.

An Analysis of Research Related to the Massage as the Nursing Intervention (간호중재로서의 마사지 적용연구 분석)

  • Purpose: The purpose of this study was to identified patterns of studies and to analyze the nursing research related to the massage intervention in Korea and to improve direction of nursing research in massage intervention in Korea. Methods: The research studies related to the massage were selected from the Korean Nurses Academic Society Journal and dissertation which were conducted experimental research design the last 7 years in Korea. The total numbers of the studies were 58. Result: The study subjects were post operational state in 34.5%, healthy adult in 17.2% studies. The others dealt with infant, cancer, dementia, hypertension, preterm neonates, elderly. Types of massage were whole body in 31.1%, foot in 22.6%, hand in 17.2%, back in 13.8% studies. And Meridian massage in 17.2%. Length of massage was below 5 minutes in 17.2%, 6~10 minutes in 29.4% and 11~20 minutes in 32.8% studies. Frequency of massage was only 1 time in 25.9%, 2~5 times in 31.1% studies. Massage produced psychological and physiological effects. Positive psychologic effects were anxiety relief, pain relief, mood enhancement and fatigue relief. Positive physiologic effects on body systems were blood pressure, heart rate, edema, skin temperature, immune system, blood glucose, excretion, PMS syndrome and growth and development. Conclusion: As a result, effects of massage were almost positive. Therefore massage is useful alternative nursing intervention. But more attention research is needed to identify the objectivity instrument to measure and repeat research.

Retethering : A Neurosurgical Viewpoint

  • During the follow-up period after surgery for spinal dysraphism, a certain portion of patients show neurological deterioration and its secondary phenomena, such as motor, sensory or sphincter changes, foot and spinal deformities, pain, and spasticity. These clinical manifestations are caused by tethering effects on the neural structures at the site of previous operation. The widespread recognition of retethering drew the attention of medical professionals of various specialties because of its incidence, which is not low when surveillance is adequate, and its progressive nature. This article reviews the literature on the incidence and timing of deterioration, predisposing factors for retethering, clinical manifestations, diagnosis, surgical treatment and its complications, clinical outcomes, prognostic factors after retethering surgery and preventive measures of retethering. Current practice and opinions of Seoul National University Children's Hospital team were added in some parts. The literature shows a wide range of data regarding the incidence, rate and degree of surgical complications and long-term outcomes. The method of prevention is still one of the main topics of this entity. Although alternatives such as spinal column shortening were introduced, re-untethering by conventional surgical methods remains the current main management tool. Re-untethering surgery is a much more difficult task than primary untethering surgery. Updated publications include strong skepticism on re-untethering surgery in a certain group of patients, though it is from a minority of research groups. For all of the abovementioned reasons, new information and ideas on the early diagnosis, treatment and prevention of retethering are critically necessary in this era.

Development of Wearable Body Weight Support System to Reduce Muscle Activity in Various Upright Tasks (다양한 직립 작업의 근육 활성도 경감을 위한 착용형 체중지지 시스템 개발)

  • While working in an industrial environment which requires extended periods of upright posture; workers tend to develop muscle fatigue due to the constant load on lower-limb muscles. In addition, when working while bending knees; muscle fatigue of lower back and hamstrings is increased due to the abnormal posture. This can lead to damage of muscles, induce musculoskeletal disorders, and reduce long-term working efficiency. Recent medical studies have shown that long-term working in an upright posture can induce musculoskeletal disorders such as foot fatigue, edema, pain and varicose veins. Likewise, medical and rehabilitation expenses have grown due to the increase in musculoskeletal conditions suffered by workers. For this problem, we aim to develop a device that can reduce the physical fatigue on the lower limbs by supporting the weight of workers during the extended periods of upright and bending postures in the industrial environments. In this paper, we have designed and manufactured a wearable weight support system; with a user intention algorithm that the users can maintain various postures. For validation of the developed system, we measured the muscle activity of the users wearing the system with EMG sensors.
