The Effect of the Streamlined Shoe on Dynamic Gait Change and Foot Plantar Pressure in Healthy Young Adults

  • Objective: This study compared the effects of streamlined shoes on dynamic gait and foot plantar pressure in healthy young adults. Background: With the importance of ankle and lower extremity mechanism, streamlined shoes are contributing to a static gait factors. However, the study of dynamic gait factor is still insufficient. Method: Sixty subjects were randomly allocated to two groups: experimental group(n=30) and control(n=30), respectively. The experimental group performed streamlined shoes, whereas the control group applied usual shoes. Main outcome measurements were assessed contact time, step length and foot plantar pressure using gait analysis with the treadmill. Independent t-test was used to compare the both groups. Results: Compared with control group, contact time of forefoot, midfoot and hindfoot decreased significantly in experimental group(p<.05), Step length increased significantly in experimental group(p<.05). Foot plantar pressure of midfoot and hindfoot increased significantly in experimental group(p<.05), whereas that of forefoot did not show significantly in experimental group(p>.05). Conclusion: Our findings suggest that streamlined shoes was more effective than usual shoes in dynamic gait change including contact time and step length and foot plantar pressure in healthy young adults. Application: The results of streamlined shoes might help to control for the gait of industrial workers.

Do N37 and P37 Potentials Have Different Generators in Somatosensory Evoked Potential? - Analysis Using Gating Mechanism - (체성감각 유발전위에서 N37과 P37은 다른 발생기를 가지고 있는가? - gating 현상을 이용한 분석 -)

  • Backgroud : The generators of N37 and P37 of posterior tibial nerve somatosensory evoked potential(PTSEP) have not been exactly known. Recently, some reports suggested that P37 and N37 might have different generator. We conducted a study to know the generators of P37 and N37 of PTSEP using gating mechanism. Methods : We evaluated subcortical and cortical somatosensoy evoked potentials(SEPs) in response to posterior tibial nerve stimulation in 3 experimental conditions of foot movement and compared them with PTSEPs in full relaxation of foot. The experimental conditions were: (a) active flexion-extention of stimulated foot, (b) isometric contraction of the stimulated foot, (c) passive flexion-extention of the stimulate foot. We analyzed the latencies and amplitudes of following potentials; P30, N37, P37, and N50. Results : The amplitude of P30 potential did not change during at any paradigms. The amplitudes of P37 and N50 were significantly attenuated in all condition. However, the amplitude of N37 showed no significant change during at any paradigms. Conclusions : These results suggest that the generators of P37 and N37 of PTSEP be different in cortex.

A Study on Cosmetic Acupuncture Through Anatomy and Physiology Interpretation (해부생리학 해석을 통한 미용침의 연구)

  • Objectives : The purpose of this study is to investigate the mechanism of Cosmetic Acupuncture through reinterpretation of anatomy and physiology. Methods : The causes of wrinkle increases and rapid aging of facial skin were studied and the theoretical system of Cosmetic Acupuncture treatment was analyzed through anatomy and physiology reinterpretation. Results and Conclusions : An increase in wrinkles and rapid aging of facial skin is caused by xerosis. Skin condition represents the condition of subcutaneous muscle. The reason why skin becomes easily dry is the heat produced by craniofacial part. Craniofacial part always generates lot of physiological fever because of the muscles. This physiological fever is produced from the muscles that are responsible for maintaining skull suture, controlling the movement of temporomandibular joint, maintaining head and neck posture. Controlling this fever is the crux of Cosmetic Acupuncture mechanism. These muscles correspond to Foot Taeyang meridian-muscle, Foot Soyang meridian-muscle and Foot Yangmyung meridian-muscle. Cosmetic Acupuncture is effective for preventing facial skin from aging and wrinkle increase by mechanical stimulus on facial muscles, and for controlling craniofacial part meridian-muscle system producing the heat.

  • Ankle fracture and Achilles tendon rupture are common as an isolated injury. However, Achilles tendon rupture with ipsilateral ankle fracture is uncommon, and occurs by a different injury mechanism with a risk of negligence. We report a case of Achilles tendon rupture with ipsilateral medial malleolar fracture.

Surgical Treatment of Maisonneuve Fracture Accompanied by Tillaux-Like Fracture: A Case Report (Tillaux 골절과 유사한 골절이 동반된 Maisonneuve 골절 환자의 수술적 치료: 증례보고)

  • Tillaux fractures, a rare type corresponding to Salter-Harris type 3, are observed in approximately 3% of pediatric ankle fractures. They are known to occur primarily via a supination-external rotation mechanism. Maisonneuve fractures, on the other hand, are proximal fibula fractures involving injury on syndesmosis, deltoid ligament, or medial malleolus fracture, occurring via a pronation-external rotation mechanism. These two types of fractures occur through different mechanisms of injury. In this study, a nine-year-old female pediatric patient presented to the outpatient department with ankle pain following a fall from a trampoline and was diagnosed with concurrent Tillaux-like fracture and Maisonneuve fractures. Surgical treatment was performed, resulting in successful recovery.

The Differences in Foot Type According to Major in Left and Right Foot for Female College Students (전공과 좌우 양 발에 따른 여대생의 발 유형 분석)

  • The purpose of this study is to analyze the differences in foot type of female college students according to academic major in both feet. The subjects for this study were 216 female students who took liberal arts classes in Seoul. Dependent variables were students' field of study -specifically whether or not they majored in physical education. Analysis of students' foot type and Malalignment Syndrome were measured using Resting Calcaneous Stance Position (RCSP). There were five categories for RCSP angle: Severe Pes Planus (< $-5^{\circ}$), Pes Planus ($-3^{\circ}{\sim}-4^{\circ}$), Pes Rectus (${\pm}2^{\circ}$), Pes Cavus ($+3^{\circ}{\sim}+4^{\circ}$), Severe Pes Cavus (> $+5^{\circ}$). ${\chi}^2$ analysis was used for statistical analysis. RCSP for all subjects (432 feet) occurred at the following frequency: Pes Planus(43.9%), Pes Rectus(43.8%), and Pes Cavus(12.3%). These levels were different for physical education majors, with Pes Planus at 42.6%, Pes Rectus at 49.4%, and Pes Cavusat 8.0%. Non-physical education majors exhibited Pes Planus at 45.0%, Pes Rectus at 39.9%, and Pes Cavus at 15.1%. 15.3% (33 subjects) of all students had Malalignment Syndrome. In conclusion, 56.2% of female college students had a foot deformity. There was nearly four times more Pes Planus than Pes Cavus. According to these results, exercise can be prescribed to alleviate foot deformities, especially supination. Severe pronation and supination problems appeared less amongst students not majoring in physical education. Thus, although exercise might be one cause of foot deformity, it can also help resolve problems with over-supination. Further study will be needed to understand and resolve the specific mechanism of over-supination.

Predicting Factors on Surgical Management of Unilateral Calcaneal Fracture (편측 종골 골절의 수술적 치료의 예후 관련 인자)

  • Purpose: To study prognostic factors of unilateral calcaneus fracture underwent surgery. Materials and Methods: We selected appropriate 60 cases of 236 calcaneus fracture cases between March 1985 and March 2004, and analyzed the correlation between sex, age, smoking, injury mechanism, Essex-Lopresti classification of calcaneus fracture, preoperative Bohler angle, postoperative Bohler angle, postoperative 1 year Bohler angle and Visual Analogue Scale (VAS), P.S. Kerr's Calcaneal Fracture Score (CFSS). The average age was 41.4 and average follow up period was 74 (12 to 240) months. Results: For follow up period, average VAS is 3.43 and CFSS is 81.23. The sex, age, smoking, injury mechanism, and preoperative, postoperative, postoperative 1 year Bohler angle had no correlation with the prognosis. But the Essex-Lopresti classification of calcaneus fracture, tongue type had better prognosis than joint depression type (VAS : p=0.041, CFSS : p=0.021). Conclusion: In unilateral calcaneus fracture, the sex, age, smoking, injury mechanism, preoperative Bohler angle, postoperative Bohler angle, postoperative 1 year Bohler angle had no correlation with the prognosis of fracture, but in Essex-Lopresti classification, the tongue type fracture had better prognosis than the joint depression type.

  • The midtarsal joint is composed of the talonavicular and calcaneocuboid joints. It is also known as the Chopart joint. Midtarsal joint fracture and dislocation are relatively rare and frequently missed or misdiagnosed. A proper understanding about the anatomy of the midtarsal joint is an essential part in comprehending the mechanism of injury and rationale for treatment. Anatomical reduction of midtarsal joint with correction of the column in length and shape are important; however, it is technically challenging and may require open procedure. Herein, we described a case of initial open reduction and internal fixation for midtarsal joint fracture and dislocation with a brief literature review.

Friction Characteristics of an Hydraulic Cylinder for an Automotive Manual Clutch (자동차 수동 클러치 유압 실린더의 마찰 특성)

  • A clutch hydraulic system for automotive manual transmissions transfers hydraulic pressure generated by driver's pedal manipulation to the clutch mechanism. The foot effort when the clutch pedal is pushed is different than that when the clutch is returned. The effort or load difference, called hysteresis, is caused by the friction produced between rubber seal and inner wall inside the hydraulic cylinder. This clutch pedal travel foot effort hysteresis is essential for a clutch hydraulic system design and analysis. The dynamic model for a clutch hydraulic system is developed and a simulation analysis is performed to estimate the fiction coefficient as a function of the cylinder pressure. The simulation result is then compared to the measurements obtained from a clutch hydraulic system tester to ensure the reliability of the dynamic model and the coefficients estimated. Also the estimated friction coefficients at various pressure values are compared to those reported by an independent study.

  • Traumatic dislocation of the peroneal tendon is an infrequent injury. This injury is caused by forceful dorsiflexion of the foot accompanied by a powerful contraction of the peroneal muscles. This mechanism of injury tears the superior peroneal retinaculum and allows the tendons to snap anteriorly. We experienced a case of chronic recurrent dislocation of the peroneal tendon, which was treated by bone block surgery using autograft of lateral fibula and reattachment of the superior peroneal retinaculum. The clinical result was satisfactory.
