Purpose : Sanhupung(産後風) is not completed golden standards of diagnosis therefore doctors diagnose through symptoms and signs of patients in clinic same as progress of treatments. We polled oriental gynecologic specialists and non-specialisst to find out cognitive differences on Sanhupung. Subjects : We surveyed 83 specialists and 87 non-speciailist from May 15 2005 to Oct 15. We analyzed the result of respondants, 39 (47%) specialists and 65 (75%) nono-specialits). Methods : We sorted all symptoms in previous studies then we categorized them into 4 fields. The results are analyzed by frequency, importance and impact(frequency percentage${\times}$average importance). Results : Except back pain, all pain symptoms were lower evaluated in non-specialist on frequency. Except inguinal region pain, pain of lower limbs and back pain, all pain symptoms were lower evaluated in non-specialist on importance. Except the numbness of limbs, all items of whole body were lower evaluated in non-specialist on frequency. Except edema, numbness of hand and foot and discharge from the uterus, all items were lower evaluated in non-specialist on importance. All items of neuropsychotic symptoms were lower evaluated in non-specialist on frequency and importance.
Purpose: The purpose of this study was to identified patterns of studies and to analyze the nursing research related to the massage intervention in Korea and to improve direction of nursing research in massage intervention in Korea. Methods: The research studies related to the massage were selected from the Korean Nurses Academic Society Journal and dissertation which were conducted experimental research design the last 7 years in Korea. The total numbers of the studies were 58. Result: The study subjects were post operational state in 34.5%, healthy adult in 17.2% studies. The others dealt with infant, cancer, dementia, hypertension, preterm neonates, elderly. Types of massage were whole body in 31.1%, foot in 22.6%, hand in 17.2%, back in 13.8% studies. And Meridian massage in 17.2%. Length of massage was below 5 minutes in 17.2%, 6~10 minutes in 29.4% and 11~20 minutes in 32.8% studies. Frequency of massage was only 1 time in 25.9%, 2~5 times in 31.1% studies. Massage produced psychological and physiological effects. Positive psychologic effects were anxiety relief, pain relief, mood enhancement and fatigue relief. Positive physiologic effects on body systems were blood pressure, heart rate, edema, skin temperature, immune system, blood glucose, excretion, PMS syndrome and growth and development. Conclusion: As a result, effects of massage were almost positive. Therefore massage is useful alternative nursing intervention. But more attention research is needed to identify the objectivity instrument to measure and repeat research.
당뇨병성 신병증은 최근 우리나라를 포함한 서구 사회에서는 만성 신부전의 가장 많은 원인 중 하나이며, 꾸준히 증가추세이다. 조직학적으로는, 사구체, 세뇨간질, 소동맥 등 신장의 주요 부위에 병변이 나타나는데, 특징적으로 사구체 기저막의 비후와 족세포의 변화와 함께 사구체 경화, 소동맥의 유리질 경화와 세뇨간질 부위에 섬유화 등이 나타난다. 당뇨병성 신병증은 혈역학적 인자들과 대사성 인자들 간의 복잡한 상호작용의 결과로 일어난다. 고혈당은 혈류역학적 요소로 고혈압과 함께 사구체내압의 증가와 혈관활성물질의 활성화에 함께, 비혈역학적 경로들, 예를 들면, 후기 당화합 최종생성물 생성, 세포 내 신호전달체계와 물질, 시토카인, 산화 스트레스 등 다양한 원인들을 활성화시킨다. 이러한 대사적, 혈류역학적인 인자들은 신장의 알부민 투과성을 증가시키고 세포 외 기질의 축적을 일으키며 결과적으로 증가된 단백뇨, 사구체경화와 세뇨관간질의 섬유화를 일으키게 된다.
While working in an industrial environment which requires extended periods of upright posture; workers tend to develop muscle fatigue due to the constant load on lower-limb muscles. In addition, when working while bending knees; muscle fatigue of lower back and hamstrings is increased due to the abnormal posture. This can lead to damage of muscles, induce musculoskeletal disorders, and reduce long-term working efficiency. Recent medical studies have shown that long-term working in an upright posture can induce musculoskeletal disorders such as foot fatigue, edema, pain and varicose veins. Likewise, medical and rehabilitation expenses have grown due to the increase in musculoskeletal conditions suffered by workers. For this problem, we aim to develop a device that can reduce the physical fatigue on the lower limbs by supporting the weight of workers during the extended periods of upright and bending postures in the industrial environments. In this paper, we have designed and manufactured a wearable weight support system; with a user intention algorithm that the users can maintain various postures. For validation of the developed system, we measured the muscle activity of the users wearing the system with EMG sensors.
감국 추출물 0.5~10 g/kg을 투여한 급성독성 실험에서는 고용량인 10 g/kg 경구투여에서도 급성독성을 발생시키지 않은 것으로 나타나 안전성이 높은 물질로 확인되었다. 만성독성 실험에서도 최대 2 g/kg까지 13주간 경구 투여하였으나 간 독성이 없는 것으로 나타났다. 감국 추출물을 2~4 g/kg 투여한 그룹에서는 통풍의 염증 및 부종 억제 효과가 있었으나, 10 g/kg 투여군에서는 다소 염증이 증가되는 경향을 나타내었다. 그러므로 통풍억제 효과가 있으나 염증성 사이토카인을 더 증가시키지 않는 4 g/kg의 농도 이하로 감국 추출물을 투여하는 것이 적당한 농도라고 판단되었다. 따라서 감국 추출물은 고용량에서도 비교적 안전한 물질로 monosodium urate crystal에 의한 염증작용을 억제하므로 통풍예방물질로서의 가치가 있다고 생각되었다.
Anti-nociceptive and anti-inflammatory effects of Clematidis Radix (CR)-distillates were investigated in three different pain animal models. Highly purified distillate of CR was injected to Zusanli (ST36) acupoint, called CR herb-acupuncture in the Korean traditional medicine. In tail flick latency test, the CR herb-acupuncture treatment did not show a significant effect of relieving acute phasic pain. To investigate the anti-inflammatory effect of CR herb-acupuncture, inflammatory pain was induced by subcutaneous injection of formalin to the plantar tissue or intra-articular injection of carrageenan to the tibio-tarsal joint in the rats. And the medicinal effect of CR herb-acupuncture was evaluated by analyzing pain behavior such as licking or biting behavior, or by measuring weight distribution ratio between two foot and ankle circumference. In the rat formalin test, the analgesic effect of CR herb-acupuncture was more pronounced in the late phase (for 20 min after the early phase) than in the early phase (for the first 10 min post formalin injection). It also significantly alleviated the carrageenan-induced monoarthritis, in terms of weight distribution ratio and ankle edema. These results revealed that CR herb-acupuncture was effective to treat the inflammatory pain and could be used as an analgesic treatment with an antiinflammatory effect.
Purpose: Distally based superficial sural artery island flap has some disadvantages such as postoperative flap edema, congestion, and partial necrosis of the flap margin. Venous congestion is an area of considerable concern in distally based superficial sural artery fasciocutaneous flap and is one of the main reasons for failure, particularly when a large flap is needed. However, we could decrease these disadvantages by means of venous superdrainage. Methods: From June of 2006 to June of 2007, a total of two patients with soft tissue defects of lower one third of the leg underwent venous supercharging distally based superficial sural artery island flap transfer. The distal pivot point of this flap was designed at septocutaneous perforator from the peroneal artery of the posterolateral septum, which was 5 cm above the tip of the lateral malleolus. Briefly, this technique is performed by anastomosing the proximal end of the lesser saphenous vein and collateral vein to any vein in the area of the recipient defect site. Results: No venous congestion was noted in any of the two cases. No other recipient or donor-site complications were observed, except for minor wound dehiscence in one case. In 3 to 6 months follow-up, patients had minor complaints about lack of sensation in the lateral dorsal foot. Conclusion: The peroneal artery perforator is predictable and reliable for the design of a distally based superficial sural artery island flap. Elevation of the venous supercharging flap is safe, easy, and less time consuming. In conclusion, the venous supercharging distally based superficial sural artery island flap offers an alterative to free tissue transfer for reconstruction of the lower extremity.
Purpose: We reconstructed the skin defect of lower legs exposing muscles, tendons and bone with fasciocutaneous sural artery flap and report our cases. Materials and Methods: Between March 2005 and September 2006, 8 cases of skin defect were reconstructed with fasciocutaneous sural artery flap. Defect site were 4 case of ankle and foot and 4 cases of lower leg. The average defect size was $4{\times}4\;cm^2$. There were 5 men and 3 women and mean age was 52.2 years. We evaluated the viability of flap, postoperative complication, healing time, patient's satisfaction. Results: There was no flap failure in 8 cases. But recurrent discharge in 2 cases was healed through several times adequate debridement and delayed suture without complication. Flap edema may be due to venous congestion was healed through leg elevation and use of low molecular weight heparin. Mean time to heal the skin defect was 4 weeks. No infection and recurrence in follow up period. Cosmetic results as judged by patients were that 5 cases are good and 3 cases are fair. Conclusion: Sural artery flap is good treatment method among the numerous methods in the cases of skin defect, with soft tissue exposed, which is not covered with debridment and skin graft. Sural artery flap is useful method for the skin defect of lower legs because it is simple procedure, has constant blood supply and relatively good cosmetic effect.
In order to access the suppressive effects of piroxicam (PX) and hydrolyzed products of Scutellariae Radix (PSH) on arthritis, we investigated whether PSH gel could suppress the progression of collagen-induced arthritis. PX, one of nonsteroidal anti-inflammatory drugs has been used in the systemic and topical treatment in a variety of inflammatory conditions. Scutellariae Radix, one of the herbal medicines, was used for the purpose of anti-inflammatory and anti-bacterials. For the purpose of transdermal absorption of the hydrogel preparations, two classes of hydrogels (PX, PSH) were formulated with carbomer 940, diethylene glycol monoethyl ether, polyethylene glycol-8-glyceryl caprylate/caprate and triethanolamine. In carrageenan-induced edema in rat hind paws, inhibition of foot swelling was more increased in PSH than PX hydrogel. Rheumatoid factors including serum IgG, IgM and collagen specific antibody were present much lower in PSH gel treated mice than control. Histological examination revealed that PSH hydrogel inhibited infiltration of inflammatory cells into affected paw joint, compared with control. The PSH hydrogel would be a suitable preparation to increase transdermal treatment for anti-inflammatory effects on collagen-induced arthritis.

Objectives: Gout is an inflammatory arthritis of the joints and soft tissues occurring due to deposition of monosodium urate (MSU) crystals, which are caused by persistent hyperuricemia. Soyeom pharmacopuncture is one treatment method that has been traditionally used for pain management in Oriental medicine. However, studies on its effect in reducing gout pain have been insufficient. Therefore, we selected Soyeom pharmacopuncture among natural products used in Korea as the new target of our study. Methods: The effects of Soyeom pharmacopuncture were examined in mouse models of acute gout induced by injection of MSU crystals into footpads. IL-1β, IL-6, and TNF-α production were examined by immunoblotting and enzyme-linked immunosorbent assay as hallmarks of NLRP3 inflammasome and cytokine activation. Results: Soyeom pharmacopuncture reduced foot edema in gout-induced mice, as well as IL-1β, nitrite, IL-6, and TNF-α production. Moreover, Soyeom pharmacopuncture also reduced MSU-induced gout inflammatory gene expressions, specifically those in the NF-kB pathway. Conclusion: Pharmacopuncture may serve as a new solution for other inflammatory diseases as well. Through active follow-up studies, we could thoroughly understand the clinical value of Soyeom pharmacopuncture.
