불안정한 자세에서 하지에 인가한 진동자극이 자세 안정성 개선에 미치는 영향 (The Effect of Human Lower Limb Vibration on Postural Stability during Unstable Posture)

  • We studied the effect of vibratory stimulations of different leg muscles, tibialis anterior(TA) and triceps surae(TS), and plantar zones in ten healthy subjects during 1) quiet standing, 2) forward lean of body, 3) backward lean of body, 4) right lean of body, and 5) left lean of body. The experiments were performed on the force platform. The effect of vibration were measured by monitoring the area of COP(Center of pressure) sway. The subjects wore a vibratory stimulation system on foot and ankles and were given the instruction not to resist against the applied perturbations. The results show that all vibratory stimulations to lower limb muscles and plantar zones reduced the COP sway area. This reduction of the COP sway area occurred also in partial vibratory stimulations during quiet standing. In forward lean of body, vibratory stimulations to TA reduced the COP sway area. During backward lean of body, vibratory stimulations to TS reduced the COP sway area. When the subject was tilted right, vibratory stimulations to left plantar zone reduced the COP sway area. During left lean of body, vibratory stimulations to right plantar zone reduced the COP sway area. Thus, the influence of vibratory stimulations to leg muscle and plantar zones differed significantly depending on the lean of body. We suggest that the vibration stimuli from leg muscles and plantar zones could be selectively used to help maintaining postural balance stable.

  • Purpose : The purpose of this study was to analyze the coordination of the trunk tilting angle and bilateral lower limbs according to the stirrups length during trot in equestrian. Methods : Participants selected as subject were consisted of adult male(n=7, mean age: $45.00{\pm}3.78yrs$, mean height: $172.50{\pm}2.44cm$, mean body mass: $76.95{\pm}4.40kg$, mean, mean leg length: $97.30{\pm}2.60cm$). They were divided into 3-types of stirrups lengths(67 cm, 72 cm, 77 cm) during trot. The variables analyzed were consisted of the trunk front-rear angle, lower limb joint(Right Left hip, knee, ankle), overall movement index(OMI) of the lower limbs(thigh, shank, foot) and asymmetry index(AI%) during trot. Results : The average angle in hip and knee joint showed more extended posture according to the increase of stirrups lengths and ankle angle showed more plantarflexion posture according to increase of stirrups length during 1 stride in trot. Also, average angle showed more extended posture in right hip and ankle joint than that of left. The angle of knee joint didn't show significant difference statistically between right and left. Also asymmetric index in average angle of hip, knee and ankle joint didn't show significant difference statistically in between lower limbs, but hip joint showed higher asymmetric index in stirrup length of 77 cm and ankle joint showed higher asymmetric index in stirrup length of 67 cm than that of the others respectively. The FR angle in trunk of horse-rider showed relative backward leaning motions at stirrup length of 67 cm and 77 cm than that of stirrup length of 72 cm during stance and swing phase. OMI in thigh, shank, and foot limbs didn't show significant difference statistically according to the stirrups length of right and left lower limbs, but left lower limbs showed higher index than that of right lower limb. Stirrup length of 72 cm in shank and foot limbs showed higher index than that of stirrup length of 67 cm and 77 cm. But stirrup length of 72 cm showed higher asymmetric index than that of stirrups length of 67 cm and 77 cm. Conclusions : When considering the above, 72 cm(ratio of lower limb 74.04%) stirrup lengths could be useful in posture correction and stabilization than 67cm(ratio of lower limb 68.69%) and 77 cm(ratio of lower limb 79.18%) stirrup lengths during trot in horse back riding.

  • It was to study a following research of "A Kinematical Traits Analysis when Performing Uchimata(inner thigh reaping throw) by Posture and Voluntary Resistance Levels(VRL) of Uke in Judo[1]" and. "A Case Study of Center of Gravity(COG) when Performing Uchimata(inner thigh reaping throw) by Posture and Voluntary Resistance Levels(VRL) of Uke in Judo[II]". The purpose of this study was to analyze an angular momentum of trunk and lower extremity when performing uchimata by two postures and voluntary resistance levels(VRL) of uke(reciver) in Judo. The subjects, who were one male judoka(YH) for 1992 Barcelona Olympic Games Olympian(silver medalist), was filmed on two S-VHS 16mm video cameras(60fields/sec.) through 3-dimensional motion analysis methods, that postures of uke were shizenhontai (straight natural posture:NP) and jigohontai (straight defensive posture:DP), VRL of uke were 0% and 100%, respectively. The variables were angular momentum of trunk, lower extremity of attacking leg and supporting leg of tori(the thrower). The data of this study collection were digitized by SIMI Motion Program computed the mean values and the standard deviation calculated for each variables. When performing uchimata according to each posture and VRL of uke and classifying. From the data analysis and discussion, the conclusions were as follows : Angular momentum of trunk when performing uchimata was showed the largest among another angular momenta, and the posture displayed more different than resistant of uke(reciver), but the pattern similar in judo. Angular momentum of trunk of X axis was the largest and Y, Z axis order. Angular momentum of attacking the thigh-leg when performing uchimata was showed the largest among another angular momenta, and the posture displayed more different than resistant of uke(reciver), X axis and Y axis similar, but angular momentum of Z axis of thigh-leg the largest, in kake(application) event in 0% resistance of DP than other variables. Angular momentum in X,Y axis of attacking the lower-leg when performing uchimata was showed that the resistance level displayed more different than posture, but Z axis the largest, in kake(E3) phase in 0% resistance of DP than other variables as same thigh-leg, and the largest from tsukuri(set-up:E2) to kake(E3) phase. X and Z axis Angular momentum of supporting the thigh-leg were similar, regardless of posture and resistance of uke, but Y axis was resistance level. Angular momentum of supporting the thigh-leg was showed the largest in X axis, increased from EO event to E2, and decreased in E3, and angular momenta of Y, X axis were showed the largest in kuzushi(balance breaking) phase when performing uchimata. Angular momentum of supporting the lower leg were similar pattern, regardless of posture and resistance of uke, in Y axis, resistance displayed more difficult the position in NP, and showed opposite angular momentum in tsukuri phase. In conclusion, angular momentum of trunk when performing uchimata was showed the largest, and pattern was similar, regardless of posture than resistant of uke(reciver), magnitude and direction were different each other, and uchimata was Ashi -waza(foot and leg techniques) division but important of trunk action.

외반슬 환자의 교정운동의 효과: 단일 피험자 A-B-A 실험 설계 (The Effect of Corrective Exercise in a patient with knee joint valgus deformity: A single-subject A-B-A experimental design)

  • PURPOSE: The purpose of this study was to determine the effect of corrective exercise on hip joint range of motion, lower limb alignment, radiographs of hip and knee joints, and quadriceps muscle activity in a patient with knee joint valgus deformity. METHODS: A single-subject A-B-A experimental design was used to determine the effects of therapeutic exercise. The single-subject was a 27-year-old male, who presented with knee joint valgus deformity. Corrective exercise program was performed for 40 min/day twice a week for 12-week. Range of motion (hip flexion, extension, abduction, adduction, internal rotation, and external rotation), lower limb alignment test (Q-angle, rear foot alignment, and leg length), standing anterior-posterior radiographs (neck shaft angle and knee joint space), and quadriceps muscle activity of both lower limbs were measured before (A 1), after 6 weeks (B 1) and after 12-weeks (B 2) of corrective exercise and after 6 weeks of exercise completion (A 2). RESULTS: Hip range of motion increased in all directions of both sides at B 2 and A 2 compared to at A 1. Q-angle of both side and leg length discrepancy decreased at B 2 and A 2 compared to at A 1. Neck shaft angle and knee joint space of both sides improve at B 2 and A 2 compared to at A 1. Quadriceps of both side muscle activities improved at B 2 and A 2 compared to at A 1. CONCLUSION: We demonstrated that corrective exercise increases range of motion, and improves lower limb alignment and muscle activity in a patient with knee joint valgus deformity.

태권도, 축구, 체조 선수들의 균형능력과 근력, 근지구력의 상관분석 (Correlation between Balance Ability, Muscle Strength, and Muscle Endurance, in Taekwondo, Soccer, and Gymnastics Athletes)

  • The purpose of this study was to investigate a relationship among Taekwondo, soccer and gymnastic athletes their balance ability and lower extremity muscle strength, muscle endurance and also whether these variables show differences by sports type. For this purpose, 10 Taekwondo athletes, 10 soccer athletes and 10 gymnastic athletes that is a high school in I area were selected and their balance ability, isokinetic muscle strength and muscle endurance were measured by using force platform (AMTI) and isokinetic measurement (HUMAC NORM). Then the following results were obtained by conducting pearson product-moment correlation analysis and one-way ANOVA. In case of both right and left foot one-leg standing with open-eyes, there was a relation between both knee extension maximal muscle strength of Taekwondo, soccer, gymnastic athletes and balance ability, and in case of right foot one-leg standing with open-eyes, there was a relation between that is right plantar flexion and dorsi flexion maximum muscle strength and muscle endurance of Taekwondo, soccer, gymnastic athletes and their balance ability. Furthermore, in case of left dorsi flexion maximum muscle strength, there were significant differences in order of Taekwondo, gymnastics and soccer, and for isokinetic plantar flexion maximum muscle endurance, both right and left showed significant differences in order of Taekwondo, gymnastic and soccer.

한 다리 서기 동안 매끈눈따라보기와 신속눈운동이 균형과 근활성도에 미치는 영향 (The Effects of Smooth Pursuit Eye Movement and Saccadic Eye Movement on Balance and Muscle Activation during One Leg Standing)

  • PURPOSE: This study is examined the effects of vestibular stimulation through eye movement on balance and muscle activity. METHODS: In 42 healthy adults, no eye exercise was applied to both feet and one foot. The speed of smooth pursuit eye movement (.2 Hz, .3 Hz, and .5 Hz) and saccadic eye movement (.5 Hz and 1.1 Hz) were randomized. The measurements were taken three times for 30 seconds while standing on two feet and measured three times for 10 seconds while standing on one foot. The muscle activity measurement equipment was used to measure the electromyogram signals of the tibialis anterior, peroneus longus, gastrocnemius medialis, vastus medialis, vastus lateralis, biceps femoris, abdominal internal oblique, and erector spinae muscle. RESULTS: As a result of this study, when applying smooth pursuit eye movement on one leg, the pressure center movement increased, the muscle activity of the lower extremity increased, in the saccadic eye movement, and the center of pressure decreased. CONCLUSION: Accordingly, the smooth pursuit of eye movement, the intervention of this study, affects balance. Through this, the balance can be improved by applying eye movement to the target who needs to improve the balance ability.

  • In Oriental Medicine, mononeuropathy belongs to or . Common peroneal neuropathy(CPN) is the most frequently encountered mononeuropathy in the lower extremity. It is usually caused by direct surgery injury, compression, leg crossing, trauma, traction etc, occasionally by nerve tumor. A 47-year-old healthy man was complained of the sudden development of left foot drop and sensory manifestation owing to suspected compression and habitual leg-crossing. Acupuncture along with bee-venom acupuncture, moxibustion was performed mainly at Stomach and Gallbladder Meridian specially ST36, ST37, ST40, GB34 and GB39. Nerve conduction study and electromyography was also performed three times. Symtoms was relieved fast, and full recovery took about 110 days. Acupuncture and bee-venom acupuncture are considered to be beneficial to CPN. More clinical trials and studies are needed.

  • Background: Short foot exercise (SFex) is often prescribed and performed in the sport and rehabilitation fields to strengthen intrinsic foot muscles. However, SFex is difficult to perform because of lack of feedback methods. Objects: The aim of this study was to compare the effects of SFex with and without electromyography (EMG) biofeedback on the medial longitudinal arch (MLA) of healthy individuals who maintained a static standing position. Methods: All participants (14 males and 12 females) were randomly divided into two groups (biofeedback and non-biofeedback groups). The EMG activity of the abductor hallucis (AbdH) and tibialis anterior (TA) and the MLA angle on the dominant leg side were measured with the participant in the standing position in the pre- and post-intervention conditions. The intervention session consisted of 15 minutes of SFex with (biofeedback group) or without (non-biofeedback group) EMG biofeedback. The groups were compared using two-way repeated measures analysis of variance. Results: The post-intervention activities of the AbdH muscle (p<.05) and the AbdH/TA ratio (p<.05) were significantly greater in the biofeedback group than in the non-biofeedback group. The activity of the TA (p<.05) and the MLA angle (p<.05) in the biofeedback group were significantly lower in the post-intervention condition than in the pre-intervention condition. Conclusion: The present findings demonstrate that the combination of SFex and EMG biofeedback can effectively facilitate the muscle activity of the AbdH and strengthen the medial longitudinal arch.

정상 성인 휴지종골 입각각도와 자세동요와의 관련성 연구 (A Study of the Relationship Between Normal Adults' Resting Calcaneal Stance Position and Postural Sway)

  • The purpose of this study was to investigate the relationship between normal adults' resting calcaneal stance position(RCSP) and postural sway. Subjects were 70 normal adults(34 men and 36 women) in their twenties who attend S. University. Postural sway during a single limb stance was measured using the CMS 10 Measuring System when subjects positioned on the balance trainer in their bare foot. RCSP while subjects were standing on the glass plate was measured using the angle tinder after subjects were positioned in prone to divide equally lower leg and calcaneus using the goniometer. The result was as follows. There were significant weak positive correlations between RCSP and postural sway(r=0.362, p<0.01), the leg of the small RCSP within a subject has the small postural sway index($X^2=43.758$, p<0.001). There was no significant difference between groups of rearfoot valgus(RCSp<$2^{\circ}$) and those of rearfoot varus (RCSp>$2^{\circ}$) in the postural sway. In conclusion, there is a weak relationship between increasing the absolute value of RCSP and increasing postural sway.

한랭환경에서 손발노출에 의한 청년과 노인의 체온조절반응 비교 (Temperature Regulation of the Young and the Aged during Hands and Feet Exposure to the Cold)

  • A study was conducted to investigate the effect of cooling hands of feet on human thermoregulation in the cold. Eight young (22$\pm$1 yr) and eight aged (69$\pm$4 yr) men volun-teered as subjects They stayed at 1$0^{\circ}C$ in the supine posture during the experiment which included hands or feet exposure to the air for 20 minutes. Hand Exposure (HE) and Foot Exposure (FE) were conducted in radomized order and the baseline was kept before HE and FE. Core temperatures, limb skin temperatures adn thermal sensations were measured. Obtained data were analyze using t-test and correlation. Rectal and esophageal tem-peratures increased in the young (YG) and in aged (AG). Change rate of esophageal temperature (Tes) was maintained higher during FE than HE while rectal temperature showed no differences between YG and AG, and between HE and FE. Hand and foot skintemperature in YG and AG decreased similarly during HE and Fe. Forearm skin temperature during HE decreased while leg skin temperature during FE showed no change HE and Fe. It was concluded that the lower cooling. Furthermore, the increase of Tes was greater in young men than aged men. It is also suggested that the wearing behavior can be differently modified between young and aged men.
