Analgesic effect of Clematidis Radix (CR) herb-acupuncture in a rat model of pain and inflammation

  • Anti-nociceptive and anti-inflammatory effects of Clematidis Radix (CR)-distillates were investigated in three different pain animal models. Highly purified distillate of CR was injected to Zusanli (ST36) acupoint, called CR herb-acupuncture in the Korean traditional medicine. In tail flick latency test, the CR herb-acupuncture treatment did not show a significant effect of relieving acute phasic pain. To investigate the anti-inflammatory effect of CR herb-acupuncture, inflammatory pain was induced by subcutaneous injection of formalin to the plantar tissue or intra-articular injection of carrageenan to the tibio-tarsal joint in the rats. And the medicinal effect of CR herb-acupuncture was evaluated by analyzing pain behavior such as licking or biting behavior, or by measuring weight distribution ratio between two foot and ankle circumference. In the rat formalin test, the analgesic effect of CR herb-acupuncture was more pronounced in the late phase (for 20 min after the early phase) than in the early phase (for the first 10 min post formalin injection). It also significantly alleviated the carrageenan-induced monoarthritis, in terms of weight distribution ratio and ankle edema. These results revealed that CR herb-acupuncture was effective to treat the inflammatory pain and could be used as an analgesic treatment with an antiinflammatory effect.

Limited Open Reduction and Internal Fixation of the Tibial Pilon Fractures (제한 절개를 통한 관혈적 정복 및 내고정술을 이용한 경골 Pilon 골절의 치료)

  • The tibial Pilon fracture, which is defined as a comminuted intraarticular fracture of the distal tibia, is difficult to manage because high axial compression and rotational forces to the ankle joint result in impaction, severe comminution, metaphyseal disruption and soft tissue damage. There are variable methods of treatment such as manipulation and cast, calcaneal traction and cast, external fixation, pin and plaster, limited open reduction and external fixation, and open reduction and rigid internal fixation. Though most of authors reported better result after a surgical treatment. than that of conservative treatment, many complications such as posttraumatic arthritis and soft tissue problem still remain troublesome. We have reviewed 19 cases of the tibial Pilon fractures in 18 patients which were treated with limited open reduction and internal fixation from September 1993 to May 1996. The results were as follows: 1. The fractures were classified into five types according to the system of Ovadia and Beals, and the most frequent type was type 3 (53%). The most common cause of injury was traffic accident (47%). 2. All of the cases of type 1 and 2, in which the injury of the ankle joint was less severe, revealed good or excellent clinical results. But in type 4 and 5, because the injury is much severe and accurate reduction is difficult, the clinical results were unsatisfaetory. 3. The most frequent complication was posttraumatic osteoarthritis, and which developed in second frequent complication, was developed m the three cases of type 3 in which the radiographic results were less than fair, but there were no correlation with the clinical results. 4. We could markedly reduce the complications related to the soft tissue problem of Pilon fracture by treatment with limited open reduction and internal fixation, and consider that this is a good method of treatment of Pilon fracture when the injury is less severe and accurate reduction is possible.

Comparative Study of Open Reduction and Internal Fixation and Primary Subtalar Arthrodesis for Sanders Type 4 Intra-Articular Calcaneal Fractures (Sanders 4형 종골 골절에 대한 관혈적 정복술 및 내고정술과 일차성 거골하 관절 유합술의 치료 결과 비교)

  • Purpose: To compare clinical outcome of Sanders type IV intra-articular calcaneal fracture treated with open reduction and internal fixation (ORIF) versus ORIF and primary subtalar arthrodesis (PSTA). Materials and Methods: Between March 2003 and November 2013, 22 patients with 22 Sanders type 4 intra-articular calcaneal fractures were included in this study. Of these, 11 were treated with ORIF (ORIF group), and 11 were treated with ORIF and PSTA (PSTA group). The mean follow-up period was 34.6 months (range, 18-72 months). Clinical outcomes were assessed along with the American Orthopedic Foot and Ankle Society's ankle-hindfoot scale (AOFAS score), and the visual analogue scale pain score (VAS score) at 6-month, 12-month, and last follow-up. Patient satisfaction, return to previous occupation and postoperative complications were also investigated. Results: The results for ORIF did not differ from those for PSTA based on the last follow-up AOFAS scores or the VAS scores (p>0.05). However, patient satisfaction was significantly higher in the PSTA group (p=0.008). Secondary subtalar arthrodesis was conducted in five patients (45.5%) of the ORIF group within 2 years postoperatively. Conclusion: We were unable to demonstrate a significant difference in clinical outcomes between ORIF and PSTA; however, the patient satisfaction was higher in the PSTA group. PSTA may be a suitable choice for patients who need fast recovery to daily activity and to prevent the need for secondary subtalar arthrodesis.

  • The female's part-cold hypersensitivity syndrome is a disease which the oriental people better than the western people suffer from and if it would be sick for a long time, it could bring into the other diseases. In the Oriental Medicine, the coldness is regarded as one of the most important causes which induce female's diseases. Nowadays the number of the female patients are ascending that serously appeal the coldness-sense and pain-sense at the particular part of the body, for example, hand, foot, abdomen, waist, external genital portion and so on. These are caused by female's own characteristics of the menstration and delivery, and the influence of the circumstances. The Oriental Medicine have better effects by the diagnosis and treatment according per symptom in the contrast with the Western Medicine having difficulties in the point of accurate diagnosis and treatment. This epidemiologic study was undertaken by using specifically designed questionair and physical examination to evaluate the incidence and degree of female's part-cold hypersensitivity syndrome in 362 parous women, Who were randomly selected among the patients visiting the Department of Gynecology, Kyung Hee Medical Center from Feb. to Jul. 1995. The results were as follows: 1. The overall incident of female's part-cold hypersensitivity syndrome was 92.8%. 2. Subdivision of 336 patients was made according to severity Grade I - 31.9%, severity Grade II - 46.9%, severity Grade III - 21.4%. 3. As for the appealed portion of female's part-cold hypersensitivity syndrome, foot and hand, lower abdomen, knee, waist, hip, shoulder, leg, and ankle were in desending order. 4. As for the causes of female's part-cold hypersensitivity syndrome, it was deeply associated with the management of the abortion, delivery and puerperium. 5. The incidence of female's part-cold hypersensitivity syndrome did not show incerased tenency according to the quantity and quality of female's menstration, leukorrhea, and the increase of parity. 6. 165 patients among 336 patients wanted the treatments of the Oriental Medicine.

Characteristics of Patients with Skeletal Fracture Admitted in Oriental Hospital (한방병원에 내원한 근골격계 질환 환자중 골절 발생 현황)

  • Objective : The purpose of this study is to investigate the characteristics of patients with fracture admitted in oriental hospital. Methods : From october 1996 to october 2001, we retrospectively evaluated the simple radiographic proven 8441 patients with musculoskeletal pain in oriental hospital. The patients included 3899 men and 4542 women. All fractures were analyzed in terms of location, age and sex. Results : 1. The fracture rate of radiologic study was relatively high in foot, chest, upper extremity and hand. The frequent fractures were in spine (257 cases), foot (119 cases), upper extremity (83 cases). and chest (68 cases). 2. The male patients with fracture were significantly higher than female in 0-40 years group: in 0-20 years group(8.5 % vs 4.0 %, p<0.01), in 21-40 years group (4.1 % vs 2.6, p<0.05), while female patients with fracture were significantly higher than male patients in more than 60 years group( 10.1 % vs 15.4 %, p<0.01). 3. The incidence of fracture showed significant association with increased age except 0-20 years group(p<0.01). Spine and chest cage in mate, and upper extremity, pelvis, spine, ankle and chest cage in female were significantly higher on older age groups. Conclusion : The overall fracture rate of radiologic study was 6.6 % for patients with musculoskeletal problem admitted in oriental hospital. The distribution of fracture showed significant difference in terms of age, sex and site. So we believe the radiologic study is helpful to evaluate fracture in oriental hospital.

Electrophysiological Study of Medial Plantar Nerve in Idiopathic Tarsal Tunnel Syndrome (특발성 발목터널 증후군에서 내측 발바닥 신경의 전기 생리학적 검사)

  • Background: Tarsal tunnel syndrome (TTS) is an entrapment neuropathy of the tibial nerve within fibrous tunnel on the medial side of the ankle. The most common cause of TTS is idiopathic. This is a retrospective study to define the electrophysiological characteristics of idiopathic TTS. Methods: We reviewed the medical and electrophysiological records of consecutive patients with foot sensory symptoms referred to electromyography laboratory. Inclusion of patients was based on clinical findings suggestive of TTS. Among them, patients with any other possible causes of sensory symptoms on the foot were excluded. Control data were obtained from 19 age-matched people with no sensory symptoms or signs. Routine motor and sensory nerve conduction study (NCS) including medial plantar nerve (MPN) using surface electrodes were performed. Result: Twenty one patients (13 women, 8 men, 9 unilateral, 12 bilateral) were enrolled to have idiopathic TTS (total 31 feet). Tinel's sign was positive in 16 feet (51.6%) of TTS and four feet (10.5%) in control group. The statistically significant electrophysiological parameter was difference of sensory conduction velocity (SCV) between sural nerve and MPN. Amplitude of sensory nerve action potential and SCV of MPN were not different significantly between idiopathic TTS feet and controls. Conclusion: Bilateral development in idiopathic TTS was more common. Tinel's sign and difference of SCV between sural nerve and MPN may be helpful for the diagnosis of idiopathic TTS.

Outcome of Conservative Treatment of the Zone I, II 5th Metatarsal Base Fracture under Early Weight-Bearing (제5 중족골 제1, 2 구역 골절의 조기 체중부하의 비수술적 치료 결과)

  • Purpose: To determine how the location, displacement, intra-articular involvement, comminution of a 5th metatarsal base fracture affect results of early weight-bearing treatment. Materials and Methods: From January 2013 to July 2017, 34 cases of 34 patients diagnosed with a fracture of the zone I and II 5th metatarsal base were enrolled. The mean follow-up period was 13 months (6-15 months). One patient was excluded as a refracture during the follow-up period, and 33 patients underwent conservative treatment. Anteroposterior, lateral, and simple oblique radiography and computed tomography of the foot were performed to evaluate the location and displacement of the fracture, the degree of joint involvement, and comminution. In all 33 patients, a short leg cast or boot brace was selected immediately after the injury, tolerable weight bearing was allowed. If the pain disappeared, full weight bearing was performed after wearing a plain shoe or postoperative shoe. As a clinical result, the American Orthopedic Foot and Ankle Society (AOFAS) score was evaluated at the final follow-up. During outpatient follow-up, a simple radiograph of the foot was taken to confirm the time of radiological bone union and return to work. Results: Nine males and 24 females, with an average age of 48.7 years, were enrolled in the study. Twenty-four patients had zone I fractures, and nine patients had zone II fractures. Twenty-two out of 33 patients had a fracture displacement of 2 mm or more. Nine and five patients had joint involvement and comminution, respectively. There was a statistically significant return to work from zone I to zone II. The AOFAS score was excellent at the final follow-up and there was no significant difference. When classifying and comparing the degree of fracture displacement, joint involvement, and comminution, there were no significant differences in the radiological union time and return to work. In all cases, satisfactory results were obtained at the final follow-up. Conclusion: Satisfactory clinical results can be obtained by allowing early weight-bearing regardless of the fracture location, displacement, joint involvement, or comminution in zone I and II 5th metatarsal base fractures.

The Effect of Ankle Joint Taping on Balance in the Elderly (발목관절 테이핑이 고령자의 균형능력에 미치는 영향)

  • Background : The purpose of this study is to understand the effects of ankle joint taping on balancing abilities of the elderly and to provide preliminary data on preventing falls among the elderly. Methods : In this study, 20 students(8 male, 12 female) in their 20s from D University as well as 14 elderly(4 male, 10 female) in their 70s from S community center have participated. The participants were well informed about the experiment and voluntarily participated. We measured the difference in balancing ability before and after taping between the two groups using methods such as standing on single-leg stance, TUG, and plantar pressure. Kinesio taping, a stop-watch, and the EMED-system were used as equipment along with the SPSS 12.0 program, with a significance level of 0.05, to test the difference before and after taping between the two groups. Result : First, the 70s group showed a significant increase in the dorsal flexion angle after taping. Second, the 70s group showed a significant increase in duration time of standing on one foot after taping. Third, the 70s group's TUG time was significantly shorter. Fourth, the maximum peak value for static and dynamic plantar pressure showed a significant decrease after taping; and dynamic plantar pressure area value decreased as well within significant range. Conclusion : The study has shown that taping is effective on static dynamic balancing ability of the elderly.

A Study on Formative Characteristics of the Leggings Design in Contemporary Fashion (현대패션에 나타난 레깅스 디자인의 조형적 특성 연구)

  • Leggings are recognized as unique fashion items that meet the needs of customers to express individuality due to their protective and thermal functions. They are highly practical fashion items that show a casual, street and sportive style with a noted influence on outdoor leisure fields. This study analyzes the sociocultural background for the spread of leggings as well as their design characteristics, coordination characteristics and authentic characteristics. The formative characteristics of modern legging designs are as follows. First, the shapes of modern leggings are ankle-length, calflength, above-knee, knee-length, stirrup and top of the foot-length. Leggings are transformed in many ways: length-expansion, side-slit, unbalanced length, variations of straps, cut-out, trimmings, pattern transformation and complex form. Second, modern leggings contain achromatic colors in modern and sensual image, vivid colors in sportive image and metallic colors futuristic and technical image. Third, leggings patterns value originality, uniqueness and rarity as shown by printed geometric patterns, hand crafted geometric and natural patterns, and abstract patterns expressed through various materials and themes. Fourth, modern leggings' textures are categorized into erotic, simply-functional, seasonless, glossy, and metallic. Functional and fashionable leggings reflect the psychological needs of modern society. This study suggests a direction for the development of legging designs and provides a theoretical database for higher value-added leggings business.

A Study on the Preferences to the Functionality and the Level of the Discomfort to the Various Materials of the Footwear when Wearing (신발의 소재별 특성이 기능성 선호도와 착용시 불편 정도에 미치는 영향)

  • This study was focused on how the various materials of footwear affect the preferences of functionality and the level of discomfort when wearing. Datas were collected using questionnaire, age of 20's to 50's of female and male. Results were analysed using SPSS WIN 2.0 through t-test and ANOVA test. Results are as follows: In the preferences of materials of footwear, datas showed the shoes are of genuine leather, the sneakers are of textiles, the sandals are of man-made leather and the boots are of genuine leather. The important factor to the preferences of the materials were the leathers in the durability, the man-made leathers in easy-care and maintenance, the rubber and the complex materials in comforts. The level of the discomforts of the footwear were highly ranked in genuine leather, the man-made leather and the complex material with man-made leather and textiles followed. Man-made leather and the textiles represented the discomfort in the ankle. Mostly discomforts of the footwear showed in the sole and the top side of the foot in all kinds of footwear. The preferences in functionality were collected by the female, also female preferred the comfort the first, male preferred the durability. And the texture and the durability were highly preferred in aged female and male.
