일 대학병원 간호사의 발 건강 상태 (Foot Health Condition and Related Characteristics of Nurses)

  • Purpose: This descriptive study aimed to identify the foot health condition and related characteristics of nurses. Methods: A literature-based questionnaire was developed and distributed to 472 nurses in a university hospital in B city, and data were collected from August 1 to August 15, 2020. The Foot Health Status Questionnaire (FHSQ) was used to measure foot health condition. Results: The most common foot diseases among the participants were plantar fasciitis (7.8%) and hallux valgus (7.0%). To relieve foot pain, most nurses used relaxation techniques, stretching, elevating the legs, and foot massage. The level of foot health condition among nurses was lower than that of older persons or the general public with chronic foot disease. The foot health condition of the nurses showed that the lower the age and the greater the number of foot diseases, the more severe the pain and dysfunction. Conclusion: The study shows that there is a need for interventions that will raise nurses' awareness about foot health and induce better foot health management.

  • Objectives: To evaluate the effectiveness of reflexology foot massage (RFM) to improve quality of life and reduce anxiety for hospitalized cancer patients undergoing chemotherapy. Methods: This study was conducted at the East-West Cancer Center at Daejeon University; Using an waiting list and non-treatment control research design, we compared the results of control group and to that of the experimental group. The study consisted of 14 hospitalized cancer patients being admitted to the East-West Cancer Center of Daejeon University Dunsan Oriental Hospital in Korea. We measured FACT-G (Functional Assessment Cancer Therapy-General) and STAI (Spielberger State-Trait Anxiety Inventory) scale before, after and 48 hours after the intervention. The patients received 30 minute RFM 5 times a week. Results: There was an average increase on the FACT-G, Physical, total scale and decrease of STAI 1 scale in the treatment group compared with control group (P < 0.05). Anxiety in the treatment group showed significant decrease compared to control group. It suggests that RFM may alleviate cancer-related anxiety and contribute to the improvement in quality of life among cancer patients. Therefore, there was limited equivalences with experimental group. Conclusions: We concluded that RFM can be considered a supportive treatment used in combination with chemotherapy.

항암화학요법 관련 말초신경병증에 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석 (Effectiveness of Non-Pharmacologic Interventions in Chemotherapy Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis)

  • Purpose: This study was conducted to evaluate the effectiveness of non-pharmacologic interventions in chemotherapy-induced peripheral neuropathy (CIPN). Methods: PubMed, Cochrane Library CENTRAL, EMBASE, CINAHL, and several Korean databases (Until August 2017) were searched. The main search strategy combined terms for peripheral neuropathy and presence of neoplasms. The risk of bias was assessed using the Cochrane's Risk of Bias tool for randomized studies and the Risk of Bias Assessment tool for non-randomized studies. To estimate the effect size, a meta-analysis of the studies was performed using the Rev Man 5.3 program of the Cochrane Library random-effects models were used in the analyses. Results: Twenty-two studies with a total of 954 participants met the inclusion criteria. Of the 22 studies, 12 were used to estimate the effect size of the non-pharmacologic interventions. The non-pharmacologic interventions used in patients with CIPN were exercise, acupuncture, massage, and foot bath. The acupuncture significantly reduced CIPN symptoms and signs (d=-0.71) and CIPN pain (d=-0.73) (p<.001). Massage and foot bath were also effective in reducing CIPN symptoms (d=-0.68; 95% CI=-1.05, -0.30; p<.001; $I^2=19%$).Exercises were effective in improving muscle strength and endurance(d=-0.55) and quality of life (d=-2.96), but they were not significantly effective in improving CIPN. Conclusion: Although these results provide little evidence of the effectiveness of acupuncture, massage, and foot bath in the treatment of CIPN, they suggest that these interventions can reduce CIPN symptoms in patients with cancer. However, the findings of this study should be interpreted with caution as there is a relative lack of data in this field, and additional well-designed studies are needed. 아로마테라피가 노인의 만성통증과 수면에 미치는 효과 (The Effects of Aromatherapy on Elders' Chronic Pain, Sleep)

  • 본 연구는 지역사회 노인을 대상으로 아로마테라피 발마사지를 중재하여 만성통증과 수면에 미치는 효과를 규명하기 위한 실험연구이다. 대상자는 총 43명으로 실험군 22명, 대조군 21명이었다. 실험군에게 적용한 에센셜오일의 구성은 케모마일 로만, 라벤더, 마조람 스위트, 오렌지 스위트를 캐리어 오일인 호호바오일 100ml에 2%로 희석한 것으로 하였다. 중재는 실험군에게 2주 동안 주당 3회, 1회 10분, 총 6회 시행하였다. 자료분석은 SPSS/WIN 21.0을 이용하여 $x^2$-test, independent t-test로 분석하였다. 본 연구결과 실험군은 대조군보다 만성통증은 실험군에서 통계적으로 유의하게 감소(t=-17.527, p=.000)가 있었고, 수면의 질은 실험군에서 통계적으로 유의하게 향상(t=38.125, p=.000)되었으며, 수면 중 소변횟수도 실험군에서 통계적으로 유의하게 감소(t=-10.088, p=.000)가 있었다. 따라서 아로마테라피 발마사지는 약물복용 없이 지역사회 노인의 만성통증 완화와 수면이 향상되어 건강관리와 증진을 위한 유용한 중재방법으로 활용 가능할 것으로 생각된다.

호스피스 자원봉사자의 발마사지와 지지적 의사소통이 외래 항암화학요법 환자의 우울, 불안 및 기분에 미치는 효과 (Effects of Foot Massage and Supportive Communication by Hospice Volunteers on Depression, Anxiety, and Mood of Cancer Patients Who Undergo Intravenous Chemotherapy at Out-patient Department)

  • 목적: 종합병원 외래 주사실에서 항암화학요법을 받는 암환자를 대상으로 발마사지와 지지적 의사소통 제공 후 우울, 불안, 긍정적 및 부정적 기분의 변화를 확인하기 위함이다. 방법: 비동등대조군 전후 시차 유사 실험설계로 연구의 대상은 원주시의 일 3차 종합병원 외래 주사실에 항암화학요법을 받는 암환자를 편의표집하여 대조군 34명 실험군 30명 총 64명이었다. 연구도구는 일반적 특성과 질병특성, 최근에 경험하는 신체적 증상 측정 문항, 그리고 각각 10 cm 시각상사척도를 이용하여 우울, 불안, 활기, 의욕, 희망, 긴장 및 무기력을 측정하였다. 결과: 제 1 가설인 '발마사지와 지지적 의사소통을 받은 실험군은 대조군보다 처치 후에 우울과 불안이 더 많이 감소될 것이다'는 처치 후 실험군에서 우울(t=5.26, P<0.001)과 불안(t=5.07, P<0.001)이 유의하게 감소되어 지지되었다. 제 2 가설인 '실험군은 대조군 보다 처치 후에 활기, 의욕 및 희망이 더 증가될 것이다'는 두 군에서 활기, 의욕 및 희망의 처치 전후의 변화를 비교한 결과 실험군에서 유의한 변화가 없어 기각되었다. 제 3 가설인 '실험군은 대조군보다 처치 후에 긴장과 무기력이 더 감소될 것이다'는 처치 후 두 군 모두에서 긴장과 무기력이 유의한 감소를 나타냈으므로, 중재 전후 변화의 정도에 차이가 있는지를 확인하기 위해 두 변수의 중재 전과 후의 차이값의 평균을 비교하였다. 검정결과 대조군에 비해 실험군에서 긴장(t=5.64, P<0.001)과 무기력(t=5.38, P<0.001)이 더 큰 감소를 보여 제 3 가설은 지지되었다. 결론: 본 연구의 결과는 외래 주사실에서 항암화학요법을 받는 동안 호스피스 자원봉사자가 제공한 발마사지와 지지적 의사소통이 암환자의 우울, 불안, 긴장, 무기력을 감소에 유용함을 제시하였다. 연구 결과의 타당성을 높이기 위해 대상자 수를 확대하고 좀 더 정련화된 중재의 개발과 효과검증을 위한 추후 연구가 필요하다.

자가 발반사마사지가 당뇨병 환자의 말초순환과 말초신경증에 미치는 효과 (Effect of Self-Foot Reflexology on Peripheral Blood Circulation and Peripheral Neuropathy in patients with Diabetes Mellitus)

  • Purpose: This study was done to investigate the effect of self-foot reflexology on peripheral blood circulation, peripheral neuropathy and to determine the feasibility of self-foot reflexology as a nursing intervention. Method: This was nonequivalent control pretest-posttest study with 76 patients with type 2 diabetes mellitus (ages between 40-79) recruited from public health centers in Busan city. Intervention was a 6 week self-foot reflexology, and outcome variables were peripheral blood circulation and peripheral neuropathy(tactile response to monofilament, intensity of symptoms of peripheral neuropathy). ANCOVA was used to do the statistical analysis. A.05 significance level was set for evaluating the effects of self-foot reflexology. Results: The self-foot reflexology was relatively effective not only in reducing peripheral neuropathy(especially tingling sensation and pain) but also in improving ability to sense the 10-g force monofilament. Conclusion: Even though self-foot reflexology was not effective in improving peripheral circulation, it had good effect on improving peripheral neuropathy. Therefore self-foot reflexology can be used as a nursing intervention program for promoting foot care for patients with DM patients.

음파진동 안마의자제품의 성능향상을 위한 판스프링의 효율적 설계 (Efficient Design of Plate Spring for Improving Performance of Sound Wave Vibration Massage Chair)

  • The customer of massage chair is expanding day by day from middle age to all ages. In 2018, the market size was 700 billion KRW, an increase of 30 times over 10 years. However, most related SMEs suffer from excessive competition by the market monopoly of some major companies. In this situation, in order for a related company to survive, it is necessary to steadily research and develop new products. Founded in 2009, company L produces massage chairs for health and relaxation of customers. L's products use a sound wave vibration module that is favorable for human body, unlike other products that use vibration motor type. However, frequent breakdowns of massage chair due to the vulnerability of plate (leaf) springs, which play an important role in sound wave vibration modules, made sap its competitiveness. In this paper, we propose a method to design desirable plate spring structure by sequentially experimenting with five different plate springs. The results of this study are expected to contribute to improve the quality of plate spring and the reliability of sound wave vibration module. In the future, it is necessary to find a way to use it in the development of foot massage or scalp management device as well as continuous research to find optimal plate spring structure through various analysis.

갱년기 여성을 위한 발건강 관리방법에 관한 문헌연구 (A Study on Foot Reflexology Therapy for Climacteric Women)

  • The purpose of this study is to present various ways for women to prevent and remedy the climacteric symptoms and to increase the health and quality of life of the women. The reason of this study focuses on this matter is that the conditions of climacteric era is connecter into the health of golden age. After examining the symptoms of the period, the researcher went over the foot heal care even recognized to the public. When the study considered the foot care as a medical service for the climacteric women, the method was expected as a way to improve the symptoms related to the climacteric diseases. This effect is also supported by the results of the clinical experiment and book media materials concerned. The main symptoms of climacteric period such as insomnia, irregular menstruation, stomach chill, stress were able to be decreased by feet bathing, and massage therapy on feet could also decrease pain of arms and legs, menstruation, headache and insomnia. The foot reflexology therapy made strengthen the periphery nerve, capillary vessels, and immune system, and increased the elasticity of blood vessels and the speed of the blood corpuscle, so they could enable to the blood circulation. The study expects the foot care to be positively method for climacteric symptoms with regular breathing, muscle relaxing exercise, acupuncture, exercise and alimentotherapy. The foot care doesn't have any side effects and could ives up to the modern people who want to keep their health and live long.

발의 형태, 구두 디자인, 구두 착용태도에 따른 발의 불편감 (Foot discomfort by foot shape, shoe design and wearing attitude)

  • As women wear shoes for a long time due to aesthetic elements and working environment, many women experience discomfort and deformation in their feet due to their shoes and the production of comfortable shoes is becoming an important issue. The purpose of this study is to investigate the factors affecting the grip of shoes by foot type, shoe design, and wearing attitude of shoes. Through this, we suggest solutions for foot discomfort due to wearing shoes and help to prevent foot related diseases. The study results are as follows; first, a wide foot, square-type, and high or low arch (hollow foot or flat foot) among foot shapes influenced the fatigue and pain of feet, and big feet over 250mm-long, wide feet, square-type feet, and high-arch feet (hollow foot) had an influence on distortion and side effects. Second, among the characteristics of wearing shoes influencing foot discomfort, the higher the shoe heels were, the narrower the surface covering the feet was (pumps and mule), and the shoes with high front heels and narrow back heel area, the shoe wearer had a higher possibility of experiencing fatigue and pain and even had not only fatigue and pain but also side effects when she wore pointed shoes. Third, a shoe wearer experiences fatigue and pain if she wears shoes for a shorter period of time, stay stood while wearing shoes for a short time, and wears shoes that are bigger or smaller than the actual shoe size. Fourth, fatigue and pain experiencers and distortion and side effects experiencers all responded that they change into other shoes to deal with foot discomfort and that they directly massage their feet.

족저 근막염의 진단과 치료 (The Diagnosis and Treatment of Plantar Fasciitis)

  • Plantar fasciitis is the most common cause of heel pain. The diagnosis of plantar fasciitis is primarily based on the presentation of symptoms and physical examination. Patients usually complain of heel pain at the medial calcaneal tubercle when taking their first step in the morning or when walking after resting. Diagnostic imaging is rarely required for the initial diagnosis of plantar fasciitis; however, it can be used for differential diagnosis. Conservative treatments, such as stretching, rest, ice massage, oral analgesics, foot orthotics, use of night splint, and corticosteroid injection, may be effective. The majority of patients report improvement with conservative treatments, and those who show no response from conservative treatments for a duration of six months or longer can consider extracorporeal shock wave therapy or surgery.
