The Clinical Evaluation of The Reconstruction of Radial Forearm Free Flap in the Head and Neck Cancer Surgery (두경부 악성 종양 절제술후 요골 전완 유리피판을 이용한 재건술의 평가)

  • Background and Objectives: The reconstruction is very important in Head and neck cancer surgery to repair the defect created by resection of tumors, to enable successful wound healing, to restore function and to provide acceptable cosmesis. The radial forearm free flap has been the most useful reconstructive flap because it provides a moderate amount of thin, pliable, relatively hairless skin and comparatively simple to do with minimal morbidity. The aims of this study is to estimate the outcome of the reconstruction with radial forearm free flap with the several factors in 140 head and neck cancer cases in our hospital for last 10 years. Materials and Methods: Retrospective review of the records of 140 patients underwent resection of the head and neck tumors and reconstruction with a radial forearm free flap from 1993 to 2003. The age, sex of the patients, Primary site, the complication of donor and recipient site, flap survival rate, median time to start diet, patient subjective symtoms about swallowing and articulating and the fact of revision reconstructive surgery were analyzed. Results: In primary pathologic site, 56 cases were oral cavity cancers, 44 cases, oropharyngeal cancers and 22 cases, hypopharyngeal cancers. Flap survival rate was 93.6% (13 leases). On donor site, wound dehiscence, hematoma, sensory change and infection were noted and on recipient site, most common complication were fistula and wound dehiscence. The complication rate of recipient's site was 19.1 % and donor site, 3.5%. In 118 cases (84.3%), the patients could take all kinds of food. Swallowing difficulty were noted in 22 cases 05.7%). In 5 cases, there was articulation difficulty but most of patients except patients having total laryngectomy (18 cases) couldn't have any difficulty in articulation and speaking. Conclusion: We conclude that the radial forearm free flap is the most appropriate reconstructive material for treating the defect in head and neck reconstruction.

Effects of Scutellariae Radix-acupuncture solution On Anti-inflammatory in RAW 264.7 macrophages (황금약침액(黃芩藥針液)이 LPS로 유도(誘導)된 RAW 264.7 대식세포(大食細胞)에서의 항염증효과(抗炎症效果))

  • Objective : Recently, Herbal-acupuncture therapeutics has been used for the treatment of inflammatory diseases such as rheumatoid arthritis. Especially, we have been interested in chemical mediators concerned with inflammation such as prostaglandin, cytokine, nitric oxide. The purpose of this study is investigated that the effect of Scutellariae Radix-acupuncture solution in lipopolysaccharide-stimulated RAW 264.7 macrophages, performed several expeimental items : those are prostaglandin E. nitric oxide and cyclooxygenase-2. Method : The cytotoxicity of Scutellariae Radix-acupuncture solution in RAW 264.7 macrophages were measured by MTT-based cytotoxicity assay. In order to observe cyclooxygenase-2 mRNA expression in lipopolysaccharide-stimulated RAW 264.7 macrophages, RT-PCR was used. Prostaglandin $E_2$ formation and nitric oxide production was measured by competitive enzyme immunoassay and Griess assay. Results : 1. The MTT assay demonstrated that cytotoxic effect of Scutellariae Radix-acupuncture solution in RAW 264.7 macrophages were not appeared before concentration of 5mg/mL 2. Scutellariae Radix-acupuncture solution inhibited cyclooxygenase-2 mRNA expression in lipopolysaccharide-treated RAW 264.7 macrophages. 3. Scutellariae Radix-acupuncture solution inhibited nitric oxide production in lipopolysaccharide-treated RAW 264.7 macrophages. 4. Scutellariae Radix-acupuncture solution inhibited prostaglandin $E_2$ formation in lipopolysaccharide-treated RAW 264.7 macrophages. Conclusion : On the basis of these results, It was shown that Scutellariae Radix-acupuncture solution is significantly able to inhibit the production of $PGE_2$ and NO, as well as COX-2 mRNA expression. Our results may provide new mechanism by which Scutellariae Radix-acupuncture solution accounts for its beneficial effect on accelerating wound healing and anti-inflammation.

Concept and Policy Developments on Eco-welfare of National Parks based on Ecosystem Service (생태계서비스 기반 국립공원 생태복지 개념정립 및 정책방향 설정)

  • The concept of Eco-welfare emerged in the process of discovering the relationship between a healthy ecosystem and human wellbeing. The objective of this study is to offer basic data for eco-welfare policy realization by conceptualizing the 'National Park Eco-welfare' since national parks are appropriate places for eco-welfare to be optimally implemented. A pre-workshop and two rounds of Delphi surveys were conducted to determine the concept of the 'National Park Eco-welfare ' and the main elements to be included in ecosystem service. Through this study, the concept of 'National Park Eco-welfare' was defined as efforts to conserve the biodiversity and sustainability of the ecosystem and create harmony between 'National Park Ecosystem Welfare' and 'National Park Human Welfare'. To establish the direction of the National Park Eco-welfare policy, we identified the main elements of ecosystem services which are appropriate for the National Park. These are comprised of 3 elements (food, fresh water and genetic resources) from supply service, all elements from regulating service and supporting/habitat service and 4 elements (aesthetic information, recreation-based ecotourism, healing, and knowledge systems with educational values) from culture service. In this study, the concept of National Park Eco-welfare was established, and its policy objectives and scope were suggested. However, further studies are necessary to develop action plans areand thereby realize the policy.

Bioequivalence Test of Rebamipide 100 mg Tablets (레바미피드 100 mg 정제의 새울학적동등성)

  • Rebamipide, ($\pm$)-2-(4-chlorobenzoylamino)-3-[2(1H)-quinolinon-4-yl] propionic acid, is used for mucosal protection, healing of gastroduodenal ulcers, and treatment of gastritis. It works by enhancing mucosal defense, scavenging free radicals and temporarily activating genes encoding cyclooxygenase-2. The purpose of the present study was to evaluate the bioequivalence of two rebamipide tablets, $Mucosta^{(R)}$ (Korea Otsuca Pharmaceuticals Co., Ltd.) and Mustar (Korean Drug Co., Ltd.), according to the guidelines of the Korea Food and Drug Administration (KFDA). The release of rebamipide from the two rebamipide formulations in vitro was tested using KP VIII Apparatus II method with pH 6.8 dissolution medium. Twenty six healthy male subjects, $23.46{\pm}2.63$ years in age and $66.62{\pm}8.97\;kg$ in body weight, were divided into two groups and a randomized $2{\times}2$ cross-over study was employed. After a single tablet containing 100 mg as rebamipide was orally administered, blood samples were taken at predetermined time intervals and the concentrations of rebamipide in serum were determined using HPLC with fluorescence detector. The dissolution profiles of two formulations were similar in the tested dissolution medium. The pharmacokinetic parameters such as $AUC_t$, $C_{max}$ and $T_{max}$ were calculated, and ANOVA test was utilized for the statistical analysis of the parameters using logarithmically transformed $AUC_t$, $C_{max}$ and untransformed $T_{max}$. The results showed that the differences between two formulations based on the reference drug, $Mucosta^{(R)}$ were -5.08, 3.52 and -9.71 % for $AUC_t$, $C_{max}$ and $T_{max}$, respectively. There were no sequence effects between two formulations in these parameters. The 90% confidence intervals using logarithmically transformed data were within the acceptance range of log 0.8 to log 1.25 (e.g., log 0.84$\sim$log 1.07 and log 0.90$\sim$log 1.17 for $AUC_t$ and $C_{max}$, respectively). Thus, the criteria of the KFDA bioequivalence guideline were satisfied, indicating Mustar tablet was bioequivalent to $Mucosta^{(R)}$ tablet.

Comparison of Growth Characterisitics, Asiaticoside Content and Antioxidant Activities of Centella asiatica (L.) Urb. (병풀의 생육 특성, Asiaticoside 함량 및 항산화 활성 비교)

  • Centella asiatica (L.) Urban is a medicinal plant containing madecassic acid, triterpenoids and asiaticoside, and has a wound healing effect. However, since it grows only in some regions of Korea, it is necessary to breed stable varieties. In this study, it were analyzed for the growth characteristics, asiaticoside, anxiodant components and 2,2-diphenyl-1picryl hydrazyl (DPPH) radical scavening activity of C. asiatica collected from domestic natural habitat (Hapcheon, Gyeongnam). In the growth characteristics of each resource, CA9 and CA10 were superior in characteristics of aerial part and in dry weight. In asiaticoside content analysis, CA5 and CA3 were high with 53.96 mg/g and 52.40 mg/g, respectively. CA12 showed the possibility of development because the resource showed high in total flavonoid (7.3 mg/g), total polyphenol (10.64 mg/g) and DPPH activity (11.96 mg AAE/g) among other resources. However, it is necessary to select a resource suitable for each characteristic of C. asiatica and to study charateristics through additional resources collection since the excellent growth characteristics of the collected resources do not necessarily coincide with the content and antioxidant activities.

Antioxidant Effect of Hot Water Extracts from 3 Types Indonesia Plants (Hibiscus Petals, Moringa Oleifera Gymnosperm, and Nipa Fruticans Wurmb) (인도네시아 식물 3종(히비스커스 꽃잎, 모링가 겉씨, 해죽순) 열수추출물의 항산화 효과)

  • Purpose: This study investigated the antioxidant activities of water extracts from Hibiscus petals, Moringa oleifera gymnosperm, and Nipa fruticans wurmb. Also, the possibility of their use as a functional cosmetic material and food were searched. Methods: We extracted Hibiscus petals, M. oleifera gymnosperm, and N. fruticans wurmb with water. And then, we measured the content of total polyphenols and flavonoids and the ability to scavenging free groups of ABTS and DPPH to study the antioxidant function. The toxicity of samples evaluated by measuring cell viability. Results: The polyphenol content of the water extract of N. fruticans wurmb was 109 ㎍/mg, which was significantly higher than that of Hibiscus petals (13 ㎍/mg) and M. oleifera gymnosperm (19 ㎍/mg). Radical scavenging ability was also excellent in N. fruticans wurmb, and the cytotoxicity test results of the samples were similar. Conclusions: The water extracts showed antioxidant activity to use for cosmetic materials or natural healing foodstuff.

Analysis of Mediating Effects of Eating Habits on the Relationship between Stress and Depression in the Elderly (노인의 스트레스와 우울 간의 관계에서 식습관의 매개 효과 분석)

  • Background: It is necessary to understand the correlation between stress and depression according to eating habits in the elderly. Purposes: To empirically verify the mediating effect of eating habits in the relationship between stress and depression in the elderly. Methods: we distributed an 'online questionnaire' to men and women aged 60 or more living in large cities, medium, and small-sized cities in Korea using Google questionnaires, and 365 replies were collected and used for analysis. Results: First, we found that stress in the elderly directly affected depression. In summary, an increment in the stress level of the elderly induces an increment in the depression level. Second, the eating habits of the elderly indirectly mediated the relationship between stress and depression. Conclusions: This study is meaningful in that it has verified eating habits have a mediating effect on the relationship between stress and depression in the elderly, and healthy eating habits of individuals can reduce depression. Therefore, counseling institution must check their daily eating habits when counseling the degree of depression of the elderly. In addition, eating habits are affected by stress, and it is necessary to grasp the individual stress index for the elderly who has terrible eating habits. Furthermore, it is needed to provide continuous nutrition education in institutions and communities to have proper eating habits.

A study on antioxidant and anti-inflammatory effects of domestic blended essential oils (국내산 블렌딩 정유의 항산화 및 항염 효과 연구)

  • Previous studies have been conducted on domestic materials as a single extract. Research on complex mixtures for maximizing plant characteristics by individual extraction and potential interference with effects is insufficient. Therefore, this study confirmed the GC-MSD according to the extraction of essential oils for Agastache rugosa O. Kuntze(AR), Pinus densiflora Sieb. et Zuccarini(PD), Curcuma longa, Curcuma domestica(CC), Zingiber officinale Roscoe(ZR), Foeniculum vulgare Miller(FV), and Citrus medica L. var. sarcodactylis Swingle(CS). The cytotoxicity, antioxidant, and anti-inflammatory properties of the blending oil were confirmed to confirm its potential as a cosmetic material. As a result of analyzing GC-MSD aroma components, the main components were estragole of AR, à-Pinene for PD, Zingiberene for CC and ZR, Anethole from FV, and D-Limonene for CS. At a concentration of 100 uL/mL with no confirmed cytotoxicity, NO production was inhibited by 70.62%, DPPH radical scavenging activity was 64.03%, and ABTS radical scavenging activity was 89.55%. Through this, blended essential oil suggests the possibility of useful application as a raw material with antioxidant and anti-inflammatory effects in the cosmetic and food industries.

The Current State of Intended Equipment for Heating in Medical Use Based on Domestic Licensed Medical Devices (국내 인·허가 온열의료기기 기술 현황 조사 및 분석)

  • Objectives : This study aimed to determine the status of thermal stimulation devices approved in Korea for medical applications over the past 10 years, and based on this, to obtain insight for future thermal treatment in Korean medical institutions. Methods : We searched the item classification list entitled "Regulations on Medical Device Items and Rating by Item" from the Ministry of Food and Drug Safety Notice No. 2021-24, 2021 (Enforced March 19, 2021; www.mfds.go.kr) for individually licensed heaters using the terms "heat" and "heating". Results : We identified 17 items of thermal stimulation product group, of which 1,308 devices were licensed by February 4, 2022, and 53.2% of them (n=696) were devices with valid permits for distribution in Korea. Among the licensed devices, heating pad systems under/overlay (electric, home use) were approved the most, but combinational stimulator (for medical use, home use; Grade 2) accounted for the highest percentage among the current valid permission. Moxibustion apparatuses were licensed separately for electrical use and non-electrical use, and occupied a low percentage of the total devices. We analyzed 307 devices that were accompanied by technical documents and found that the heat sources were wires in 145 (47.2%), infrared rays in 44 (14.3%) and ultrasonic waves in 42 (13.7%) devices. Most (83.1%) devices were used for pain relief, while other applications included beauty, cancer treatment, maintenance of infant body temperature, and healing fractures. Conclusions : Thermal stimulation devices accounted for about 0.9% of all medical devices, and among them, combinational stimulators and heating pad systems under/overlay had the most valid permits. Thermal stimulation devices using heating wires and infrared rays were the most prevalent, and most were used to relieve pain. In order to develop a range of thermal stimulation devices that can be utilized in Korean medical institutions, it is imperative that they have potential applications beyond pain management, addressing various medical purposes. To achieve this, foundational research is necessary to effectively apply diverse heat sources based on medical objectives.

DEU-7 Derived from Ulmus macrocarpa Improved Immune Functions in Cyclophosphamide-treated Mice (면역억제 마우스 모델에서 왕느릅나무 유래 DEU-7의 면역기능 증강)

  • The present study investigated the immunomodulatory properties of four different medicinal plants in a cyclophosphamide-treated Balb/c mouse model. One of the four plants, Ulmus macrocarpa, showed partial resistance against immune suppression induced by cyclophosphamide. The bark of U. macrocarpa, commonly known as the Chinese elm, has been used as a pharmaceutical material in Korean traditional medicine to treat bacterial inflammation and induce wound healing. In this study, water extract of U. macrocarpa, named DEU-7, was used for its immunomodulating functional activity. DEU-7 increased the weight of the spleen and the number of splenocytes but did not significantly affect the liver, kidney, and thymus in vivo. A splenocyte viability assay confirmed that DEU-7 influenced ex vivo splenocyte survival. DEU-7 also increased the levels of cytokines, such as IL-2 and IL-4, and immunoglobulins, such as IgM, IgG, and IgA. These results indicated that DEU-7 is involved in the activation of T and B lymphocytes. In addition, DEU-7 was able to maintain the production of cytokines, such as TNF-α, IL-12, and IFN-γ, in the condition of cyclophosphamide-induced immune suppression, suggesting that DEU-7 activated innate immune cells, even under immune suppression. We concluded that DEU-7 aids immunological homeostasis, thereby preventing immune suppression, and aids both innate and adaptive immune response by maintaining the levels of various cytokines and immunoglobulins. Consequently, it is worth investigating the potential of DEU-7 as a supplemental source for immune-enhancing agents.
