  • Throwing motion divided by 6 phases such as wind-up, (early and late) cocking, acceleration, deceleration, follow-through. Overuse injuries in baseball result most often from pitching. Poor mechanics is the leading cause of arm injuries. The purpose of this article is to analyze the each joint motion and evaluate the biomechanics and pathomechanics during pitching.

  • From August 1988 to August 1991 40 automatic staplers[four TA 30 Premium and 36 TA 55 Premium] were used to close the bronchial stump in 38 patients. There were 8 pneumonectomies, 5 bilobectomies, 23 lobectomies, 1 lobectomy and segmentectomy, and 1 segmentectomy due to various pulmonary diseases. Among 40 bronchial stumps, 15 stumps were covered with the pleural flaps for reinforcing the stump and remained 25 stumps left uncovered. The patients of the 2 groups were selected randomly regardless of disease entities. The duration of follow-up was from 1 month to 38 month-average 16 months, and there were no bronchopleural fistulas in all cases during the follow-up period. Especially in the uncovered bronchial stumps group, 3 preoperative sputum AFB[+] patients and 2 patients of stage IIIa lung cancer underwent pneumonectomy with ipsilateral radical lymph node dissection were included. So, surgical stapling is the safe method and is recommended in closing the bronchial stump in various pulmonary resections.

  • This is case report of total anomalous pulmonary venous return with atrial septal defect which were corrected surgically by intracardiac procedure under total cardiopulmonary bypass. Two patients were supracardiac type, cardiac and mixed type was each one.The mixed type was three years old female patient. She was diagnosed as atrial septal defect with partial anomalous pulmonary venous return[right pulmonary vein drains into superior vena cava and right atrium] and corrected as usual. After operation, she underwent exertional dyspnea and frequent tachycardia.Chest x-ray film showed pulmonary congestion.Follow up cardiac cineangiogram revealed that left pulmonary vein also anomalously drained into left innominate vein through vertical vein.Through left thoracotomy, anastomosis was successfully carried between left atrium and vertical vein without cardiopulmonary bypass and there was no sign of pulmonary artery obstruction for two years follow up.The other three patient were corrected successfully without complication and got good result.

  • Purpose : The purpose of this study was to investigate the effect of mat activities in PNF on improvement of balance performance in stroke patients. Methods : Included 12 persons with stroke who were living in the community. They participated in exercise class for 60minutes duration three times a week for 6 weeks. Mat activities in PNF program was modified PNF patterns and techniques on various posture. Balance indexes were measured at pre-treatment, post-treatment and follow-up by K.A.T.3000. Results : Balance index scores had larger improvements after 6 weekends of treatment(p<.05) than pre-treatment, also follow-up test(p<.05). Conclusion : The result of this study showed that PNF mat activities intervention can improve balance performance in disabled persons after stroke.

  • A bronchopleural ~stula[BPF] is a major therapeutic challange to the thoracic surgeons due to difficulties in its management. We have experienced 9 cases of postpneumonectomy BPF, and all of them occured after the right side pneumonectomy. Seven patients underwent pneumonectomy due to lung cancer, and one patient each received pneumonectomy due to bronchiectasis and pulmonary tuberculosis. The most frequently used method of treatment was Eloesser operation which was done in 7 patients, and one had open thoracostomy and one had betadine irrigation until he died suddenly due to pulmonary vein rupture. The result of treatment was poor with five mortalities including one operative death. Two patients are in good condition in 28 months and 4 years of follow-up, but two are lost in follow-up. Tissue sealant such as Tisseel was used in three patients but seems to have no effect at all in the treatment of postoperative BPF.

  • This paper summarized the results of previous and recent studies on cumulative trauma disorders (CTD) and provides a prevention strategy for CTD and showed the results from a quite extensive survey with tables and figures. This is a follow-up study of Kwon (1993) 〔2〕. From the previous studies, Kwon (1993) showed that many people are exposed on CTD, but they did not only know the clear reasons why they got CTD, but also how to prevent or change their workstation〔2〕. Lee (1996) showed the result from both field survey and medical examination. He found that most frequent symptoms are CTS (nerve disorders) and tendon disorders 〔6〕. From these previous results, it is obvious that CTD patients will increase in Korean industries if we did not pay attention to this CTD problem. Therefore a proper education of Ergonomics, CTD prevention for workers and moderate changes in workplace are necessary. From 1998 and 1999 follow-up studies, there are still many people(23%) did not hear about CTD, CTS, or MSD. Only 42% of respondents is exercising during or after work for short-time. Therefore, there are many rooms for prevention strategy of CTD and education for preventing CTD.

  • Investigating LVF directly, quantitatively and non-invasively by MUGA helps to validate and to follow up the effectiveness of new kinds of therapy. This is performed in controlled studies comparing groups of patients as demonstrated in the case of intracoronary thrombolysis. But it is also applicable on an individual basis. Contrary to the protocol in controlled studies medication was not discontinued in our patients before and after bypass surgery. This resembles the situation under routine conditions. Therefore the number of patients with reduced LVF was rather small before and after bypass operation. But, and this in the main result of our study, implementing MUGA for follow-up under this conditions can help: 1. To find out early those patients in whom LVF deteriorates postoperatively despite adequate medication. 2. To better define whether in cases of persistent or recurrent angina there is also deterioration of LVF. By this means MUGA can significantly contribute to find out whether reangiography is necessary or not, a question which often is difficult to answer in those patients.

  • An odontoid process fracture is a serious type of cervical spine injury. This injury is categorized into three types based on the location of the fracture. Severe or even fatal neurological deficits can occur due to associated cord injury, which can result in complete quadriplegia. Computed tomography is the primary diagnostic tool, while magnetic resonance imaging is used to evaluate any associated cord injuries. These injuries can occur either directly from the injury or during transportation to the hospital if mishandled. There are two main treatment approaches: surgical fixation or external nonsurgical fixation, with various types and models of fixation devices available. In this case study, computed tomography follow-up confirmed that external fixation can yield successful results in terms of complete healing, even in cases complicated by other factors that may impede healing, such as pregnancy.
