The Effect of Indwelling Silk Suture Following Aspiration in the Treatment of Chronic Olecranon Bursitis (흡입 후 견 봉합사 거치를 통한 만성 주두 점액낭염의 치료)

  • The purpose of this study was to evaluate the drainage effect of silk suture following aspiration of the bursa as an early treatment of chronic olecranon bursitis. Eleven cases, which have over two weeks of history and over one year of follow-up, were investigated. The average duration of follow-up was 17.5 months. The average symptom duration was 1.8 months. With an aseptic technique, the aspiration of the bursa was done with 18gauge needle and syringe and then the insertion of silk suture through the aspiration needle was performed. The amount of drainage was regularly checked $2{\sim}3$ days interval and stitch out was done at the cessation of drainage. At the follow-up, recurrence, infection, pain, and limitation of range of motion were investigated by telephone interview. Redness around the insertion site of silk suture was found in all cases, but there was no active infection or recurrence. The results were satisfactory in all cases and the average time for recovery was 10.5 days. The drainage with silk suture following aspiration of the bursa is less invasive and very effective method in the early treatment of chronic olecranon bursitis.

System Representation for the Control System of the Follow-up Mechanism on the Marine Gyro Compass

  • It does not seem necessarily practicable to keep the system always in optimal condition, athough the control system of the follow-up mechanism on the most marine gyro compasses is to be adjusted by the operator through the gain adjustment. Sometimes a sustained oscillation or an incorrect gyro reading occurs to the system. For such a system any systematical research or theoretical basis of the guide for the optimal gain adjustment has not been reported yet. As a basic investigation of the theoretical system analysis to solve the problems concerned, the author attempts in this paper to express the system in a mathematical model deduced from the results of the theoretical approach and the experimental observation of each element contained in the follow-up mechanism of Hokshin D-1 gyro compass, and to constitute an over-all closed loop transfer function. This funciton being reverted to a fourth orderlinear differential equation, the first order simultaneous differential equations are obtained by means of the state-variables. The latter equations are solved by the Runge-Kutta method with digital computer. By comparing the characteristic of the simulated over-all output with that of the experimental result, it is shown that both outputs are nearly consistent with each other. It is also expected that the system representation proposed by this paper is valid and will be a prospective means in a further study on the design and optimal adjustment of the system.

  • Trade finance promotes export performance, and every small- and medium-sized export business has the right to use policy finance. The credit line is also stipulated by relevant laws and regulations. However, trade finance has repeatedly been misused, so these matters can lead to substantial financial loss and damage to the related financial institutions. The lack of expertise of K-SURE and the backwardness of banks represent even bigger problems in the follow-up management. The existing trade finance system should be improved in the following ways from the institutional and legal perspectives. Firstly, follow-up management of beneficiary companies should be reinforced and systematized through examining business tendencies, financial status, and other important factors. There is also a need for advancement of following up management. An effective and streamlined financing system must be established by abolishing the Korea Trade Insurance Corporation. This study presents improvements and their implications by looking into the main issues under the current trade finance system. This study is based on documentary research and practical cases.

Implementation of Ship Trajectory Following Algorithm

  • As interest in autonomous ships continues to grow, researchers around the world are dedicating themselves to the development of relevant technologies. However, these technologies are not yet perfect. Several technical problems remain unresolved. To address these problems, this study presents the implementation of a ship trajectory algorithm for group navigation, where followers can navigate by following the trajectory of a leader. The algorithm works by storing the leader's trajectory as a follow-point and by calculating the azimuth using the line-of-sight guidance law to reach it. A course-keeping controller based on PD control is implemented to follow the target course and a speed control algorithm is designed to prevent collisions. Sea experiments were conducted using 1 m class small RC model boats to verify the proposed algorithm. The follower successfully navigated by following the leader's trajectory and maintained the designated distance to the forward boat. This study is significant in that it implements an algorithm for the follower to follow the trajectory of the leader rather than directly following it as in conventional methods, and verifies it through sea experiments.

Longitudinal Relationships between Cigarette Smoking and Increases Risk for Incident Metabolic Syndrome: 16-year Follow-up of the Korean Genome and Epidemiology Study (KOGES)

  • This study aimed to determine whether smoking affects the metabolic syndrome and its components through long-term follow-up. Of the 10,030 cohort subjects in the community-based Korean Genome and Epidemiology Study (KoGES) from 2001 to 2018, 2,848 people with metabolic syndrome and 4,854 people with insufficient data for analysis were excluded for this study. The study population comprised 2,328 individuals (1,123 men, 1,205 women) who were eligible for inclusion. The mean age of the participants was 49.2±7.5 years, and 21.9% were current smoker. In log rank test, current smoker had a significantly higher cumulative incidence of metabolic syndrome compared with non smoker (P<0.001). In the Cox proportional hazards model adjusted for key variables, metabolic syndrome (hazard ratio [HR] 1.57, P<0.001), high fasting glucose (HR 1.40, P<0.01), hypertriglyceridemia (HR 1.60, P<0.001), low HDL-cholesterol (HR, 1.30, P<0.01), and abdominal obesity (HR 1.32, P<0.01) in current smoker compared with non smoker were statistically significant, respectively, but not hypertension (HR 1.00, P>0.05). After adjustment for confounders, the time (P-time<0.001) and group (P-group<0.001) effects on metabolic syndrome score change were statistically significant. Furthermore, the interaction analysis of time and smoking group on the change in metabolic syndrome score was statistically significant (P-interaction<0.001). In long-term follow-up, smoking worsens metabolic syndrome.

On the Flowering Dates of the Woody Plant Species in the Hongneung Arboretum, Seoul (홍릉수목원수종의 개화기에 관하여)

  • Flowering dates of 389 plant species in the Hongneung Arboretum, Seoul, had been recorded from 1968 through 1975. The thermal analysis on the air temperature as the key factor determining the first flowering date, with climatological data obtained in the Arboretum, were undertaken by Nuttonson's Index (1948) and Lindsey & Newman's Index (1956). The results and conclusion in this study are as follow; Peak in the bell shape distribution curve of the species and first flowering dates, largely, was early May. Flower spans of most species were 10 to 20 days, neverthless, some species flower only a few days while others may stay flowering a hundred days even more. Increase-curves of summation temperature from early spring through late-summer showed almost the same mode in both Nuttonson;s Index (Tn) and Lindsey & Newman's Index (T1). These Indices manifested the exponential curve, increasing slowly at the beginning of spring chiefly but rapidly from the middle part of April. The equation of the linear relationahip between Tn and Tl as far as in thisstudy is as follow. Tl=230Tn It appears that the distribution of summation temperature, below Tn=400°C·day, affects the first flowering, even though it could be modified somehow by the distribution of precipitation, day length and others. Nuttonson's Index (Tn.f) and Lindsey & Newman's Index (Tl.f) upon the thermal amount first flowering dates have been respectively simulated as follow. Tn.f=θa + C Tl.f=230θa + 230C where θ is air temperature 10°C, a and C are a constant.

An Anterosuperior Deltoid Splitting Approach for Plate Fixation of Proximal Humeral Fractures

  • Background: The purpose of this study was to evaluate the usefulness and safety of the anterosuperior deltoid splitting approach for fixation of displaced proximal humeral fractures by analyzing the surgical outcomes. Methods: Twenty-three patients who could be followed-up for at least 8 months after the treatment of displaced proximal humeral fractures through the anterosuperior deltoid splitting approach were enrolled. We evaluated the reduction of the fractures and surgery-related complications at the last follow-up using X-ray results and clinical outcomes comprising the University of California at Los Angeles (UCLA) scoring system and the Korean Shoulder Society (KSS) score. Results: At the last follow-up of patients treated using the anterosuperior deltoid splitting approach for internal fixation of proximal humeral fractures, we found 22 cases (95.6%) of bone union, a mean UCLA score of 28.3 (range, 15 to 34) and a mean KSS score of 82.1 (range, 67 to 95). Various surgery-related complications were noted; a case of varus malunion after fracture displacement, a case of nonunion, a case of delayed union, two cases of impingement, and a case of partial axillary nerve injury, which recovered completely through the follow-up. Conclusions: Plate fixation using the anterosuperior deltoid splitting approach could be another reliable option for treating displaced proximal humeral fractures.

The Effects of a Cognitive-Behavioral Group Therapy Program for Social Anxiety in Female Adolescents (여자 청소년의 사회불안 감소를 위한 인지행동집단치료 프로그램의 개발과 효과 - 2년 추후연구 -)

  • The aim of the present study was to develop a cognitive-behavioral group therapy program for social anxiety in Korean adolescents and to examine its efficacy through pre-, post- and 2-year follow-up tests. The program included cognitive restructuring, exposure training and social skills training. KSAS-A and SASC-R were administered to 588 1st grade middle-school female students to assess their social anxiety levels. On the basis of double criteria procedure(top 10% scores on both scales), 59 students were selected initially. Individuals currently receiving clinical treatment were excluded. Finally, 40 students were selected and randomly assigned to either a treatment group or a wait-list control group. The program consisted of 12 weekly sessions, approximately 1.5-2 hours in duration. Whereas pre- and post-tests were administered to both groups, follow-up tests were administered to the treatment group only. The collected data were statistically analyzed through independent t-test and paired t-test. The results of the study were as follows: 1) At post-test students in the treatment group showed a very significant reduction in social anxiety and fear of negative evaluation by others in comparison with those in the wait-list group. 2) At post-test students in the treatment group showed a significant reduction in negative automatic thoughts in comparison with those in the wait-list group. 3) At post-test students in the treatment group showed a significant improvement in overall social skills in comparison with those in the wait-list group. 4) At post-test students in the wait-list group showed a significant increase in both social anxiety and negative automatic thoughts in comparison with those in the treatment group. 5) At 3-month, 1-year and 2-year follow-ups, the program's effectiveness was maintained.

Safety and efficacy of fimasartan with essential hypertension patients in real world clinical practice: data from a post marketing surveillance in Korea

  • The safety and efficacy of fimasartan have been evaluated through post-marketing surveillance in real world clinical practice. The multi-center, prospective, open-label and non-interventional study. A total of 3,945 patients (3,729 patients for safety assessment and 3,473 patients for efficacy assessment) were screened in patients with essential hypertension in 89 study centers from 9 September 2010 through 8 September 2016. Among the total patients, 2,893 patients (77.6%) were administered fimasartan for 24 weeks or longer and were classified as 'patients with long-term follow-up', and the additional safety and efficacy analysis were performed. The improvement was defined as systolic blood pressure (SBP) controlled to ${\leq}140mmHg$ or decreased SBP differences ${\geq}20mmHg$ after treatment or diastolic blood pressure (DBP) controlled to ${\leq}90mmHg$ or decreased DBP differences ${\geq}10mmHg$ after treatment. Adverse drug reactions (ADRs) were reported in 3.8% patients; dizziness, and hypotension were the most frequently reported ADRs in total patients. The results of patients with long-term follow-up were comparable with total patients. The overall improvement rate in all efficacy assessment at the last visit was 87.1% (3,025/3,473 patients). The overall improvement rate of the patients with long-term follow-up was 88.9%. Fimasartan was well tolerated, with no new safety concerns identified and an effective treatment in the real world clinical practice for Korean patients with hypertension.
