Progressive Manifestations of Reversible Cerebral Vasoconstriction Syndrome Presenting with Subarachnoid Hemorrhage, Intracerebral Hemorrhage, and Cerebral Infarction

  • Reversible cerebral vasoconstriction syndrome (RCVS) is characterized by sudden-onset headache with focal neurologic deficit and prolonged but reversible multifocal narrowing of the distal cerebral arteries. Stroke, either hemorrhagic or ischemic, is a relatively frequent presentation in RCVS, but progressive manifestations of subarachnoid hemorrhage, intracerebral hemorrhage, cerebral infarction in a patient is seldom described. We report a rare case of a 56-year-old woman with reversible cerebral vasoconstriction syndrome consecutively presenting as cortical subarachnoid hemorrhage, intracerebral hemorrhage, and cerebral infarction. When she complained of severe headache with subtle cortical subarachnoid hemorrhage, her angiography was non-specific. But, computed tomographic angiography showed typical angiographic features of this syndrome after four days. Day 12, she suffered mental deterioration and hemiplegia due to contralateral intracerebral hematoma, and she was surgically treated. For recurrent attacks of headache, medical management with calcium channel blockers has been instituted. Normalized angiographic features were documented after 8 weeks. Reversible cerebral vasoconstriction syndrome should be considered as differential diagnosis of non-aneurysmal subarachnoid hemorrhage, and repeated angiography is recommended for the diagnosis of this under-recognized syndrome.

The Effects of tDCS and Montoya Stair Task on Sensorimotor Recovery and GFAP Expression in MCAo induced Stroke Rat Model

  • This study is intended to examine the tDCS and Montoya stair task(MST) on sensorimotor recovery and glial scar expression in MCAo induced stroke model of rat. To achieve this goal, this study selected 80 SD rats of 8 weeks. The experiment groups were divided them into four groups, and assigned 20 rats to each group. Group I was a experimental control group; GroupII was a tDCS application group after MCAo; Group III was a MST application group after MCAo; Group IV was a tDCS and MST application group after MCAo. In each group, neurological function test measurement, motor behavior test, montoya stair task test, immunohistochemistric finding of GFAP expression finding were analyzed. In motor behavior test, the outcome of group I was significantly difference than the other group, especially from 14days. In montoya stair task test, the outcome of group I was significantly lower than the other group especially, group II were significantly different on 14days and group IV was most significantly difference than the other group. In immunohistochemistric finding, group II, III, IV were decrease GFAP expression on depend on time stream. These results throughout the MCAo due to focal ischemic brain injury rat model four weeks tDCS and MST was applied, when the neurobehavioural, upper extremity function and ability, histopathologic data suggest that sensorimotor function recovery and a positive influence on glial scar decrease and confirmed that.

Semi-Quantitative Analyses of Hippocampal Heat Shock Protein-70 Expression Based on the Duration of Ischemia and the Volume of Cerebral Infarction in Mice

  • Objective : We investigated the expression of hippocampal heat shock protein 70 (HSP-70) infarction volume after different durations of experimental ischemic stroke in mice. Methods : Focal cerebral ischemia was induced in mice by occluding the middle cerebral artery with the modified intraluminal filament technique. Twenty-four hours after ischemia induction, both hippocampi were extracted for HSP-70 protein analyses. Slices from each hemisphere were stained with 2,3,5-triphenyltetrazolium chloride (2%), and infarction volumes were calculated. HSP-70 levels were evaluated using western blot and enzyme-linked immunosorbent assay (ELISA). HSP-70 subtype (hsp70.1, hspa1a, hspa1b) mRNA levels in the hippocampus were measured using reverse transcription-polymerase chain reaction (RT-PCR). Results : Cerebral infarctions were found ipsilateral to the occlusion in 10 mice exposed to transient ischemia (5 each in the 30-min and 60-min occlusion groups), whereas no focal infarctions were noted in any of the sham mice. The average infarct volumes of the 2 ischemic groups were $22.28{\pm}7.31mm^3$ [30-min group${\times}$standard deviation (SD)] and $38.06{\pm}9.53mm^3$ (60-min group${\times}$SD). Western blot analyses and ELISA showed that HSP-70 in hippocampal tissues increased in the infarction groups than in the sham group. However, differences in HSP-70 levels between the 2 infarction groups were statistically insignificant. Moreover, RT-PCR results demonstrated no relationship between the mRNA expression of HSP-70 subtypes and occlusion time or infarction volume. Conclusion : Our results indicated no significant difference in HSP-70 expression between the 30- and 60-min occlusion groups despite the statistical difference in infarction volumes. Furthermore, HSP-70 subtype mRNA expression was independent of both occlusion duration and cerebral infarction volume.

성심산(醒心散)의 중대뇌동맥 폐쇄로 유발된 허혈성 뇌손상 백서(白鼠)에 대한 인지 및 운동기능 회복 촉진효과 (Sungshim-san-mediated Recovery of Cognition and Motor Function in the Severe Rat Stroke, Permanent Middle Cerebral Artery Occlusion Model)

  • Objectives: The object of this study was to evaluate the cognition and motor function recovery effects of Sungshim-san (SSS), a traditional Korean cardio-protective polyherbal formula in the severe rat stroke, permanent middle cerebral artery occlusion (pMCAO) model. Methods: The experimental animals were divided into 6 groups. SSS aqueous extracts (yield=16.82%; 400, 200 and 100 mg/kg) were administered orally by using Sonde, once daily, for 28 continuous days from 24 hrs post-pMCAO. Donepezil 10 mg/kg, a representative drug for dementia, was used as a reference drug. The body weight changes, infarct/defect sizes, sensorimotor function and cognitive motor behavior were serially monitored. Limb placing and body-swing test for sensorimotor functions were conducted at 1 day before operation (base line), and 1, 3, 7, 14, 21 and 28 days post-pMCAO; and water maze test for the cognitive motor behavior was conducted at 14 and 28 days post-pMCAO, respectively. Results: Focal cerebral cortex infarct and defects due to pMCAO resulted in marked decreases of body weight, disorders of sensorimotor functions and cognitive motor behaviors. However, the pMCAO-related ischemic damages were markedly and dose-dependently inhibited by treatment with SSS 400 and 200 mg/kg, respectively. Donepezil markedly decreased the body weight and gains, as compared with pMCAO control rats; however, SSS 400 and 200 mg/kg favorably ameliorated the pMCAO-induced decreases in body weight and gains. SSS 100 mg/kg treated rats did not show any favorable effects on the pMCAO-related ischemic damages, as compared with pMCAO control rats. Conclusions: The results of the study indicated that oral administration of SSS 400 and 200 mg/kg accelerated cognition and motor function recovery in the rat pMCAO model. The treatment effect was potentially mediated by neuroprotection via the known augmentation of cerebral antioxidant defense system of SSS itself or its individual herbal components. Especially, the overall effects of SSS 200 mg/kg were similar to those of donepezil 10 mg/kg, but less toxic.

뇌경색 환자에서 초음파로 측정한 경동맥 내막-중막두께와 죽상경화반의 관련성에 대한 연구 (Study on Relationship Between Intima Medial Thickness and the Plaque in Stroke Patients on Carotid Artery Sonography)

  • 초음파에 의한 경동맥 내막-중막 두께의 측정은 동맥경화의 조기발견, 뇌혈관질환, 죽상동맥경화증의 지표로서 사용되지만, 죽상경화증이 내막에 국한되어 발생하는 현상임에도 불구하고 경동맥 내막-중막 두께의 측정은 혈관 벽의 전반적인 두께를 측정하며 일차적으로 중막의 변화를 측정한다는 문제점을 가지고 있다. 이에 반해 죽상경화반의 경우는 혈관벽을 따라 넓게 분포한 것이 아니라 일부분의 위치에 국한되어 나타나는 현상으로 죽상경화증을 더 반영한다고 볼 수 있다. 따라서 본 연구는 뇌경색환자에 대해 경동맥 초음파 검사를 실시하여 경동맥 내막-중막 두께와 죽상경화반의 관계를 파악하고 뇌경색 예측인자인 죽상경화반의 선별검사로 경동맥 초음파 검사가 유용한지 알아보고자 하였다. 2008년 1월에서 3월까지 뇌졸중 전문병원 신경과에 입원한 환자를 대상으로 B-mode 초음파를 이용한 경동맥 내막-중막 두께와 죽상경화반을 검사하였다. 뇌경색환자는 자기공명촬영을 통해 확인하고 나이, 성별과 두개강외 혈관 협착의 위험 인자들을 분석하였다. 대상자의 연령이 높은 군에서 경동맥 내막-중막 두께가 이상이 있는 사람의 분포가 높은 것으로 나타났고(p = 0.004), 경동맥 분지에서의 죽상경화반은 연령이 높은 군에서 그 분포가 높은 것으로 나타났다(p = 0.006). 경동맥 내막-중막 두께가 정상인 사람 총 131명 중 죽상경화반이 발견된 경우는 90명(69%)이었고, 경동맥 내막-중막 두께가 이상인 사람 총 91명 중 죽상경화반이 발견된 경우는 84명(92%)이어서 경동맥 내막-중막 두께가 이상적으로 두꺼운 사람에서 죽상경화반이 발견된 경우가 유의하게 높게 나타났다(p < 0.05). 따라서, 경동맥 내막-중막 두께와 죽상경화반은 밀접한 관련성이 있고, 뇌혈관 질환발생 여부 미치는 영향을 보기 위해 정상인을 대상으로 한 전향적 연구가 필요한 것으로 사료된다.

Statins Have No Role in Preventing the Progression of Aortic Valve Sclerosis

  • BACKGROUND: Statins are thought to have little effect on the progression of aortic stenosis, but the data on their role in patients with aortic valve sclerosis (AVS) are limited and inconsistent. METHODS: We retrospectively analyzed 541 consecutive patients (214 men, age: $70{\pm}11$ years) with AVS. Each patient underwent two or more electrocardiography examinations at least 6 months apart at Kangwon National University Hospital from August 2010 to August 2015. AVS is defined as irregular thickening of the leaflets, focal increases in echogenicity and minimal elevation of the peak aortic valve velocity (> 1.5 and < 2 m/s). The progression rate of AVS was expressed as the increase in peak velocity per year (m/s/yr). RESULTS: The mean follow-up duration was $24.9{\pm}13.3$ months in the statin-treated group and $24.1{\pm}12.4$ months in the non-statin-treated group (p = 0.460). There were no differences between the statin-treated and non-statin-treated groups in mean age, gender or smoking status. Relative to the non-statin-treated group, a higher number of patients in the statin-treated group had hypertension, diabetes, ischemic heart disease, and stroke. The progression rate of AVS did not differ between the statin-treated and non-statin-treated groups ($0.012{\pm}0.340m/s/yr$ vs. $0.014{\pm}0.245m/s/yr$, p = 0.956). Multivariate analysis showed initial peak aortic jet velocity was significantly associated with AVS progression (${\beta}=0.153$, p = 0.009). CONCLUSIONS: Our study demonstrated that statins had no effect on the progression of AVS. However, well-designed studies are needed to define the prognosis and management of AVS.

Cilostazol에 의한 뇌혈관내피세포의 세포이동 증진 효과연구 (Cilostazol Promotes the Migration of Brain Microvascular Endothelial Cells)

  • Cilostazol은 phosphodiesterase III의 선택적 저해제로 알려져 있으며, 뇌졸중 치료에 일반적으로 사용되고 있다. Cilostazol을 처리한 경우, 국소 뇌허혈이 발생한 후에 혈관신생을 통해서 혈관형성이 향상된다는 것을 본 연구자들이 발표하였다. 혈관신생은 조직의 허혈상태를 극복하기 위해서 혈관재생을 촉진하는 중요한 과정으로써, 혈관내피세포의 증식, 이동, 모세관구조 형성의 다단계 과정으로 구성되어 있다. 이에 본 연구에서는 인간 뇌혈관내피세포를 이용하여 cilostazol이 혈관신생의 각 단계들에 어떤 영향을 미치는지 조사하였다. Cilostazol은 농도의존적으로 뇌혈관내피세포의 이동성을 촉진하였으나, 뇌혈관내피세포의 증식과 모세관구조 형성에는 영향을 미치지 않았다. Cilostazol이 세포이동을 조절하는 기전을 분석하기 위해서 cDNA microarray를 수행하였고, 세포이동에 관련성이 있는 5종의 후보 유전자들을 선택하여 real-time PCR을 통해 해당 유전자의 발현을 검증하였다. Cilostazol에 의해서 발현양이 조절되는 유전자들로써, phosphoserine aminotransferase 1 (PSAT1)와 CCAAT/enhancer binding protein ${\beta}$ ($C/EBP{\beta}$)은 발현이 증가하였고, tissue factor pathway inhibitor 2 (TFPI2), retinoic acid receptor responder 1 (RARRES1), RARRES3는 발현이 감소하였다. 이상의 결과를 통해서 cilostazol이 혈관내피세포의 이동을 촉진하여 혈관신생을 향상시킬 수 있음을 제안할 수 있으며, 뇌혈관내피세포에 대한 cilostazol의 조절기전에 대해서 더욱 상세히 규명을 한다면 혈관형성을 통하여 허혈성 질환을 치료할 수 있는 유용한 정보가 될 것으로 기대한다.

성향정기산(星香正氣散)이 뇌허혈(腦虛血)을 유발(誘發)시킨 백서(白鼠)의 신경전달물질(神經傳達物質)에 미치는 영향(影響) (The Protective Effects of Sunghyangjeongki-San on Middle Cerebral Artery Occlusion)

  • Objectives : The aim of this study is to investigate that Sunghyangjeongki-San which has been frequently medicated in the early stage of stroke can protect against ischemic damage in rat brain Methods : Extracellular levels of amino acids(glutamate, aspartate, GABA, glycine, taurine, tyrosine, alanine), organic acids(pyruvate, lactate), and cerebral infarction volume were measured at the striatum of rats subjected to permanant focal cerebral ischemia induced by 2 hours of middle cerebral artery occiusion(MCAO). Rats were orally administered with Sunghyangjeongki-San at 30mins before MCAO and the microdialysate was collected by intracerebral microdialysis three times before MCAO and six times after MCAO at 20mins interval and analyzed by HPLC. After a microdialysis study, the brain was sliced and stained with cresyl violet buffer for the measurement of cerebral infarcted area and volume by image analyzer system Results : The concentrations of glutamate, aspartate, and tyrosine known as excitatory neurotransmitters were significantly decreased in Sunghyangjeongki-San group compared with control group, The concentrations of GABA, glycine, taurine and alanine known as inhibitory neurotransmitters were significantly increased in Sunghyangjeongki-San group compared with control group. The concentrations of pyruvate and lactate showed little significant change in Sunghyangjeongki-San group compared with control group. The measurement of cerebral infarcted area and volume by image analyzer system were significantly decreased in Sunghyangjeongki-San group compared with control group. Conclusions : Sunghyangjeongki-San can affect on protecting against cerebral ischemia by regulating extracellular levels of excitatory and inhibitory amino acid neurotransmitters and improve the conditions of the patients in the early stage of stroke.

국소뇌허혈에서 NMDA 수용체 길항제가 국소 뇌포도당 대사율에 미치는 영향 (Effect of NMDA Receptor Antagonist on Local Cerebral Glucose Metabolic Rate in Focal Cerebral Ischemia)

  • 국소뇌허혈에서 MK-801의 신경세포방어효과를 검토하기 위하여 흰쥐의 중뇌동맥을 폐색시켜 뇌허혈 및 경색을 유발하기 전과 후에 MK-801을 투여한 후 국소뇌포도당이용률의 변화를 2-deoxy-D-(14C)glu-cose 자가방사기록법을 이용하여 측정하고 이를 대조군과 비교하였다. 중대뇌동맥폐색 시술 전과 후에 평균동맥압, 직장체온, 동맥혈 PH, $pCO_2,\;pO_2$의 유의한 차이는 없었다. [$^{14}C$]DG 실험 중 대조군, MK-801 전처처군과 MK-801 후처치군 사이에서 평균동맥압, 직장 체온, 동맥혈장 당농도, 동맥혈 pH, $pCO_2,\;pO_2$의 유의한 차이는 없었다. 대조군에서 중대뇌동맥폐색 24시간 후에 폐색된 대뇌반구의 전두엽피질, 두정엽피질, 담창구, 편도, 해마 CA-2 지역, CA-3 지역, 내비회, 후두엽피질, 측두엽피질의 국소뇌포도당이용률은 반대쪽정상 대뇌반구 상동부위의 국소뇌포도당이용률보다 유의하게 낮았다. MK-801 전처치군에서 전두엽피질, 두정엽피질, 편도, 해마 CA-2 지역, 내비회, 후두엽피질, 측두엽피질의 병변/정상 국소뇌포도당이용률 비는 대조군의 그것보다 유의하게 높았다. 한편 MK-801 후처치군에서는 전두엽피질, 두정엽피질, 해마 CA-2 지역, 후두엽피질, 측두엽피질의 병변/정상 국소뇌포도 당이용률 비가 대조관의 그것보다 유의하게 높았다. MK-801 전처치군 정상 대뇌반구의 미상핵-피각, 전두엽피질, 두정엽피질, 대상회전, 담창구, 시상, 뇌량, 해마 CA-1, CA-2, CA-3 지역, 후두엽피질, 측두엽피질의 국소뇌포도당이용률은 대조군 정상 대뇌반구의 그것보다 유의하게 높았다. 반면 MK-801 후처치군 정상 대뇌반구의 국소뇌포도당이용률은 대조군 정상대뇌반구의 국소뇌포도당이용률과 유의한 차이가 없었다. 뿐만 아니라 폐색된 대뇌반구에서도 정상 대뇌반구와 매우 유사한 결과를 보였다. 즉, MK-801 전처치군에서는 미상핵-피각, 전두엽피질, 두정엽피질, 대상회전, 담창구, 시상, 뇌량, 해마 CA-1, CA-2, CA-3 지역, 내비회, 후두엽피질, 측두엽피질의 국소뇌포도당이용률이 대조군보다 유의하게 높은 반면, MK-801 후처치군에서는 대조군과 유의한 국소뇌포도당이용률의 차이를 나타내지 않았다. 이상의 결과로부터 국소뇌허혈에서 MK-801은 신경세포방어효과를 나타낼 것으로 생각된다. MK-801은 뇌포도당이용률을 국소적으로 증가시키며, 이는 NMDA 수용체 길항제 투여와 뇌기능 변화의 유관성을 시사하는 것이다.
