Characteristics and Treatment of Temporomandibular Disorder in Children and Adolescents: An Analytic Review

  • Purpose: The purpose of this study is to investigate the prevalence of temporomandibular disorders (TMDs) in children and adolescents, their characteristic contributing factors, the characteristic features of symptoms and symptoms, and the response to treatment. Methods: We studied the researches, that were the results of the searches for words such as temporomandibular disorder, TMD, children, adolescents, and juvenile through PubMed and DBpia. Results: According to a study conducted in Busan, the ratio of adolescents increased from 18.3% to 21% in 2008 compared to 2000, and the proportion of boys increased from 38.58% to 45.38%. One of the characteristic contributing factors for adolescents is the macrotrauma such as jaw trauma, vehicle accidents, sports, physical abuse, forceful intubation, and third molar extraction. The second is a microtrauma from parafunctional habit such as bruxism, clenching, hyperextension, wind instrument, and fingernail biting that can cause joint overload, cartilage breakdown, synovial fluid alterations, and other changes within the joint. The diagnosis of TMDs in juvenile adolescents is not significantly different from that of adults. Medical history, clinical examination and radiological examinations are required. Conclusions: In the temporomandibular joint history and assessment, all comprehensive dental history examination is required, including head and neck pain, mandibular dysfunction, previous orofacial trauma, history of present illness with an account of current symptoms. For the treatment and management of temporomandibular arthritis in juvenile adolescents, understanding the characteristics of TMDs in juvenile adolescents and thoroughly analyzing appropriate diagnosis and possible contributing factors through comprehensive history taking & examination, conservative treatment, including fast and active cautions education, will be essential.

  • Personal Rapid Transit (PRT) is the emerging personal transport vehicle operating on the loop automatically. The PRT system utilize the electrical power from super capacity or battery, it is important to manage the power or energy. In this regards, the management of high temperature occurred by the operation of system is significantly important to prevent from serious damage of component. In this study, we studied the adequate shape of underbody which can reduce the heat accumulation by pickup coil and condenser using natural air cooling. We suggested the additional air pathway, air inlet and flow separator to decrease the temperature of the heat source components. It was found that suggested system can decrease the temperature of PRT under body by 16% during the static mode and by 10% during the running mode at 30km/h. It is expected that the findings of this study will feed into final design of newly built Korean PRT vehicle.

  • The hemolytic uremic syndrome (HUS) is a rare disease of microangiopathic hemolytic anemia, low platelet count and renal impairment. HUS usually occurs in young children after hemorrhagic colitis by shigatoxin-producing enterohemorrhagic E. coli (D+HUS). HUS is the most common cause of acute renal failure in infants and young children, and is a substantial cause of acute mortality and morbidity; however, renal function recovers in most of them. About 10% of children with HUS do not reveal preceding diarrheal illness, and is referred to as D- HUS or atypical HUS. Atypical HUS comprises a heterogeneous group of thrombomicroangiopathy (TMA) triggered by non-enteric infection, virus, drug, malignancies, transplantation, and other underlying medical condition. Emerging data indicate dysregulation of alternative complement pathway in atypical HUS, and genetic analyses have identified mutations of several regulatory genes; i.e. the fluid phase complement regulator Factor H (CFH), the integral membrane regulator membrane cofactor protein (MCP; CD46) and the serine protease Factor I (IF). The uncontrolled activation of the complement alternative pathway results in the excessive consumption of C3. Plasma exchange or plasma infusion is recommended for treatment of, and has dropped the mortality rate. However, overall prognosis is poor, and many patients succumb to end-stage renal disease. Clinical presentations, response to plasma therapy, and outcome after renal transplantation are influenced by the genotype of the complement regulators. Thrombotic thrombocytopenic purpura (TTP), another type of TMA, occurs mainly in adults as an acquired disease accompanied by fever, neurologic deficits and renal abnormalities. However, less frequent cases of congenital or hereditary TTP associated with ADAMTS-13 (a disintegrin and metalloprotease, with thrombospondin 1-like domains 13) gene mutations have been reported, also. Recent advances in molecular genetics better allow various HUS to be distinguished on the basis of their pathogenesis. The genetic analysis of HUS is important in defining the underlying etiology, predicting the genotype-related outcome and optimizing the management of the patients.

초임계 CO2 발전용 파워 터빈의 회전체 동역학 해석 및 구동 시험 (Rotordynamic Performance Analysis and Operation Test of a Power Turbine for the Super critical CO2 Cycle Application)

  • This paper presents a rotordynamic analysis and the operation of a power turbine applied to a 250 kW super-critical $CO_2$ cycle. The power turbine consists of a turbine wheel and a shaft supported by two fluid film bearings. We use a tilting pad bearing for the power turbine owing to the high speed operation, and employ copper backing pads to improve the thermal management of the bearing. We conduct a rotordynamic analysis based on the design parameters of the power turbine. The dynamic coefficients of the tilting pad bearings were calculated based on the iso-thermal lubrication theory and turbine wheel was modeled as equivalent inertia. The predicted Cambell diagram showed that there are two critical speeds, namely the conical and bending critical speeds under the rated speed. However, the unbalance response prediction showed that vibration levels are controlled within 10 mm for all speed ranges owing to the high damping ratio of the modes. Additionally, the predicted logarithmic decrement indicates that there is no unstable mode. The power turbine uses compressed air at a temperature of $250^{\circ}C$ in its operation, and we monitor the shaft vibration and temperature of the lubricant during the test. In the steady state, we record a temperature rise of $40^{\circ}C$ between the inlet and outlet lubricant and the measured shaft vibration shows good agreement with the prediction.

Immediate Debridement and Reconstruction with a Pectoralis Major Muscle Flap for Poststernotomy Mediastinitis

  • Background : Poststernotomy mediastinitis is a rare, but life-threatening complication, thus early diagnosis and proper management is essential for poststernotomy mediastinitis. The main treatment for mediastinitis is aggressive debridement. Several options exist for reconstruction of defects after debridement. The efficacy of immediate debridement and reconstruction with a pectoralis major muscle flap designed for the defect immediately after the diagnosis of poststernotomy mediastinitis is demonstrated. Methods : Between September 2009 and June 2011, 6 patients were referred to the Department of Plastic and Reconstructive Surgery and the Department of Thoracic and Cardiovascular Surgery of Ajou University Hospital for poststernotomy mediastinitis. All of the patients underwent extensive debridement and reconstruction with pectoralis major muscle flaps, advanced based on the pedicle of the thoracoacromial artery as soon as possible following diagnosis. A retrospective review of the 6 cases was performed to evaluate infection control, postoperative morbidity, and mortality. Results : All patients had complete wound closures and reduced severity of infections based on the erythrocyte sedimentation rate and C-reactive protein levels and a reduction in poststernal fluid collection on computed tomography an average of 6 days postoperatively. A lack of growth of organisms in the wound culture was demonstrated after 3 weeks. There were no major wound morbidities, such as hematomas, but one minor complication required a skin graft caused by skin flap necrosis. No patient expired after definitive surgery. Conclusions : Immediate debridement and reconstruction using a pectoralis major muscle flap is a safe technique for managing infections associated with poststernotomy mediastinitis, and is associated with minimal morbidity and mortality.

  • Anorexia is a common symptom in the elderly patients. Causes of anorexia in the elderly are very diverse and multifactorial. Causes include physiological changes associated with aging, mental disorders such as depression, anorexia tardive, dementia, medical diseases such as cancer(lung and gastrointestinal cancer), benign gastrointestinal disorders, cardiac disorders, pulmonary disease, thyroid disorders, infection. Medications such as digoxin, theophylline have also been implicated in the problem. No cause is found in about one quarter of patients. Management is directing at treating causes and providing nutritional support. In Oriental medicine, the appetite has close relation to Biwi. The main cause of anorexia is the insufficiency of Biwi. The physiology of Biwi is that Bi sends clarity(food essence) upward and Wi sends digested food downward. Specially if the physiologic function of Wi is disordered by various factors, Wi cannot send digested food downward. As a result, the anorexia can present by the disorder of Wi function. We experienced a case of an 74 years old female patient with ill-defined severe anorexia differentiated as Wijoongheohan. The patient was managed with fluid therapy and Jeonghyangsiche-tang. The anorexia and other symptoms improved continuously during hospitalization.

히트파이프 작동유체 개발을 위한 에탄올-탄소나노튜브 나노유체의 열전도도 및 점도 특성 연구 (Study on the Thermal Conductivity and Viscosity of Ethanol with Carbon Nanotubes for the Development of a Heat Pipe Working Fluid)

  • Nanofluids using Carbon Nanotubes have a excellent thermal characteristic. In this study, for increasing the efficiency of solar collector, the thermal conductivity and viscosity of Ethanol-Oxidized Multi-walled Carbon Nanofluids were measured. Nanofluids were manufactured by ultra-sonic dispersing Oxidized Multi-walled Carbon Nanotubes(OMWCNTs) in ethanol at the rates of 0.0005 ~ 0.1 vol%. The thermal conductivity and viscosity of manufactured nanofluids were measured at the low temperature($10^{\circ}C$), the room temperature($25^{\circ}C$) and the high temperature($70^{\circ}C$). For measuring thermal conductivity and viscosity, we used transient hot-wire method and rotational digital viscometer, respectively. As a result, under given temperature conditions, thermal conductivity of the 0.1 vol% nanofluids improved 33.74% ($10^{\circ}C$), 33.14% ($25^{\circ}C$) and 32.36% ($70^{\circ}C$), and its viscosity increased by 37.93% ($10^{\circ}C$), 31.92% ($25^{\circ}C$) and 29.42% ($70^{\circ}C$) than the base fluids.

파랑-구조물-지반 상호작용에 의한 혼성제 주변 해저지반의 3차원 동적응답 특성 (3-D Dynamic Response Characteristics of Seabed around Composite Breakwater in Relation to Wave-Structure-Soil Interaction)

  • If the seabed is exposed to high waves for a long period, the pore water pressure may be excessive, making the seabed subject to liquefaction. As the water pressure change due to wave action is transmitted to the pore water pressure of the seabed, a phase difference will occur because of the fluid resistance from water permeability. Thus, the effective stress of the seabed will be decreased. If a composite breakwater or other structure with large wave reflection is installed over the seabed, a partial standing wave field is formed, and thus larger wave loading is directly transmitted to the seabed, which considerably influences its stability. To analyze the 3-D dynamic response characteristics of the seabed around a composite breakwater, this study performed a numerical simulation by applying LES-WASS-3D to directly analyze the wave-structure-soil interaction. First, the waveform around the composite breakwater and the pore water pressure in the seabed and rubble mound were compared and verified using the results of existing experiments. In addition, the characteristics of the wave field were analyzed around the composite breakwater, where there was an opening under different incident wave conditions. To analyze the effect of the changed wave field on the 3-D dynamic response of the seabed, the correlation between the wave height distribution and pore water pressure distribution of the seabed was investigated. Finally, the numerical results for the perpendicular phase difference of the pore water pressure were aggregated to understand the characteristics of the 3-D dynamic response of the seabed around the composite breakwater in relation to the water-structure-soil interaction.

건식 듀얼 클러치의 열해석 모델에 대한 연구 (A Study on Thermal Analytical Model for a Dry Dual Clutch)

  • The stability of friction characteristics and thermal management for a dry type dual clutch transmission (DCT) are inferior to those of a wet clutch. Too high temperature resulting from frequent engagement of DCT speeds up degradation or serious wear of the pressure plate or burning of the clutch disk lining. Even though it is significantly important to estimate the temperature of a dry double clutch (DDC) in real-time, few meaningful study of the thermal model of DDC has been known yet. This study presented a thermal analytical model of lumped parameters for a DDC by analyzing its each component firstly. Then a series of experimental test was carried out on the test bench with a patented temperature telemetry system to validate the proposed thermal model. The thermal model, whose optimal parameter values were found by optimization algorithm, was also simulated on the experimental test conditions. The simulation results of DDC temperature show consistency with the experiment, which validates the proposed thermal model of DDC.

구강악안면영역의 치성감염으로 인한 근막간극 감염에 대한 회귀적 연구 (THE RETROSPECTIVE STUDY OF INTRAFASCIAL INFECTION FROM ODONTOGENIC INFECTION IN ORAL AND MAXILLOFACIAL REGION)

  • Infections in oral and maxillofacial region are relativley common and self-limiting, but in some cases, infections spread to adjacent hard and soft tissue and to cause any complication, even threaten life. So we made retrospective study of patients with interfascial infection who had been hospitalized and been treated by surgical treatment in Dankook university about 10 years. We reviewed the charts of patient with interfascial infection from 1995 to 2005. The result were as follows: 1. In gender & age distribution, male(54.2%) & fouth decade were most frequently. 2. The most common cause of infection was dental caries(55.2%) and the most of involving teeth was lower posterior teeth(44.1%). 3. Submandibular space is most frequently involving space and most infection involved mainly one space. 4. The patients with systemic disease were 38.2%. Diabetic mellitus was 87.2% of systemic diease. The admission period was 19.5 days in systemic disease. 5. The most microorganism in culture was Streptococcus Viridans(36.2%) in all patient. Klebsiella Pneumoniae was found most in Diabetic Mellitus. 6. The patient were mainly treated I&D on admission day. Of them 5(1.1%) patients were received tracheostomy. 7. Serum albumin, CRP and body weight are associated with Nutritional Risk Index(NRI). High risk patient group according to NRI classification showed higher rate of complications & mortality. 8. The patients with complication were 28(6.7%) persons. 4(0.9%) patients were expired. Nutritional Risk Index was helpful to predict the prognosis. When interfascial infection starts to spread, we must pay attention to airway management. Fluid therapy with nutritional may support to healing of wound.
