Pancreatico-pleural Fistula: A Rare Cause of Hemorrhagic Pleural Effusion - A case report - (췌흉강루에 의한 혈성 흉수의 치험 - 1예 보고 -)

  • A pancreatico-pleural fistula (PPF), caused by rupture of a pancreatic pseudocyststectomy or obstruction of the pancreatic duct, is a rare condition. A 48-year-old man with chronic alcoholism was admitted with a massive pleural effusion. Pleural fluid studies revealed elevated amylase and lipase. A PPF complicated by a ruptured pancreatic pseudocyststectomy was diagnosed by computerized tomography scan. Although the symptoms improved with conservative management, (chest tube drainage, NPO, total parenteral nutrition, and a pancreatic secretion inhibitor), a distal pancreatectomy, including a pseudocystectomy and thoracotomy, were performed for an increasing size of the hemorrhagic pancreatic pseudocyststectomy and a recurrent hemorrhagic pleural effusion. There were no post-operative complications and the patient was discharged on post-operative day 27.

Application of EFDC Model to an Agricultural Reservoir for Assessing the Effect of Point Source Bypassing (농업용 저수지의 점오염원 바이패스 효과 평가를 위한 EFDC 모델의 적용)

  • Agricultural reservoirs in Korea have been recognized as an emerging resource for recreational and cultural activities for residents. However, most of the reservoirs are eutrophic and showing high level of contamination with nuisance algal bloom and offensive odor during the summer. For better management and restoration of the reservoirs' water quality, scientific modeling approaches could be used to diagnose the problems and evaluate the efficacy of alternative control measures. The objectives of this study were to validate the performance of a three-dimensional (3D) hydrodynamic and water quality model (Environmental Fluid Dynamics Code, EFDC) for a eutrophic agricultural reservoir and assess the effect of bypassing of the effluent from a wastewater treatment plant on the reservoir water quality. The 3D model successfully simulated the temporal variations of water temperature, DO, TOC, nitrogen and phosphorus species and Chl-a observed in 2014 and also captured their spatial heterogeneity in the reservoir. The simulation results indicated that the point source bypassing may reduce the T-N and T-P concentrations of the reservoir by 6.6 ~ 8.2 %, and 1.7 ~ 16.8 %, respectively. The bypassing, however, showed a marginal effect on the control of TOC due to the increased algal biomass associated with the increased water retention time after bypassing as well as the lower TOC level of the effluent compared to the ambient reservoir water.

  • The efficiency of artificial insemination (AI) for horses remains unsatisfactory. It is mainly because each process of AI causes a detrimental effect on semen quality. To sustain quality of semen properly, several factors including libido of stallions and sperm damage during sperm processing and preservation should be considered. Stallions with decent libido produce a high ratio of sperm to seminal plasma in their ejaculates, which is the ideal semen composition for maintaining sperm quality. Thus, to maximize the fertility rate upon AI, stallions should be appropriately managed to enhance their libido. Seminal plasma should have a positive effect on horse fertility in the case of natural breeding, whereas the effects of seminal plasma on both sperm viability and quality in the context of AI remain controversial. Centrifugation of semen is performed during semen processing to remove seminal plasma and to isolate fine quality sperm from semen. However, the centrifugation process can also result in sperm loss and damage. To solve this problem, several different centrifugation techniques such as Cushion Fluid along with dual and single Androcoll-E$^{TM}$ were developed to minimize loss of sperm and to damage at the bottom of the pellet. Most recently, a new technique without centrifugation was developed with the purpose of separating sperm from semen. AI techniques have been advanced to deliver sperm to optimal region of female reproductive tract at perfect timing. Recombinant equine luteinizing hormone (reLH) and low dose insemination techniques have been developed to maximize both fertility rate and the efficiency of AI. Horse breeders should consider that the entire AI procedure should be optimized for each stallion due to variation in individual horses for a uniformed AI protocol.

Wind-induced response of open type hyperbolic-parabolic membrane structures

  • In this paper, the mechanical characteristics of the open type hyperbolic-parabolic membrane structure under wind load were investigated. First, the numerical simulation of a typical plane membrane structure was performed based on the Large-Eddy Simulation method. The accuracy of the simulation method was validated by the corresponding wind tunnel test results. Then, the wind load shape coefficients of open type hyperbolic-parabolic membrane structures are obtained from the series of numerical calculations and compared with the recommended values in the "Technical Specification for Membrane Structures (CECS 158: 2015). Finally, the influences of the wind directions and wind speeds on the mean wind pressure distribution of open type hyperbolic-parabolic membrane structures were investigated. This study aims to gain a better understanding of the wind-induced response for this type of structure and be useful to engineers and researchers.

Modeling Sedimentation Process in Ipjang Reservoir using SWAT and EFDC (SWAT-EFDC 연계 모델링을 통한 입장저수지의 유사 해석)

  • Reservoir sedimentation is a major environmental issue, and various sediment load controls and plans have been proposed to secure clean and safe water resources. The objectives of this study were to estimate soil loss in the upper basins and predict sediment deposition in Ipjang reservoir using hydrologic and hydraulic model. To do so, SWAT (Soil and Water Assessment Tool) and EFDC (Environmental Fluid Dynamics Code) was used to estimate soil loss in two upper basins and to predict spatial distribution and amount of sediment deposition in the Ipjang reservoir, respectively. The hydrologic modeling results showed that annual average soil loss from the upper basins was 500 ton. The hydraulic modeling results demonstrated that sediment particles transported to the reservoir were mostly trapped in the vicinity of the reservoir inlet and then moved toward the bank over time. If long-term water quality monitoring and sediment survey are performed, this study can be used as a tool for predicting the dredging amount, dredging location and proper dredging cycle in the reservoir. The study findings are expected to be used as a basis to establish management solutions for sediment reduction.

Verification of SPACE Code with MSGTR-PAFS Accident Experiment (증기발생기 전열관 다중파단-피동보조급수냉각계통 사고 실험 기반 안전해석코드 SPACE 검증)

  • The Korean nuclear industry developed the SPACE (Safety and Performance Analysis Code for nuclear power plants) code and this code adpots two-phase flows, two-fluid, three-field models which are comprised of gas, continuous liquid and droplet fields and has a capability to simulate three-dimensional model. According to the revised law by the Nuclear Safety and Security Commission (NSSC) in Korea, the multiple failure accidents that must be considered for accident management plan of nuclear power plant was determined based on the lessons learned from the Fukushima accident. Generally, to improve the reliability of the calculation results of a safety analysis code, verification work for separate and integral effect experiments is required. In this reason, the goal of this work is to verify calculation capability of SPACE code for multiple failure accident. For this purpose, it was selected the experiment which was conducted to simulate a Multiple Steam Generator Tube Rupture(MSGTR) accident with Passive Auxiliary Feedwater System(PAFS) operation by Korea Atomic Energy Research Institute (KAERI) and focused that the comparison between the experiment results and code calculation results to verify the performance of the SPACE code. The MSGR accident has a unique feature of the penetration of the barrier between the Reactor Coolant System (RCS) and the secondary system resulting from multiple failure of steam generator U-tubes. The PAFS is one of the advanced safety features with passive cooling system to replace a conventional active auxiliary feedwater system. This system is passively capable of condensing steam generated in steam generator and feeding the condensed water to the steam generator by gravity. As the results of overall system transient response using SPACE code showed similar trends with the experimental results such as the system pressure, mass flow rate, and collapsed water level in component. In conclusion, it could be concluded that the SPACE code has sufficient capability to simulate a MSGTR accident.

The Avoidance of Microsurgical Complications in the Extradural Anterior Clinoidectomy to Paraclinoid Aneurysms

  • Objective : Paraclinoid segment internal carotid artery (ICA) aneurysms have historically been a technical challenge for neurovascular surgeons. The development of microsurgical approach, advances in surgical techniques, and endovascular procedures have improved the outcome for paraclinoid aneurysms. However, many authors have reported high complication rates from microsurgical treatments. Therefore, the present study reviews the microsurgical complications of the extradural anterior clinoidectomy for treating paraclinoid aneurysms and investigates the prevention and management of observed complications. Methods : Between January 2004 and April 2008, 22 patients with 24 paraclinoid aneurysms underwent microsurgical direct clipping by a cerebrovascular team at a regional neurosurgical center. Microsurgery was performed via an ipsilateral pterional approach with extradural anterior clinoidectomy. We retrospectively reviewed patients' medical charts, office records, radiographic studies, and operative records. Results : In our series, the clinical outcomes after an ipsilateral pterional approach with extradural anterior clinoidectomy for paraclinoid aneurysms were excellent or good (Glasgows Outcome Scale : GOS 5 or 4) in 87.5% of cases. The microsurgical complications related directly to the extradural anterior clinoidectomy included transient cranial nerve palsy (6), cerebrospinal fluid leak (1), worsened change in vision (1), unplanned ICA occlusion (1), and epidural hematoma (1). Only one of the complications resulted in permanent morbidity (4.2%), and none resulted in death. Conclusion : Although surgical complications are still reported to occur more frequently for the treatment of paraclinoid aneurysms, the permanent morbidity and mortality resulting from a extradural anterior clinoidectomy in our series were lower than previously reported. Precise anatomical knowledge combined with several microsurgical tactics can help to achieve good outcomes with minimal complications.

A Suggested Method for Predicting Permeability of Porous Sandstone Using Porosity and Drying Rate (공극률과 건조율을 이용한 다공질 사암의 투과도 추정방법 제안)

  • As the permeability is an important parameter to characterize the ease with which a porous medium transmits fluids, it is usually obtained by fluid flow experiment using core samples. In order to measure the permeability, however, an experimental apparatus is required and it might take long measurement time, especially for tight samples. In this study, the relationship between permeability and porosity as well as drying rate has been investigated to predict the permeability without a series of measuring experiments. Porosity is measured by drying monitoring method, which measures weight variation continuously while drying surface-dried saturated sample, and drying rate is obtained from weight variation ratio with respect to the water saturation. The total of 6 Berea sandstone samples, which have a permeability range of 70 to 670 mD, were used in this work, and a new and empirical equation which could predict permeability of porous sandstone by using porosity and drying rate were obtained through regression analysis.

  • Acute gastric dilatation (AGD), one of the common causes of emergency occurs in macaca monkeys that are accommodated as laboratory-housed nonhuman primates. This report introduces some cases of occurrence in raising primates. The primates revealed an acute gastric dilatation, including the histories that were trained by monkey chair, anesthetized for the study or intact case. The clinical signs were comatose condition with sever abdominal distension, dehydration, cyanosis and apnea. One case died by deterioration of systemic body condition and performed necropsy. The other cases recovered from the AGD by the emergency treatment using the gastric tube and fluid therapy. Necropsy revealed the huge stomach filled with water, gas and ingesta. This report suggests that etiologic factors of AGD may include non-specific factors like these cases, with special emphasis on the incidence and management of AGD in nonhuman primates.

City center Regeneration in Busan Metropolitan Using GIS (GIS를 활용한 부산시 도심재생에 관한 연구)

  • Donut phenomenon of city centers has resulted from the decreasing residential population and the fluid population with curved commercial function, but the analysis can bring other problems owing to the simplified method. And As the survey and the analysis the research can offer the characteristic methods for regeneration in the city of Busan. To figure out the functional regions of city center the research has analyzed 15 Dongs in city center of the three years of 1996, 2000 and 2004 to find out the donut phenomenon and the regeneration of city center. The survey has chosen 20 variation factors using factor analysis and cluster analysis. Major factors in each year are shown 3 factors, presenting 84.2%, 87.1% and 85.5% of the accumulated explanation rate. These factors explain residential centered, commercial centered, and management centered factor. The results are as following. First, it is necessary to plan the regeneration of the total city center owing to the total donut phenomenon regarding the functions. Second, the methods to regenerate city centers should be established according to the various regional characterizations.
