The complications of a tracheotomy are caused by inappropriate surgical techniques, unsuitable cannula selections, and improper wound care. Among these, the solutions to problems of surgical technique and wound care have been reported in many articles. Detailed methods for preventing complications by the cannula are rare. The authors tried to find a way of preventing complications by the cannula Materials and Methods : The authors analized complications in 70 patients who had a temporary tracheotomy and were wearing a cannula. And the complications were compared between 4 commercial cannulas used in our institute. The examination methods used were a simple neck lateral radiogram and flexible endoscopy. Results: The order of most commonly found complications were as followed; at the suprastoma, end of cannula, level of tracheotomy, and infrastoma. Among 4 cannulas, a particular product had so many complications compared to the other 3 cannulas. The most common cause of complications was unsuitable cannula. All complications were cured with no sequelae. Flexible endoscopy is far superior to radiologic exam for detecting tracheal complications. Conclusion: Flexible endoscopy through the tracheostoma is very helpful for detecting complications early and determining if a proper cannula is used, which can prevent further complications such as stenosis or innominate artery rupture. The authors, therfore, recommend using the flexible endoscopy to all patients wearing tracheotomy tubes. Some complications can simply be prevented by replacing the one to another cannula properly fit for the individual patients. Various cannulas should be prepared at the hospital because the tracheal curvature and distance of skin to trachea are individualized.
목 적 : 식도 이물의 내시경적 적출의 최근 경향과 내시경적 적출의 유용성에 대해 알아보고 소아에서 유연형 내시경을 통한 식도 이물 제거술이 정착되기를 바라는 마음으로 이 연구를 시작하였다. 방 법 : 1987년 1월부터 2001년 7월까지 14년 6개월간 경상대학교병원 소아과와 이비인후과에서 식도 이물로 진단하여 유연형 내시경과 강직형 내시경으로 치료하였던 127례를 대상으로 하였다. 유연형 내시경의 경우는 소아과에서 Olympus사의 XP-20, XQ-240과 Pentax사의 2731와 다양한 보조 기구를 사용하고 전신마취나 midazolam이나 diazepam 등의 약제는 사용하지 않았다. 결 과 : 1) 연도별 분포 : 1998년까지는 이물의 제거에 강직형 내시경이나 유연형 내시경이 비슷하게 이용되어 왔다. 1999년 이후부터 유연형 내시경에 의한 이물의 제거가 활발히 이루어졌다. 2) 연령별, 성별 분포 : 유연형 내시경을 사용한 경우는 1세 미만의 경우 4례(6%)였고, 1-5세의 경우 43례(65.1%), 5-10세의 경우 17례(25.7%), 10-15세의 경우 2례(3.2 %)로 총 66례였다. 강직형 내시경을 사용한 경우는 1세 미만의 경우 8례(13.5%), 1-5세는 42례(68.8%), 5-10세는 7례(11.5%), 10-15세는 4례(6.6%)로 총 61례였다. 두 경우 모두 1-5세가 호발 연령이었다. 남녀 비를 보면 유연형 내시경을 사용한 경우 1.64 : 1이었고 강직형 내시경을 사용한 경우에는 1.13:1로 남아에서 다소 많았다. 3) 이물의 종류, 증상, 제거까지 걸린 시간 및 합병증 : 이물의 종류로는 유연형 내시경으로 제거한 경우는 54례, 강직형 내시경으로 제거한 경우 42례로 동전이 제일 많았고 임상 증상은 유연형 내시경의 경우에는 무증상이, 강직형 내시경의 경우에는 연하 곤란이 제일 많았다. 식도 이물을 제거까지 걸린 시간은 대부분 이물을 삼킨 후 24시간 이내였다(108례, 85%). 24시간 이후에 이물을 제거 한 경우는 병원에 늦게 온 경우이며 17례(5%)였다. 이중 합병증이 발생한 경우는 11례로 미란이 7례, 궤양소견을 보인 1례, 출혈 2례, 천공 소견을 보인 1례가 있었다. 4) 동반 질환 : 동반된 질환아 있었던 8례(7.5%)였으며 식도 협착이 5례, 뇌성 마비가 3례였다. 결 론 : 본 병원에 소아 내시경실이 생긴 이후 과거 10년 동안 소아 내시경에 의한 이물의 제거가 많이 이루어졌지만 뚜렷한 이점을 발견 할 수 없었던 강직형 내시경을 이용한 이물의 제거가 최근 2-3년 전까지도 활발히 이루어져 왔다는 것을 알 수 있었다. 유연형 내시경은 전신 마취의 불편함 없이도 쉽게 할 수 있는 안전하고 효과적인 방법이며, 최근 2-3년 간 유연형 내시경에 의한 식도 이물의 제거가 빈번해졌다. 앞으로도 내시경의 발달과 그 보조 기구의 발달로 인하여 전신 마취 없이 유연형 내시경을 사용한 이물의 제거는 활발히 이루어져야 할 것이고 처음에 시도하는 치료 방법으로 선택되어져야 할 것이다.

In general, gastroesophageal reflux disease (GERD) is diagnosed clinically based on typical symptoms and/or response to proton pump inhibitor treatment. Upper gastrointestinal endoscopy is reserved for patients presenting with alarm symptoms, such as dysphagia, odynophagia, significant weight loss, gastrointestinal bleeding, or anorexia; those who meet the criteria for Barrett's esophagus screening; those who report a lack or partial response to proton pump inhibitor treatment; and those with prior endoscopic or surgical anti-reflux interventions. Newer endoscopic techniques are primarily used to increase diagnostic yield and provide an alternative to medical or surgical treatment for GERD. The available endoscopic modalities for the diagnosis of GERD include conventional endoscopy with white-light imaging, high-resolution and high-magnification endoscopy, chromoendoscopy, image-enhanced endoscopy (narrow-band imaging, I- SCAN, flexible spectral imaging color enhancement, blue laser imaging, and linked color imaging), and confocal laser endomicroscopy. Endoscopic techniques for treating GERD include esophageal radiofrequency energy delivery/Stretta procedure, transoral incisionless fundoplication, and endoscopic full-thickness plication. Other novel techniques include anti-reflux mucosectomy, peroral endoscopic cardiac constriction, endoscopic submucosal dissection, and endoscopic band ligation. Currently, many of the new endoscopic techniques are not widely available, and their use is limited to centers of excellence.
The diagnosis and treatment of early gastrointestinal cancers is the gastroenterologists' mission because of national cancer screening program in South Korea. The detection of early cancers is emphasized, because these were previously treated with surgical treatment can be currently cured with endoscopic treatment. Gastroenterologists who achieved at least on some level can make an exact diagnosis regardless of what type of endoscopy, but generally, there are some required conditions for an optimal diagnosis. First, clinically important lesions have to be detected easily. Second, the border and morphology of lesions have to be characterized easily. Third, lesions have to be diagnosed exactly. Precancers and early cancers are often subtle and can pose a challenge to gastroenterologists to visualize using standard white light endoscopy. The use of dye solutions aids the diagnosis of early gastrointestinal cancers, however, it is a quite cumbersome to use dye solutions all the time and the solution often bothers the exact observation by pooling into the depression or ulceration of the lesion. To overcome this weakness, newer endoscopes are now developed so called "image enhanced endoscopy" using optical and/or electronic methods such as narrow band imaging (NBI), autofluorescence imaging (AFI), i-scan, flexible spectral imaging color enhancement (FICE) and confocal endomicroscopy (CLE).
Although colonoscopy was originally a diagnostic imaging procedure, it has now expanded to include an increasing range of therapeutic interventions. These procedures require precise maneuvers of instruments, execution of force, efficient transmission of force from the operator to the point of application, and sufficient dexterity in the mobilization of endoscopic surgical instruments. The conventional endoscope is not designed to support technically demanding endoscopic procedures. In case of colonoscopy, the tortuous anatomy of the colon makes inserting, moving, and orientating the endoscope difficult. Exerting excessive pressure can cause looping of the endoscope, pain to the patient, and even perforation of the colon. To mitigate the technical constraints, numerous technically enhanced systems have been developed to enable better control of instruments and precise delivery of force in the execution of surgical tasks such as apposing, grasping, traction, counter-traction, and cutting of tissues. Among the recent developments are highly dexterous robotic master and slave systems, computer-assisted or robotically enhanced conventional endoscopes, and autonomously driven locomotion devices that can effortlessly traverse the colon. Developments in endoscopic instrumentations have overcome technical barriers and opened new horizons for further advancements in therapeutic interventions. This review describes examples of some of these systems in the context of their applications to advanced therapeutic colonoscopy.
In this report, we present a case in which good results were achieved by treatment using a free jejunal patch graft with virtual endoscopy (VE) assistance in a patient whose swallowing had failed to improve for 40 years after he mistakenly swallowed sulfuric acid, despite pectoralis major myocutaneous flap grafting and frequent balloon dilatation surgery. During the last 20 years, virtual computed tomography imaging has improved remarkably and continues to be used to address new challenges. For reconstructive surgeons, the greatest advantage of VE is that it is a noninvasive modality capable of visualizing areas inaccessible to a flexible endoscope. Using VE findings, we were able to visualize the 3-dimensional shape beyond the stenosis. VE can also help predict the area of the defect after contracture release.
Background/Aims: In March 2022, the Association for the Advancement of Medical Instrumentation (AAMI) released the American National Standards Institute (ANSI)/AAMI ST91:2021, their latest update on comprehensive, flexible, and semirigid endoscope reprocessing. These updated standards recommend the sterilization of high-risk endoscopes when possible and provide new recommendations for the precleaning, leak testing, manual cleaning, visual inspection, automated reprocessing, drying, storage, and transport of endoscopes. Methods: ANSI/AAMI ST91:2021 was compared with ANSI/AAMI ST91:2015 for major reprocessing differences that result in either time and/or cost increases. Time estimates were captured by explicit recommendation inclusion or taken from the literature. All the costs were estimated using publicly available resources. Results: The updated standards represent a potential 24.3-minute and 52.35 to 67.57 United States dollars increase per procedure in terms of reprocessing time and spending, respectively, not including capital investments. Capital costs per procedure were highly dependent on the procedure volume of the facility. Conclusions: The new AAMI standards recommend several major changes, such as sterilization, for facilities to reprocess and manage endoscopes between uses. As more facilities increase their reprocessing methods to reflect the updated standards, they do so at a cost and introduce several delays. As the reprocessing landscape evolves, facilities should consider their true costs and alternative solutions, such as single-use endoscopes.
Osteoma is one of the most common tumors of the cranial vault and the facial skeleton. For osteoma in the facial region, endoscopic resection is widely used to prevent surgical scarring. Tumors in a total of 14 patients were resected using an endoscopic holmium-doped yttrium aluminium garnet (Ho:YAG) laser with a long flexible fiber. Aside from having the advantage of not leaving a scar due to the use of endoscopy, this procedure allowed resection at any position, was minimally invasive, and caused less postoperative pain. This method yielded excellent cosmetic results, so the endoscopic Ho:YAG laser is expected to emerge as a good treatment option for osteoma.
목적: 소아의 위장관 이물은 일상 생활에서 흔히 경험하는 질환으로 대개의 경우는 합병증 없이 자연 배출된다. 저자들은 내시경과 Foley 도관을 이용한 위장관 이물 적출술에 관한 최근 경향을 알아보고자 한다. 방법: 1996년 1월부터 1999년 12월까지 4년간 원주기독병원 소아과에 위장관 이물로 내원한 60례를 대상으로 하였으며 성별, 연령, 임상 증상, 이물의 종류와 위치, 이물의 치료 및 제거 방법, 합병증 등에 대하여 후향적으로 조사하였다. 결과: 위장관 이물 환자의 연령은 7개월에서 13세까지였으며, 5세 이하가 57례(95.0%)를 차지하였으며, 남녀비는 1.07:1이었다. 임상 증상은 무증상이 45례(75.0%)로 가장 많았고 연하곤란 8례(13.3%), 인후통 5례(8.3%), 구토, 복부 불쾌감이 각각 1례(1.7%)였다. 이물의 종류는 동전이 43례(71.7%)로 가장 많았고 구슬, 바둑알, 반지 등이 각각 3례(5.0%), 수은 건전지가 2례(3.3%)였으며 그 외 금속 clip, 나사못, 커튼핀, 머리핀, 오디오 열쇠, 스티커 등이 각각 1례(1.7%)씩 있었다. 이물의 위치는 식도 31례(51.6%), 위 25례(41.7%), 소장 3례(5.0%), 인후 1례(1.7%)였고, 이 중 식도 이물은 경부식도 24례(40.0%), 원위식도 5례(8.3%), 흉부식도 2례(3.3%)에서 관찰되었다. 유연성 내시경적 이물 제거를 시행한 경우가 22례(36.7%)로 가장 많았고, Foley 도관을 이용한 경우가 18례(30.0%), Forcep를 이용한 경우가 1례(1.7%)였다. 18례(30.0%)에서는 자연 배출되었고, 1례(1.7%)에서는 수술을 시행하였다. 결론: 소아에서 상부 위장관 이물은 조기에 제거하여 이환율과 합병증을 감소시켜야 하며, 식도이물의 제거시 X-선 투시하의 Foley 도관과 내시경은 소아에서 안전하고, 효과적인 이물적출방법이다. 얇고, 날카로운 이물은 주위 깊은 관찰을 요하며, 하부 위장관에 위치시 장천공에 대한 주위 깊은 관찰을 요한다.
FHD급 고해상도 연성 내시경을 설계하기 위한 wedge prism 적용 방법을 연구하였다. 기존의 연성 내시경 광학계의 경우, 넓은 피사계 심도 범위에서 동일한 결상 성능을 얻기 위해 F 넘버를 크게 가져가거나 액체 렌즈를 적용하였다. 하지만 이는 추가적인 light guide와 기구물을 필요로 하여 광학계의 직경이 커진다는 문제점이 있다. 이를 해결하기 위해, 연성 내시경 광학계에 2매의 wedge prism을 적용하여 각 물체거리별로 상거리를 조절하였다. 먼저 설계한 내시경 광학계에 2매의 wedge prism을 대칭으로 배치하고, 각 물체거리별로 목표 결상 성능을 만족하는 상거리 값을 도출하였다. 그 다음, 상거리를 조절하기 위한 wedge prism 디센터 값을 도출하였다. 이 두 가지 데이터를 조합하여, 각 물체거리에서 목표 결상 성능을 만족하는 wedge prism 디센터 값을 다중구성으로 적용하였다. Wedge prism을 적용한 최적 설계 결과, 100 mm-7 mm의 전체 피사계 심도 범위에서, 분해능 178 cycles/mm에 대한 변조전달함수 20% 이상의 목표 결상 성능을 만족하였다.
