A Report on the Effect of Jowisengchung-tang in 2 Cases of Precocious Puberty (조위승청탕(調胃升淸湯)을 투여한 성조숙 증상을 주소로 한 여아의 한방치료 2례)

  • Objectives: These cases are that of three girls who are diagnosed as precocious puberty. It seems that the incidence of precocious puberty is rapidly increasing these days. In addition to the psychosocial disturbances associated with precocious puberty, the premature pubertal growth spurt and the accelerated bone maturation result in reduced adult height. The aim of this study was to report the effect of Jowisengchung-tang in 3 cases of precocious puberty. Methods: Retrospective and comparative analysis of 3 children who had been diagnosed with preco cious puberty. Anthropometric measurements including height, weight, body fat, body fat percent, AHP and body mass index were measured. Endocrine investigations including estradiol, luteinizing hormone(LH), FSH were conducted. Pubertal stages were determined with a questionnaire using Tanner stages. Jowisengchung-tang was given to 3 precocious puberty girls. Results: After treatment, the hormone(estradiol index) and accompanying symptoms (breast bud with elevation of breast and papilla; enlargement of areola) were reduced compared with first visit day. Conclusions: We may conclude that Korean traditional treatment of Jowisengchung-tang is effective in patients with precocious puberty and menopausal symptoms.

The Effect of 8 Constitution Acupuncture on degenerative arthritis of knee joint (퇴행성(退行性) 슬관절염(膝關節炎)에 체질침요법(體質鍼療法)이 미치는 영향)

  • Objective: This study was designed to compared the effect of general acupuncture on degenerative arthritis of knee joint with 8 constitution acupuncture by Visual Analogue Scale(VAS). Methods: VAS was used for this research, with the pain indicator of 10 and 0 which represents a pain of first visit and a status of no pain respectively. Two groups, consisting of 40 patients, was randomly allocated for this research. SPSS(Statistic Program for Social Science) 10.0 for window was used for a statistical analysis. Independent T-test and Mann Whitney U test was performed to evaluate the improvement of each group by the treatment number and to compare that of two groups. Results: As a result of the comparison analysis between two groups after 20th treatment, the improvement of subjective symptoms in 8 constitution acupuncture group was shown more effective than that in general acupuncture group, while, After 10th treatment, there was no significant difference between two groups in VAS. Conclusion: It is suggested that the effect of 8 constitution acupuncture should be significantly different from that of general acupuncture on degenerative arthritis of knee joint. Further study is needed to confirm the effectiveness of 8 constitution acupuncture.

A Study for Satisfaction and Expectation of Effect on the Growth of Children Treated with Herbal Medicine (소아 성장을 위한 한약투여에 대한 만족도와 기대치 조사)

  • Objectives The purpose of this study is to evaluate the effect of herbal medicine in children's growth and to report the children's expectation. Methods 171 children visited the department of Pediatrics, Kyung-Hee Oriental Medical Hospital from June 2009 to December 2009 were studied. They were treated more than 6 months and they visited our clinic more than 3 times. Every time when patients visited our hospital, their height, body weight and obesity degree was examined using DS-102(dszenix, Seoul). Also, height percentile of first and the last visit was measured, and those were compared by the Growth Statistics Curve made by the Korean Association of Pediatrics, 2007. Results and Conclusions The mean growth of total children increased by 4.42 percent; the increased percent for boys was 4.17 and that of girls was 5.00. Herbal medical treatment helped growth in children who took medicine for growth retardation problem. Plus, majority of parents who visited the department of Pediatrics, Oriental Medical Hospital satisfied with the service and their children's height. Although thinking genetic factor is important for their height, but they think that herbal medical treatment help the growth with changing eating habit.

The Effect of Continuous Epidural Block for Herpes Zoster Opthalmicus (안 대상포진 환자에서 지속적 경부 경막외차단의 효과 -증례보고-)

  • A 34 year old male patient visited to our neuro-pain clinic with symtoms of a left frontal headache, eyeball throbbing and occipital pain. Two days after the first visit to our clinic. pain was aggrevated and the skin eruption appeared on the left forehead. He was diagnosed as raving Herpes Zoster Opthalmicus(HZO). We performed stellate ganglion block(SGB), but pain did not subsid. So a continuous cervical epidural block was perfomed(CCEB) and it could relieve the pain promptly. In this case, VAS(visual analogue scale) was diminished from 10 to 3 and the skin eruption was healed 24 days after the treatment with CCEB and SGB. We experienced that CCEB is more effective rather than intermittent SGB in intractable HZO. CCEB should be considered to the treatment of choice in patients with HZO.

A Clinical Study of Acupuncture Effect on Children's Myopia (소아근시의 침치료 효과에 대한 임상적 연구)

  • Objectives This study was performed to evaluate the clinical effect of acupuncture on children's myopia. Methods Among the outpatients with myopia who visited to Department of Acupuncture & Moxibustion, National Medical Center from June 2000 to May 2001, we selected 50 cases that did not involve ,any other pathological condition of eyes and patient age was below 17, whose eyes including with glasses or naked examined by monoocular and biocular visual acuity test per each visit and followed up for 6 weeks. Results We investigated 29 female and 21 male patients. The most distribution of age was 12.26. First mean biocular visual acutity was $0.2{\pm}0.074$. Last mean biocular visual acuity after 6 weeks was $0.8{\pm}0.046(p<0.001). Improved cases on biocular test were 38 cases($76.00\%$). Conclusions These results suggest that the acupuncture therapy is effective treatment modality on children's myopia.

  • Objectives: This case is that of a woman in her late forties who was diagnosed as conversion disorder. She suffered the climacteric while being treated with conversion disorder and the respective symptoms got worse. We investigated whether Korean traditional treatment is effective in patient with conversion disorder and menopausal symptoms. Methods: The prescription of Banhabaekchulchunma-tang, Gamisoyo-san, Bunsimgieum, acupuncture therapy and moxibustion therapy were used for the treatment. Results: After treatment, the chief complaint and accompanying symptoms were reduced to 40% compared with first visit day. Conclusions: We may conclude that Korean traditional treatment is effective in patients with conversion disorder and menopausal symptoms.

Full mouth rehabilitation of an asthma patient with severe tooth wear and occlusion disharmony (심한 마모 및 교합 부조화를 가진 천식 환자의 전악 수복 증례)

  • Objectives: Patients with excessive tooth wear should first be diagnosed for the etiology of the tooth wear. Causes of tooth wear include bruxism, clenching, and taking medications for systemic diseases. After identifying the cause of tooth attrition, the final prosthesis should be restored with an appropriate vertical dimension. Methods: A 79-year-old man with worn out teeth desired a whole dental treatment. He was on medications for high blood pressure and asthma. The treatment proceeded with a consultation with a medical doctor. The medications for asthma evoked multiple teeth wear and a loss of the vertical dimension. After recovery of 3 mm of vertical dimension, 2 months of evaluation was followed by an interim prosthesis. Results: The increased vertical dimension caused no problem in function and esthetics, and the final restoration was performed with a full monolithic zirconia crown. Group function, adequate anterior guidance, and the occlusal plane were determined. Conclusions: After the final restoration, the patient was both esthetically and functionally satisfied, and a night guard splint was delivered to prevent prosthesis fracture. The patient was informed about the potential tooth wear associated with asthma drugs and educated to visit the clinic regularly.

A Case Report on the Immediate Effects of Cytoskeletal Manual Therapy on Pain, Muscle Thickness, and Pressure Pain Threshold in a Patient with Scoliosis

  • Objective: Patients with scoliosis complain of various symptoms such as muscle imbalance, dysfunction, back pain, abnormal posture and gait abnormality. The most basic treatment for scoliosis is to observe the progress based on conservative treatment. Therefore, in this case report, the effect of cytoskeletal manual therapy (CMT), a soft tissue mobilization technique, on pain intensity, muscle thickness, and pressure pain threshold (PPT) in a patient with scoliosis was investigated. Design: A case report Methods: A 25-year-old male diagnosed with scoliosis visited the Neuromusculoskeletal Science Laboratory with chronic back pain. In the laboratory, scoliosis was confirmed through the X-ray image used for his diagnosis, and it was confirmed again through Adam's forward bending test. Pain, pressure pain threshold and muscle thickness were measured to compare the immediate effects of CMT applied in the laboratory for 40 minutes. Treatments were visited two weeks after the first visit and outcome measures were assessed after a total of two visits. Results: After receiving CMT up to the second session, the pain intensity decreased by 4 points and the screening angle decreased by 15 degrees. Muscle thickness decreased in all but 10 mm on the dominant side of the thoracic spine. All of the PPTs increased, and the greatest increase was 3.1 lb on the dominant side of the thoracic spine. Conclusions: CMT showed positive improvement in pain during trunk flexion, spinal curvature, muscle imbalance, and pressure pain, which is considered as an ancillary treatment option for scoliosis management.

Lupus anticoagulant hypoprothrombinemia syndrome associated with a hemorrhagic ovarian cyst in a girl with systemic lupus erythematosus: a case report

  • Lupus anticoagulant hypoprothrombinemia syndrome (LAHPS) is a rare entity characterized by the presence of lupus anticoagulant (LA) and prothrombin (factor II) deficiency. It may cause severe bleeding contrary to classical antiphospholipid syndrome. Here, we report a case of LAHPS presenting with a hemorrhagic ovarian cyst in a 17-year-old girl with systemic lupus erythematosus (SLE) nephritis. She had been followed up for 8 years. Her first manifestation of SLE was prolonged gingival bleeding after tooth extraction at 9 years of age. During the follow-up period, she had neither severe bleeding nor thrombotic complications despite a positive LA and a prolonged activated partial thromboplastin time (aPTT). At this visit, the patient presented with colicky abdominal pain, a hemorrhagic ovarian cyst, a prolonged prothrombin time, a prolonged aPTT, a low factor II level, and a positive LA, leading to the diagnosis of LAHPS. While a hemorrhagic ovarian cyst resolved completely in 3 months, she received oral pill, transfusions of red blood cells and plasma, and intravenous cyclophosphamide pulse therapy in combination with glucocorticoids due to persistent menorrhagia, anemia, prolonged aPTT, and lupus flaring. Thus, LAHPS needs to be considered in SLE patients with positive LA and prolonged aPTT.

Variables Affecting Long-Term Compliance of Oral Appliance for Snoring (코골이 치료용 구강장치의 지속적 사용에 영향을 주는 요인의 분석)

  • The mandibular advancement device(MAD) has been used to help manage snoring and obstructive sleep apnea. The aims of this study were to specify the demographic and clinical characteristics of the patients receiving long-term treatment with MAD and to quantify the compliance with and side effects of the use of the device. Of 103 patients who were treated with MAD for at least one full year after delivery date, 49 were able to be contacted with telephone and complete follow-up questionnaires were obtainable. They were telephoned to determine whether they were still using the device. If not, they were asked when and why they stopped using it. Patients were also asked how much effectiveness of the MAD in decreasing snoring and how much they and their bed-partners were satisfied with the MAD therapy. The initial respiratory disturbance indices and pre-treatment snoring frequency and intensity were obtained from the medical records of initial visit. All the data were compared between users and nonusers. The results were as follows: 1. Of 49 patients 25 are still using the device, but 24 stopped using it. Among nonusers nobody stopped wearing the device within first 1 month, but 37.5% of nonusers stopped wearing it in the following 6 months, and another 4.2% before the end of the first year. 2. The one-year compliance of the MAD therapy was 79.59%. 3. There were no significant differences in mean age, mean body mass index, and gender distribution between users group and nonusers group. 4. There was no significant difference in mean respiratory disturbance index at initial visit between users group and nonusers group. 5. There was no significant difference in pre-treatment snoring frequency and intensity between users group and nonusers group. 6. The degree of decrease in snoring with use of MAD was significantly higher in the users when compared to nonusers. 7. Patient's overall satisfaction with treatment outcome was significantly higher in the users when compared to nonusers. 8. Bed partner's satisfaction with treatment outcome tended to be higher in the users when compared to nonusers. 9. The most frequent reasons why patients discontinued wearing the MAD were: jaw pain(25%), dental pain(20.83%), broken appliance(20.83%), hassle using(16.67%), lost weight(8.3%), dental work(8.3%), no or little effect(4.17%), sleep disturbance(4.27).
