  • A chimeric gene comprising murine whey acidic protein(mWAP) and human growth hormone(hGH) was used to produce transgenic rats express hGH and secrete it into the blood. Two lines of transgenic rats carrying the mWAP/hGH construct were established; High line was characterized by relatively high levels of serum hGH, and low line had relatively low levels. The secretory profiles of rat GH(rGH) as well as hGH, the transgene product, were obtained in transgenic males and females of low line; both hGH and rGH serum levels were flattened with no episodic fluctuations, and the overall mean concentration of rGH was significantly lower than in normal littermates. Although the animals of High line showed an acceles, as assessed by vaginal opening and occurrence of first ovulation, advanced by 7∼8 days in both lines of animals. Accordingly, the body weight at puberty of low line transgenic females was much lower than that of normal littermates, indicating that continuous hGH expression could induce precocious puberty without enhancing the growth rate.

  • Progressive hemifacial atrophy, also known as Parry-Romberg syndrome, is an uncommon degenerative condition which is poorly defined. It is characterized by a slow and progressive atrophy affecting one side of the face. The onset usually occurs during the first two decades of life. Characteristically, the atrophy progresses slowly for several years, and then it becomes stable. Ophthalmic involvement is common, with progressive enophthalmos which is a frequent finding. Cutaneous pigmentation is common in such conditions, however its extension to the conjunctiva is rarely reported. We report a case of Parry Romberg syndrome with characteristic clinical and radiographic presentation accompanied with rare ocular findings. The clinical features, radiological findings, and differential diagnoses to be considered, and the available treatment options are discussed in this report.

The Retrospective Review of 410 Shoulder Pain Inpatients in Korean Medicine Hospital (어깨 통증으로 한방병원에 입원한 환자 410명을 대상으로 한 후향적 연구)

  • Objectives The purpose of this study is to investigate the characteristics of inpatients who were admitted to Korean medicine hospital due to shoulder pain. Methods Characteristics of four hundred ten shoulder pain inpatients of single institution from 2011 to 2013 were retrospectively reviewed through medical charts. Results Patients showed different characteristics in sex, age, motives, interval between onset and admission day, first occurrence or not, related medical history before admission, usage of west medication, admission days, treatment results, radiological examinations by shoulder pain disease groups. Accordingly, Korean medical treatment such as acupuncture, moxibustion, external treatment, cupping therapy, physiotherapy, herb-medication varied with the characteristics of shoulder pain disease groups. Conclusions Through retrospective medical charts review, characteristics of 410 single institution shoulder pain inpatients showed distinctive features and versatile Korean medicine treatments by disease groups. It is expected that this study would accelerate multi-institutional and large scale characteristic review of shoulder pain patients, which would raise reconsideration and expand boundary of Korean medicine.

Acoustic Characteristics of Korean Stops in Korean Child-directed Speech (한국어 아동 지향어에 나타난 폐쇄음의 음향 음성학적 특성)

  • A variety of cross-linguistic studies has documented that the acoustic properties of speech addressed to young children include exaggeration of pitch contours and acoustically salient features of phonetic units. It has been suggested that phonetic modifications of child-directed speech facilitate young children's learning of speech sounds by providing detailed phonetic information about the target word. While there are several studies reporting vowel modifications in speech to infants (i.e., hyper-articulated vowels), there has been little research about consonant modifications in speech to young children (except for VOT). The present study examines acoustic properties of Korean stops in Korean mothers' speech to their children (seven children aged 27 to 38 months). Korean tense, lax, and aspirated stops are all voiceless in word-initial position, and are perceptually differentiated by several acoustic parameters including VOT, $f_0$ of the following vowel, and the amplitude difference of the first and second harmonics at the voice onset of the following vowel. This study compares values of these parameters in Korean child-directed speech to those in adult-directed speech from same speakers. Conclusions focus on the acoustic properties of Korean stops in child-directed speech and how they are modified to help Korean young children learn the three-way phonetic contrast.

Verification of the Calendar Days of the Joseon Dynasty

  • Astronomical data such as calendar day and time of rising/setting of the sun and onset of twilight are essential in our daily lives. Knowing the calendar day of the past is particularly crucial for studying the history of a clan or a nation. To verify previous studies on the calendar day of the Joseon dynasty (1392 - 1910), we also investigated the sexagenary cycle of the new moon day (i.e., the first day in a lunar month) using different sources: results of the calculation by the Datong calendar (a Chinese Calendar of the Ming Dynasty) and data of Baekjungryeok (a Perpetual Calendar - literally, a one hundred-year almanac). Compared with the study of Ahn et al., we have found that as many as 17 sexagenary cycles show discrepancies. In the case of nine discrepancies, we found that the sexagenary cycles of this study are identical to those of the almanacs at that time. In addition, we study six sexagenary cycles by using the historical accounts of Joseon Wangjo Sillok (Annals of the Joseon Dynasty), Seungjeongwon Ilgi (Daily Records of Royal Secretariat), Chungung Ilgi (Logs of Crown Prince), and so forth. We present historical materials supporting the results of this study for the remainder. In conclusion, we think that this study will greatly contribute to the comparison between luni-solar calendar days during the Joseon dynasty and those in the modern (i.e., Gregorian) calendar.

  • A low delay code excited linear predictive speech coder(LD-CELP) at bit rates under 8kbps is considered. We try to improve the perfomance of speech coder with frame type dependent modification of synthesis filter. We first classify frames into 3 groups: voiced, unvoiced and onset. For voicedand unvoiced frame, the spectral envelope of the synthesis filter is adapted to the phonetic characteristics. For transition frame from unvoiced to voiced, the synthesis filter which has been interpolated with the bias filter is used. The proposed vocoder produced more clear sound with similar delay level than other pre-existing LD-CELP vocoders.

  • Hallervorden-Spatz disease (HSD) is a rare autosomal recessive disorder associated with excessive iron deposition in the basal ganglia. In general, HSD is characterized by onset in first two decade of life and by the presence of extra-pyramidal dysfunction including dystonia, rigidity, choreoathetosis. Other associated features include gait and posture disturbance, intellectual decline, seizure, tremor, dysarthria. These signs and symptoms are progressive. MRI is often demonstrated hypodensity in the basal ganglia which is probably suggestive of accumulation of iron. There is no specific treatment for HSD and 45% of patients die before reaching the age of 20 years. The managements directed at specific symptoms are often helpful. Especially, some surgical procedures like pallidotomy and gastrostomy are performed under general anesthesia. There is special need for careful management because of numerous anesthetic challenges like difficulty in cooperation, life-threatening airway obstruction and possibility of aspiration. We report a successful anesthetic management in a patient with HSD for dental procedures.

Painful legs and moving toes syndrome in a 16-year-old girl

  • Painful legs and moving toes (PLMT) syndrome is characterized by spontaneous movements of the digits and pain in one or both lower extremities. Of the reported cases, a majority of the patients was female, and the mean age of onset was 58 years. Only one pediatric case has been reported so far. Herein, we report the first adolescent case of PLMT in Korea. A 16-year-old girl complained of tingling pain in the left leg and involuntary movement of the ipsilateral great toe one month after a second untethering surgery. Three years ago, she had undergone untethering surgery to correct lipomeningomyelocele at the S2 level of the conus medullaris. At that time, she was diagnosed with polyradiculopathy at the left L5 level with axonal involvement. We diagnosed her with PLMT syndrome and prescribed gabapentin. Her symptoms diminished within a day. Complete relief from involuntary movement of the toe was achieved within four months. PLMT is a rare syndrome but it should be considered in the differential diagnosis of children and adolescents with limb pain and spontaneous movement in their toes.

  • Objective : Lately variable oriental-western medicine treatment have been used for Bell's palsy. The purpose of this study is comparison of clinical treatment outcome between oriental medicine group and oriental-western medicine group. Materials and Methods : From 08-01-2000 to 07-30-2001, 30 Bell's palsy out-patients who visited within 5days after onset and treated more than 3weeks at the Department of Acupuncture & Moxibustion, Bundang CHA Oriental Medicine Hospital were selected for two groups. One group(A group) was treated by oriental medicine therapy(Acupunture, Herb, Physical therapy), the other group(B group) was treated by oriental-western medicine therapy(Acupunture, Herb, Physical therapy, Administration of prednison). Two group was composed of 15 patients respectively and measured by HBGS(House-Brackmann Grading System) and DEFS(Detailed Evaluation of Facial Symmetry of Pillsbury and Fisch) at first visit and 3weeks after. Result and Conclusion : A group was marked more higher than B group in treatment outcome. But we discovered that it is not significant differences between two groups.

  • Multiple vertebral compression fractures are a rare condition and are a serious consequence of postpartum osteoporosis. This report describes the case of a 35-year-old woman who had given birth to her first child 2 months before the onset of pain. Magnetic resonance imaging showed compression fractures of 7 vertebrae. The patient was treated with both conventional, and Korean medicine methods including acupuncture and herbal prescriptions. The patient's progress was assessed using self-reported symptoms, scale scores and laboratory test results. Her pain was gradually alleviated and biochemical inflammation marker levels improved, but her functional status remained severely impaired. Clinical practitioners who treat women in the postpartum period must be aware of osteoporosis and potential vertebral fractures and need to consider Korean medicine as an alternative therapy to help such patients.
