응급구조과 학생의 임상실습 중 경험한 죽음의 의미에 대한 질적 연구 (A qualitative study on the meaning of death experienced by paramedic students during clinical practice)

  • Purpose: This study explored the values and attitudes toward the death of paramedic students. It comprehensively investigated paramedic students' first encounters with death during their clinical practice. Methods: Between December 2022 and January 2023, in-depth interviews were conducted with seven paramedic students who underwent clinical practice. Their experiences with death and grievances were explored by applying Colaizzi's (1978) phenomenological research method. Results: After exploring and analyzing the collected data, the context and meaning of the phenomenon were integrated. Resultingly, five major categories and 16 subcategories were derived. Conclusion: Universities should introduce a practical curriculum on experiences with death to reduce the negative emotions associated with death that paramedic students experience during their clinical practice and convert them into opportunities for reflection and growth.

죽음준비교육 프로그램이 간호학생의 생의 의미, 죽음 불안 및 임종간호 태도에 미치는 영향 (Influence of the Death Education Program on Meaning in Life, Death Anxiety and Attitude Toward Nursing Care of the Dying Patients among Nursing Students)

  • Objectives : This study aims to examine the effects of death education program on meaning in life, death anxiety and attitude toward nursing care of the dying patients among nursing students. Methods : Subjects were 155 nursing students of the K college. Before and after the intervention, students responded a questionnaire developed to measure meaning in life, death anxiety and attitude toward nursing care of the dying patients. To analyse the data, Cronbach's ${\alpha}$, t-test, and paired t-test were used with an SPSS 12.0 program. Results : 1. Meaning in life and attitude toward nursing care of the dying patients levels significantly increased, death anxiety levels significantly decreased. 2. Meaning in life levels increased significantly in the 20-24 age group, female, first grade, no religion, no death experience of relatives. 3. Death anxiety levels decreased significantly in the 20-24 age group, female, no religion, no death experience of relatives, but increased significantly in a buddhist group. 4. Attitude toward nursing care of the dying patients levels increased significantly in the 20-24 age group, female, first grade, won buddhist, no death experience of relatives. Conclusions : This study, through the above result, shows that the death education program can be an effective nursing education to improve meaning in life and attitude toward nursing care of the dying patients and to decrease death anxiety. These results suggest that the death education program will be helpful for recognizing the values of themselves and their current lives and improving their nursing intervention care of the dying patients.

  • Until 1970's, mankind have not had definite perspectives on what life after the death was like, which is one of the most important problems for them. Their concepts of the 'life and death problem' has been distorted by the dogmas of the established religions such as Buddhism or Christianity on heaven and hell. But the mankind came to have wholly different views on the life after death thanks to the studies by Raymond Moody Jr. or Elizabeth Quebler-Ross in the mid-1970's. This is the studies on the so-called 'near-death experience(NDE)' which made humankind be able to have scientific approach to the life after death for the first time in their history. What attracts our attention at this point is, however, that the arguements of the NDErs on humman destiny accurately coincide with those of the established religions. In the NDE, most of the experiencers have an encounter with the personal being, symbolized by the Light, through whom they learnt that the devotion(or love) to the neighborhood and the gain of the wisdom are the sole meaning of life. With this result, we can recognize why essential virtues maintained by the established religions until now such as ultimate compassion, unconditional love, forgiveness, or insightful learning are so significant, and that our studies of the NDE are very important in this respect.

Nursing Students' First Clinical Experiences of Death

  • 목적: 본 연구는 "병원 실습 중 간호대학생의 환자의 죽음에 대한 첫 경험은 어떠한가?"라는 질문을 통해 실습 중 간호대학생이 직면하는 환자의 죽음에 대한 경험을 포괄적이고 심층적으로 파악하고자 실시되었다. 방법: 본 연구는 2012년 1월 27일로부터 시작하여 2012년 3월 6일까지 양산시 부산대학교 간호학과 4학년에 재학 중인 여학생으로, 병원실습 중 환자의 죽음을 처음 경험한 실습생들 8명에게 실시되었다. 연구방법은 Colaizzi의 현상학적 연구 방법이 이용되었다. 결과: 17개의 주제, 15개의 주제묶음과 8개의 범주가 도출되었다. 8개의 범주는 '피하고 싶은 죽음현실', '무력감', '건강회복의 기대감에서 공포로', '죽음의 다양한 해석', '간호학의 한계', '간호사 부족에 대한 원망', '갈증의 증폭', '성장의 계기'이다. 결론: 간호대학생들은 환자의 죽음에 대한 첫 경험 과정에서 다양한 감정을 경험하고, 임종간호를 감당하고 있는 간호사의 모습을 미래의 간호사라는 자신의 입장에서 해석해가고 있었다. 이와 동시에 그들은 죽음을 간호의 영역으로 받아들이고, 공부하고 익혀나가는 성장의 노력을 하고 있었다. 따라서, 본 연구의 결과를 통해 죽음관련 내용이 간호대학 교육과정에 꼭 포함되어야 함을 알 수 있었고, 이는 신규간호사 시기에 큰 부담이 되고 있는 임종간호의 스트레스를 줄여서 업무 적응을 높이고, 이직 의도를 낮추는데 긍정적 영향을 미칠 것이다.

중년여성의 호스피스 자원봉사 경험자와 비경험자의 죽음의식과 자아존중감 비교 (A Comparative Study on Death Orientation and Self-Esteem between Hospice Volunteer Experience and Hospice Volunteer Non-experience of Middle-Aged Women)

  • This study was carried out to identify how a middle-aged woman's consciousness of the death and self-esteem was affected by whether the hospice volunteer's experience exist or not. As a form of descriptive research, this study, which is based on a structured questionnaire intended for the 191 middle aged women doing volunteer work at the non-profit women's organization, had been conducted twice from May 10, 2010, to May 21, 2010. The results of my research work is followed ; First, a middle-aged woman's consciousness of the death and self-esteem showed significant differences depending on whether the hospice volunteer's experience exist or not. Experienced hospice volunteer group showed a lower consciousness of the death and a higher self-esteem compared to non-experienced hospice volunteer group's case. Second, in case that the results show a high self-esteem, the consciousness of death reach at the low stage. Especially, the experienced hospice volunteer group showed an opposite correlation compared to non-experienced group. Third, Anyone's consciousness of death appeared at a low level when she had a religion and higher education background. Anyone's self-esteem oriented sense appeared at low level when she didn't have religion but rather appeared at high level when the health status of the targeted group was in a good shape. As a result, the hospice volunteer activities can be assumed to increase the middle-aged woman's self-esteem, therefore, turn the consciousness of death into a positive attitude.

병원직원들의 죽음 및 호스피스 인식에 관한 연구 (A Study on Hospital Staff's Perception of Death and Hospice)

  • The purpose of this study was to survey the hospital staff's perception of hospice and death and thereby, suggest the ways to help them have a mature attitude towards and a better understanding of death through an effective education on hospice. For this purpose, this study was designed to provide some data useful for the hospital staff not experienced in facing the dying patients to handle the desperate situation skillfully and engage themselves more effectively in their hospice services. For this study, the researcher conducted a questionnaire survey for the staff of 'C' hospital in Seoul about their hospice philosophy, attitudes towards hospice and perception of death for the period from February, 2006 to March, 2006. A total of 751 subjects responded effectively to the survey. The data collected were analyzed using the SF55 11.0 for ANOVA and T-test in order to test the relationships among subjects' perception of death, their demographic variables, their health condition, their hospice philosophy formed from their experiences of patients' death and hospice services and their attitudes towards hospice. Besides, the correlations among their hospice philosophy, perception of death and attitudes towards hospice were tested. The results of this study can be summarized as follows; First, as a result of testing the relationships among subjects' experiences of health consulting. their experiences of patients' death and hospice and their hospice philosophy, it was confirmed that their experiences of health and death consulting and their experiences of having been educated were relevant. Second, it was found that such variables as health condition, death, experience of hospice and attitude towards hospice were not significantly correlated with each other. Third, as a consequence of testing the relationships among health condition, death, experience of hospice and perception of death, it was disclosed that only the physical health condition was significant. Fourth, it was confirmed that subjects' hospice philosophy, perception of death and attitudes towards hospice were at the usual level on average. Fifth, hospice philosophy, perception of death and attitudes towards hospice were found correlated significantly with each other. Sixth, as a result of the stepwise variable adjustment for such variables as hospice philosophy, perception of death, attitudes towards hospice, it was found that the adjusted r-square value was 0.347 when departments, experience of having been requested by dying patient for consulting, experience of having been educated on hospice, religion and marital status were set as independent variables. And the estimated value of each variable was found significant. Seventh, as a consequence of conducting the multiple regression analysis by setting 'religion' as significant independent variable, it was found that the estimated value of physical health condition was not significant statistically. This, as a result of the simple regression analysis for 'religion' only, its explanatory power was found .197, while its adjusted r-square value was 0.20. Eight, it was found that subjects' attitude towards hospice was significantly correlated with such variables of experience of patient's or relative's death, experience of having been requested for consulting about death, gender, marital status and departments. As a result of the multiple regression analysis and the subsequent stepwise adjustment for this variable, it was found that only 'experience of having been requested for consulting' had some explanatory power: its adjusted r-square value was 0.089. As discussed above, this study tested the correlations among various variables including hospital staff's attitude towards hospice and perception of death and thereby, provided for the data useful for their education on hospice. This study may be significant in that it proved that it would be essential to educate hospital staff on hospice for more effective care of hospice patients and their family members at hospital.

Nature Death Act -Taiwan Experience-

  • Hospice movement in Taiwan emerged early in 1983. There was a nurse visiting terminal cancer patients by herself in Taipei city. It was ceased after one year. This stage of hospice movement might be called as "compassionate era". In early 1990, the first in-patient hospice ward was set up in north Taiwan. She demonstrated high touch in the high technology medical atmosphere. There was a great echo in Taiwan society to this action. In the following years, quite a few new hospice settings were founded. Medical professionals were aroused again to talk and think about life and death, dignity of dying and holistic care. This stage of hospice movement might be called as "ethical stage". Around 2000, obstructions were discovered in our development. We do need system and rules. Standard of setting and care, Curriculum of education and training, Accreditation system and specialist system and Nature Death Act are some of the systems we approached. This stage of hospice movement might be called as "Act stage". Among the "Act stage", the Nature Death Act is actually the mile stone in our history. 존엄사의 법제화에 대한 태도 (Attitudes Toward Legalization of Death with Dignity)

  • 본 연구는 일반성인의 존엄사의 법제화에 대한 의견을 살펴보고자 서울 경기지역에 거주하는 20세이상 남녀 561명을 대상으로 설문조사를 실시하였다. 응답자의 개인적 특성과 죽음과 관련한 개인적 경험, 죽음에 대한 생각, 존엄사 및 법제화에 대한 찬반여부, 법제화 요건에 대한 의견 등의 총 28개 항목에 대해 백분율, 평균분석 및 교차분석을 실시하였다. 조사결과 응답자의 87.3%가 존엄사법제화에 찬성하였으며, 연령이 높아질수록, 기혼인 경우, 부부가구의 경우, 불교를 믿는 경우, 자영업자나 생산직 종사자의 경우, 가족 친지의 투병 및 임종경험, 간병경험이 있는 경우, 사후세계에 대한 믿음이 강한 경우, 유언의 필요성을 느끼고 있는 경우 존엄사의 법제화 찬성율이 더 높은 것으로 조사되었다. 법제화를 위한 요건으로 '본인의사 판단의 근거'를 가장 중요시하였으며, '존엄사의 결정자'에 대해 본인 다음으로 가족의 의견을 중요시하는 것으로 나타났다. 필요서비스로 존엄사 결정시 '본인, 가족과 의사와의 의견조율'에 대한 도움과 '질병경과나 연명치료 과정에 대한 의학적 정보'와 '우울 등 심리상담'을 필요로 하는 것으로 나타났다.

신생아 외과 환자의 수술 후 사망률 변화에 대한 연구 (Mortality Analysis of Surgical Neonates: A 20-year Experience by A Single Surgeon)

  • Pediatric surgery could establish a definitive position in the medical field on the basis of a stable patient population. Neonatal surgery, the core of pediatric surgery, requires highly skilled surgeons. However, recent advancement of prenatal diagnosis followed by intervention and decreased birth rate has resulted in a significant decrease in the neonatal surgical population and the number of surgical operations. The purpose of this study is to examine the outcome of neonatal surgeries and to propose a guide for the future surgeries. A total of 359 neonatal surgical patients operated upon at the Department of Surgery, Ewha Medical Center, during past 21 years were studied. The study period hasbeen divided into two time periods: from 1983 to 1993 and from 1994 to 2004. Analysis was based on the Clinical Classification System and mortality pattern, frequency of disorders, occurrence and cause of death, and other changes. Neonatal surgery was 6.4 % of all pediatric surgery during the total 21 year period, 9.9 % in the first period and 4.8 % in the second. Male to female ratio increased from 2.7:1 to 2.1:1. The overall mortality was 6.7 %, and there was significant decrease from 7.4 % in the first period to 6.0 % in the second. The clinical classification system (CCS) for death cases included class II 2, III 4, and IV 7 during the first period and class III 3, and IV 8 during the second, respectively. According to the mortality pattern by Hazebroek, there were 6 preventable death cases during the first period, and only one in the second, and 2 non-preventable death cases during the first period and 8 in second, respectively. Although the patients in the second period had more serious diseases, surgical mortality has been decreased in the second period, which may be the result of improved surgery methods for newborns and advanced patient care.

응급구조과 학생의 첫 임상 실습 경험에 대한 연구 (An Initial Clinical Experience of EMT Students)

  • This study was conducted to identify an initial clinical experience of EMT students, so to better understand their' experience in clinical training. The subjects were 30 EMT students of C department of Emergency Medical technology in C city, who were demonstrating at the emergency room in C, T, S, W city. This study was approached by phenomenological method, collected data were analyzed by Colaizzi's method. the results were as a follows. From the protocol, significant statements were organized into 34 formulated meanings. from the formulated meanings, 21themes were identified, organized into 8 theme clusters, and then categories. EMT students got experienced 'tension resulting from new situation', 'fear in contacting with patients', 'lack of knowledge and skill', 'confidence feeling from being adapted', 'facing up to Paramedic role', 'experience of death' and 'stress'. The results of this study are to use as basic data for students attending clinical experience for the first time.
