Finger Tip Replantation (수지 첨부 절단의 재접합술)

  • It has been known that the vessels of the finger tip are very small and have many branches, so the anastomosis of this vessel is very difficult. But when this operation achieves success, the replanted finger tip provides adequate soft tissue, restoration of the length, sensory and nail. The authors describe the result of finger tip replantation in 94 patients, 103 finger tips, from October 1999 to June 2002. We performed the salvage procedure in all cases. The success rate of finger tip replantation was 74.8%. We achieved higher success rate, when we succeeded anastomosis of the central artery. The function of the replanted fingers was good. The average TAM was 80% and patient satisfaction was high. We be live that we can achieve higher success rate by anastomosing central artery and performing delicate salvage procedure.

Dynamic Analysis of Finger Joint Torque for Tip Pinch Task (두 점 집기 작업 시 손가락 관절토크의 역학적 해석)

  • This paper presents the dynamic analysis on the joint torque of a finger for the tip pinch task. The dynamic model on finger movement was developed in order to predict the joint torques of an index finger, and the finger was assumed as a three-link planar manipulator. Analysis of the model revealed that the joint stiffness was one of the most important parameters affecting the joint torque. The stiffness of the finger joint was experimentally measured, and it was used in analyzing the finger joint torque required for performing the tip pinch task. The obtained joint torque for the tip pinch task will be used as the design requirements of the finger exoskeletal orthosis actuated by the polymer actuator whose allowable torque limit is relatively low compared to that of a mechanical actuator.

  • A digital micromirror system was proposed for future smart learning. This system is the compact micro-projector with a built-in CMOS sensor modules. It can provide the various interfaces. The basis of interface is to recognize the finger tip on projected image. But the recognition rate of finger tip is very low due to various image degradations. In this paper, we propose the finger tip recognition algorithm that minimize the image degradation factors by using the Retinex transform and IR structuring light. By verifying the availability of the algorithm through experiment, the performance of finger tip recognition was confirmed. Therefore, the user interface can be able to be enhanced significantly in DMS.

Neurovascular Island Graft for Finger Tip Loss (도서형 신경 혈관 피판을 이용한 수지의 피부 및 연부조직 결손의 재건술)

  • Purpose : Loss of sensibility over the finger tip resents a grave deficit and is an indication for sensible soft tissue reconstruction. This paper was performed to assess the long term results obtained by nerovascular island flap. Material and Methods : We performed neurovascular island graft for defective sensibility of finger tip loss in 94 cases since 1979 to 2000. The recipient sites were the thumb pulp defect in 79 cases, the amputated thumb in 9 cases, the amputated index in 4 cases, and the velar aspect of interphalangeal joint of thumb in 2 cases. The donor flaps were obtained from the radial side of ring finger in 63 cases, the ulnar side of the ring finger in 21 cases, and the ulnar side of the middle finger in 10 cases. A mean follow-up period was 5.7 years. Results : The flap quality was well vascularized and survived in 89 cases. The two-point discrimination was average 8.7mm. Because of scar contracture, the range of motion of the donor finger was decreased 3.5% of the normal finger in the distal interphalangeal joint, 8,2% in the proximal interphalangeal joint. A phenomenon of double sensibility occurred in 66 cases. Conclusion : This technique was excellent both aesthetically and functionally as a reconstruction of the Loss of fingertip.

Effect of Trigger Finger on Pain, Grip Strength and Function of Upper Limb of Patients with Carpal Tunnel Syndrome: A Cross-sectional Study (방아쇠수지가 손목터널증후군 환자의 악력, 통증 및 상지기능에 미치는 영향: 단면적 연구)

  • PURPOSE: The purpose of this study was to investigate the effects of trigger finger on pain, muscle strength and function in carpal tunnel syndrome (CTS) patients. METHODS: A total of 60 subjects (30 carpal tunnel syndrome with trigger finger and 30 carpal tunnel syndrome without trigger finger) were assessment for pain, muscle strength (power grip, key pinch , tip to tip pinch, three jaw pinch) and function. The effect sizes of the two groups were compared, and the correlation between the trigger finger and each variable was analyzed. RESULTS: The results showed that there were significantly difference in the pain, muscle strength excluding three jaw pinch and function (p < .05). The results also showed correlation between trigger finger and pain (r = .552), muscle strength excluding three jaw pinch (power grip r = -.296, key pinch r = -.260, tip to tip pinch r = -.285), and function (r = .375). The function of carpal tunnel syndrome patients was related to pain (r = .550) and power grips (r = -.324) of muscle strength. CONCLUSION: In carpal tunnel syndrome patients with trigger finger compared to carpal tunnel syndrome, muscle weakness, pain increase, and function reduction were shown. In addition, trigger finger are correlated with muscle strength, pain and function, and muscle weakness and increased pain affect the daily living of carpal tunnel syndrome patients with triggers finger. Therefore, physical therapy interventions of carpal tunnel syndrome patients with trigger finger should be combined with treatment for muscle strength enhancement as well as pain reduction.

A Case of Finger Tip Metastasis in Patient with Double Primary Cancer of Lung and Lower Lip (폐와 입술의 이중 원발암을 가진 환자에서 손가락 끝으로의 전이 1례)

  • Lung cancer is one of high mortality malignancy. It is known that skin metastasis from lung cancer is uncommon. We report a very rare case of finger tip metastasis from double primary cancer of the lung and lower lip. A 79 year-old man diagnosed with non small cell lung cancer presented with protruding solid mass in his lower lip. It showed central necrosis with purulent discharge. It had appeared rapidly growing features. Simultaneously, another solid mass accompanying painful swelling without skin lesion was found in his left middle finger tip. Both two solid masses were moderately differentiated squamous cell carcinomas. Lower lip mass was a primary cancer, while middle finger tip mass was diagnosed with clinically metastatic cancer from lung or lower lip, which means that it had double primary cancer origin.

  • Although finger tip replantation is popularized nowadays, it is not easy operations even for experienced surgeons. The indication for replantation to treat fingetip amputation is still controversial, but I think replantation for function and cosmetic absolute indication. This paper will discuss about methods of fingertip replantation and unexpected complications on soft tissue after operation.

AR-based Tangible Interaction Using a Finger Fixture for Digital Handheld Products (손가락 고정구를 이용한 휴대용 전자제품의 증강현실기반 감각형 상호작용)

  • In this paper, we propose an AR-based tangible interaction using a finger fixture for virtual evaluation of digital handheld products. To realize tangible interaction between a user and a product in a computer-vision based AR environment, we uses two types of tangible objects: a product-type object and a finger fixture. The product-type object is used to acquire the position and orientation of the product, and the finger fixture is used to recognize the position of a finger tip. The two objects are fabricated by RP technology and AR markers are attached to them. The finger fixture is designed to satisfy various requirements with an ultimate goal that the user holding the finger fixture in his or her index finger can create HMI events by touching specified regions (buttons or sliders) of the product-type object with the finger tip. By assessing the accuracy of the proposed interaction, we have found that it can be applied to a wide variety of digital handheld products whose button size is not less than 6 mm. After performing the design evaluation of several handheld products using the proposed AR-based tangible interaction, we received highly encouraging feedback from users since the proposed interaction is intuitive and tangible enough to provide a feeling like manipulating products with human hands.

Analysis of String Vibraion in Cello (첼로 현의 진동분석)

  • In viol family string instruments vibration of string is transmitted to the front plate through bridge to cause resonance of the instrument body. Therefore, the vibration of string is the source of sound and the properties, the structure, and the boundary conditions of string are expected to have significant effect on the sound. In this study the vibration of strings in cello when one end is fixed and the other end is pressed with finger tip is studied and compared with the case of the fixed-fixed ends. The result of analysis and experiment shows that the vibration of the string that is fixed at one end and pressed with finger tip at the other end is significantly influenced by the contact condition of the string with the finger tip and also quite different from that of the fixed-fixed string, It also suggests that the sound of a cello can be significantly influenced by the contact condition between the string and the finger tip in the fingered string.

Rosition control of a Flexible Finger Driven by Piezoelectric Bimorph Cells Using Fuzzy Algorithms

  • This paper dealt with the position control of a flexible miniature finger driven by piezoelectric bimorph cells, cemented on both side of the finger. Bending moments generated by cells drives the finger, and end-point of the finger is controlled, so as to move in synchrony with fluctation of target and maintain a constant distance between target surface and inger's tip. The voltage applied for the cell is controlled by tip displacement error and error rate. We proposed a PD-Fuzzy controller under conception of PD control strategy. It brought and advantage which reduce number of rules than that of same type conventional fuzzy system and more correct redponse than PID control results.
