We develop a stochastic model to predict the score of a soccer match. We describe the scoring process of the soccer match as a markovian arrival process (MAP). To do this, we define a two-state underlying Markov chain, in which the two states represent the offense and defense states of the two teams to play. Then, we derive the probability vector generating function of the final scores. Numerically inverting this generating function, we obtain the desired probability distribution of the scores. Sample numerical examples are given at the end to demonstrate how to utilize this result to predict the final score of the match.
Objective : This study was done for reporting the effect of $Sa-Am$ acupuncture bladder reinforcing method to $Ryodoraku$ on the patients with chronic low back pain Methods : We investigated 49 cases of patients with chronic low back pain, and devided patients into two groups : We treated one group by needling with $Sa-Am$ acupuncture bladder reinforcing method, and did the other group by needling without $Sa-Am$ acupuncture bladder reinforcing method. And we analyzed the $Ryodoraku$ score (F4). Results : 1. In $Sa-Am$ acupuncture bladder reinforcing method group and non-$Sa-Am$ acupuncture bladder reinforcing method group, compared with baseline, at final, $Ryodoraku$ score (F4) was significantly increased. 2. At final, $Sa-Am$ acupuncture bladder reinforcing method group showed significant increase on $Ryodoraku$ score (F4) score compared with non-$Sa-Am$ acupuncture bladder reinforcing method group. Conclusions : $Sa-Am$ acupuncture bladder reinforcing method showed significant improvement in $Ryodoraku$ score (F4) on chronic low back pain.
Objective : The purpose of this study is to report the effectiveness of a long-term Korean Medicine Treatment on 1035 atopic dermatitis patients Methods : Objective SCORAD Index(OSI) is used for measuring the degree of atopic dermatitis with photos of total 1035 atopic dermatitis patients who has visited oriental clinic from January 9, 2011 to October 31, 2013. Then, OSI was estimated according to gender, age, period of treatment and intensity. Results : 1. The ratio of male to female was 44.4 : 55.6. All patients were constituted with 26.5% of 0~11 year-old group, 15.7% of 12~18 year-old and 57.9% of 19 and more year-old. 2. The ratio of OSI improvement was significantly higher in women than in men after 3 month treatment, but there was no significant difference after 6, 9 and 12 months. 3. The OSI improvement of 0~11 year-old group was significantly higher than the other groups after 3 month treatment. But at the other periods of treatment, there was no significantly difference of the improvement among 3 different groups of age. 4. After 3, 6, 9 and 12 month treatment, the score of OSI was significantly lower than before treatment. After 12 month treatment, the score of OSI was lowest compared to the other periods of treatment. 5. The final OSI score of 87.7% patients was decreased compared to the initial. 6. All patients had been divided with 1.93% of the mild, 22.7% of the moderate and 75.4% of the severe by OSI intensity at initial visit. At final visit, the composition of patients was changed to 14.6% of the mild, 53.7% of the moderate and 31.7% of the severe. 7. The mild patients had got 42.56% of OSI improvement at the final. And the moderate had got 37.72% of OSI improvement at the final. 8. The severe patients had got 36.76% of OSI improvement at the final. And 89.2% of the severe had got decreased final OSI score compared to the initial. Conclusion : There was a definite decreasing of OSI score on the significant rate of Atopic dermatitis patients who had been treated by Korean medicine treatment at least for 3 months.
The purpose of this study is to evaluate the final outcome after arthroscopic calcific removal in the calcific tendinitis of the shoulder joint and to analyze the influencing factors to affect the surgical treatment on the final results. From September, 1993 to March, 2000, arthroscopic removal of the calcific deposit in the shoulder joint was performed in 34 consecutive patients who had had typical symptoms and failed with the conservative treatment and 21 cases of 20 patients could be followed up at least 2 years. Fourteen cases(67%) were located in the supraspinatus, 5 cases(24%) in the infraspinatus and 2 cases(9%) in the subscapularis. Preoperative severity of symptoms was correlated with higher postoperative score. Pain was relieved from 7.6 to 0.9(Visual Analogue Scale:VAS) and UCLA score improved from 13.9 preoperatively to 32.0 postoperatively, but there was no statiscally significant difference in according to the deposit size(P=0.386). Pain and UCLA score improved from 7.9 to 0.4 and from 12.7 to 33.0 respectively when a calcific deposit was located in the supraspinatus, from 7.6 to 1.0 and from 14.8 to 33.4 in the infraspinatus but pain relieved from 6.5 to 4.0 and UCLA score improved from 20.0 to 22.5 in the subscapularis and these outcomes were shown a statiscal significance(P=0.001). Completeness of removal did not affect the final results(P>0,05). Excellent was 23.8% in 5 cases, good 66.7% in 14 cases, fair 4.8% in 1 and poor 4.8% in 1, and patients were satisfied with their final results in 81 %.
Background: The purpose of our study was to investigate short-term outcomes of two-stage reverse total shoulder arthroplasty (RTSA) with an antibiotic-loaded cement spacer for shoulder infection. Methods: Eleven patients with shoulder infection were treated by two-stage RTSA following temporary antibiotic-loaded cement spacer. Of the 11 shoulders, nine had pyogenic arthritis combined with complex conditions such as recurrent infection, extensive osteomyelitis, osteoarthritis, or massive rotator cuff tear and two had periprosthetic joint infection (PJI). The mean follow-up period was 29.9 months (range, 12-48 months) after RTSA. Clinical and radiographic outcomes were evaluated using the visual analog scale (VAS) score for pain, American Shoulder and Elbow Surgeons (ASES) score, subjective shoulder value (SSV), and serial plain radiographs. Results: The mean time from antibiotic-loaded cement spacer to RTSA was 9.2 months (range, 1-35 months). All patients had no clinical and radiographic signs of recurrent infection at final follow-up. The mean final VAS score, ASES score, and SSV were significantly improved from 4.5, 38.6, and 29.1% before RTSA to 1.7, 75.1, and 75.9% at final follow-up, respectively. The mean forward flexion, abduction, external rotation, and internal rotation were improved from 50.0°, 50.9°, 17.7°, and sacrum level before RTSA to 127.3°, 110.0°, 51.8°, and L2 level at final follow-up, respectively. Conclusions: Two-stage RTSA with antibiotic-loaded cement spacer yields satisfactory short-term clinical and radiographic outcomes. In patients with pyogenic arthritis combined with complex conditions or PJI, two-stage RTSA with an antibiotic-loaded cement spacer would be a successful approach to eradicate infection and to improve function with pain relief.
Objectives : To review surgical results of post-fusion lumbar flatback treated with pedicle subtraction osteotomy (PSO) or Smith-Petersen osteotomies (SPOs). Methods : Twenty-eight patients underwent osteotomies. Radiological outcomes by sagittal vertical axis (SVA), and pelvic tilt (PT), T1 pelvic angle (T1PA), and pelvic incidence (PI)-lumbar lordosis (LL) at preoperative, postoperative 1 month, and final were evaluated. Oswestry Disability Index (ODI), visual analog scale (VAS) score of back pain/leg pain, and Scoliosis Research Society-22 score (SRS-22r) were analyzed and compared. Patients were divided into 2 groups (SVA ${\leq}5cm$ : normal, SVA >5 cm : positive) at final and compared outcomes. Results : Nineteen patients (68%) had PSO and the other 9 patients had SPOs with anterior lumbar interbody fusions (ALIFs) (Mean age : 65 years, follow-up : 31 months). The PT, PI-LL, SVA, T1PA were significantly improved at 1 month and at final (p<0.01). VAS score, ODI, and SRS-22r were also significantly improved at the final (p<0.01). 23 patients were restored with normal SVA and the rest 5 patients demonstrated to positive SVA. SVA and T1PA at 1 month and SVA, PI-LL, and T1PA at final were significantly different (p<0.05) while the ODI, VAS, and SRS-22r did not differ significantly between the groups (p>0.05). Common reoperations were early 4 proximal junctional failures (14%) and late four rod fractures. Conclusion : Our results demonstrate that PSO and SPOs with ALIFs at the lower lumbar are significantly improves sagittal balance. For maintenance of normal SVA, PI-LL might be made negative value and T1PA might be less than $11^{\circ}$ even though positive SVA group was also significantly improved clinical outcomes.
이 연구의 목적은 2018 아시안게임 남자 기계체조 마루운동 결승에 진출한 8명의 최우수 체조선수들을 대상으로 연기내용을 분석하는데 있었다. 공인된 디지털 영상자료를 수집하여 마루운동 국제심판인 감독관 1명과 난도심판 2명이 교차 확인하는 연구 설계를 적용하였다. 난도점수를 충족하기 위해 뒤 공중 요소그룹들을 가장 많이 실시하였고 앞 공중 1/1턴, 뒤 공중 5/2턴, 그리고 뒤 공중 3/1턴 동작을 8명의 모든 선수들이 연기한 것으로 나타났다. 점수분포에서 평균 D점수는 6.0, E점수는 7.9, 그리고 연결가점은 0.25점으로 나타났다. 우수한 성적을 거두기 위해서는 D점수는 최소 6.0점 이상, E점수는 8.5 이상, 그리고 연결가점은 0.3 이상의 연기를 실시하는 것이 필요하다. 향후 연구방향은 난도분석 뿐만 아니라 실시점수에 따른 감점분석에 대한 연구가 필요하다.
목적: 소아청소년 환자에서 발생한 개방안구손상의 임상 양상을 분석하여 최종시력을 예측할 수 있는 인자를 알아보고, 최종시력을 예측하는 방법으로 안외상 점수(ocular trauma score, OTS)와 소아 안외상 점수(penetrating ocular trauma score, POTS)를 평가해 보고자 한다. 대상과 방법: 1993년 5월부터 2014년 4월까지 개방안구손상으로 본원에 내원한 18세 이하 소아청소년 환자 77명을 대상으로 의무기록을 후향적으로 분석하였다. 최종시력에 영향을 미칠 수 있는 인자들을 조사하였고, 최종시력을 예측하는 방법으로 OTS와 POTS를 receiver operating characteristic 곡선을 이용하여 평가해 보았다. 결과: 단변량 분석에서 20/200 미만의 초기시력, 안구파열, 7.0 mm 이상의 손상 크기, 망막박리, 수정체이탈, 총 수술 횟수는 불량한 최종시력(<20/200)과 유의한 관련성이 있었다. 반면에 20/32 이상의 좋은 최종시력의 경우 각막중심 침범 유무, 외상백내장 유무, 7.0 mm 미만의 손상 크기, 20/200 이상의 초기시력과 관련성이 있었다. OTS와 POTS 모두 최종시력을 예측하는 방법으로 진단적 가치가 있었으며, 통계적으로 두 점수 체계에 유의한 차이는 없었다. 결론: 소아청소년에게 발생한 개방안구손상에서 초기시력과 안구손상 크기는 최종시력에 영향을 주는 중요한 예측인자이다. 소아청소년 개방안구손상 환자의 최종시력 예측에 있어 기존의 안외상 점수(OTS)뿐만 아니라 소아 안외상 점수(POTS) 또한 좋은 예측도를 보여주어 둘 모두 유용한 평가 방법으로 생각된다.
Objective : This study was to evaluate the effectiveness of Bee Venom acupuncture therapy on external epicondylitis. Methods : We divided chronic arthritis of ankle patient into 2 groups; one group combined bee venom acupuncture therapy and acupuncture therapy, another group was only acupuncture therapy. To estimate the effectiveness of treatment that applied for two groups, we used visual analog scale(VAS). We compared the VAS score of two groups statistically. Results : 1. As a result of evaluation by using visual analog scale(VAS), treatment score at final was marked more higher than score before treatment on each groups. 2. treatment at final, acupuncture and bee venom acupuncture therapy group had significant result on visual analog scale(VAS) compared with acupuncture therapy group. Conclusion : Bee Venom acupuncture therapy can be used with acupuncture therapy for highly effective treatment for external epicondylitis.
The purposes of this study are (1) to investigate the correlation between English word-final stop and the duration of vowels before word-final stop and (2) to suggest a way to detect pronunciation errors and teach the pronunciation of English word-final stops. For these purposes, 18 Korean speakers' production was recorded and analysed using Speech Analyzer and their production was compared with that of native English speakers. In addition, two native English speakers evaluated the subjects' pronunciation. The major findings are the voicing dependent effect of the English vowels produced by native Korean speakers is lower than that of native English speakers; Korean speakers release English word-final stops less than native English speakers; and the pronunciation of English word-final stops and the duration of adjacent vowels are closely related in that the pronunciation score of final stops and the ratio of vowels between the vowels before voiced stops and voiceless stops are correlated. In addition, this study concludes with pedagogical suggestions that may be useful for English pronunciation teaching.
