An Experience of Management of Homeless Neurosurgical Patients

  • Objective : Homeless patients probably have epidemiologic features that are different from those of general population. However, there have been no published articles about clinical characteristics of neurosurgical homeless patients. The authors tried to assess the clinical characteristics and treatment outcome of homeless neurosurgical patients. Methods : We retrospectively reviewed the medical records and radiological films of 76 homeless patients and 72 non-homeless patients following head trauma who were admitted to our neurosurgical department between June 2001 and June 2005. We compared two groups of the patients with the demographics, the clinical and laboratory characteristics. Also, 3D-day mortality was determined according to Glasgow Coma Scale (GCS) score. Results : Age of homeless patients was younger than that of non-homeless patients. Homeless patients had previous craniotomy evidences in skull x-rays more frequently (10.5% vs. 1.4%). Acute subdural hematoma was the most common type of head injury in the two groups. Moderate and severe head injury, based on GCS score on admission was more frequent in homeless patients (64% vs. 39%). Fifty percent of homeless patients underwent operation for traumatic head injury. However, 3D-day mortality according to GCS score was not significantly higher in homeless patients. Conclusion : Most homeless neurological patients were relatively young men. Also, moderate or severe brain injuries were observed more frequently. However, mortality rate of homeless patients in neurosurgical field is not significantly higher in the present study.

Purification and Characterization of an Antilisterial Bacteriocin Produced by Leuconostoc sp. W65

  • This study was carried out to characterize the antilisterial substances produced by Leuconostoc sp. W65 and to evaluate the effects of pH, temperature, and time on inhibitory activity using response surface methodology. Leucocin W65, an antilisterial substance produced by Leuconostoc sp. W65, markedly inhibited the growth of Listeria monocytogenes, L. innocua, and L. ivanovii, whereas other pathogens including Gram-negative bacteria were not susceptible. The pH was the most effective factor with regard to bacteriocin activity, while temperature and time of heat treatment had no significant effect. Fifty percent of inhibitory activity remained after 22.8 min at pH 4.2 and $121^{\circ}C$. Leucocin W65 was purified by ammonium sulfate precipitation, hydrophobic interaction chromatography, and tricine-SDS-PAGE. Compositional analysis originally estimated the peptide to be 56 amino acids in length without asparagine, glutamine, and tryptophane. The sequence of partial N-terminal amino acid residues of purified bacteriocin was identified as follows: $NH_{2}-XGXAGVXPXGGQQPXVPLXYP$.

A Study on the Quality Characteristics of Fish Cakes Containing Rice Flour (쌀가루 첨가 어묵의 품질 특성에 관한 연구)

  • To encourage rice consumption, this study investigated the quality characteristics of fish cakes containing four different amounts (0%: control, 50%, 75%, 100%) of rice flour. The moisture and pH of fish cake dough and fat contents, color values, folding score, texture profiles and consumer acceptability of fried fish cakes containing rice flour were measured. There were no significant difference on moisture and pH of fish cake dough containing rice flour. There was significant decrease in terms of fat contents, folding score of the fish cakes with increasing rice flour(p<.05). While a and b values were increased, L value decreased significantly, as the ratio of rice flour increased(p<.05). Hardness, cohesiveness, gumminess and chewiness of fish cakes with rice flour decreased significantly (p<.05), but no significant difference in springiness. There were no significant differences in overall acceptance, appearance, flavor, color and taste between the control group and treatment groups(p<.05). Texture was decreased significantly as the rice flour increased(p<.05). This study suggests that adding fifty percent rice flour to fish cakes could be a good way to increasing rice flour without decreasing texture acceptability.

  • The purpose of this study was to apply a PERT-type system, a combination of the project evaluation and review technique(PERT) and critical path method(CPM) on the employees' work time management of flood preparation, assembly, transportation and cleaning in home-delivered meals program for elderly The resources allotment heuristic program was developed by considering the number of employees and cooking utilities, being limited resources of home- delivered meals program. This program could assign the employees to perform the works included in flood preparation, assembly, transportation ind cleaning. Critical path and activities ware identified by PERT-type system on the basic of work time investigation in five senior centers. Work sheets were invented to perform the work by the shortest path with flexible employees'maximum flow As a result of the work time investigation, the most prevalent activities were ones of preparation in center C and E. Besides, the preparation(over fifty percent) was the most proportion among flood preparation, assembly, transportation and cleaning in center C and E. Critical path and activities of 'C'center were cucumber in sauce preparation path and assembly, wrapping in assembly path and case delivery in transportation path. Critical path and activities of 'E'center were Pan-fried Potato Preparation Path and assembly, case covering, wrapping in assembly Path. The work sheet invented by the heuristic program and PERT-type system reduced the work completion time and man hours in both centers.

A Study on Work Condition, Stress, Role Conflict and Job Satisfaction of Infection Control Nurses Working in General Hospitals (종합병원 감염관리간호사의 업무실태와 스트레스, 역할갈등 및 직무만족에 관한 연구)

  • Purpose: The purpose of this study was to investigate the work conditions, reported stress, role conflict and job satisfaction of infection control nurses working in general hospitals. Methods: The subjects were 125 infection control nurses from 102 general hospitals. Data were collected using a questionnaire consisting of questions about general characteristics, job characteristics, work condition, stress, role conflict, and job satisfaction. Data were collected from July to September 2011 and analyzed by descriptive statistics, independent t-test, one-way ANOVA, Scheffe test, Pearson's correlation, and Stepwise multiple regression in SPSS/WIN 18.0. Results: Over fifty percent (56.0%) of infection control nurses reported the majority of their time was monitoring intensive units. The average reported stress was 7.20, role conflict was 3.67, and job satisfaction was 3.14. The factors which influenced job satisfaction were stress, enough information, annual income, and the number of hospital beds. Conclusion: The factors contributing to job stress of infection control nurses need to be specifically identified. Education and training for infection control personnel should be provided to enhance job satisfaction.

Attitude of dietitians working for elementary schools on meat products (학교급식 영양사의 육가공식품에 대한 인식 및 사용실태에 관한 조사 연구)

  • This study was conducted to investigate the attitude of dietitians on meat products and consumption of meat products in meal service of elementary school. Eight hundred and forty two questionnaires collected from each district of the country were statistically analyzed. Dietitians showed preference of ham and packed meat to other meat products. Dietitians had a good image on meat product regarding it as a good source of protein, imported food and diversely used food for cooking. Whereas 50% and 25.8% of dietitians concerned about the high content of preservative and sodium, respectively. More than 96% of dietitians suggested the food processing company to decrease the addition of preservatives, coloring agents, color formers and sodium. Fifty nine percent of dietitians responded they would increase the consumption of meat product if it is processed to food with low sodium, low cholesterol, low fat and no persavatives. Dietitians used ham for cooking once or twice a month. The reason they used the meat product for meal service was the preference of students and readiness of meat product for cooking. The dietitian's favorite cooking method was roasting ham or sausage with vegetable. Dietitians responded the first thing they checked for purchasing meat product was the date of processing and the term of validity.

Use of GIS to Develop a Multivariate Habitat Model for the Leopard Cat (Prionailurus bengalensis) in Mountainous Region of Korea

  • A habitat model was developed to delineate potential habitat of the leopard cat (Prionailurus bengalensis) in a mountainous region of Kangwon Province, Korea. Between 1997 and 2005, 224 leopard cat presence sites were recorded in the province in the Nationwide Survey on Natural Environments. Fifty percent of the sites were used to develop a habitat model, and the remaining sites were used to test the model. Fourteen environmental variables related to topographic features, water resources, vegetation and human disturbance were quantified for 112 of the leopard cat presence sites and an equal number of randomly selected sites. Statistical analyses (e.g., t-tests, and Pearson correlation analysis) showed that elevation, ridges, plains, % water cover, distance to water source, vegetated area, deciduous forest, coniferous forest, and distance to paved road differed significantly (P < 0.01) between presence and random sites. Stepwise logistic regression was used to develop a habitat model. Landform type (e.g., ridges vs. plains) is the major topographic factor affecting leopard cat presence. The species also appears to prefer deciduous forests and areas far from paved roads. The habitat map derived from the model correctly classified 93.75% of data from an independent sample of leopard cat presence sites, and the map at a regional scale showed that the cat's habitats are highly fragmented. Protection and restoration of connectivity of critical habitats should be implemented to preserve the leopard cat in mountainous regions of Korea.

Trends of Occupational Health Nursing Research in Korea (한국 산업간호 연구동향: 20년간 한국산업간호학회지 게재논문 분석)

  • Purpose: The purpose of this study was to identify the trends of occupational health nursing research by analyzing the Korean Journal of Occupational Health Nursing for the last 20 years. Methods: A total of 268 published papers during the 20 year period were analyzed with structured analysis forms. Results: Most of the articles were descriptive, but the number of experimental studies increased. Fifty-five percents of the subjects were healthy workers. Most common selection of subjects appeared to be based on convenience. Sixty-eight percent of research instruments were questionnaires and the reliability and the validity of instruments were described in approximately half of the studies. Parametric statistics were the main methods of data analysis, but some advanced statistics were often used rather than simple descriptive statistics. Mainly "health" has been studied (47.9%) among four meta-paradigms of nursing science. Conclusion: It is necessary to develop more rigorous and various research methodology such as longitudinal design, experimental study, or randomized controlled trial. Also, special attention must be paid to studying the environment of workers.

Nitrate Content and Organophosphorus Pesticide Residues in Edible Part of Organic Farming Vegetables (시판 유기농법 재배 채소류의 질산염 및 유기인계 잔류농약의 함량)

  • The NO3 contents and organophosphorus pesticide residues in edible part of vegetables cultivated by a conventional or an organic farming methods were determined. The NO3 contents of vegetables cultivated by the organic farming method were between 120ppm and 4,523ppm, whereas its contents of vegetables cultivated by the conventional farming method were between 89ppm and 1,575ppm. Fifty two percent of vegetables cultivated by hte organic farming method accumulated NO3 content over than 2,000ppm, whereas 82% vegetables cultivated by the conventional farming method accumulated NO3 content below than 1,000ppm. The NO3 contents of lettuce dependent on the cultivation method obviously. The NO3 contents of lettuce cultivated by a hydroponic farming method were between 4,800 and 6,500ppm, whereas those cultivated by the conventional method were between 630 and 750ppm. The organophosphorus pesticide residues in edible part of vegetables cultivated by the conventional or the organic farming methods were not detected. The NO3 contents in edible part of vegetables cultivated by the organic farming method should be considered as one of several parameters to judge a real safe vegetable to be certified by goverment.

The Solution of High Vibration of the Secondary Cooling Pump in HANARO (하나로 2차 냉각펌프의 고진동 해소방안)

  • The heat produced by the fission in the fuel of HANARO, 30 MW of research reactor, was transferred from the primary cooling water to the secondary cooling water through heat exchangers. The secondary cooling water absorbed the heat was circulated by secondary cooling pumps and cooled through 33 MW of cooling tower. Each capacity of the three secondary cooling pumps was fifty percent ($50\%$) of full load. The two pumps were normally operated and the other pump was standby. One of the secondary cooling pumps has often get troubles by high vibration. To release these troubles the pump shaft has been re-aligned, the pump bearing has been replaced with new one, the shaft sleeve has been replaced with new one, the shaft and the impeller have been re-weight balanced representatively or the vibration of motor has been tested by disconnecting the shaft of pump. But the high vibration of pump cannot be cleared. We find out the weight balance trouble of the assembly that the impeller is installed in the shaft. After clearing the trouble, the high vibration is released and the pump is operated with smooth. In this paper the trouble solution of secondary cooling pump is described including the reason of high vibration.
