유방 섬유낭성 질환의 세포학적 진단의 분류 - 세포학적 소견에 의한 등급점수표의 이용에 대한 제안 - (Cytologic Classification of Fibrocystic Disease of the Breast - A Proposal for Use of Cytologic Criteria Grading System -)

  • Fine needle aspiration biopsy has been proved as a safe, accurate and cost-effective diagnostic modality in palpable breast lesions. Cytologically, fibrocystic disease can be classified into 3 categories as nonproliferative breast disease, proliferative breast disease without atypia, and proliferative breast disease with atypia. This terminology for fine needle aspirates is compatible with that of diagnostic histopathology. Cytologic differentiation of nonproliferative disease from proliferative breast disease is important, since the risk of cancer development in cases of atypical hyperplasia is 4-5 times higher than that of general population. Twenty five fine needle aspirates of fibrocystic disease confirmed by subsequent histopathology were re-evaluated and classified into 3 categories depending on their architectural and nuclear features. In addition, these aspirates were scored according to the cytologic grading system, devised by Masood et al. and based on six cytologic criteria. Concordance rates between cytomorpholgic diagnosis and cytologic diagnosis using the cytologic criteria grading system and histologic diagnosis were 88% and 92%, respectively.

유방종괴의 세침흡인세포학의 진단적 가치에 관한 연구 (A Study of Diagnostic Value on Fine Needle Aspiration Cytology of the Breast Masses)

  • This study was performed in order to evaluate the accuracy and the usefulness of the fine needle aspiration cytology (FNAC) on the breast lesions, to compare the FNAC findings between fibroadenoma and fibrocystic disease, and to determine the accuracy of cytologic Black's nuclear grading. The subjects in this study were 110 cases of FNAC, later confirmed by biopsy, between January 1988 and December 1991. The results are as follows ; 1 Comparison between the results of the FNAC and the histologic findings revealed that FNAC had a sensitivity of 96.6%, a specificity of 100%, a false negative rate of 3.4% a false positive rate of 0.0%, and an overall diagnostic accuracy of 98.2%. 2 Semi-quantitative evaluation of epithelial celluarity, stroma, and naked nuclei in the smears of aspirate showed high celluarity in 56.7% of the aspirates from fibroadenoma and in 0% of those from fibrocystic disease. Abundant stroma was found in 46.7% of the fibroadenoma and none of fibrocystic disease. Numerous naked nuclei were found in 60% of the fibroadenoma and 4.5% of the fibrocystic disease. The overall diagnostic accuracy was 98% 3. In order to determine the accuracy of Black's nuclear grading of FNAC on breast carcinoma, we retrospectively studied 38 cases of ductal carcinomas diagnosed by FNAC with subsequent histologic confirmation. The concordance rate with histology was 94.7%. These results suggest that FNAC of breast is a diagnostically accurate method, and provide for the preoperative differential diagnosis between fibroadenoma and fibrocystic disease. Our results also suggest that the evaluation of nuclear grading of FNAC can predict clinical outcome and decide the way of management of breast cancer.

  • Ankylosing spondylitis is a chronic inflammatory multisystem disease that primarily affects the axial joints. Pleuropulmonary involvement is an uncommon extra-articular manifestation of ankylosing spondylitis. There is a wide spectrum of pulmonary parenchymal changes in ankylosing spondylitis, beginning in the early stages of the disease and increasing over time. The lesions are usually asymptomatic, and not visible on chest radiographs in early stages. We reported a case of advanced ankylosing spondylitis in a 56-year-old man with progressive pulmonary bullous fibrocystic changes on both upper lobes that were misdiagnosed as tuberculosis in the early stages of the disease.

유방 종괴의 감별진단을 위한 탈륨 스캔의 유용성 (Usefulness of Thallium Scan for Differential Diagnosis of Breast Mass)

  • 유방종괴의 감별진단을 위해 탈륨스캔을 시행하여 다음과 같은 결과를 얻었다. 1) 유방암 11예 중 10예에서 양성소견을 보였다(sensitivity 90.9%). 2) 위음성 1예는 화학요법을 받았던 환자로 종괴의 크기는 직경 1cm이하였다. 3) 병리조직학적으로 양성인 17예와 추적관찰상 악성을 의심할 수 없었던 3예 등 20예 중 17예에서 탈륨섭취가 없었다(specificity 85% ) 4) 섬유낭성 질환(fibrocystic disease)13예 중 3예에서 위양성 소견을 보였으며 섬유선종(fibroadenoma)3예와 1예의 엽상낭육종(cystosarcoma phyllodes)에서는 섭취를 보이지 않았다. 5) 이상의 결과 악성과 양성 감별의 정확도는 87.1%로써 유방종괴의 감별진단에 탈륨 스캔이 한 방법이 되리라 생각된다.

  • Fine needle aspiration cytologic findings in 14 cases of gynecomastia are described. General cytomorphologic features resemble those of fibrocystic disease in women than those of fibroadenoma. Among the cytologic parameters, three-dimensional structure of epithelial cell clusters, presence of micronucleoli and irregularities of nuclear size and shape are suggestive of epithelial proliferative activity. In audition, 4 cases are proliferative breast disease without atypia and 10 cases are nonproliferative breast disease depending on cytologic criteria grading system.

Lack of Evidence for a Relationship between High Risk Human Papillomaviruses and Breast Cancer in Iranian Patients

  • Background: Whether there is any relationship between human papilloma virus (HPV) and breast carcinoma is not clear. Some previous studies have indicated a possible role in oncogenesis in the breast. In this study, we therefore analyzed the presence of HPV infection in breast tissues of Iranian women from Yazd city. Materials and Methods: In a cross-sectional study, formalin-fixed paraffin-embedded tissues from 87 patients with breast cancer and 84 cases with breast fibrocystic lesions (control group) were selected from a tissue archive. Grade of tumors and fibrocystic tissues were determined by two pathologists. The nested-PCR method was performed for detection of HPVs in samples. HPV genotypes were determined by sequencing and the phylogenetic tree depicted by MEGA software. Results: Of the 87 women with breast cancer, 22.9% (20 isolates) had positive results for HPV DNA. In the control group no HPV was detected. The HPV genotypes in positive samples were HPV-16 (35%) HPV-18 (15%), HPV-6 (45%) and HPV-11 (5%). The data did not approved a significant correlation between tissue pathology of breast cancer and the HPV genotype frequency. Conclusions: The data did not provide any evidence for a role of high risk HPV types in oncogenesis in the breast.

  • Sclerosing polycystic adenosis(SPA) is a pathology of salivary gland which occurs infrequently. It has a controversial etiology. It is a recently described and extremely rare disease entity. Its reactive, sclerosing and inflammatory process is similar to fibrocystic changes and adenosis tumor of breast. Most cases has been reported to be occurred within major salivary glands. This disease is frequently confused with other salivary gland carcinomas, especially because of its histomorphological similarity. Here, we report a case of SPA within the submandibular gland in a 62-year-old woman.

진단이 애매했던 유방 세침흡인 세포검사에서 등급 점수표의 역할 (Role of Cytologic Scoring System in Minimizing "Gray Zone" in Breast Aspiration Cytology)

  • 세침흡인 검사는 촉지되거나 혹은 촉지되지 않는 양성과 악성 유방 질환을 인지하는 매우 유용한 방법이나 진단이 애매한 회색 지역이 존재한다. 1년동안 시행한 697예의 유방 세침흡인 검사 중 111예가 비정형적 혹은 악성이 의심된 다고 보고되었으며 이 중 조직학적으로 확진된 74예를 재검색하여 Masood 등 (1990)이 제안한 등급 점수표가 회색 지역을 줄이는데 유용한지 살펴보았다. 기술적 문제가 19예에서 나타났으며 나머지 55예 중 18예는 양성으로, 37예는 악성으로 조직학적으로 진단되었다. 양성 질환 중 섬유선종(5예)및 섬유선종의 성상을 지닌 섬유낭성 질환(3예)이 가장 많았고 악성은 대부분 침윤성 관암종이였으며 (29예), 관상암종(3예), 사상암종(2예) 및 점액 암종(2예) 등 분화가 좋은 암종들이 상대적으로 많았다. 등급 점수표는 회색 지역을 줄이는데 상당히 유용하여 29예의 관암종중 27예가 세포학적 악성 범주에 속했다. 그러나 분화가 좋은 암종은 7예중 2예만이 세포학적 악성 범주에 속했고 섬유선종 및 섬유선종의 성상을 지닌 섬유낭성 질환은 비교적 높은 점수를 보였다. 따라서 등급 점수표의 응용은 유방 세침흡인 검사에서 양성과 악성 병변의 감별에 매우 유용하게 이용될 수 있으나 분화가 좋은 암종들과 섬유선종의 진단에는 주의가 필요할 것으로 사료되며 유방 세침흡인 세포학적 검사에는 세포 병리 의사의 경험 및 유방질환의 세포학적 변조에 대한 친숙성이 강조되어야 할 것이다.

Breast Screening in North India: A Cost-Effective Cancer Prevention Strategy

  • Objectives: Breast cancer is a leading cause of morbidity and mortality in women worldwide. Breast screening in normal and/or asymptomatic women is essential to reduce the burden of breast malignancies. Our study aimed to identify possible risk- and/or co-factors associated with breast screening in North Indian women. Methods: A public health research survey was conducted among 100 women of North Indian ethnicity during clinic visits in a 6-month timeline (April-October 2012). Demographic and clinical data, including mammography screening, were recorded in the questionnaire-based proforma after conducting a 10 minute interview. Written informed consent was taken from all the participants. Results: The mean age of the participants was $32.2{\pm}9.9$ years. Out of 100 women, 6% had family history of breast disease. Breast-related complaints/malignancy, including galactorrhoea, mastitis, axillary lump, fibrocystic disease, fibroadenosis and adenocarcinoma were observed in 41% participants; age stratification revealed that 82.9% of this group (n=41) were <30 years, while 9.7% and 7.3% were >30 years and 30 years of age, respectively. 32% participants underwent mammography screening and 8% had breast ultrasound imaging. Age stratification in the mammography screening group demonstrated that 24 women were <40 years, while 7 women were >40 years. Conclusions: Our pilot study identified possible co-factors affecting breast screening in North Indian women. These findings may be beneficial in early detection of breast abnormalities, including malignancies in women susceptible to breast cancer, and thus aid in future design of cost-effective screening strategies to reduce the increasing burden of breast carcinoma in women worldwide.

Spectrum of Cytological Findings in Fine Needle Aspiration Cytology of Breast Lumps with Histopathology Correlation: Experience in a Tertiary Care Rural Hospital in India

  • To determine the pattern of disease in patients presenting with breast lumps and to determine the sensitivity and specificity of fine needle aspiration cytology of benign and malignant diseases as a diagnostic tool by correlating with histopathology findings. This retrospective study was carried out in the Department of Pathology, Maharaja Agrasen Medical College, Agroha, from Jan 2008 to April 2012. Fine needle aspiration cytology was performed on 370 cases and out of these 52 cases were received in the Department for histopathological examination. Fibroadenoma was the most common disease encountered, in 88 (24%), with a peak incidence in second and third decade of life. Fibrocystic disease was second, being common in the third and fourth decades of life. Peak incidences of duct ectasia, granulomatous and tubercular mastitis were seen in the third decade. Gynaecomastia showed two peak incidences in second and sixth decades of life. Out of total 370 cases undergoing fine needle aspiration, benign cases were 316 (85.4%), malignant and suspicious were 54 (14.6%) and 10 (2.70%) respectively. Out of total 22 histological confirmed malignant lesions 19 were interpreted as malignant cytologically while two as suspicious and one as benign. All thirty histologically confirmed benign cases were diagnosed as benign cytologically. The sensitivity, specificity, positive and negative predictive values were 98%, 100%, 100% and 96.4% respectively. FNA cytology is highly accurate for diagnosis of breast masses. However, the clinician should correlate FNA cytological results with physical examination and imaging findings to prevent false negative and false positive events and to obtain optimal management of their patients.
