In this paper, we propose impact factors and validity of blood variables on death of COVID-19 patients. The clinical-epidemiological data of 5628 COVID-19 patients, provided from Korea Disease Control and Prevention Agency as day of 30th April 2020, were used. As results, impact factors of death were dementia, older age, high lymphocyte, cancer, dyspnea, COPD, change of consciousness, heart disease, high platelets, abnormal diastolic pressure and fever. The validities of blood variables for death were high in the order of lymphocyte, hemoglobin, hematocrit, platelet and WBC. Therefore, risk factors such as initial clinical characteristics, underlying disease and blood test results, could be regarded for efficient management of COVID-19 patients.
Multiple species of muridae and arvicolidae rodents serve as the natural reserviors of hantaviruses. Hantaviruses are distributed in rodent populations world-widely even in geographical areas where hemorrhagic fever with renal syndrome (HFRS) has not been reported. Serologic diagnosis of infection, using hantaviral antigen, indicates that hantaviruses are widey distributed in wild rodents. This study was designed to intended the hantavirus infection among wild rodents captured in Kyebang mountain, Kangwon-do in Korea. A total of 216 wild rodents in 3 species were trapped in July and September in 1995. Serological evidence for hantaviruses infection were tested against five hantavirus antigens by indirect immunofluorescent antibody technique (IFA). Among 100 Eothenomys regulus, 78 Apodemus peninsulae and 38 Apodemus agrarius; 12 C. regulus, 15 A. peninsulae and 6 A. agrarius were IF antibody positive against hantaviruses. This data suggest that Eothenomys regulus and Apodemus peninsulae would be a natural reservoir of hantaviruses.

Ectodermal dysplasia is a hereditary disease characterized by a congenital dysplasia of one or more ectodermal structures and their accessory appendages. At least 120 subtypes of ectodermal dysplasia have been reported. The disease is usually transmitted as an X-linked recessive trait in which the gene is carried by the female and manifested in the male. Hypohidrotic ectodermal dysplasia is manifested as a triad of defects that include hypohidrosis, hypotrichosis and hypodontia. The characteristic facial features consist of asteatosis, onchodysplasia, sparse and fine blond hair, prominent forehead, a depressed nasal bridge, thick everted lips. The patient may suffer from dry skin, hyperthermia and unexplained high fever as a result of the deficiency of sweat glands. This case report presents detailed procedures of rehabilitating functional and esthetic defect of a 6-year-old boy with hypohidrotic ectodermal dysplasia along with the review of relevant literatures.
Background: Having been known as a virulent disease in 1970s, cancer is now onsidered a chronic disease and 64% of cancer patients live for five years after diagnosis. Home care has gradually gained more importance and it is a great burden on the shoulders of caregivers. Caregivers have to undertake the responsibility of the cancer patient's home management, and organize care and arrange health care services according to the ever-changing condition of patients. Caregivers should be prepared for home care so they can provide accurate and complete care to patients. This descriptive study aims to investigate challenges that caregivers encounter in the home care of patients and the reasons for these challenges. Materials and Methods: The research group consisted of caregivers of outpatients in a daily treatment center in a university hospital. The research sampling consisted of 137 voluntary caregivers of patients who attended the Daily Treatment Center for control, chemotherapy or other supportive cares services between January-June, 2011. Data were collected with face-to-face interviews in the Daily Treatment Center. Ethics Committee approval was taken university hospital; caregivers and their patients were informed about the research and their approval was taken as well. Results: It was found that 54.01% of caregivers help patient's nutrition, 50.36% help medicine use, 26.28% help oral hygiene, 26.28% help to meet urinary needs and 51.82% help to change clothes, 69.34% of caregivers help to change bed sheets, 38.69% help the patient to communicate with their environment and 71.53% help to bring the patient to hospital or outside. Conclusions: This study, it was found that caregivers experience challenges due to following factors: patient nutrition, medicine use, oral and body hygiene, colostomy maintenance and stomach tube feeding, concern of dropping the patient, feeling incompetency in body temperature and fever control, fatigue, and lack of personal time.
The hemolytic uremic syndrome (HUS) is a rare disease of microangiopathic hemolytic anemia, low platelet count and renal impairment. HUS usually occurs in young children after hemorrhagic colitis by shigatoxin-producing enterohemorrhagic E. coli (D+HUS). HUS is the most common cause of acute renal failure in infants and young children, and is a substantial cause of acute mortality and morbidity; however, renal function recovers in most of them. About 10% of children with HUS do not reveal preceding diarrheal illness, and is referred to as D- HUS or atypical HUS. Atypical HUS comprises a heterogeneous group of thrombomicroangiopathy (TMA) triggered by non-enteric infection, virus, drug, malignancies, transplantation, and other underlying medical condition. Emerging data indicate dysregulation of alternative complement pathway in atypical HUS, and genetic analyses have identified mutations of several regulatory genes; i.e. the fluid phase complement regulator Factor H (CFH), the integral membrane regulator membrane cofactor protein (MCP; CD46) and the serine protease Factor I (IF). The uncontrolled activation of the complement alternative pathway results in the excessive consumption of C3. Plasma exchange or plasma infusion is recommended for treatment of, and has dropped the mortality rate. However, overall prognosis is poor, and many patients succumb to end-stage renal disease. Clinical presentations, response to plasma therapy, and outcome after renal transplantation are influenced by the genotype of the complement regulators. Thrombotic thrombocytopenic purpura (TTP), another type of TMA, occurs mainly in adults as an acquired disease accompanied by fever, neurologic deficits and renal abnormalities. However, less frequent cases of congenital or hereditary TTP associated with ADAMTS-13 (a disintegrin and metalloprotease, with thrombospondin 1-like domains 13) gene mutations have been reported, also. Recent advances in molecular genetics better allow various HUS to be distinguished on the basis of their pathogenesis. The genetic analysis of HUS is important in defining the underlying etiology, predicting the genotype-related outcome and optimizing the management of the patients.

Purpose: The purpose of this study is to investigate the incidence of acalculous cholecystitis after multiple trauma with fractures and to analyze the characteristics of cholecystitis. Methods: We performed a retrospective study of multiple trauma patients with fractures between April 2010 and April 2012. Sixty- nine patients were identified, and the average age was 46.8(range: 15-74) years. Data were collected regarding associated injury, injury severity score (ISS), the diagnosis time after trauma, diagnostic tool, and management Results: There were three cases(4.3%) of cholecystitis among the 69 cases, and cholecystitis was diagnosed an average of 20.7(range: 8-33) days after injury. Two patients complained of abdominal pain at diagnosis, but the other patient who had undergone surgery for small bowel perforation at the time of the injury had no abdominal pain. All three patients had abnormal liver function tests (LFTs) at diagnosis. The cholecystitis was confirmed with ultrasonography or computed tomography, and all cases were acalculous cholecystitis. At first, percutaneous transhepatic gallbladder drainage was performed; then, laparoscopic cholecystectomy (LC) was tried an average of 12(range: 11-13) days later. An laparoscopic cholecystectomy was successfully done in only one case, the other cases being converted to an open cholecystectomy due to severe inflammation. Conclusion: The incidence of acaculous cholecystitis was 4.3% after multiple trauma with fractures. We should consider cholecystitis in patients with abdominal pain, fever and elevated LFTs after multiple trauma.
Objectives : Halitosis is a common human condition, exact pathophysiological mechanisms of which are unclear. This study, which investigates halitosis patients' characteristics and their related factors, was done with intent to establish a foundation for the Oriental Medical treatment of halitosis. Methods : 329 patients were surveyed by reviewing medical charts and questionnaires from the East-West Halitosis Clinic of Kyunghee Medical Center from May 1, 2001 to December 31, 2002. Sex, age, illness duration, coating on the tongue, postnasal drip, globus pharyngeus, indigestion, condition of feces, halimeter measurements and results of the salivary scans were analyzed. Results : The thick and yellowish coating on the tongue was not a major cause of halitosis in the patients studied. Among the halitosis patients, 21.6% complained of postnasal drip, 15.6% of globus pharyngeus, 37.4% of indigestion, and 23.8% of diarrhea or constipation, suggesting the possibility of extra-oral origin in oral malodor. Conclusions : This study suggests that, when treating halitosis patients, intra-oral causes of halitosis aside, Oriental Medical treatments such as solving qi-stasis(氣鬱), augmenting deficiency of the spleen qi(碑氣虛) and treating gastric fever(胃熱) constitute effective management for oral malodor patients.
Resistance of Plcsmodium species to antimalarial agents has become increasingly challenging to the management and prevention of malaria. We experienced an imported case of tertian malaria due to Plasmodum viuax relapsed after a seemingly successful treatment with conventional course of hydroxychloroquine and primaquine. A 35-year-old man developed fever three days after return from India and mainland China. After his illness was diagnosed as tertian malaria, he was managed with hydroxy- chloroquine and then primaquine (primaquine base 15 mg/day for 14 days). Thereafter peripheral blood smears showed no malarial parasites, and there was no relapse of symptom until the 55th post-treatment day, however, six months after the above treatment tertian malaria relapsed. He was managed with the same medications again un malaria did not relapse for 10 months.
Background. Infective endocarditis s an uncommon but serious disease in children. Optimal treatment strategy, especially surgical indications, continues to evolve. Method. In this report, we retrospectively reviewed 19 patients who underwent operations in Seoul National University Children's Hospital for infective endocarditis between September 1986 and February 1996. There were 8 male and 11 female patients(mean age 70.6 months) and their main symptoms were fever and dyspnea. Preoperative echocardiography detected vegetation in 14 cases out of 19. Congenital anomalies were in 15 cases and acquired forms were in 4 cases. Causative organisms were identified in 12 cases and Gram-positive cocci were in 7 cases. Adequate preoperative antibiotic management was done. Result. With cardiopulmonary bypass in all cases, surgical corrections including vegetation removal were performed. 4 patients died after operations(21.1%). All the mortality cases were having underlying complex cardiac anomalies. There were no late deaths. Act arial survival rate was 79.0% at 1 year and 79.0% at 5 year and reoperationfree survival rate was 73.7% at 1 year and 73.7% at 5 year. Collclusion. Despite advances in antibiotic therapy, early surgical treatment is imperative in a significant subset, and concurrent intracardiac repair should be appropriate.
This study on the visits of the oriental medical night clinic of patients, was made to be used as reference data by examining and analyzing statistically the many actual conditions of patients who had been visited in Semyung University Attached Oriental Medical Hospital during the period from April, 1998 to March, 1999. The purpose of this survey was to understand the characteristics of patients, to evaluate the roles of Night Clinic of oriental medical hospital, and contribute to the systemic and efficient management of night clinic service. The results obtained were as follows: I. Distribution of sex: male 53% (421 cases), female 47% (379 cases) 2. The age distribution of patients showed the highest in under ten, followed by the thirties and fifties, forties and twenties in order. 3. The monthly distribution of patients showed the highest in October 1998, followed by May 1998 and February 1999 in order. 4. The daily distribution of patients showed the highest in Sunday, followed by Saturday, Monday, Friday and Tuesday in order. 5. The regional distribution revealed the highest in Jecheon with 76% of all patients, followed by Danyang in order. 6. The distribution of arrival time showed the highest in 7:00 pm~0:00 am (27%), followed by 5:30 pm~7:00 pm (23%) in order. 7. The admission rate in patients was 17%. Among them direct visiting rate was 85%. Circulatory systemic disease was the highest. 8. The highest incidences by disease were of motor system with 48% (394cases), followed by diseases in circulatory system with 19% (l48cases). The majority of the patients 67% was connected with two diseases. 9. The chief complaint of pediatric diseases was febrile seizure with 64% (32cases), digestive disease was abdominal pain with 44% (90cases), circulatory diseases was motor disturbance with 43% (83cases), motor system disease was leg pain with 37% (l19cases), respiratory disease was fever with 46% ( 41 cases). 10. In the treatment method, a major portion of treatment methods was acupuncture treatment with 32% (373cases), followed by acupuncture & herbal medicine treatment with 23% (275cases). The most commonly used herbal medicines were Hangsapyunguisan(l6%) and Ojeoksan(l4%).
