  • After ACL reconstruction. abrasion or wear of graft appeared frequently because of contact stresses between femoral tunnel and ACL. To minimize these problems. optimal shape of femoral tunnel is necessary. In this study. we evaluate friction force by degree of wear due to abrasion of soft tissue and develop 3-dimensional FEM model using ANSYS 5.5.1 version to analyze stress growths between femoral tunnel and ACL, We conclude that femoral tunnel angle must be slacked parallel to tunnel direction to minimize contact stress.

  • Gait control capacity for most trans-femoral prostheses is significantly different from that of a normal person, and training is required for a long period of time in order for a patient to walk properly. People become easily tired when wearing a prosthesis or orthosis for a long period typically because the gait angle cannot be smoothly adjusted during wearing. Therefore, to improve the gait control problems of a trans-femoral prosthesis, the proper gait angle is estimated through surface EMG(electromyogram) signals on a normal leg, then the gait posture which the trans-femoral prosthesis should take is calculated in the neural network, which learns the gait kinetics on the basis of the normal leg's gait angle. Based on this predicted angle, a postural control method is proposed and tested adaptively following the patient's gait habit based on the predicted angle. In this study, the gait angle prediction showed accuracy of over $97\%$, and the posture control capacity of over $90\%$.

  • Recent clinical studies have shown that computer navigation for total knee arthroplasty (TKA) provides improved component alignment accuracy. However, femoral stress fracture after computernavigated TKA have been reported due to the pin hole and we hypothesized that osteoporosis would be one of the key factors in pin hole fracture after computer-navigated TKA. We investigated the von-Mises stress around the femoral pin-hole for different elastic modulli and ultimate stresses and four different pin penetration modes to understand the aging effect on femoral stress fracture risk after computer-navigated TKA by finite element analysis. In this study, aging effect was shown to increase the femoral stress fracture risk for all pin penetration modes. Especially, aging effect was shown dramatically in the transcortical pin penetration mode.

A Case Report of Delayed Healing in Femoral Shaft Fractured Child (대퇴골 간부 골절 후 골절유합이 지연된 환아의 한의치료 증례보고)

  • Objectives: The purpose of this study is to report the clinical effects of oriental medical treatment on delayed healing in femoral shaft fractured child. Methods: We treated the patient with herb medicine named Kamiboatang and Pyritum. Child took Kamiboatang and Pyritum for 2 months, and Kamiboatang for 1 month. After 3 months treatments, we examined the case with radiological findings. Results: The patient's delayed femoral shaft fracture was improved after 3 months oriental medical treatment. Conclusions: This case showed that oriental medical treatment on delayed healing in femoral shaft fractured child was effective. To prove the effectiveness of Kamiboatang and Pyritum, the more clinical study of Oriental medical treatment for this case might be also needed.

  • Ipsilateral fractures of proximal femur with shaft and condylar region are very rare. Current concept of management is based on fixation of each fracture as independent entity using separate fixation modalities for proximal and distal parts of femur. However, we considered that antegrade femoral nailing with cephalomedullary screw fixation is a good option for ipsilateral multi-level femoral fractures. Here, we present an experience of satisfactory treatment for ipsilateral femoral neck fracture, subtrochanteric fracture, comminuted shaft fracture with supracondylar fracture following road traffic accident.

  • The measurement of the fermoral anteversion is an important factor in the practice of orthopedic surgery. The anteversion is measured by means of the axis of head and neck of the femur. In this study. widely used computed tomography method of measuring anteversion was tested on femoral necks of patients. Measurement by the manual method and image viewer of computed tomography to determine the anteversion of femoral head were carried out on both femurs. In January and February 2001, 30 patients 3 to 6 years of age were randomly selected from Seoul National University Hospital. the purpose of this paper was to introduce a new method to measure femoral anteversion angle utilizing image viewer program. This new method was more accurate and convenient than the conventional manual method. No significant difference was observed between the manual method and the image viewer measurement of femoral anteversion. In conclusion, image viewer program was statistically more reliable in determining the angle of the fermoral anteversion.

  • The Purpose of this study was ta investigate the change of bone density by Partial weight bearing and non-weight bearing exercise. Twelve female volunteers in good health (between 20 and 30 years of age) were studied as subjects. Subjects were divided into three groups; an experimental group 1(n =4 swimming group), group 2(n = 4 bicycle group) and control group=4. Before and after 11weeks(five times a week), the subjects were examined for change of bond density using a dual energy X-ray absorptiometry. The results were summarized as fellows T . There was increase in bone density of femoral neck in the group that swam but there was no difference in lumbar, femoral neck and femoral ward's triangle region (P<0.05) 2. There was increase in hone density of femoral ward's triangle in the group that swam but there was no difference in lumbar, femoral neck and femoral ward's triangle region(P<0.05) 3. After a aerobic exercise by partial weight bearing and non-weight bearing , there was no difference in the part of excessive muscle's exercise but there is the increase of the bone density in figures.

  • Femoral hernia is very rare in children and is easily misdiagnosed. During a period of three years, three children with femoral hernia were treated by one pediatric surgeon at Severance Hospital. Only one case was diagnosed correctly before surgery, and the others were thought to be either an indirect inguinal hernia or groin mass. Curative hernioplasty (McVay hernioplasty) could be done in only one case at the time of first operation. Diagnosis of femoral hernia in children is a challenge because of rarity and similarity of clinical presentation to indirect inguinal hernia. Co-incidental findings of indirect inguinal hernia sac or patent processus vaginalis during surgery can perpetuate the misdiagnosis. In case of absence of expected indirect inguinal hernia or apparent recurrence of indirect inguinal hernia, one should consider the possibility of femoral hernia.

A Case Report of Avascular Necrosis of the Femoral Head (대퇴골두 무혈성 괴사로 진단받은 고관절 통증 1례에 대한 증례보고)

  • Objective : The etiology and treatment of the avascular necrosis of the femoral head continiue to be controversial and the pathogenesis remains obscure. And its clinical study and report about oriental treatment are absolutely insufficient. Therefore, we report a case of avascular necrosis of the femoral head. Methods : The patient was treated with Bee-Venom therapy, Electro Acupuncture and Sopungwhalhyul-tang(疎風活血湯). Results : After 26days of Oriental treatments, there was remarkable improvement in condition of the patient. Conclusions : In this case, oriental treatments for avascular necrosis of the femoral head was effective. Futher studies are required to prove effectiveness of oriental treatments for treating avascular necrosis of the femoral head.

Oblique Single-Cut Rotation Osteotomy for Correction of Femoral Varus-Torsional Deformities in 3D-Reconstructed Canine Bone Models

  • The purpose of this study was to report the reliability and validity of oblique single-cut rotation osteotomy (OSCRO) in 3D-reconstructed canine bone models with femoral varus and torsional deformities. A healthy adult male beagle was recruited to create a 3D bone model, and this bone model was modified by using a 3D program. Fifteen bone models were constructed for this study. OSCRO simulation was performed in accordance with the plan after printing using a 3D printing machine. The anatomical lateral distal femoral angle (aLDFA), anteversion angle (AA), anatomical caudo-distal femoral angle (aCdDFA), mechanical caudo-distal femoral angle (mCdDFA) and pre- and postoperative bone length were calculated. There were no significant differences between the target values and postoperative values. In addition, the difference between pre- and postoperative bone length was small (p = 0.001). Our findings suggest that OSCRO could be an effective surgical option for MPL with bone deformities in small-breed dogs that often undergo conventional distal femoral osteotomy.
