Long-term Experience with the Bjork-Shiley Monostrut Valve (Bjork-Shiley monostrut 판막의 임상 치험)

  • One hundred and fourty-one Bjork-Shiley Monostrut valves were implanted in 105 consecutive patients from November 1983 to February 1990. There were 61 male and 44 female patients with a mean age of 33.6 years at the surgery. The cummurative follow-up was 370.3 patient-years with a mean of 44.0 months per patient. The operative mortality rate was 3.8%, and the linealized late mortality was 1.18%/patient-year. The incidence of major complications were 4.59%/patient-year, and the actuarial rate of freedom from valve-related morbidity was 80.2$\pm$4.4% at 7 years. The 7-year survival rate was 90.5$\pm$4.1%, and the actuarial rate of freedom from thromboembolism at 7 years was 93.3$\pm$2.3%. The linealized annual rates of complication were ; structural valve failure 0.27% /patient-year, non-structural valve failure 0.54% /patient-year, thromboembolism 1.62% /patient-year; bleeding 1.62% /patient-year; endocarditis 0.54% /patient-year. On the basis of our experience, we judged the Bjork-Shiley Monostrut valve reliable, with similar incidence of valve-related morbidity of other mechanical prosthetic valves.

A Study of Irritable Bowel Syndrome and Stress on Female College Students in Korea (국내 여자 대학생에서 과민성 장증후군과 스트레스에 관한 연구)

  • Objective: The purpose of this study was to provide a database for making better decisions of the treatment of the irritable bowel syndrome (IBS) patient, and for improving the recognition of IBS in the Korean society. Methods: The survey was conducted on 174 female college students in Korea from January 2017 to January 2018. Out of the total students surveyed, 160 questionnaires were analyzed after excluding 14 uncompleted questionnaires. Results: Based on the survey, the prevalence of IBS among female college students in Korea was found to be 13.8%. Statistically, the results proved that there were significant differences in the stress levels between the students with IBS and non-IBS diagnosed by ROME IV criteria. Students with IBS had higher stress levels than students without the ailment. Conclusions: Based on this study, stress management related treatment should be actively considered. Furthermore, it is necessary to develop more efficient and updated guidelines for the treatment of IBS and new patient care programs.

Surgical Treatment of Primary Cardiac Tumor (원발성 심장종양에 대한 외과적 치험)

  • We experienced 6 cases of primary cardiac tumor, all received operation for removal of tumor. Mean age was 43.8 years-old ranging from 17 years-old to 66 years-old. Five cases were female, one case was male. Five cases were benign, myxoma, all located within left atrium. One case was malignant, angiosarcoma within right atrium. All patient showed cardiac manifestations. One case was in NYHA functional class II, two were in III, three were in IV. Four patients showed constitutional symptoms, but no one showed evidence of embolic phenomenon. All case of myxoma showed cardiomegaly except one malignancy. Only one case was regular sinus rhythm, three were sinus tachycardia 8z two were atrial fibrillation. The most common site of tumor origin was fossa ovalis limbus[four of all]. Two of five myxomas received emergency operation, one patient died postoperatively. Lived four patients showed no evidence of recurrence[mean follow-up, 3,5 years], but one patient has Grade II /IV mitral regurgitation & in OPD follow-up now, One malignant case, 17 years-old cerebral palsy female, was angiosarcoma occupied most of right atrial chamber originated from anterior wall of right atrium, received emergency operation which was removal of mass & reconstruction of right atrium with artificial pericardial patch. This patient died on postoperative 36th day due to persistent LCOS[low cardiac output syndrome] with combined sepsis.

Case of Combined Eastern-Western Medicine Treatment on a Patient Diagnosed with Benign Paroxysmal Positional Vertigo after Acute Vestibular Neuritis (양성 돌발성 체위 현훈으로 이행된 급성 전정신경염 환자의 한양방 협진 치료 1례)

  • Objective: This clinical study was performed with a female patient to evaluate the effects of combined Eastern-Western medicine treatment on benign paroxysmal positional vertigo after acute vestibular neuritis.Method: We used acupuncture, herbal medication, Western medication, fluid therapy, and the canalith repositioning maneuver to treat a female patient suffering from dizzy spells. The vertigo scale was checked to assess any improvement in symptoms.Results: The patient had originally recovered from acute vestibular neuritis, but after the first discharge, she came down with benign paroxysmal positional vertigo and had to be rehospitalized. She eventually got better, however, and her score on the vertigo scale improved.Conclusion: This study demonstrates that combined Eastern-Western medicine treatment may be an effective option for treating benign paroxysmal positional vertigo after acute vestibular neuritis, despite the difficulty of the treatment in this case.

A Clinical Case Report of Soyangin Female Patients with Flushing (전신열감을 호소하는 소양인 여환 치험 3례)

  • Objectives The aim of this study was to report improvements on the symptoms of Flushing for three soyangin female patients after using sasang constitutional medicines. Methods The patients were diagnosed with Soyangin Symptomatology and treated with Soyangin sasang constitutional medicines. The patient's subjective symptoms of flushing and discomfort in sleeping were observed using the Visual Analogue Scale(VAS) respectively during the treatment period. And the patient's subjective symptoms of the pain were observed using Numerical Rating Scale(NRS). The period of the patient's defecation interval were also observed. Results and Concluions The symptoms of flushing, discomfort in sleeping and the pain improved remarkedly in all respects. In conclusion, this study shows that Sasang Constitutional Medicine can be an effective treatment for Soyangin patient with Flushing.

Gigantomastia as a Cause of Pulmonary Hypertension

  • Reduction mammaplasty is the gold standard treatment for gigantomastia. We report one female patient with juvenile gigantomastia associated with severe pulmonary hypertension where her pulmonary pressure decreased significantly after the surgery, improving her quality of life. A 22-year-old female patient with gigantomastia since 10 years old, tricuspid regurgitation, and pulmonary thromboembolism antecedent was admitted to the emergency department. Her oxygen saturation was 89%. Acute heart failure management was initiated. An echocardiogram reported left ventricle ejection fraction (LVEF) of 70% with severe right heart dilation, contractile dysfunction, and arterial pulmonary pressure (PASP) of 110 mm Hg. A multidisciplinary team considered gigantomastia could generate a restrictive pattern, so a Thorek reduction mammoplasty with Wise pattern was performed. Presurgical measurements were: sternal notch to nipple-areola complex, right 59 cm, left 56 cm. Three days after surgery, the patient could breathe without oxygen support. In the outpatient follow-up, patient referred reduction of her respiratory symptoms and marked improvement in her quality of life. Six months after surgery, a control echocardiogram showed a LVEF of 62% and PASP of 85 mm Hg. Pulmonary hypertension may be present in patients with gigantomastia. Reduction mammoplasty may be a feasible alternative to improve the cardiac signs and symptoms in patients with medical refractory management.

A case of disseminated strongyloidiasis diagnosed by worms in the urinary sediment

  • Strongyloidiasis is a chronic infection caused by the intestinal nematode parasite Strongyloides stercoralis and is characterized by a diverse spectrum of nonspecific clinical manifestations. This report describe a case of disseminated strongyloidiasis with urination difficulty, generalized weakness, and chronic alcoholism diagnosed through the presence of worms in the urinary sediment. A 53-year-old man was hospitalized for severe abdominal distension and urinary difficulties that started 7-10 days prior. The patient also presented with generalized weakness that had persisted for 3 years, passed loose stools without diarrhea, and complained of dyspnea. In the emergency room, approximately 7 L of urine was collected, in which several free-living female adult and rhabditiform larvae of S. stercoralis, identified through their morphological characteristics and size measurements, were detected via microscopic examination. Rhabditiform larvae of S. stercoralis were also found in the patient's stool. During hospitalization, the patient received treatment for strongyloidiasis, chronic alcoholism, peripheral neurosis, neurogenic bladder, and megaloblastic anemia, and was subsequently discharged with improved generalized conditions. Overall, this report presents a rare case of disseminated strongyloidiasis in which worms were detected in the urinary sediment of a patient with urination difficulties and generalized weakness combined with chronic alcoholism, neurogenic bladder, and megaloblastic anemia.

Development and Evaluation of Cardiovascular Disease Prevention Education Materials for Middle-aged Korean-Chinese Female Workers: Applying Patient Education Materials Assessment Tool for Printable Materials (PEMAT-P) (조선족 중년여성 근로자의 심혈관질환 예방 교육자료 개발 및 평가: Patient Education Materials Assessment Tool for Printable Materials (PEMAT-P) 이용)

  • Purpose: The purpose of this study was to develop and evaluate the quality (understandability and actionability) of health education materials for Korean-Chinese (KC) female migrant workers, using Patient Education Materials Assessment Tool for Printable Materials (PEMAT-P). Methods: Educational needs assessment was conducted with 3 focus groups with 20 KC women and a focus group with 4 community stakeholders. The quality of the educational materials was evaluated by 3 experts and a community stakeholder, followed by a survey with 15 KC women using 17 items for understandability and 7 items for actionability by means of a Korean version PEMAT-P. Results: The health educational calendar consists of 12 subjects out of 9 topics related to healthy lifestyles for preventing cardiovascular diseases. The overall mean understandability score was 98.8% and the overall mean actionability was 100%. Conclusion: Involvement of KC women and community stakeholders in the development of educational materials was found to be an effective strategy for increasing understandability and actionability of educational materials for KC female migrant workers. This study also demonstrates the PEMAT-P is a useful evaluation tool, emphasizing the actionability of educational materials.

Fatigue Diagnostic Measure Research Through The Heart Rate Variability of Chronic Fatigue Patients and Healthy Students in Korean Medical Hospital (한방병원에 내원한 만성 피로환자와 건강인의 심박변이도 측정을 통한 피로 진단 지표 연구)

  • This study compared the heart rate variability tests of healthy college students and chronic fatigue patients for 2 years. Study subjects were idiopathic chronic fatigue patients who carried out the HRV tests and students conducted HRV test when health screening test. Study subjects were 250 people, 104 people and men (41.6%), women were 146 patients (58.4%). In patient group, the autonomic nervous system activity was decreased, the activity of the sympathetic nerve and the parasympathetic nerve were significantly reduced compared to the control group(Patient:84.17±16.27, Control:98.33±17.28). Regardless of gender, patient group's autonomic nervous system activity was decreased, the activity of the sympathetic nerve and the parasympathetic nerve were significantly reduced, compared to the control group(Patient Female:84.17±16.27, Patient Male:84.07±14.96/Control Female:98.33±17.28, Control Male:96.45±16.92). Even though same ages, patient group's autonomic nervous activity was reduced compared to control group(Patient:89.36±12.43, Control:97.39±16.91). Fatigue patients' activity of the sympathetic nerve and the parasympathetic nerve were significantly reduced, regardless of the fact that activity of the sympathetic nerve is increased and the parasympathetic nerve is decreased in stress state like an impassioned injury(七情傷). Therefore HRV test is useful to diagnose Fatigue from this study.
