Case Report of Each Example of a Female and a Male Patient Who Were Infertile and Failed in In Vitro Fertilization(IVF) Treatment (IVF 시술 실패한 여성 불임환자 1례와 남성 불임환자 1례의 임상보고)

  • Purpose: The purpose of this paper is to report the effect of Korean medicine treatments on a female and a male patient who were infertile and failed to in vitro fertilization(IVF) treatment. Methods: The patients are a 30-year-old female who had been diagnosed with the ovulation dysfunction caused by the depression of ovarian function, and a 33-year-old male who had been diagnosed with sperm defect. The female patient alone underwent Korean medicine treatment after the eighth failure in IVF treatment and the male patient received Korean medicine treatment along with his wife after he failed in the first IVF treatment. The Korean medicine applied include herb-medicine administration, acupuncture, moxibustion, Jwa-Hun, and Du-Han-Jok-Yeol treatment. Results: Through Korean medicine treatment the female patient had succeeded in the IVF treatment and she also recovered completely from allergic rhinitis. The male patient whose wife also received Korean medicine treatment with him got his wife pregnant naturally. Conclusions: This result concludes that Korean medicine treatment is effective in infertile patients. Therefore, there needs to be more of subsequent research on infertile patients that is based on Korean medicine treatment.

A Study on Korean Medicine College Students' Perception of Korean Medicine Doctors: Focus on Gender Differences (한의사 직종에 대한 한의대생의 인식 연구 - 성별에 따른 차이를 중심으로)

  • Objectives: This study aimed to investigate gender differences in the Korean Medical Community. Methods: Participants in the research study included 90 students regularly attending the fourth grade in Korean Medicine College, of which 48 were male and 42 were female. Results: 1. Patient's preferences for female KMD (Korean Medicine Doctor) had the lowest response rate in Korean Medical Society. Preferences for male KMD were due to factors including comfort level, rationality, good training, and patient's preference. 2. Patient's preferences for female KMD had the highest response rate in Gynecology, and Pediatrics. On the other hand, patient's preferences for female KMD had the lowest response rate in the other 7 specialty subjects. 3. The reasons for gender discrimination against male KMD were lack of subtlety, lack of empathy, authoritative attitude, and lack of patient management skills. The reasons for gender discrimination, against female KMD were childcare burden, social prejudices, lack of physical strength, and housekeeping burdens. Conclusions: The study results indicated that there was a lower preference for female KMD than male KMD among the Korean Medical Society and in patients' consciousness. Male-centered culture of Korean Medical Society had no role in this finding, but masculine image for KMD jobs and work-family double burden for female KMD were important contributing factors.

Solid and Papillary Cystic Neoplasm of Pancreas in Children (소아에서 발생한 췌장의 유두상 낭성 종양 2예)

  • Solid and papillary cystic neoplasm of pancreas is an uncommon low grade malignant tumor. It is predominant in young female between the second and third decades of life, and amenable to cure by surgical treatment. The authors report two cases of solid and papillary neoplasm of pancreas pathologically verified at Kyung Hee University Hospital. The first case was an 11-years old female patient and the other case was a 12-years old male. Symptoms were abdominal discomfort, nausea and vomiting in both cases and abdominal pain in the female patient. CT finding included a solid and papillary neoplasm of pancreas. The mass was well-demarcated with solid and cystic necrosis components. In the female patient, a large hematoma was found. Gross findings revealed apparent encapsulation, cystic degeneration and hemorrhagic necrosis. Microscopically the tumors were characterized by distinctive solid and papillary patterns of cellular arrangement without local invasion. Both patients were discharged after surgery and followed up without any problem.

Subjectivity of Female Nursing Student Providing Intimate Care to Patient (여자 간호대학생의 친밀한 신체간호에 대한 인식)

  • Nursing students are often embarrassed about offering an intimate care to patients in a clinical environment, and experience stress. In particular, nursing students is often a low level of interaction skill and nursing school has no education program for this intimate care. Therefore, these difficulties are increased while the clinical practice. The purpose of this study was to identify the attitude of female nursing students regarding intimate care to patient and the characteristics of the type by applying Q-methodology, and to create the basic data for the development of education programs for nursing student to prevent stress and embarrassed feeling. Data were collected by applying 35 Q statements to 32 female nursing students. Collected data were analyzed with the PC QUANL program. Results indicate that female nursing students' attitudes of intimate care were divided into four types: 'considering situation-professional obligation type' 'gender related risk avoidance type' 'transcendent professional care type' and 'pursuit of the ideal professional type.' The types of female nursing students' attitudes of intimate care to patient were discussed, focusing on the attitude type identified in this study.

A Study on Gender Difference in Assessing Emotional Design Characteristics of General Hospital's Wards (사용자 성별에 따른 종합병원 병동부 감성디자인 특성 평가에 관한 연구)

  • Purpose: This research was for understanding the attitudinal difference by gender towards emotional design through questionnaire survey with female and male inpatients on environmental characteristics of wards in general hospitals. Methods: The survey was conducted by questioning inpatients at two general hospitals on the importance rating on emotional design elements of patient rooms, lounges, and hallways. Eighty questionnaires were returned and used for data analyses through SPSS windows version 15.0 statistic package program. Results: 1) In general, female patients considered emotional design of wards as more important than male patients and the most outstanding difference was indicated for patient rooms among patient rooms, lounges, and hallways. For patient rooms the comfortability index was rated as the most important to both female and male inpatient groups, and for lounges and hallways the safety index was evaluated as the most important to both gender groups. 2)For lounges, while male patients rated 'prevention of infection' important among safety relating items, women considered 'accident prevention' more important. It is inferred that female patients have more safety needs and anxiety about physical injury or accidents than male ones do. Implications: It is considered that there need to be further succeeding in-depth studies, e.g. research interviews with inpatient;s family members or other caregivers as well as patients themselves.

  • Objectives This study was designed to report treatment of Soyangin Mangeum with gastroesophageal reflux disease. Methods We treated two patients suffering from dyspepsia and chest discomfort. A female patient was treated with Hyeongbangjiwhang-tang. A male patient was treated with Hyungbangsabaek-san. The progress was evaluated with the visual analogue scale for dyspepsia, chest discomfort, and other symptoms. Results A female patient's symptoms such as dyspepsia, chest discomfort were almost cured. but they increased with patient's emotional state repeatedly. A male patient' symptoms such as abdominal pain, dyspepsia, nausea, heart burn was successfully disappeared. Conclusions Hyeongbangjiwhang-tang and Hyungbangsabaek-san can be considered as a treatment for patient with Soyangin Mangyin diagnosed as gastroesophageal reflux disease.

  • Purpose: The purpose of this study is to evaluate the effects of Gyejigabuja-tang on a menopausal female patient complained of excessive sweating after bilateral salpingo-oophorectomy (BSO). Methods: We studied one menopausal female patient complained of excessive sweating who visited OO University Hospital from 10th July 2013 to 23th July 2013. We only treated her with Gyejigabuja-tang. The climacteric symptoms of the patient had been estimated with Numeric Rating Scale (NRS) and Menopause Rating Scale (MRS). Results: After treatment, climacteric symptoms were improved and the score of MRS was reduced from 20 to 10 during 12days. NRS score of sweating was reduced from 10 to 2. NRS scores of fatigue, hot flush, thirst, anorexia, dyspepsia, back pain and cramp of calves also reduced after treatment. Conclusions: This study suggests that Gyejigabuja-tang significantly reduced the climacteric symptoms of patient.

Vietnamese Health Care Providers' Preferences Regarding Recommendation of HPV Vaccines

  • Physician recommendation is an important predictor of HPV vaccine acceptance; however, physician willingness and preferences regarding HPV vaccination may be influenced by factors including patient age, vaccine type, and cost. A cross-sectional survey was administered to a convenience sample of health care providers in Da Nang, Vietnam, to evaluate awareness, perceptions about HPV and HPV vaccines, and willingness to vaccinate a female patient. Willingness to vaccinate was evaluated using a full-factorial presentation of scenarios featuring the following factors: vaccine cost (free vs 1,000,000 VND), patient age (12, 16, or 22 years), and HPV vaccine type (bivalent vs quadrivalent). Responses from 244 providers were analyzed; providers had a mean age of $34{\pm}11.9$ years; a majority were female, married, and had children of their own. Thirty-six percent specialized in obstetrics/gynecology and 24% were providers in family medicine. Of the three factors considered in conjoint analysis, vaccine cost was the most important factor in willingness to vaccinate, followed by patient age, and vaccine type. The most favorable scenario for vaccinating a female patient was when the vaccine was free, the patient was 22 years of age, and the HPV4 vaccine was described. In multivariable analysis, older age, being a physician, being married, and having children were all associated with increased willingness to recommend HPV vaccination (p<0.05). Provider willingness is an important aspect of successful HPV vaccination programs; identifying preferences and biases in recommendation patterns will highlight potential areas for education and intervention.

A Study on the Aspects of Chronic Circulatory Disease Through Treatment Analysis of Employee s Medical Insurance (직장의료보험 수진분석을 통한 만성 순환기계질환의 실태연구)

  • Based on the statistical data of FKMIS during five years from 1981 to 1985, the major findings of studying on the treatments of employee's modical insurance covered people were made as follows. 1. During five years, the total number of trratment case was increased 25.14% annually, and so doubled. The consultation rate was increased from 2.086 to 2.856, showng annual increase of 8.17% and total increase of 36.91%. In the cafe, of in-patient, the rate was from 0.056 to 0.602, . showing annual increase of 2.58%. And in out-patient, the rate was from, 2.030 to 2.794, showing annual increase of 8.31%. The male: female ratio of treatment case was changed from 1 : 1.1 to 1 : 1.2. 2. Case of chronic disease was on the increase .every year. The ratio of medical expenditure of that disease to the total medical expenditure was increased from 22.99% in 1984 to 25.0% in 1985. 3. As a whole, the consultation rate of circulatory disease was increased from 26.10 in 1981 to 46.53 in 1985, showing an nual increase of 15.55 %. The rate of in-patient was increased from 2.06 to 2.94, showing annual increase of 9.30%. The rate of out-patient wag from. 24.04 to 43.59, showing annual increase of 16.04%. 4. The duration (days) of circulatory disease in 1985 by types is as follows. In the case of in-patient, rheumatic fever rheumatic heart disease, (22. 67), ischaemic disease (17.39), cerebrovascular disease (17.18), disease of pulmonary circulation and other from of heart disease (15.82), hypertensive disease (13.18), other disease of circulatory disease(11.55). In the case of out-patient, visiting day (11.57 day) and medical expenditure per case (7,853 won) is lower than that of other diseases (4.39 day, 4,361 won). 5. Cases of circulatory chronic disease were two times as many as those of non-chronic disease. Incidence of the out-patient was shown higher than that of in-patient. In the case of duration per case, the chronic disease(12.92 days) was longer that of non-chronic disease (9.8 day). 6. The male: female ratio of chronic rheumatic heart disease is 34.56 : 65.44 (in-patient) and 34. 67 : 65.33 (out-patient). The consultation rate(case per 1,000 persons) was increased from 1.11 in 1983 to 1.30 in 1985, showing annual increase of 8.22 %. The duration, visiting day, was decreased slightly, but medication day wasincreased from 13.93 in 1983 to 16.72 in 1985, showing annual increase of 9.56%. 7. The male: female ratio of hypertensive disease (case) was 39.36 : 60.64(in-patient) and 40.67 : 59.33 (out-patient). The consultation rate was increased from 19.59 in 1983 to 25.36 in 1985, showing annual increase of 13.78%. Duration, visting day was decreased slightly, but medication day was increased from 11.82 in 1983 to 12.77 in 1985, showing annual increase of 3.94%. 8. The male: female ratio of chronic pulmonary-ischaemic heart disease (case) was 48.90 : 51.10 (in-patient) and 43.66 : 56.34 (out-patient). The consultation rate of chronic pulmonary-ischaemic heart disease was increase from 0.69 in 1983 to 1. 12 in 1985, showing annual increase of 27.40%. Duration, visiting day, was decreased from 2.67 in 1983 to 2.36 in 1985, and medication day was decreased from 0.69 in 1983 to 1.12 in 1985, showing annual decrease of 2.09%. 9. The male: female ratio of cerebrovascular disease (case) was 47.90 : 52.10 (in-patient) and 52.28 : 47.72 (out: patient). The consulatation rate was increased from 2.12 in 1983 to 2.89 in 1985, showing annual increase of 16.76%, Duration, visiting day, was decreased slightly, but medication day was increased from 12. 67 in 1983 to 13.85 in 1985, showing annual increase of 4.55%. 10. In case of artery and capillary disease, the male: female ratio of case was 61.80 : 38.20 (in-patient) and 51.77 : 48.23 (out-patient). But durntion, visiting day, was increased from 3.45 in 1983 to 3.60 in 1985, showing annual increase of 2.15 % and the medication day was increased from 10. 06 to 10.18, showing annual increase of 0.59%.

The Design Project Based on the Proference and the Actual Condition of Patient's Clothes (환자복(患者腹)의 실태(實態)및 선호도(選好度)에 따른 디자인 기획(企劃))

  • This paper has the meaning to plan new design of patient's cloth focusing on its aesthetic aspect through surveying actual condition of use of patient's cloth and reflecting color or pattern that patient prefers to it. Substantial purpose of this paper is 1) to survey actual condition of patient's cloth on the basis of general hospital of downtown of Busan City, 2) to revise difficulty of patient's cloth and survey and analyze color and pattern that patient prefers and 3) to suggest new patient's cloth design using color and pattern that meets function that is not difficult for patient's activity and cure and stabilizes patient's mind on the basis of the result of analysis. The result of this paper is like follow. 1. Problem of current patient's cloth is that most hospital uses patient's cloth having white background and hospital logo of blue or green color and its length is too long so it requires adjustment of length of sleeves and pants 2. The result from analyzing preferred patient's cloth is that patient prefers patient's cloth classified by man and woman, one that its length of sleeves and pants are adjusted and one that there is no collar in its neckline. Regarding color, male patient prefers mild indigo color(5PB7/7) and mild green color (5G9/2) and female patient prefers bright purple color and bright scarlet color(5YR8/7). Regarding pattern, both of man and woman prefers natural pattern. 3. This paper planned total 6 kinds of patient's cloth (common patient's cloth: 2 kinds, male patient's cloth: 2 kinds and female patient's cloth: 2 kinds) through revising difficulty and using new color and pattern according to result of preference.
