요추추간판탈출증군과 단순요통군간의 Current Perception Threshold상의 차이에 관한 연구 (The study of difference between HIVD group and simple low back pain group by Current Perception Threshold test)

  • Objective : The aim of this study is to assess the ability of the CPT test to dignosis radiculopathy in Korean with NeuvalTM CPT database. Method : Electrical stimulation(at 2000, 250, and 5Hz) using Neurometer was applied to the 19 patients who felt back and sciatic pain with herniated intervertevral disc(HIVD) of L-spine, dignosisd by lumbar CT or MRI, and the 33 patients who felt only back or hip or leg pain without HIVD, stenosis, spondylolisthesis which causing radiculopathy. The test sites were toe1, toe3 and toe5 related to L4, L5 and S1 nerve roots. Results : The mean values of the CPT of HIVD group was stiatically lower than LBP group at toe1-250Hz, toe3-2000Hz, 250Hz and toe5-250Hz, 5Hz. The grading CPT score of HIVD group was stastically higher than LBP group at the toe3(L5). The VAS of HIVD group was stastically higher than LBP group. Conclusion : These results suggest that the CPT test can be a valuable testing for diagnosing radiculopathy in Korean. In using CPT test further study is needed for the diagnosis and evaluation of sensory nerve dysfunction in the musculoskeletal disease.

한방병원에 내원한 파킨슨병 환자의 통증의 임상적 특성에 관한 후향적 의무기록 분석 연구 (Clinical Characteristics of Pain in Patients with Parkinson's Disease Who Have Visited a Korean Medical Hospital : A Retrospective Chart Review)

  • Objectives: To investigate the prevalence of pain, clinical characteristics of pain, association between clinical features and pain of patients with Parkinson's Disease(PD). Methods: We undertook a retrospective review of the medical records of patients diagnosed with PD between 2012 and 2019 at Kyung Hee University Korean Medicine Hospital at Gangdong in South Korea. Results: A total of 172 PD patients met entry criteria and 147 out of 172 patients(85.5%) reported pain. In comparison with general population, PD patients has high prevalence of pain. Female PD patients more frequently reported pain than male (P=0.03). 102 out of 147patients(69.3) complained of musculoskeletal pain, and musculoskeletal pain show significant difference depending on the PD motor subtypes (P=0.039). Pain was mainly locatedin the leg (57.8%) in all PD motor subtypes. Tremor-dominant PD more frequently felt pain in upper limb than postural instability-gait difficulty dominant(PIGD) PD, but it was not statistically significant. Conclusions: These findings showed high prevalence of pain in PD patients, the correlation between female and pain, and the relationship between PD motor subtype and pain type. Our study can contribute to the clinical approach based on a more in-depth understanding of PD patients with pain.

도서지역여성의 요실금 체험 (Lived Experience of Women체s Urinary Incontinence in Small Island)

  • This study adopts the phenomenological approach in order to explore the experience of urinary felt by the small island women and to find the meaning and structure of their experience, for the further understanding of them. This study succeeded in detecting five topics and three basic structure from eight participants, and followings are the comprehensive statement of them. The five topics include neglect of care after childbirth, unavoidable life in the tidal flat, shame which cannot be expressed even to their husbands, endless anxiety toward the expected future, and sad(dilemmatic) lived experience. The basic structure is that small island women who have urinary incontinence are apt to regard their disease as a natural destiny of women who fail to get adequate care after childbirth, and something to be endured to live in the seashore. They think of urinary incontinence as something so shameful that they cannot reveal it even to their husband and family. They believe that it even changes their personality since they must always stay alert in order to cope with the situation; for example, when it takes place unexpectedly, like too often to go to toilet, to change the underwears, to wake up in the middle of the night to go to toilet, to try not to laugh loudly, or to have showers. In addition, they accept it as a natural process of aging and incurable disease, and they consider themselves already ruined on the way of becoming uglier. They show dilemmatic abandonment: give it up unwillingly but at the same time think it is natural for others too. The unique experience of small island women with urinary incontinence implied in those statement are inseparable with the specific conditions for survival in the island. Unlike other diseases, it is considered the result of traditionally poor care after childbirth. However this misunderstanding that it is a natural phenomena for all the women who experience childbirth and aging and thereby incurable leads to an undesirable attitude toward urinary incontinence. According to the analysis, environmental conditions specific for small islands make the women there have distinct and unique experience concerned with urinary incontinence. Consequently, the future nursing plan for urinary incontinence in the small island area must be made and enforced with the consideration of these specific phenomenological meanings. Modern Korean nursing has basically been centered to hospital or urban areas. Besides, nursing intervention has long depended upon the research of western countries. This research, however, shows how greatly the regional and cultural characteristics influence the understanding of a certain disease, and is expected to make more specific and in-depth nursing approach enable for those who have urinary incontinence in small islands.

조혈줄기세포이식후 발생한 이식편대숙주병의 구강병소에 대한 최소침습적 레이저조직생검 증례 (Minimally Invasive Laser-Assisted Biopsy of the Oral Lesions for Oral Graft-Versus-Host Disease after Hematopoietic Stem-Cell Transplantation)

  • 조혈줄기세포이식은 면역 결핍 질환, 자가 면역 질환, 악성 조혈 종양의 주된 치료법이다. 이종 조혈줄기세포이식의 주요한 합병증은 이식편대 숙주질환이다. 구강 점막 생검은 이식편대 숙주 질환의 확진과 치료계획 수립을 위해 필요하나 영양 결핍과 화학 요법으로 인해 전신상태가 불량한 환자에게 출혈과 균혈증을 일으킨다. 본 논문에서는 레이저를 이용한 최소 침습적 조직생검의 효능을 평가하였다. 모두 3개의 증례가 소개되었고 모든 환자의 의료 기록, 임상 사진, 조직병리학적 결과가 레이저를 이용한 조직생검의 효능을 평가하기 위해 검토되었다. 모든 환자는 생검 후 불편감이 없었고 심각한 합병증이 발생하지 않았다. 생검 조직의 질이 이식편대 숙주 질환을 확진하는 데에 적당했다. 구강 점막의 레이저를 이용한 최소 침습적 생검은 통상의 조직병리학적 생검에 비해 출혈을 일으키지 않아 감염과 균혈증, 술 후 흉터 생성을 줄이므로 이식편대 숙주 질환의 확진에 이로울 것이다.

When Disease Defines a Place: Batavia in British Diplomatic and Military Narratives, 1775-1850

  • The full impact of COVID-19 has yet to be felt: while it may not define the new decade, it is clear that its immediate significance was to test many of the basic operating assumptions and procedures of global civilization. Even as vaccines are developed and utilized and even as it is possible to see the beginning of the end of COVID-19 as a discrete historical event, it remains unclear as to its ultimate importance. That said, it is evident that the academic exploration of Southeast Asia will also be affected by both the global and regional experiences of the pandemic. "Breakthroughs of Area Studies and ASEAN in the Era of Homo Untact" promises to help reconceptualize the study of the region by highlighting the importance of redefined spatial relationships and new potentially depersonalized modes of communication. This paper acknowledges these issues by suggesting that the transformations caused by the pandemic should motivate scholars to raise new questions about how to understand humanity-particularly as it is defined by societies, nations and regions. Given that COVID-19 (and the response to it) has altered many of the fundamental rhythms of globalized regions, there is sufficient warrant for re-examining both the ways in which disease, health and their related spaces affect the perceptions of Southeast Asia. To achieve "breakthroughs" into the investigation of the region, it makes sense to have another glance at the ways in which the discourses about diseases and health may have helped to inscribe definitions of Southeast Asia-or, at the very least, the nations, societies and peoples who live within it. In order to at least consider these larger issues, the discussion will concentrate on a formative moment in the conceptualization of Southeast Asia-British engagement with the region in the late 18th and early 19th centuries. To that end three themes will be highlighted: (1) the role that British diplomatic and military narratives played in establishing the information priorities required for the construction of colonial knowledge; (2) the importance not only of "colonial knowledge" but information making in its own right; (3) in anticipation of the use of big data, the manner in which manufactured information (related to space and disease) could function in shaping early British perceptions of Southeast Asia-particularly in Batavia and Java. This discussion will suggest that rather than see social distancing or increased communication as the greatest outcome of COVID-19, instead it will be the use of data-that is, big, aggregated biometric data which have not only shaped responses to the pandemic, but remain likely to produce the reconceptualization of both information and knowledge about the region in a way that will be at least as great as that which took place to meet the needs of the "New Imperialism." Furthermore, the definition and articulation of Southeast Asia has often reflected political and security considerations. Yet, the experience of COVID-19 could prove that data and security are now fused into a set of interests critical to policy-makers. Given that the pandemic should accelerate many existing trends, it might be foreseen these developments will herald the triumph of homo indicina: an epistemic condition whereby the human subject has become a kind of index for its harvestable data. If so, the "breakthroughs" for those who study Southeast Asia will follow in due course.

국내 메르스(MERS) 사태가 남긴 과제와 법률에 미친 영향에 대한 소고(小考) (The Study of Effectiveness of MERS on the Law and Remaining Task)

  • 2015년 5월 중동 사우디아라비아와 아랍에미레이트를 다녀온 68세의 남자가 고열, 근육통, 기침 호흡곤란으로 자택부근 2개 병원)과 삼성서울병원(SMC) 응급실을 거치면서 시작된 메르스(MERS)라는 전염병은 거의 3개월에 걸쳐 최근까지 대한민국을 유행성 전염병의 공포로 몰아넣었다. 특히 이 전염병은 중동지역 아라비아 반도를 중심으로 2012년 9월부터 현재까지 총 26개국에서 1,392명에게 발생하여, 538명의 사망환자가 보고(유럽질병센터, 2015.7.21. 기준)되었고, 우리나라의 경우에도 총 확진자 186명, 사망자 36명(치사율 19.4%) 격리자 16,693명이라는 기록을 남기고 점차 우리의 뇌리에서 사라져가고 있다. 이 전염병의 파급으로 인하여 현 정부의 전염병 관리체계에 대한 많은 비판과 문제점을 제기하는 계기가 되었다. 특히 병원내 감염문제, 민간의료기관-지역 보건소-질병관리본부를 연결하는 감염병 관리전달체계의 미작동, 정부의 전염병과 관련된 정보 공개 거부, 냉난방 공조설비의 환기 등과 관련된 시설문제, 감염병 격리병상의 절대부족 문제, 지역보건소의 기능 및 권한의 제한문제, 격리자 및 의료기관에 대한 보상문제, 제도적으로는 감염병에 대한 건강보험 수가 개선문제, 법정감염병 재분류 문제, 질병관리본부의 역할 재정립 문제, 전염병 관련 전문인력 양성을 위한 제도적 보완문제, 병원 이용문화의 개선으로서 다인실 조정과 간병인 대체문제, 사회적 후유증을 치유하기 위한 정책적 배려문제 등이 있으며, 이에 대한 광범위한 논의와 해결방안이 필요한 때이다. 국민들은 2015년 5월 이전에는 아주 낯설었던 메르스라는 외부유입 전염병에 국가 방역체계가 흔들리는 모습을 보고 극도의 공포감을 느꼈으며, 외국에서는 중동지방을 제외하고는 전세계적으로 유례가 없는 메르스라는 전염병이 대유행하는 대한민국에 발길을 끊었다. 국민의 건강권이 심각하게 위협받았음은 물론이고 관광산업과 유통업계 등의 경제도 얼어붙었다. 정부는 메르스라는 전염병 확산의 초기대응 실패를 인정하고, 사태를 악화시킨 점에 대한 책임을 통감하여야 한다. 국민들에 대한 신뢰상실을 반성하고 향후 각종 필요한 정보를 공개함으로써, 메르스와 같은 전염병에 대한 정부정책을 투명하게 집행하고, 지방자치단체와 국민들이 지역사회 감염에 대처하고 확산방지에 나설 수 있도록 제도를 개선하며, 변경된 제도를 홍보하여 전염병 전파 예방에 노력해야 할 것이다.

심혈관대사질환이 있는 지역사회 거주 환자의 질환경험 및 완화의료 요구 (Illness Experiences and Palliative Care Needs in Community Dwelling Persons with Cardiometabolic Diseases)

  • 목적: 본 질적 연구는 비암성 만성 질환이자 중 심혈관대사 질병(심장병, 신장병, 말기 당뇨병)을 가지고 있는 환자들의 "질환 경험"을 이해하고 이들의 완화의료에 대한 요구를 파악하기 위해 수행되었다. 방법: 서술적 질적 연구방법을 통해 환자들의 질환경험 및 자가간호행태, 질병 진행에 대한 준비 등을 알아보았다. 질적 연구 훈련을 받은 연구자가 반구조화된 면담 지침을 통해 연구참여자가 원하는 장소, 시간에 상담을 진행하였다. 면담내용은 필사되었으며, 필사된 내용은 내용분석을 통해 분석되었다. 결과: 연구 참여자는 40대에서 80대의 11명의 만성 질환이자(남성 9명, 여성 2명)였고, 세 개의 범주(같은 질병- 다른 질환경험; 나의 질병, 내 몸의 주체는 나; 질병진행에 대한 준비)가 도출되었다. 연구 참여자들은 자신의 진단명은 알고 있었지만, 의료진으로부터 질병단계 및 진행에 대한 준비, 자가간호 방법, 생애후기계획(돌봄계획) 수립과 관련한 교육을 받은 적이 없었다. 의료진은 질병진행에 따라 처방을 바꿔주는 사람, 약물은 질병치료를 하는 것, 자가간호는 단순 보조적 요법으로 인식되고 있었다. 약복용을 지시대로 규칙적으로 하고 있음에도 불구하고 질병이 진행된다고 느낄 때, 자가간호에 보다 적극적이 되었다. 모든 연구 참여자들이 질병이 진행되고 있다는 것을 느끼고 있었으나, 생애 말기 대비에는 수동적이고 소극적이었다. 많은 환자들이 돌봄계획 수립이나 성년후견인 지정의 필요성에 공감하였으나, 행동으로 옮기지는 못하고 있었다. 결론: 의료진은 비암성 만성질환자의 질환경험을 듣고 질병 관리를 도울 수 있는 치료적 파트너가 되어야 할 것이며, 환자의 건강정보이해능력을 고려한 교육이 제공 되어야 할 것으로 사료된다. 또한 비암성 만성질환자의 웰리빙에 대한 요구와 삶의 질 향상을 지지하는 측면에서 이들을 위한 완화의료 프로그램을 개발, 제공해야 할 것으로 사료된다.

예비치과위생사의 취업에 관한 의식 조사 (A Survey on Preliminary Dental Hygienists' Senses of Employment)

  • The purpose of this study was to conduct a survey on the senses that preliminary dental hygienists have on employment, who applied for the National Dental Hygienist Examination, so that it could provide a basic reference required for fostering good human resources specializing in oral health with a sense of mission and responsibility as professional. To meet these goals, a self-administered questionnaire survey was conducted for participants in a refresher education meeting for preliminary dental hygienists across three major regions(Seoul, Daejeon and Busan), which was held by the Korean Dental Hygienists Association(KDHA) on Jan. 15, 2006. Total 289 sheets of questionnaire were collected, and 262 sheets except invalid or incomplete ones were used for examination and analysis. As a result, this study came to the following conclusions: First, it was found that 40.4% respondents wanted to be employed in dental clinic or hospital. Most respondents(90.1%) answered that faithfulness is most valued as prerequisite qualification for employment in hospital, and highest percentage of respondents(39.3%) wanted to earn 16 to 17 million Korean won annually. For internship opportunities, 61.9% respondents replied that they could willingly accept the internship, provided that basic pay and switchover to regular employee are all assured. Second, as the results of survey on employment circumstances around respondents, it was found that most respondents(58.3%) were employed in dental clinic or hospital, and highest percentage of respondents(35.0%) earned 15 to 16 million Korean won. Third, it was found that interpersonal relationships among employees had most significant effects(4.81) on selection of employment, which was followed by in-house welfare benefits 1(monthly/annual vacation, resting room, etc; 4.56). For possible influential factors on the selection of employment depending on whether employed or not, it was found that there were Significant differences in pay, welfare benefit 2(seminar, orientation, refresher training opportunities, etc), post-wedding continued employment and merit system between employees and non-employees(pE0.05). Fourth, the survey on respondents' occupational senses of dental hygienist showed that major tasks of dental hygienists were represented by oral disease prevention(92.6%), case management(71.5%), oral health education(66.3%), hospital/clinic management and dental management support(10.4%) and public oral health activities(6.7%) respectively. For job satisfaction, it was found that almost half respondents(48.9%) showed satisfaction at their job and 32.6% felt satisfied at and proud of their job. That is, majority of respondents(81.5%) felt satisfied at their job as dental hygienist. For follow-up requirements to qualify for dental hygienists, it was found that 73.3% felt it necessary to give more investments to self-development, 62.2% respondents considered protection of membership's rights as one of future assignments that KDHA should be more committed to. For future social awareness about dental hygienists as occupation, 69.2% respondents expected that they would be better developed and treated as professional. And desired training courses as a part of qualification for professional dental hygienists were represented by case management(services, manners, etc; 33.3%), implant(28.9%), esthetics(correction, prosthesis, whitening; 18.9%) and so on. For an item on working years, it was noted that 75.9% respondents would keep working as dental hygienists as long as they could.

Knowledge and Opinions Regarding Oral Cancer among Yemeni Dental Students

  • Background: Oral cancer presents with high mortality rates, and the likelihood of survival is remarkably superior when detected early. Health care providers, particularly dentists, play a critical role in early detection of oral cancers and should be knowledgeable and skillful in oral cancer diagnosis. Purpose: The aim of the present study was to assess the current knowledge of future Yemeni dentists and their opinions on oral cancer. Materials and Methods: A pretested self-administered questionnaire was distributed to fourth and fifth year dental students. Questions relating to knowledge of oral cancer, risk factors, and opinions on oral cancer prevention and practices were posed. Results: The response rate was 80%. The vast majority of students identified smoking and smokeless tobacco as the major risk factors for oral cancer. Most of the students (92.6%) knew that squamous cell carcinoma is the most common form of oral cancer, and 85.3% were aware that tongue and floor of the mouth are the most likely sites. While the majority showed willingness to advise their patients on risk factors, only 40% felt adequately trained to provide such advice. More than 85% of students admitted that they need further information regarding oral cancer. As expected, students of the final year appeared slightly more knowledgeable regarding risk factors and clinical features of the disease. Conclusions: The findings of the present study suggest that here is a need to reinforce the undergraduate dental curriculum with regards to oral cancer education, particularly in its prevention and early detection.

용접공에서 발생한 구리흄에 의한 금속열 1례 (A Case of Metal Fume Fever Associated with Copper Fume in a Welder)

  • Metal fume fever has been known as an occupational disease is induced by intense inhalation of fresh metal fume with a particle size smaller than $0.5{\mu}m\;to\;1{\mu}m$. The fumes originate from heating metals beyond their boiling point, as happens, for example, in welding operations. Oxidation usually accompanies this process. In most cases, this syndrome is due to exposure to zinc oxide fumes; however, other metals like copper, magnesium, cadmium, manganese, and antimony are also reported to produce such reactions. Authors report a case of metal fume fever suspected to be associated with copper fume inhalation. The patient was a 42-year-old male and was a smoker. He conducted inert gas tungsten arc welding on copper-coated materials without safety precautions such as a protective mask and adequate ventilation. Immediately after work, he felt metallic taste in his mouth. A few hours after welding, he developed headache, chilling sensation, and chest discomfort. He also complained of myalgia, arthralgia, feverish sensation, thirst, and general weakness. Symptoms worsened after repeated copper welding on the next day and subsided gradually following two weeks. Laboratory examination showed a transient increase of neutrophil count, eosinophilia, elevated erythrocyte sedimentation rate, and positive C-reactive proteinemia. Blood and urine copper level was also increased compared to his wife. Before this episode, he experienced above complaints several times after welding with copper materials but welding of other metals did not produce any symptoms. It was suggested that copper fume would have induced metal fume fever in this case. Further investigations are needed to clarify their pathogenic mechanisms.
