미숙아의 수유 프로토콜 개발 및 적용 (Development and Effect of the Feeding Protocol for Preterm Infants)

  • 목적 본 연구는 미숙아를 위한 수유 프로토콜을 개발하고 적용하여 그 효과를 검증하기 위하여 시행되었다. 방법 미숙아 수유 프로토콜의 개발을 위해 전문가 집단을 구성하고, 문헌고찰과 협의를 통해 작성된 프로토콜에 대해 내용타당도 검증과 예비조사를 실시하여 확정하였다. 개발된 프로토콜은 비동등성대조군 전후시차설계로 B시의 P대학교병원에서 태어난 37주 미만의 미숙아 85명에게 적용되었다. 대조군(n=38)에게 2013년 1월부터 4월까지 일반적인 미숙아 수유관리가 시행되었고, 실험군(n=35)에게는 2013년 6월부터 9월까지 본 연구를 통해 개발된 미숙아 수유 프로토콜이 적용되었다. 수집된 자료는 SPSS 18.0 WIN program을 통하여 t-test와 ${\chi}^2$-test로 분석하였다. 결과 실험군은 대조군보다 첫 수유시간(t=2.22, p =.029)과 완전장관영양에 이르는 기간(t=2.28, p =.026)이 통계적으로 유의하게 감소하였다. 그러나 실험군과 대조군의 생후 7일째 체중 감소율(t=-1.23, p =.222)과 합병증 발생률(p >.05)은 통계적으로 유의한 차이가 없었다. 결론 미숙아 수유 프로토콜 적용을 통하여 미숙아들의 첫 수유시간과 완전장관영양에 이르는 기간이 단축되었다.

중환자실 경장영양 환자의 영양지원, 위 잔여량 및 영양상태 (Nutritional Support, Gastric Residual Volume and Nutritional Status during Enteral Nutrition in Intensive Care Unit Patients)

  • Purpose: The purpose of this study was to investigate the nutritional support, gastric residual volume, and nutritional status of the intensive care unit (ICU) patients on enteral feeding. Methods: A descriptive longitudinal design was used to collect 5 day data on enteral nutrition of 52 ICU patients in an university hospital. Nutritional support was calculated with actual caloric intake compared to individual caloric requirement. Residual volumes were measured prior to routine feedings, and the serum albumin levels and the total lymphocyte counts were checked to evaluate nutritional status. The data were analyzed using one group repeated measures ANOVA, paired t-test, and Spearman's bivariate correlation analysis. Results: The subjects received their first enteral feeding on the $5.75^{th}$ day of ICU admission. The mean nutritional support rate was 49.1% of the requirement, however prescription rate and support rate were increased as time goes by. Gastric residual volumes were less than 10 cc in 95% cases. A significant negative correlation was found between nutritional support and nutritional status. Conclusion: The nutritional support for ICU patient was low compared to the requirement, and their nutritional status was worse than at the time of ICU admission. Further studies are necessary to develop nursing interventions for improving nutritional support for ICU patients.

The Potential of Gastrophysa viridula as a Biological Control Agent for Rumex obtusifolius

  • This study was carried out to see whether Gastrophysa viridula (Degeer) (Coleoptera : Chrysomelidae) could be used as a biological control agent for Rumex obtusifolius L., with human manipulation of the beetle population. The study was consisted of three experimental sets : Wet-Dry experiment (the wet weight Vs dry weight relationship of Rumex obtusifolius L.), Greenhouse feeding experiment, and Field experiment. There was a significant correlation between the total wet and dry weight of Rumex obtusifolius as follows : Total dry weight : -0.23542+ (0.17514${\times}$Total wet weight) ($R^2$=0.9317, p=0.047, T=16.927 (dF=21)). In the Greenhouse feeding experiment, the result was very promising. The relationship between the density unit of the beetles and the growth of the plant is given below (20 day) : Plant growth =105.8+(-34.4${\times}$Density unit) ($R^2$=0.76, p=0.13). A repeated introduction of the beetle population into the field vegetation of R. obtusifolius from April to October is suggested to see the beetle's grazing ability on the plant. This study shows that the potential grazing power of the beetle on Rumex obtusifolius was enough to defoliate the plants, but it was able to recover from its root reserves. The practical question remains as to whether repeated additions (by man) of the beetles to Rumex obtusifolius could eliminate them.

홀 센서를 이용한 자동연료공급 제어장치에 관한 연구 (A Study on the Auto Fuel Feeding Control System using Hall Sensor)

  • 일반적으로 유체나 연료 공급 장치는 모터를 제어하기 위한 특별한 센서가 필요하고, 센서를 부착하기 위한 보조 장치와 부착하기 위한 공간이 필요하다. 또한 신호선 및 기타 배선 등이 필요하여 어려운 점이 따르게 된다. 본 논문에서는, 불편함과 문제점을 해결하기 위하여 홀센서를 이용한 제어시스템을 제안하였고, 시스템의 제어원리는 모터 동작 단계에 따라 전류흐름에 차이가 발생한다는 것을 이용하였다. 또한 제어 시스템이 우수할 뿐만 아니라 센서 설치에 따른 고장을 줄일 수 있었고, 재료비 및 인건비를 절감할 수 있다. Auto-Pumping 시스템은 Soft-Ware적응용을 통해 모든 유체 공급 시스템에 적용할 수 있으며, 차후에 시스템을 이용한 여러 종류의 장치를 개발할 수 있으리라 본다.

고속 생산형 필름 진동판 성형기 및 금형 국산화 개발(II) - 다량 생산 진동판 성형기 - (Domestic Development of Vibrational Film Forming Machine and Die in the High Speed Production(II) - Multi-production forming machine -)

  • This study consists of two parts. The first discusses the development of a single production forming machine which was reported in earlier papers. The second outlines the development of a multi-production forming machine, which consists primarily of a film feeding unit, an unwinding unit, and a heating block unit. The heating block unit of the multi-production forming machine has 30 members per die. An analysis of the stress deformation and temperature deviation of this machine is carried out using ANSYS Workbench and CFX-11 under the design conditions. According to this analysis, the maximum deflection in the Z-direction is $0.05104{\mu}m$ and the maximum temperature deviation is $0.7^{\circ}C$ when the temperature of the heating block unit is $175^{\circ}C$. It was also found that these values are structurally safe. The advantage of the developed multi-production forming machine is demonstrated to be in its offering of a proper voice test.

서울.경기지역 신샌아의 수면/활동 양상에 관한 연구 (A Study on the Sleep pattern of normal neonates in Seoul and Kyungki area)

  • The sleep is the most important problem for new parents. However, there were very rare the study about sleep of normal neonates and early infants in Korea. So we attempted pilot survey study of neonates' sleep pattern. The purpose of this study was to investigate the sleep pattern of normal neonates in Seoul and Kyungki area and to provide for basis for nursing intervention for parents of them. The subjects of this study were 37 normal full-term neonates of 2~3weeks of age who were visited by researcher or assistant at postpartum care unit, health care clinic, outpatient clinic located in Seoul and Kyungki area from April 20, 1999 to March 15, 2000. This study was descriptive survey study under the convenient sampling method. The instrument of this study was the NCASA translated by Korean Parent Child Health Academic Association. The collected data were coded as 1/4hour unit, and analyzed by MS Office Excel program. The result of this study were as followed: 1. The mean amount of total sleep was 15.1hours. The mean amount of daytime sleep was 9.5hours. The mean amount of nighttime sleep was 5.7 hours. The mean amount of longest sleep was 2.9hours. The mean amount of shortest sleep was 0.6hour. 2. The mean amount of total activity was 8.9hours. The mean amount of daytime activity was 6.5 hours. The mean amount of nighttime activity was 2.3hours. The frequency of feeding was 10.4 times a day. The frequency of daytime feeding was 7.3 times. The frequency of nighttime feeding was 3.1 times. It is suggested that various subjects and more data according to age, and comparative study among different cultures are needed for further study in the future.

Effect of Fermented Soybean "Natto" Supplement on Egg Production and Qualities

  • Natto is a Japanese traditional soybean product fermented by Bacillus natto. The effects of dried fermented soybean (natto) supplement on egg production and egg qualities of layer chickens was studied with regard to the effective use of various waste foods in Japan. Dried natto, prepared by heating at $60^{\circ}C$, was added to a basic diet at a level of up to 3%. Forty 166-wk-old layer chickens (Rhode Island Red) were randomly divided into 4 groups and five layer chickens were used in each group with two replicates. Layer chickens in group 1 were fed a basic diet as the control. The remaining 3 groups were fed the basic diet supplemented with dried natto at levels of 1, 2, and 3% (w/w), respectively. The result did not show improvements in egg production or feed conversion ratio of layer chickens even when 3% dried natto was added to the control diet. The egg qualities including egg weight, eggshell strength and thickness, yolk color, yolk weight, albumen height, and Haugh unit were also not improved. However, the feeding of dried natto changed the cholesterol content in the egg yolk. The supplementation of dried natto showed the tendency to decrease the yolk cholesterol after 12-wk of feeding compared to the control diet though it did not change plasma cholesterol levels in the blood. On the other hand, yolk cholesterol decreased significantly after 12-wk of feeding 3% dried natto (p<0.05).

도서지역 굴토끼(Oryctolagus cuniculus) 침입에 따른 피해와 관리 (Damage and Management by Invasive European Rabbit (Oryctolagus cuniculus) in Island)

  • 본 연구는 도서지역에 침입한 굴토끼(Oryctolagus cuniculus)의 서식양상과 섭식 영향을 밝혀 효과적인 관리에 필요한 기초자료를 제공하고자 수행되었다. 조사 대상지인 범섬과 까막섬은 도입식물의 식재, 답압, 경작활동 등 인위적 간섭에 의해 교란된 이차식생 유형이다. 범섬에 정착한 굴토끼는 벌채지를 중심으로 높은 흔적밀도를 보였고, 까막섬에 정착한 굴토끼는 낙엽활엽수림대를 중심으로 높은 흔적밀도를 나타내었다. 조사지역에서 확인된 굴토끼 섭식 가해 식물은 총 12과 17분류군으로 확인되었다. 범섬에서는 총 9과 11분류군의 섭식 가해 식물이 확인되었고, 먹이 가용 식물이 대부분 소실된 까막섬에서는 총 5과 6분류군의 섭식 가해 식물이 확인되었다. 범섬의 식생지수는 도서 전체 면적의 약 6.6% 감소하였고, 까막섬의 식생지수는 전체 도서지역에서 감소하였다. 까막섬은 Enhanced Vegetation Index가 -0.008unit/yr 이상 감소하는 지역이 전체의 5.2%였으며, -0.008~-0.006unit/yr으로 감소하는 지역은 전체의 13.32%로 확인되어 심각한 식생 교란과 생물다양성 손실이 예상되므로 도서생태계의 보전과 복원을 위한 즉각적인 관리가 필요하다.

신생아 집중치료실에서 침습 칸디다 감염의 예방 (Prevention of Invasive Candida Infections in the Neonatal Intensive Care Unit)

  • Invasive Candida infections (ICI) have become the third most common cause of late-onset infection among premature infants in the neonatal intensive care unit (NICU). Risk factors include birth weight less than 1,000 g, exposure to more than two antimicrobials, third generation cephalosporin exposure, parenteral nutrition including lipid emulsion, central venous catheter, and abdominal surgery. Candida colonization of the skin and gastrointestinal tract is an important first step in the pathogenesis of invasive disease. Strict infection control measures against the infection should be done in the NICU. The following practices are likely to contribute to reducing the rate of ICI: (1) restriction of broad-spectrum antibiotics, antacids and steroid; (2) introduction of early feeding and promoting breast milk. Fluconazole prophylaxis may be an effective control measure to prevent Candida colonization and infections in individual units with high incidence of fungal infection. In addition, there is a need of further data including the development of resistant strains and the effect on long-term neurodevelopmental outcomes of infants exposed to drugs before the initiation of routine application of antifungal prophylaxis in the NICU.

무심연삭 시스템의 신뢰성 예측 (Reliability prediction of Centerless grinding machine)

  • As recently optical communication industry is developed, request of optical communication part is increased. Ferrule is very important part which determines transmission efficiency and quality of information in the optical communication part. Most of ferrule processes are grinding which request high processing precision. The ultra precision centerless grinding machine for ferrule grinding was designed. The centerless grinding machine is composed of the high damping bed, grinding wheel spindle unit, regulating wheel spindle unit, feeding table and dressing unit. Reliability prediction was very important for the high quality design. In this study, centerless grinding machine was predicted reliability.
